Functional Connectivity and White Matter Microstructural Alterations of Thalamic Subregions in Benign Paroxysmal Positional Vertigo.

OBJECTIVE: Benign Paroxysmal Positional Vertigo (BPPV) is a common peripheral vestibular disorder. Although the thalamus serves as a key node in vestibular pathways, its role in the central regulation of BPPV remains unclear. This study aimed to explore the functional and structural alterations of thalamus in patients with BPPV. METHODS: The study recruited 52 BPPV patients and 52 age- and gender-matched healthy volunteers. Seed-based analysis was used to explore functional connectivity (FC) alterations between thalamic subregions and the whole brain in patient group. Probabilistic fiber trac

Voxel-Wise Case-Control Analysis of Dose, Linear Energy Transfer, and Symptomatic Brainstem Necrosis in Pediatric Brain Tumors Treated With Proton Therapy.

PURPOSE: Brainstem necrosis is a rare but serious toxicity following radiation therapy for pediatric brain tumors. In proton therapy, concerns have emerged around elevated linear energy transfer (LET) and the relative biological effectiveness (RBE) in relation to normal tissue damage. Although previous studies have largely focused on asymptomatic image changes in adults, we investigated voxel-wise correlations between dose, LET, and symptomatic brainstem necrosis in a pediatric case-control cohort. METHODS AND MATERIALS: From a cohort of exactly 1200 patients with central nervous system tumor

Computational Model for Synthesizing Auditory Brainstem Responses to Assess Neuronal Alterations in Aging and Autistic Animal Models.

PURPOSE: The auditory brainstem response (ABR) is a widely used objective electrophysiology measure for non-invasively assessing auditory function and neural activity in the auditory brainstem, but its ability to reflect detailed neuronal processing is limited due to the averaging nature of the electroencephalogram-type recordings. METHOD: This study addresses this limitation by developing a computational model of the auditory brainstem, which is capable of synthesizing ABR traces based on a large, population scale neural extrapolation of a spiking neuronal network of auditory brainstem circu

Extended high frequency hearing influences cortical response amplitudes to speech.

Since young adults hear sounds up to 20 kHz, the loss of extended high-frequency hearing (EHF; above 8 kHz) is a hallmark of age-related hearing loss, often progressing from early lifetime. However, this deficit frequently goes undetected because routine clinical hearing tests and most hearing aids are currently mostly limited to 8 kHz. EHF hearing has been linked to deficits in speech perception in noise and to self-reported hearing. However, it remains elusive how EHF hearing influences speech intelligibility. Here we recorded neuromagnetic brain responses using magnetoencephalography (MEG)

Addressing fusiform gyrus dysfunction in schizophrenia, theory of mind, and implications for antipsychotics' response and resistance: A systematic review and critical appraisal.

BACKGROUND: The Fusiform Gyrus (FG), a brain region relevant for the interaction between cognitive processes and emotional behavior, especially emotion recognition and interpretation of social cues, has been linked to the pathophysiology of schizophrenia conceptualized as a disorder of aberrant synaptic function and brain connectivity as well as characterized by disruptions in thought processes, perceptions, emotional responsiveness, and Theory of Mind (ToM). Disrupted FG connectivity has been reported in schizophrenia; however, no systematic review has addressed this issue in a comprehensive

Effect of metformin treatment on brain 18F-FDG uptake: a translational study.

INTRODUCTION: Metformin is one of the most widely used drugs to control diabetes type 2. However, there is no indication of pausing metformin treatment before an 18F-FDG PET scan. The aim of this study is to evaluate if metformin, in both patients and mice, can alter brain 18F-FDG uptake. METHODS: In vivo 18F-FDG PET scans were carried out in male mice. Animals were treated with saline or metformin (300 mg/kg) 10 min before 18F-FDG or chronically (3 weeks) in drinking water. Separately, patients with non-pathological 18F-FDG brain scans were distributed into two groups: metformin-treated (n =

Contralateral piriform cortex alterations after surgery for temporal lobe epilepsy: A pilot MRI volumetry study.

PURPOSE: Temporal lobe epilepsy (TLE) is increasingly conceptualized as a network disorder, with the piriform cortex as an implicated region. We characterized postoperative contralateral piriform cortex-cortical amygdala (PCA) structural and perfusion-related changes in a small seizure-free TLE cohort. METHODS: Nine patients with drug-resistant TLE who underwent epilepsy surgery and achieved postoperative seizure freedom were included in this single-center pre-post pilot study. Bilateral PCA was manually delineated on 3D T1-weighted MRI using ITK-SNAP, and volumes were normalized to total int

Neural correlates of self-directed violence: A large-sample resting-state fMRI study from UK Biobank.

BACKGROUND: Suicide remains a leading cause of death worldwide, underscoring the importance of identifying neurobiological markers associated with suicidal behaviors. While nonsuicidal self-directed violence (NSSDV) is an established risk factor for future suicide attempts (SAs), the underlying neural distinctions between these behaviors remain insufficiently understood. METHODS: A total of 1,070 participants with a history of self-directed violence (SDV) were stratified into SA (n = 579) and NSSDV (n = 491) groups based on self-reported questionnaires. All data were collected from the UK Bio

Utility of Vorasidenib in IDH1 R132C-Mutant WHO Grade 2 Astrocytoma: A Case Report.

WHO grade 2 astrocytomas are defined by an IDH mutation and the absence of 1p/19q codeletion. Most harbor the canonical IDH1 R132H substitution; rarer R132X variants, including R132C, remain underrepresented in prospective trials. Vorasidenib, a brain-penetrant dual IDH1/IDH2 inhibitor, prolongs progression-free survival in residual or recurrent grade 2 IDH-mutant glioma and is now included in contemporary North American guidelines when radiotherapy and chemotherapy can be deferred. We report a 25-year-old man who underwent maximal safe resection of a right temporal mass diagnosed as WHO grade

Endothelial cell-specific TGM2 deficiency promotes progression of temporal lobe epilepsy by disrupting S1P metabolism and intercellular communication.

BACKGROUND: Temporal lobe epilepsy (TLE) is a common and medically refractory neurological disorder with a complex pathogenesis. Blood-brain barrier (BBB) dysfunction is a key pathogenic factor. As a multifunctional protein, TGM2 has been shown to participate in vascular homeostasis, metabolic pathway regulation, and immune-inflammatory responses. This study aimed to systematically investigate the expression pattern, core functions, and molecular regulatory mechanisms of TGM2 in TLE, and to clarify its clinical value. METHODS: Temporal cortex tissue and plasma samples from drug-resistant TLE

Altered Cortico-cerebellar effective connectivity of the Visuo-attentional system is associated with READ1 deletion in developmental dyslexia.

Developmental dyslexia (DD) is a heritable neurodevelopmental disorder associated with altered brain connectivity. The influence of genetic risks on cortico-cerebellar effective connectivity (EC) remains unclear. The present study investigates how reading proficiency and the READ1 deletion (READ1d) in the DCDC2 gene modulate visuo-attentional network dynamics. Functional MRI data were acquired in 78 children (DD and typical readers, with/without READ1d) performing a coherent motion detection task. EC within a seven-region visuo-attentional network, including magnocellular, ventral attention, a

CAG repeat length shapes bidirectional functional connectome alterations in spinocerebellar ataxia type 3: a multimodal MRI study.

The ATXN3 gene is the primary driver of brain damage in spinocerebellar ataxia type 3 (SCA3). We aimed to elucidate the patterns of connectome alterations shaped by cytosine-adenine-guanine (CAG) repeat length in SCA3. Eighty-nine genetically confirmed patients with SCA3 and 89 age-, sex-, education-matched healthy controls underwent multimodal MRI and comprehensive clinical assessments. Connectome-based predictive modeling (CPM) was used to identify functional connectivity patterns associated with CAG repeat length. Structural brain damage was assessed using voxel-based morphometry. Mediation

Longitudinal Changes and Responsiveness of the Cerebellar Cognitive Affective Syndrome Scale in Patients with Acute Cerebellar Stroke.

Cognitive and affective disturbances due to cerebellar lesions are termed cerebellar cognitive affective syndrome (CCAS). Although CCAS is frequent after acute cerebellar stroke, CCAS-Scale responsiveness during acute hospitalization remains unclear. Objective: To examine longitudinal changes and responsiveness of the CCAS-Scale and compare its changes with global cognitive function, ataxia, and activities of daily living (ADL). In this single-center prospective observational study, 21 patients with first-ever cerebellar stroke were assessed 7 days after onset and at discharge (median, 21 days

Therapeutic Hybrid Intelligence with Neural and Knowledge-based Expert Reasoning for SRS (THINKERS): an AI Model for Functional Outcome Prediction After Gamma Knife Thalamotomy for Essential Tremor.

Outcome prediction after Gamma Knife thalamotomy for essential tremor remains largely guided by baseline tremor severity, target anatomy, radiosurgical dose parameters, and institutional experience rather than individualized estimates of functional improvement, latency to benefit, and adverse radiation effect risk. We developed THINKERS-ET, a mixture-of-experts (MoE) artificial intelligence framework for personalized functional outcome prediction and dose-policy evaluation after Gamma Knife thalamotomy. We performed a retrospective single-center treatment-level study of patients with medically

Vascular instructions for synaptic specificity.

Many signals contribute to the selective assembly of neural circuits alongside the developing brain vasculature. In this issue of Science Signaling, Lu et al. identify endothelial PRMT5 as an epigenetic brake that restrains Angptl4 expression to limit inhibitory synapse formation in the cerebellum, linking angiocrine signaling to synaptic specificity and circuit function.

Resting-state fMRI biomarkers of surgical outcome in temporal lobe epilepsy: Network reorganization, rsfMRI-based prediction models, and cognition - A systematic review.

Temporal lobe epilepsy (TLE) is a network disorder extending beyond the structurally defined epileptogenic zone. Resting-state functional MRI (rsfMRI) may provide information on brain network alterations relevant to seizure outcome and cognitive risk after epilepsy surgery. We reviewed rsfMRI studies in TLE surgical cohorts to evaluate postoperative network changes, preoperative predictors of seizure outcome, and associations between functional connectivity and cognition. This systematic review followed PRISMA guidelines and was registered in PROSPERO. Fifteen studies were included, comprising

Diaphragm 'atonia' during ictal and post-ictal apnoea: A case report.

OBJECTIVE: Ictal and post ictal central apnoea (ICA and PCA) are driven by suppression of brainstem respiratory drive and represent an electroclinical biomarker of sudden unexpected death in epilepsy (SUDEP). While animal models have demonstrated a true 'atonic diaphragm' underlying seizure-related apnoea, no human polygraphic study has yet confirmed respiratory muscle 'atonia' with direct EMG recording. METHODS: In this case report, we present a 26-year-old female with meningoencephalocele-associated temporal lobe epilepsy who underwent a video-EEG polygraphy integrated with surface EMG from
