Putative Transcriptomic Endotypes Reveal Neuronal, Immune, and Metabolic Heterogeneity in Temporal Lobe Epilepsy with Hippocampal Sclerosis.

Temporal lobe epilepsy with hippocampal sclerosis (TLE-HS) is a biologically heterogeneous disorder in which neuronal loss, gliosis, neuroinflammation, and metabolic stress may coexist but are unlikely to be uniformly represented across all patients. To investigate this heterogeneity at the molecular level, we analysed publicly available hippocampal transcriptomic data from individuals with TLE-HS. Our analyses identified three transcriptomically defined putative endotypes within this shared neuropathological diagnosis. These states were associated with distinct molecular programs, including a

Reassessment of the DTI-ALPS index in focal epilepsy.

OBJECTIVE: Recent evidence highlights the limited specificity of diffusion tensor imaging (DTI) along perivascular spaces (ALPS) as a noninvasive glymphatic biomarker because microstructural tract degeneration and crossing-fiber geometry confound it. We aimed to reassess the interpretation of the DTI-ALPS index in people with focal epilepsy. MATERIALS AND METHODS: Seventy-nine people with drug-resistant focal epilepsy (PWFE) underwent comprehensive presurgical work-up and 3-T magnetic resonance imaging (MRI). Etiologies included MRI-negative epilepsy, malformations of cortical development, te

5-Hydroxytryptamine 6 Receptor Antagonism Ameliorates Depression-Like Behaviors in Temporal Lobe Epilepsy Rats with Potential Involvement of the MEF2C/NF-κB Pathway.

This study aimed to investigate whether 5-HT6 receptor (5-HT6R) antagonism could ameliorate epilepsy-associated emotional and behavioral disturbances in rats with temporal lobe epilepsy (TLE) and to explore its potential association with the MEF2C/NF-κB pathway. Thirty rats were randomly divided into normal control (NC) group, Model group and 5-HT6R group. The TLE model was established using lithium chloride-pilocarpine. The rats in 5-HT6R group were treated with 5-HT6R antagonist SB271046. Behavioral tests were conducted to evaluate anxiety- and depression-like behaviors associated with epile

Development of a risk score to predict encephalitis by Herpes Simplex Virus and Varicella Zoster Virus.

Encephalitis guidelines recommend repeating a lumbar puncture (LP) 3-7 days after an initial negative cerebrospinal fluid (CSF) herpes simplex virus (HSV) polymerase chain reaction (PCR) test in patients with high suspicion for HSV or varicella zoster virus (VZV), but this subgroup has not been specified. Retrospective observational study of 602 adults with encephalitis from 2002 to 2023 from two large U.S. healthcare systems. Multivariate logistic regression models were developed and internally validated to identify clinically relevant and statistically significant features predicting HSV, VZ

The Mask of Delirium: Uncovering a Presumed Cardioembolic Stroke Through Comprehensive Clinical Evaluation.

Delirium and aphasia exhibit overlapping clinical features, complicating the diagnosis of acute neurological disturbances in older adults. Both conditions impair communication and orientation, leading to incoherence; however, they arise from distinct pathophysiological mechanisms and require different management strategies. This case concerns an 83-year-old woman with a 48-hour history of acute-onset confusion and abnormal speech. Magnetic resonance imaging (MRI) revealed an acute infarct in the inferior division of the left middle cerebral artery (MCA) territory, with cardioembolism considere

Serum β-synuclein predicts dementia conversion and mortality beyond amyloid-β and phospho-tau.

Synaptic dysfunction is among the earliest pathological events in Alzheimer's disease and among the strongest neuropathological correlates of cognitive impairment, yet blood-based synaptic biomarkers are lacking. Whether serum β-synuclein provides prognostic information independent of established amyloid-β (A; CSF Aβ42/Aβ40 ratio) and tau (T1; CSF pTau181) biomarkers has not been determined. We studied 259 non-demented individuals with subjective or mild cognitive impairment from a single-centre memory clinic, classified by CSF biomarkers using externally validated cut-offs into A-T1- controls

Anterior part of fronto-temporal language network as the neural basis of morphologically structured word form processing: evidence from Korean morphologically complex word.

The present fMRI study investigated which subregions of the visual word recognition network contribute to morphologically structured word form processing in Korean. Participants performed two separate lexical decision tasks in the scanner. In the Eojeol task, stimuli were three-syllable words composed of a disyllabic noun plus a case particle, whereas in the Noun task, stimuli were disyllabic (DSN) and trisyllabic (TSN) nouns. Behaviorally, reaction time was significantly longer in the Eojeol condition than in both DSN and TSN conditions, indicating greater processing demands for morphological

Cerebral blood flow as a biomarker of resilience in cognitively unimpaired APOE ɛ4 carriers.

INTRODUCTION: It remains unclear whether medial temporal lobe (MTL) hyperperfusion in cognitively unimpaired apolipoprotein E (APOE) ɛ4 carriers reflects resilience or dysfunction. We examined whether cortical MTL cerebral blood flow (CBF) relates to cognition differently in APOE ɛ4 carriers and noncarriers with and without AD pathophysiological changes. METHODS: One hundred forty-eight cognitively unimpaired older adults underwent pseudo-continuous arterial spin labeling MRI to quantify MTL CBF and assessment of memory and executive function. In a subset (n = 126), AD pathology was indexed u

Giant Multiloculated Cystic Trochlear Nerve Schwannoma Between the Tentorial Leaflets: Case Report and Systematic Review.

BACKGROUND: Trochlear nerve schwannomas (TNSs) are exceptionally rare benign tumors, accounting for a very small fraction of intracranial schwannomas. Due to their deep locations and slow growth, they often reach a considerable size before diagnosis. Giant or multiloculated cystic variants are exceptionally uncommon and present significant diagnostic and surgical challenges. METHODS: A 32-year-old female presented with headache and dizziness but no diplopia. Magnetic resonance imaging revealed a giant multiloculated cystic lesion extending from the posterior fossa to the right temporal lobe,

Neural dysfunction underlying divergent thinking and its shared mechanisms with protracted withdrawal symptoms in heroin use disorder.

BACKGROUND: Drug use may induce transient bursts of inspiration but is also associated with increasing cognitive rigidity. How neurological damage induced by drug addiction affects higher-order thinking processes in humans remains elusive. This gap limits our understanding of broader cognitive impairment in addiction and prolonged maladaptation during withdrawal. METHODS: The study investigated divergent thinking, a key component of flexible behavior, in heroin use disorder (HUD). Behavioral performance was assessed using the Realistic Presented Problem-Solving Task. Task-related neural signa

Evaluation of Macro- and Microstructural White Matter Changes in Hot Water Epilepsy.

BACKGROUND AND OBJECTIVES: Hot water epilepsy (HWE) is a form of reflex epilepsy and usually lacks definitive magnetic resonance imaging (MRI) features. Although white matter abnormalities have been extensively investigated in patients with temporal lobe epilepsy or generalized epilepsies, very few studies have examined quantitative changes in reflex epilepsy. This study aims to explore the white matter changes in HWE. The objective is to investigate the white matter micro- and macrostructural changes in patients with HWE using diffusion tensor imaging (DTI) indices and three-dimensional struc

Basal Forebrain Volume is Associated With Cortical Amyloid Burden in Cognitively Unimpaired Older Adults at Varying Genetic Risk for Alzheimer's Disease.

BACKGROUND: In mild cognitive impairment and dementia due to Alzheimer's disease (AD), postmortem and in vivo neuroimaging studies have demonstrated significant neuronal loss in the basal forebrain cholinergic system (BFCS), which provides the primary cholinergic input to the cerebral cortex. Within the BFCS, atrophy is most prominent in the nucleus basalis of Meynert (nbM), a posteriorly located cluster of magnocellular neurons. However, less is known surrounding the relationship between amyloid deposition, BFCS atrophy, and medial temporal lobe (MTL) volume loss in preclinical AD. This study

Astrocyte-Associated ABCA1/APOE Responses Accompany Hippocampal Lipid Remodeling and Ectopic Neuronal Accumulation During Epileptogenesis.

Temporal lobe epilepsy (TLE) is characterized by progressive hippocampal remodeling, including reactive astrogliosis and aberrant neuronal positioning. Although lipid disturbances are implicated in epileptogenesis, how astrocyte-associated lipid responses relate to hippocampal remodeling remains poorly understood. Here, human TLE and kainic acid (KA)-treated mouse transcriptomic datasets identified ATP-binding cassette transporter A1 (ABCA1)/Abca1 as a nominal astrocyte-associated candidate, while KA- and pilocarpine-induced status epilepticus (SE) elicited astrocyte-associated ABCA1/apolipopr

Progressive Hippocampal Volume Reduction Beyond Normal Aging in Temporal Lobe Epilepsy: A Longitudinal MRI Study.

Recent neuroimaging studies have suggested that epilepsy may show progressive brain structural changes over time. However, longitudinal hippocampal volume changes in temporal lobe epilepsy (TLE) have not been adequately compared with those observed in normal aging. In this study, we enrolled 46 patients with unilateral TLE and 25 healthy controls with longitudinal structural MRI scans using the same scanner. Hippocampal volumes were quantified using the Hipposeg software, and the annual percentage volume reduction relative to baseline (%Reduction/year) was calculated for each hippocampus. Comp

Atherosclerotic cardiovascular disease risk is associated with AT(N)V neuroimaging biomarkers and cognitive impairment in the health and aging brain study-health disparities cohort.

BACKGROUND: Cardiovascular risk is increasingly recognized as relevant to cognitive aging and dementia-related brain changes, yet its relationship to multimodal neuroimaging biomarkers of amyloid, tau, neurodegeneration, and vascular injury (AT(N)-V) remains incompletely understood in racially and ethnically diverse cohorts. We examined whether ten-year atherosclerotic cardiovascular disease (ASCVD) risk was associated with AT(N)-V neuroimaging biomarkers and cognitive domain scores. METHODS: This cross-sectional study included 1,626 Health and Aging Brain Study-Health Disparities (HABS-HD) p

Exploring the roles of ghrelin and nesfatin-1 as potential biomarkers in human epilepsy: a systematic review.

PURPOSE: Epilepsy affects approximately 50 million people worldwide, with approximately 30% of patients exhibiting pharmacoresistance. Identifying reliable biomarkers for diagnosis, predicting treatment response, and monitoring disease progression is essential for improving epilepsy management. Neuropeptides such as ghrelin and nesfatin-1 have emerged as potential candidates owing to their roles in neuroprotection and metabolic regulation. VIEWS: We systematically reviewed studies on ghrelin and nesfatin-1 in human epilepsy by searching PubMed, Embase and Web of Science. Inclusion criteria co

Malignant cerebral edema secondary to herpes simplex virus encephalitis requiring decompressive hemicraniectomy.

Herpes simplex encephalitis (HSE) is a severe viral infection of the central nervous system that most commonly affects the temporal lobes and limbic structures. Although most cases are managed medically with intravenous acyclovir, rare cases can rapidly progress to malignant cerebral edema, mass effect, and neurologic deterioration requiring surgical intervention. We report the case of a 57-year-old woman presenting with altered mental status, headache, vomiting, myalgias, and focal neurologic deficits. Initial computed tomography (CT) and CT angiography of the head and neck were unremarkable,

Network-guided Thalamic Neuromodulation for Epilepsy.

PURPOSE OF REVIEW: This review takes a network-based perspective on thalamic neuromodulation for epilepsy, examining how each target (e.g., anterior (ANT), centromedian (CM), and pulvinar (PUL) thalamic nuclei) engage distinct seizure networks, what evidence supports each, and how stimulation/electrophysiologic mapping can guide individualized targeting rather than a fixed, syndrome-based approach. RECENT FINDINGS: Randomized and prospective trials have expanded the evidence for ANT and CM stimulation, while emerging data support the pulvinar as a target for posterior quadrant and temporal lo
