A Rare Case of Solitary Brain Metastasis From Papillary Thyroid Carcinoma: The Role of Empiric I-131 Therapy and SPECT/CT (Single Photon Emission Computed Tomography/Computed Tomography) in Achieving an 11-Year Survival.

Brain metastases from papillary thyroid carcinoma (PTC) are rare and typically associated with a poor prognosis; however, early detection of solitary lesions can lead to exceptional long-term outcomes. We present the case of a 65-year-old man with a history of multicentric PTC (T1bN0M0, Stage I) treated with total thyroidectomy and postoperative radioiodine ablation 12 years earlier, who presented with rising thyroglobulin (Tg) levels (12 ng/mL) under TSH (thyroid-stimulating hormone) stimulation. Neck ultrasonography and 18F-FDG PET/CT (18-fluorodeoxyglucose positron emission tomography/compu

Recovering vision function after post-traumatic bilateral occipital infarction.

Visual impairment may occur in severe traumatic brain injury due to ischemia in the territory of the posterior cerebral arteries, usually secondary to the compressive effect of expanding collections. The resulting functional defect tends to be permanent; however, through mechanisms yet to be clarified, some patients recover aspects of visual function, with blindsight suggested as a potential mechanism. We present a peculiar case of gradual visual recovery after cortical blindness in a victim of a car accident, which resulted in bilateral occipital infarcts. We aim to raise awareness of this ra

Ultra-high field magnetic resonance spectroscopy study of transcranial magnetic stimulation treatment in adolescents with depression.

BACKGROUND: Altered glutamatergic and GABAergic signaling is increasingly implicated in the neurobiology of adolescent major depressive disorder (MDD). Transcranial magnetic stimulation (TMS) interventions may restore balance between excitatory and inhibitory neurotransmission. To explore these mechanisms, we quantified cortical GABA, glutamate, and glutamine concentrations with ultra-high field (7 T) magnetic resonance spectroscopy (MRS) in adolescents with MDD treated with TMS for six weeks. METHODS: 7 T-MRS data were collected from the occipital lobe of adolescents with MDD (n = 26) prior t

The oblique effect in visual working memory is enhanced by distraction, regardless of tDCS manipulations.

The sensory recruitment model suggests that visual working memory (VWM) relies on early visual cortex (EVC), while the alternative frontoparietal account argues VWM representation is independent from EVC. An emerging consensus suggests that VWM relies on a distributed network, with different brain loci serving behavior depending on task demands. To evaluate VWM reliance on EVC vs. parietal cortex, we applied anodal (or sham) transcranial direct current stimulation (tDCS) to either occipital cortex (Oz) or right posterior parietal cortex (P4) to manipulate cortical representations before an ori

Impact of Postlabeling Delay on Arterial Spin-Labeling MRI-Based Measures of Cerebral Blood Flow in a Simultaneous PET/MRI Study of Aging.

BACKGROUND AND PURPOSE: Arterial spin-labeling (ASL) MRI with a single postlabeling delay (PLD) of 1800 msec is recommended for quantifying CBF when the PLD is sufficiently longer than the arterial transit time. However, this condition may not be satisfied in older participants or individuals with dementia, for whom multiple PLDs may be more beneficial. This work assessed the intermediate-term (4-week) test-retest reliability and accuracy of single- versus multiple-PLD ASL for quantifying CBF relative to [15O]water PET acquired using PET/MRI imaging in a study of aging and Alzheimer disease (A

[Correlation between eye movement and white matter hyperintensity characteristics in patients with cerebral small vessel disease].

OBJECTIVES: The mechanism of abnormal eye movements in patients with cerebral small vessel disease (CSVD) remains unclear. This study aims to explore the potential link between eye movement in CSVD patients and the severity and distribution of white matter hyperintensities (WMH), and to evaluate the possibility of using eye movement assessment as a tool for specific diagnosis. METHODS: This retrospective cross-sectional study was conducted at Xiangya Hospital, Central South University between September 7th, 2022 and October 27th, 2023, enrolling a total of 161 patients with CSVD. Demographic

Rapid regression of a bulky cranial lesion in high-risk multiple myeloma with isatuximab-based quadruplet induction.

A 77-year-old woman with newly diagnosed immunoglobulin (Ig)G-κ multiple myeloma presented with a massive cranial paraskeletal (PS) lesion (84 × 59 × 62 mm) compressing the occipital lobe. Fluorescence in situ hybridization of bone marrow aspirate revealed 1q21 gain and 17p deletion. Induction therapy with isatuximab, bortezomib, lenalidomide, and dexamethasone (Isa-VRd) was initiated to avoid emergent local intervention. The response was rapid; head computed tomography on day 28 showed an approximately 80% reduction in bidimensional measurements, with near-complete radiologic resolution by th

Continuous Theta Burst Stimulation Inhibits Contralesional Parietal and Occipital Activity During Imagined Movements After Stroke.

Repetitive transcranial magnetic stimulation, when paired with skilled motor practice, shows promise for motor recovery after stroke via modulation of cortical excitability. After stroke, continuous theta burst stimulation (cTBS) has been used to reduce excitability in the contralesional cortex. In theory this increases excitability in the homologous motor area in the ipsilesional motor cortex. However, the response to cTBS is inconsistent. In turn, it is difficult to understand the broader impact of cTBS on brain activity after stroke. Here, 30 participants with middle cerebral artery stroke

Functional dual-slope frequency-domain near-infrared spectroscopy data interpreted with two- and three-layer models.

SIGNIFICANCE: Functional near-infrared spectroscopy (fNIRS) is impacted by signal contamination from superficial hemodynamics. It is important to develop methods that account for such contamination and provide accurate measurements of cerebral hemodynamics. AIM: We aim to investigate whether simulated data with two- or three-layer tissue models are able to reproduce in vivo data collected with dual-slope (DS) frequency-domain (FD) near-infrared spectroscopy (NIRS) on human subjects during brain activation. APPROACH: We performed Monte Carlo simulations to generate DS FD-NIRS data from two- a

Faces and Word Forms Can Coexist in the Right Ventro-occipital Temporal Cortex-Evidence from Left-Hemisphere Perinatal Stroke.

In most healthy adult humans, the right ventro-occipital temporal cortex (vOTC) is more involved in face perception than its left-hemisphere counterpart. Several hypotheses link this right-lateralization for face processing to left-lateralization of language and visual word form processing. In its strongest form, this account assumes an encroachment by word form processing into left vOTC territory that would otherwise be dedicated to face processing, causing face processing that would otherwise be bilateral to rely relatively more on right vOTC. By this logic, if visual word recognition came t

An interpretable deep learning diagnostic framework for early Alzheimer's disease based on EEG microstate spectra and multi-branch CNN.

BACKGROUND: The early diagnosis of Alzheimer’s disease (AD) and its preclinical stage, mild cognitive impairment (MCI), remains challenging because of the limited diagnostic efficacy of traditional EEG microstate temporal features and the generally poor interpretability of deep learning models. This study aims to explore the frequency-domain information of EEG microstates and develop an interpretable deep learning framework to enhance early diagnostic performance and elucidate the underlying neural mechanisms. METHODS: A total of 30 healthy controls (HCs), 29 individuals with MCI, and 29 indiv

Walking speed does not modulate electrocortical alpha response to intermittent visual occlusions.

Visual processing is crucial for the preparation, guidance, and stabilization of human locomotion. Responses to visual perturbations appear more adaptive at slower rather than faster walking speeds. Intermittent visual occlusions during walking cause widespread alpha synchronization across brain regions and can improve balance training. We investigated whether the response to intermittent loss of vision during human walking would change with different gait speeds. Fifteen healthy adults (5 male, 10 female) stood stationary and walked on a treadmill at speeds of 0.4, 0.8, 1.2, 1.6 m/s while vie

Aberrant local and global neural activation patterns in pediatric Prader-Willi syndrome.

PURPOSE: Although cognitive disorders in children with Prader-Willi syndrome (PWS) are linked to abnormalities in spontaneous neural activation and functional connectivity (FC), the specific neural activation patterns remain uncertain, especially in young children with PWS. METHODS: The current study set out to explore specific local and global neural activation in pediatric PWS using the amplitude of low-frequency fluctuations (ALFF), regional homogeneity (ReHo), and seed-based whole brain FC. Information was gathered from 35 pediatric PWS patients and 33 healthy controls (HC). Both groups'

Occipital interhemispheric transtentorial approach for the quadrangular lobule of the cerebellum.

BACKGROUND: The occipital interhemispheric transtentorial approach (OITT) is widely used for accessing lesions in the pineal region. Although reports are scarce, this approach can also be successfully applied to superior cerebellar lesions, involving the quadrangular lobule. METHODS: We describe the OITT approach for cerebellar quadrangular lobe lesions, providing relevant anatomy, surgical technique, and key technical considerations. CONCLUSION: Gross total resection can be achieved while preserving normal brain tissue and functionOITT is a safe and low-morbidity route for lesions within th

T2 Hyperintensity and ADC Values in Fetal Brain MRI following Prenatal Open Neural Tube Defect Repair.

BACKGROUND AND PURPOSE: T2 hyperintensity on fetal brain MRI is frequently observed in fetuses with an open neural tube defect (ONTD) and may reflect edema, impaired CSF circulation, or altered brain maturation. By characterizing the diffusion of water molecules within brain tissue, DWI may help differentiate among these etiologies by assessing water diffusion as a marker of brain microstructure. Our aim was to investigate the relationship between brain T2-hyperintensity and ADC values in fetuses following prenatal ONTD repair. MATERIALS AND METHODS: We retrospectively analyzed fetal MRI scan

Aggressive evolution of an occipital bone ABC-like lesion with dural invasion and intradural extension: a case report.

BACKGROUND: Aneurysmal bone cysts (ABCs) of the skull are rare, particularly those arising in the occipital bone. Intracranial extension or dural destruction is extremely uncommon. Here, we report an occipital ABC-like lesion that recurred 19 months after gross-total resection with dural defects, intradural extension, and molecular evidence supporting aggressive neoplastic evolution. CASE DESCRIPTION: A 37-year-old woman presented with 2 months of headache and 1 month of binocular diplopia. CT and MRI demonstrated a large expansile lytic lesion of the occipital bone with poorly defined interf

Evans' Index and Frontal and Occipital Horn Ratio in Correlation to Volumetric Data of Lateral Ventricles in a Population-based MRI Study.

PURPOSE: Evans' Index (EI) and Frontal and Occipital Horn Ratio (FOHR) are routinely used in screening for enlargement of the ventricular system. However, both lack epidemiological definition of normal values in adults. In a representative, randomized sample of adult normal population in Germany we studied the correlation of lateral ventricle volumes with EI and FOHR to evaluate the performance of pre-established cutoff values of EI > 0.3 and FOHR > 0.37, and to introduce age- and sex-specific thresholds, which for FOHR do not yet exist in adults. METHODS: In a population-based sample of 3058

A cross-sectional and longitudinal study of changes in complex networks related to brain function in temporal lobe epilepsy patients treated with levetiracetam intervention therapy.

The aim of this study was to investigate the changes in complex networks of brain functions associated with LEV treatment and the neural mechanisms underlying the effects on executive functions. Patients with TLE, including both users and nonusers of LEV, were included in this study along with healthy controls. Rs-fMRI and an attentional network test (ANT) were performed, and 9 of the 23 patients not taking LEV were selected for LEV intervention and followed up for 18 months. Rs-fMRI and ANT were reviewed, and graph theoretic analyses were applied to investigate the changes in their topologica

Abnormal temporal dynamics of high-frequency neural oscillations are associated with poor sleep quality in abstinent patients with alcohol use disorders.

BACKGROUND: Sleep disturbance is a common and clinically significant symptom in individuals with alcohol use disorder (AUD), often persisting during abstinence and increasing the risk of relapse. However, the neurophysiological mechanisms underlying sleep dysfunction in AUD remain poorly understood. Long-range temporal correlations (LRTC) in neural oscillations reflect the temporal organization and excitation-inhibition (E/I) balance of brain networks. While altered LRTC has been observed in various psychiatric and sleep disorders, its role in AUD-and its potential relationship to sleep dysfun
