Panophthalmitis With Orbital Cellulitis Following Glaucoma Drainage Implant Surgery in an Adult Patient.

BACKGROUND Glaucoma drainage implants, including the Paul glaucoma implant, are widely used in the management of refractory glaucoma when medical therapy and conventional filtering surgery prove insufficient. Although associated with favorable outcomes, rare but serious postoperative complications such as endophthalmitis and panophthalmitis can occur, resulting in significant visual morbidity. CASE REPORT We describe the case of a 77-year-old man with chronic angle-closure glaucoma who developed acute panophthalmitis with orbital cellulitis 12 days following uneventful Paul glaucoma implant su

Rare Case of Disseminated Histoplasmosis Presenting as Endogenous Histoplasma Capsulatum Endophthalmitis in an Immunocompetent Patient.

This report presents a case of a 75-year-old man living in Arkansas, United States, who presented with subacute eye pain and severe vision loss in the left eye for 5 to 6 weeks and a history of 40-lb unintentional weight loss over the previous year. Exam revealed panuveitis initially suspicious for endogenous bacterial versus fungal endophthalmitis in the context of recent intravenous (IV) vitamin infusions. However, a systemic work-up revealed a positive serum and urine Histoplasma antigen, diagnostic for disseminated histoplasmosis. Vitreous cultures were negative, but pan-fungal polymerase

A case of orbital compartment syndrome caused by orbital abscess in an immunocompetent pediatric patient.

Orbital compartment syndrome, a condition in which increased pressure within the orbit can cause permanent visual impairment, can be caused by infection or trauma. Orbital cellulitis is a spectrum of infection that can progress to abscess formation within the orbit, subsequently leading to orbital compartment syndrome. We describe the case of an immunocompetent pediatric patient who developed an orbital abscess from asymptomatic sinusitis, which led to orbital compartment syndrome. He received a lateral canthotomy and cantholysis followed by definitive management of his orbital abscess and exp

Phenotypic characterization, molecular typing, and clonal relatedness within Staphylococcus isolates in healthy ocular conditions.

This study aimed to explore the culturable bacterial community in healthy volunteers, with a focus on the staphylococcal population. First, 120 bacterial isolates were collected from conjunctival swabs of 49 healthy volunteers (aged 18 to 77 years). Isolates were characterized by phenotypic analyses, and 68 of them were confirmed as staphylococci by MALDI-TOF/MS and a multiplex PCR assay. The Staphylococcus strains (n = 68) were tested for resistance to the most relevant clinical antibiotics. Clonal relationships among isolates were evaluated by pulsed-field gel electrophoresis (PFGE), and the

Bacterial keratitis: a global review of current practices, challenges and innovations.

Bacterial keratitis is a major cause of corneal blindness worldwide, with marked differences in clinical presentation and risk factors across regions. In high-income countries, contact lens use is the leading risk factor, while in low- and middle-income countries, trauma-related infections are more prevalent. Management relies on timely diagnosis and sensitivity-guided topical antibiotic therapy. Staphylococcus and Streptococcus species are the most frequently isolated Gram-positive organisms, while Pseudomonas aeruginosa is the leading Gram-negative pathogen, especially in contact lens-relate

Acute-onset endophthalmitis following pars plana vitrectomy for symptomatic vitreous opacities: a case series.

BACKGROUND: Pars plana vitrectomy (PPV) for symptomatic vitreous opacities, commonly referred to as floaterectomy, is an increasingly performed elective procedure that can meaningfully improve quality of life. However, like all intraocular surgery, it carries the risk of endophthalmitis. To our knowledge, only 2 cases of this complication following floaterectomy have previously been reported. The purpose of this study is to report the largest series of acute-onset bacterial endophthalmitis following floaterectomy, characterizing clinical presentation, causative organisms, antibiotic susceptibi

Frontiers in Ocular Therapy: Linking Nanoparticles to Patho- Therapeutic Outcomes in Keratitis Treatment.

Keratitis is a critical inflammation of the cornea caused by chemical agents due to bacterial, fungal, or viral pathogens. It significantly contributes to the global burden of vision loss and blindness in older adults. Antimicrobial treatment helps fight different types of bacteria. Conventional treatments are less effective owing to poor ocular drug bioavailability, rapid tear clearance, and drug-resistant microbes. Recent developments in nanotechnology hold promise for circumventing these limitations via improved delivery, targeting, and combination therapies. This study provides a critical

Tetrasulfide-bridged virus-like particles enable host-sparing cascade membrane-metabolic disruption for drug-resistant bacterial keratitis.

Drug-resistant bacterial keratitis remains difficult to treat because anatomical barriers and biofilm-associated tolerance restrict antibacterial access and efficacy. Intensifying bactericidal activity can enhance bacterial clearance, but may also cause nonspecific host injury and disrupt stromal homeostasis. Thus, effective therapy requires a strategy that redirects antibacterial action from bacterial surface killing to intracellular intervention without perturbing the corneal stromal microenvironment. Herein, we developed AuNR@vTSMS-DLYS, a virus-mimetic spiky nanoplatform that couples bacte

Diagnosis of chlamydial and gonococcal conjunctivitis: performance evaluation of a nucleic acid amplification test.

UNLABELLED: No regulatory agency-approved nucleic acid amplification tests (NAATs) exist for detecting Chlamydia trachomatis (CT) or Neisseria gonorrhoeae (NG) in conjunctival samples, necessitating validation, including performance analysis. This is challenging due to case rarity and the absence of a gold standard. This retrospective case series evaluated the performance of the Roche cobas CT/NG NAAT on conjunctival samples using microbiological and clinical data. Clinical records of patients with positive CT/NG NAAT conjunctival samples processed in our laboratory between 2013 and 2023 were

Training Data Recorders with a Manual to Take High-Quality Trachoma Photographs in South Sudan.

When a country has reached a low trachoma prevalence, it can be difficult to reliably train survey graders to accurately grade trachoma signs. Conjunctival photography provides an opportunity to capture images for grading. Recently, a training manual targeting survey data recorders was published. In July 2023, this manual was used during a photography training, after which certified trainees served as data recorders and photographers during the Enhancing the 'A' in SAFE (ETAS) study in South Sudan. Five of 10 trainees passed the training and became certified. These individuals took 7,064 smart
