Histologically indolent but widely disseminated diffuse midline glioma H3K27-altered revealed by autopsy.

Diffuse midline glioma, H3K27-altered is a highly aggressive central nervous system tumor classified as grade 4 in the 2021 World Health Organization Classification of Tumors of the Central Nervous System. This malignancy typically arises in midline structures such as the thalamus, brainstem, and spinal cord, and is characterized by its infiltrative capacity. Despite the use of magnetic resonance imaging to evaluate tumor extension, its microscopic dissemination can extend far beyond radiological detection. We report the case of a 46-year-old woman with a thalamic diffuse midline glioma harbor

Bridging veins govern discrepancy of CSF efflux into dura mater in mice.

Meningeal lymphatic vessels within the dura provide a route for cerebrospinal fluid (CSF) outflow, but how CSF crosses the arachnoid barrier, particularly toward basal dural compartments, remains unclear. Using in vivo CSF tracer infusion and ultrastructural analysis in mice, we show that bridging vein-associated arachnoid cuff exit (ACE) domains provide specialized routes for CSF movement from the SAS into the dura. These ACE-associated structures are abundant in the basal than dorsal meninges, supporting preferential CSF efflux toward basal meningeal lymphatics. In contrast, spinal meninges

Decellularized CNS-Derived ECM Hydrogels as Dual-Action Scaffolds for Angiogenesis and Neurogenesis.

Neurodegenerative disorders of the central nervous system (CNS) remain a significant clinical challenge due to the limited regenerative capacity of neural tissues. Herein, bioactive scaffolds were developed from decellularized extracellular matrices (dECMs) derived from four CNS tissues: brain, spinal cord, spinal meninges, and optic nerve. The scaffolds were comprehensively characterized with respect to biochemical composition, mechanical properties, and biological performance. Optimized decellularization protocols efficiently removed cellular components while preserving key matrix constituen

Primary spinal meningeal melanoma with intramedullary and intradural extramedullary components-a case report.

Primary melanomas of the spinal meninges are exceedingly rare. While both intramedullary and extramedullary spinal melanomas have been reported, to the best of our knowledge, this is the first noted case of primary spinal melanoma that has both intramedullary and intradural extramedullary components. We present a case of a 61-year-old female presenting with a 1-year history of lower back pain, bilateral lower extremity pain, and perceived weakness of left foot. Magnetic resonance imaging of the thoracic spine suggested intramedullary and intradural extramedullary mass at levels T8-T12. A T7-T1

An integrated classification to guide the management of spinal meninges pathologies.

PURPOSE: The terminology used for spinal meningeal pathologies is often inconsistent, with multiple terms referring to similar or related anatomical anomalies. This lack of standardization leads to variability in the management of these lesions, with differing diagnostic and therapeutic approaches for similar conditions. Our aim was to revise the classification of spinal meningeal pathologies by integrating pathogenesis, diagnostic criteria, and therapeutic strategies. METHODS: We first conducted a descriptive cohort study over a ten-year period at a single neurosurgical reference center. Cli

Diabetes is associated with a decreased risk of meningiomas: a mendelian randomization study.

BACKGROUND: Meningiomas are the commonest primary intracranial tumors, but the risk factors are complex and not fully understood. Type 1 and 2 diabetes mellitus (T1DM and T2DM) are frequently occurring endocrine diseases, but their relationship with meningiomas is debated. Obesity and elevated body mass index (BMI) are known risk factors for meningiomas but often confound with diabetes. We aim to address this controversy using mendelian randomization a method inherently robust to confounding. METHODS: We used mendelian randomization on genetic instruments for T1DM and T2DM to assess the risks

Postnatal meningeal CSF transport is primarily mediated by the arachnoid and pia maters and is not altered after intraventricular hemorrhage-posthemorrhagic hydrocephalus.

BACKGROUND: CSF has long been accepted to circulate throughout the subarachnoid space, which lies between the arachnoid and pia maters of the meninges. How the CSF interacts with the cellular components of the developing postnatal meninges including the dura, arachnoid, and pia of both the meninges at the surface of the brain and the intracranial meninges, prior to its eventual efflux from the cranium and spine, is less understood. Here, we characterize small and large CSF solute distribution patterns along the intracranial and surface meninges in neonatal rodents and compare our findings to m

Effects of surface profile on porcine dural mechanical properties.

BACKGROUND: Cerebrospinal fluid leakage through the spinal meninges is difficult to diagnose and treat. Moreover, its underlying mechanism remains unknown. Considering that the dura mater is structurally the strongest and outermost membrane among the three-layered meninges, we hypothesized that a dural mechanical tear would trigger spontaneous cerebrospinal fluid leakage, especially when a traumatic loading event is involved. Thus, accurate biomechanical properties of the dura mater are indispensable for improving computational models, which aid in predicting blunt impact injuries and creating

Resection of an incidentally discovered spinal arachnoid web: illustrative case.

BACKGROUND: Spinal arachnoid webs (SAWs) are rare pathologies of the spinal meninges often associated with syringomyelia and the radiographic "scalpel sign." Patients can experience pain, numbness, gait disturbances, or no symptoms at all. They are typically diagnosed via magnetic resonance imaging and treated with laminectomy and excision. OBSERVATIONS: A 61-year-old male presented after a mechanical fall and had an incidentally discovered SAW on imaging. He was initially asymptomatic and was therefore conservatively managed. Several years later, however, the patient experienced new-onset ba

Case report: Magnetic resonance imaging features with postoperative improvement of atypical cervical glioma characterized by predominant extramedullary distribution in a dog.

INTRODUCTION: Intramedullary cord tumors present diagnostic and therapeutic challenges. Furthermore, spinal cord tumors can move across compartments, making antemortem diagnosis difficult, even with advanced imaging. This report presents a rare case of a cranial cervical spinal glioma, confirmed by surgical histopathology, with postoperative improvement in a dog. CASE DESCRIPTION: A 9-year-old female Maltese dog presented with kyphotic posture, progressive left hemiparesis, and decreased appetite. Neurological examination revealed neck pain and decreased proprioception in the left limbs along

Intrathecal IgG4 synthesis in IgG4 related spinal hypertrophic pachymeningitis: a case report.

Immunoglobulin G4 (IgG4) related hypertrophic pachymeningitis of the spinal cord is a rare condition, characterized by infiltration of the spinal meninges with IgG4-producing plasma cells and subsequent hypertrophic fibrosis. Here, we report on a 65-year-old woman with IgG4 associated hypertrophic spinal pachymeningitis, in whom cerebrospinal fluid (CSF) analysis was a decisive diagnostic tool. Not only could we demonstrate an intrathecal IgG4 production, but also IgG4 positive plasma cells in CSF. Following decompressive surgery, diagnosis of IgG4 associated hypertrophic pachymeningitis was c

Infectious meningitis. Why are the leptomeninges preferentially involved? Electron microscopic insights.

In infectious meningitis, pathogens preferentially attack the leptomeninges (pia mater and arachnoid) rather than the pachymeninges (dura mater). This study aims to provide ultra-anatomical insights from our extensive collection of electron microscopy images and propose mechanisms, highlighting structures that favor the introduction, adherence, colonization, and proliferation of microorganisms leading to spinal meningitis. Over several years, we analyzed an extensive collection of transmission and scanning electron microscopy images of human spinal meninges captured in our laboratories. Upon e

Upregulation of delta opioid receptor by meningeal interleukin-10 prevents relapsing pain.

Chronic pain often includes periods of transient amelioration and even remission that alternate with severe relapsing pain. While most research on chronic pain has focused on pain development and maintenance, there is a critical unmet need to better understand the mechanisms that underlie pain remission and relapse. We found that interleukin (IL)-10, a pain resolving cytokine, is produced by resident macrophages in the spinal meninges during remission from pain and signaled to IL-10 receptor-expressing sensory neurons. Using unbiased RNA-sequencing, we identified that IL-10 upregulated express

Clinical and pathological features of cerebrospinal meningitis caused by Pantoea agglomerans : a case report.

BACKGROUND: Pantoea agglomerans (P. agglomerans) is a gram-negative bacterium that is commonly isolated from plant surfaces, seeds, and the environment. As an opportunistic pathogen, it can cause blood, urinary and soft tissue infections in immunocompromised patients. In central nervous system, P. agglomerans infection has been report in children and immune-compromised patients, however, infection by such bacterium in nontraumatized immune competent adults has not been reported. Here, we report a case of P. agglomerans cerebrospinal meningitis accompanied by positive anti-myeloperoxidase (MPO)

The impact of adjuvant radiotherapy on overall survival in spinal low-grade gliomas: a propensity score-matched analysis.

INTRODUCTION: Spinal low-grade gliomas (sLGGs) are a group of tumors that arise from glial cells in the spinal cord. Current evidence supporting the use of adjuvant radiotherapy for the management of sLGG is lacking. We hence aimed to compare overall survival rates in patients receiving surgery alone with those receiving surgery with adjuvant radiotherapy. METHODS: The NCDB, a large, nationwide, US-based cancer registry was used. Relevant cases were identified using the following ICD-O-3 histological codes: 9382, 9384, 9400, 9410, 9411, 9420, 9421, 9424, 9425, and 9450, along with the ICD-O-3

Cervical myelopathy caused by IgG4-related hypertrophic spinal pachymeningitis: Case report and a descriptive review of the literature.

INTRODUCTION: IgG4-related disease is an immune-mediated condition characterized by tissue infiltration of IgG4-positive plasma cells. Involvement of the spinal meninges results in hypertrophic spinal pachymeningitis (HSP), causing spinal cord and nerve root compression. RESEARCH QUESTION AND CASE DESCRIPTION: In this review, we present a case of IgG4-related hypertrophic spinal pachymeningitis. Furthermore, we provide an updated literature review on IgG4-related HSP. MATERIALS AND METHODS: We describe the case of a 45-year-old male presenting with cervical myelopathy. Supercritical CO2-Mediated Decellularization of Bovine Spinal Cord Meninges: A Comparative Study for Decellularization Performance.

The extracellular matrix (ECM) of spinal meninge tissue closely resembles the wealthy ECM content of the brain and spinal cord. The ECM is typically acquired through the process of decellularizing tissues. Nevertheless, the decellularization process of the brain and spinal cord is challenging due to their high-fat content, in contrast to the spinal meninges. Hence, bovine spinal cord meninges offer a promising source to produce ECM-based scaffolds, thanks to their abundance, accessibility, and ease of decellularization for neural tissue engineering. However, most decellularization techniques i

Cerebrospinal fluid protein levels are elevated 100 times in a Leptomeningeal metastasis patient: a case report and literature review.

Leptomeningeal metastasis (LM) has a high degree of malignancy and high mortality. We describe a patient admitted to hospital with acute lower extremity weakness, dysuria, and high intracranial pressure. Enhanced magnetic resonance imaging (MRI) showed extensive enhancement of the leptomeningeal and spinal meninges with multiple nodular changes and extensive fusion. His cerebrospinal fluid (CSF) was yellow and cloudy, the Pandy test was strongly positive (++++), the protein was 46 g/L (normal range 0.15-0.45 g/L), which attracted our attention. Initially, miliary TB with associated tuberculous

Formalin-evoked pain triggers sex-specific behavior and spinal immune response.

Mounting evidence shows sex-related differences in the experience of pain with women suffering more from chronic pain than men. Yet, our understanding of the biological basis underlying those differences remains incomplete. Using an adapted model of formalin-induced chemical/inflammatory pain, we report here that in contrast to male mice, females distinctly display two types of nocifensive responses to formalin, distinguishable by the duration of the interphase. Females in proestrus and in metestrus exhibited respectively a short-lasting and a long-lasting interphase, underscoring the influenc

A Unique Case of Sarcoid-associated Myelopathy Accompanied by Lung Cancer.

The differential diagnosis of myelopathy in patients with malignancies may be challenging, as a spinal biopsy is not always applicable. A 66-year-old woman who had shown transient double vision and nausea developed spasticity and impaired deep sensation in both feet. Magnetic resonance imaging showed abnormal gadolinium enhancement of the brainstem, spinal meninges, and nerve root. Cerebrospinal fluid (CSF) revealed mild pleocytosis and elevated protein and decreased glucose levels, although CSF cytology was normal. Lung carcinoma was simultaneously detected, and noncaseating granuloma was det
