Face blindness, also called prosopagnosia, is the inability to recognize familiar faces and is often secondary to dysfunction of the fusiform gyrus. This symptom can be part of dementia or be related to a lesion in this anatomical structure. We present the case of a 78-year-old female patient, with a diagnosis of dementia without prosopagnosia, who developed an episode of transient loss of consciousness. A brain computed tomography scan revealed an extensive right hemispheric chronic subdural hematoma. Patient underwent urgent hematoma drainage after deterioration of the neurological condition

BACKGROUND: Congenital cytomegalovirus infection is the most common congenital infection. Ocular abnormalities affect approximately 14% of symptomatic children, but fewer than 1% of asymptomatic children. Prenatal valacyclovir reduces vertical transmission, but ophthalmic outcomes in exposed infants are poorly described.
METHODS: We describe 2 neonates with congenital cytomegalovirus confirmed by amniotic fluid and urine polymerase chain reaction following first-trimester maternal primary infection. Both mothers received high-dose valacyclovir (8 g daily) from 13 weeks' gestation. Infants und

BACKGROUND: Patients with chronic temporal lobe epilepsy (TLE) frequently experience various sleep disorders, characterized by reduced sleep duration, diminished sleep stages, and disrupted sleep patterns. Recent research has identified a population of GABAergic neurons in the ventral zona incerta (ZI) that is instrumental in promoting sleep, along with a neural pathway that connects the cerebellar dentate nucleus (CDN) to the ZI. However, the precise role of this circuit in regulating sleep patterns remains inadequately understood.
METHODS: This study employed anatomical, chemogenetic, optog

BACKGROUND: Aspergillosis involving Meckel's cave (MC) is rare. Only 5 previous case reports exist in the literature. The primary treatment for this disease is surgical debridement combined with antifungal therapy. The authors present a minimally invasive technique using an endoscopic transorbital approach (ETOA) to manage this condition.
OBSERVATIONS: A 26-year-old male corn farmer presented with progressive right-sided facial pain and numbness for 1 month. MRI demonstrated a markedly hypointense T2 signal lesion in the right MC with gadolinium enhancement. CT revealed sphenoid sinus opacifi

BACKGROUND: The association between temporal encephaloceles (TEs) and drug-resistant epilepsy (DRE) is well recognized, yet the literature regarding therapeutic management remains scarce and largely retrospective. Reported strategies vary from conservative approaches to extensive resection of encephaloceles including the adjacent epileptogenic zone (EZ). Surprisingly, although the EZ may be localized within or adjacent to eloquent areas, surgeries under awake conditions have not been described. One plausible explanation is the potential risk of intraoperative pain during surgical manipulation

Introduction Epilepsy is a common neurological disorder. Despite being on anti-epileptic drug therapy, a subset of patients continue to experience seizures. In such patients with drug-resistant epilepsy, surgery remains a viable treatment option. Patient-reported satisfaction provides an important perspective on post-epilepsy surgery outcomes. This study aimed to descriptively assess postoperative patient satisfaction using the validated Epilepsy Surgery Satisfaction Questionnaire-19 (ESSQ-19). Materials and methods This was a retrospective observational study conducted at a tertiary care cent

Mnemonic discrimination-the ability to distinguish between existing memories and similar new inputs-supports accurate memory in the face of interference (e.g., in a card game, recognizing that the card you are currently seeing, the King of Hearts, is distinct from the King of Diamonds you encountered earlier in the same hand). Although hippocampal pattern separation has been emphasized as a key mechanism for this process, recent theoretical frameworks and empirical findings suggest that surrounding medial temporal lobe and cortical regions also contribute by resolving interference. We examined focus-relative amyloid PET asymmetry and its associations with resting-state EEG connectivity and processing speed in mesial temporal lobe epilepsy (MTLE). Thirty adults with clinically assigned MTLE (19 right-focus, 11 left-focus) underwent 18F-florapronol PET and resting-state EEG. Regional standardized uptake value ratios (SUVRs) and sensor-level weighted phase lag index (wPLI) were evaluated using paired comparisons, rank correlations and exploratory bootstrap indirect-association models. Cognitive data were available for 26 participants, including Digit Symbol Coding (DSC) for

OBJECTIVES: To investigate alterations in CBF-ALFF spatial coupling in obstructive sleep apnoea (OSA).
METHODS: Thirty-two patients with OSA and 32 healthy controls(HCs) matched for sex, age, and education background were recruited for this work. All participants received arterial spin labelling (ASL) and resting-state functional MRI (rs-fMRI) examinations. Cerebral blood flow (CBF) and amplitude of low-frequency fluctuation (ALFF) maps were derived from ASL and fMRI data, respectively. Between-group comparisons were performed to identify differences in ALFF, CBF, and CBF-ALFF coupling metric

OBJECTIVE: Malformations of cortical development (MCDs) are a major cause of drug-resistant epilepsy (DRE) in children. A subset of MCDs, including focal cortical dysplasia type II, tuberous sclerosis complex, and hemimegalencephaly, shares characteristic histopathological features and somatic mutations in genes within the Phosphoinositide 3-kinase (PI3K)-AKT-mechanistic target of rapamycin (mTOR) signaling cascade, and is collectively termed mTORopathies. However, mechanisms driving epileptogenesis and drug-resistant seizures remain poorly understood.
METHODS: We performed integrated genomic

BackgroundAmnestic mild cognitive impairment (aMCI), a prodromal stage associated with an increased risk of Alzheimer's disease (AD), is characterized by early neural network dysfunction. The temporal lobe is among the earliest affected regions, yet conventional electroencephalography (EEG) provides limited information regarding subregional electrophysiological alterations.ObjectiveTo characterize temporal-subregional electrophysiological features in aMCI using high-density EEG (HD-EEG).MethodsFifty-five individuals with aMCI and 35 cognitively normal controls underwent neuropsychological asse

Menopause is a critical transitional stage in women's lives, yet evidence on its impact on brain structure remains inconsistent, and large-sample neuroimaging studies are limited. Here, we applied whole-brain morphometry techniques to evaluate differences between pre- and post-menopausal women in terms of gray matter volume, cortical thickness and cortical gyrification. We processed and analyzed T1-weighted brain scans of 4098 women (1033 pre-menopausal; 3065 post-menopausal) from the UK Biobank. Post-menopausal women exhibited greater gray matter volume than pre-menopausal women primarily in

INTRODUCTION: Plasma p-tau217 is associated with Alzheimer's disease pathology and memory performance, but its implications for memory may depend on fibrillar tau topography. We examined whether tau-PET subtype modified the association between plasma p-tau217 and memory.
METHODS: We analyzed 396 amyloid-positive, tau-positive participants spanning cognitively unimpaired, mild cognitive impairment, and dementia stages from six cohorts with tau-PET and plasma p-tau217; 301 had harmonized memory data. Tau-PET subtypes were derived using data-driven disease progression modelling. Primary cross-se

PURPOSE: Neuroinflammation is a key pathogenic mechanism in temporal lobe epilepsy, quantifiable in vivo using positron emission tomography targeting the translocator protein. This study integrated [18F]DPA-714 PET/MRI with serum cytokine profiling to investigate this central-peripheral immune relationship in TLE patients.
METHODS: A prospective study of 24 TLE patients and 24 healthy controls (HCs) was conducted. PET/MRI data were available for 24 TLE patients and 7 HCs, while cytokine data were available for all 24 participants in both groups. Standardized uptake value ratios of [18F]DPA-71

Recent work has shown that the brain abstracts non-spatial relationships between entities or task states into representations called cognitive maps. Here, we investigate how cognitive control enables flexible top-down selection of goal-relevant information from multidimensional cognitive maps retrieved from memory. We examine the relationship between cognitive control and representational geometry by conducting parallel analyses of functional magnetic resonance imaging data and recurrent neural network models trained to perform the same task. We find both stable map-like representations in a m

In noisy listening environments, visual cues from a speaker's face can significantly boost speech comprehension. The underlying audiovisual integration in the brain involves tracking of audiovisual speech features in the temporal lobe. Moreover, lip reading in silence is associated with tracking of the speech envelope in the low-delta frequency band (0.5-1 Hz). Recently, digital avatars have emerged that can support speech comprehension. Yet, it remains unclear how the human brain integrates such artificial visual signals with natural speech. Here, we employed magnetoencephalography (MEG) to m

The medial temporal lobe (MTL) is a key region in Alzheimer's disease (AD) pathogenesis, affected early by tau pathology. While female sex and APOE-ε4 are two known risk factors that affect tau levels, whether these factors drive tau in the MTL via amyloid-dependent or amyloid-independent mechanisms remains unclear. We investigated these relationships in two large neuropathological cohorts comprising 2550 participants from the Religious Orders Study/Memory and Aging Project (ROS/MAP; n=1587, main cohort) and Arizona Study of Aging and Neurodegenerative Disorders (AZSAND; n=963, replication coh

Temporal lobe epilepsy (TLE), the most common form of drug-resistant epilepsy (DRE) in adults, is a frequent chronic neurological disorder. Nevertheless, the precise pathophysiological mechanisms underlying TLE remain largely undefined. Our findings demonstrate that Actin-binding LIM protein 1 (ABLIM1) is broadly expressed in the adult mouse brain, with high expression in the hippocampus. The protein is primarily localized to neurons, enriched in the postsynaptic density (PSD), colocalizes with the scaffold protein PSD95, and directly binds to F-actin. Co-IP experiments confirmed that ABLIM1 i

OBJECTIVE: Social disadvantage is an important determinant of health in focal epilepsy, but its neurobiological basis remains unclear. We examined whether brain morphometric alterations represent a structural signature of deprivation.
METHODS: We performed a retrospective cohort study of 340 adults with pharmacoresistant focal epilepsy evaluated at the Cleveland Clinic (2010-2022). Age- and sex-adjusted volumetric percentiles were derived from 3-T brain magnetic resonance imaging. To account for hemispheric network differences, analyses were stratified by epilepsy laterality (right vs. left),

Activation of the NLRP3 inflammasome pathway and dysfunction of brain-derived neurotrophic factor (BDNF) are both implicated in the pathogenesis of Alzheimer's disease (AD). While in vitro and in vivo models support these mechanisms, there is little research investigating their relationship in human brain tissue, and whether this relationship may be abnormal in the AD brain. Temporal cortex tissue taken post-mortem from AD patients and control subjects was analysed for expression of genes for the NLRP3 pathway (NLRP3, PYCARD, CASP1), the potassium channel THIK1 (KCNK13), and related pro-inflam
