The relationship between the cerebellum and cerebrum remains poorly understood in the neuroimaging literature, despite decades of functional magnetic resonance (fMRI) research. In this opinion article, we argue that this gap reflects fundamental methodological constraints rather than merely a lack of scientific interest. We first discuss challenges in acquiring fMRI data and analysis, with an emphasis on the cerebellum. We then critically evaluate the most common analysis approaches including functional connectivity, univariate contrasts, multivariate pattern analysis, and encoding models, exa

BACKGROUND AND OBJECTIVE: Neurodevelopmental disorders (NDDs)-including autism spectrum disorder (ASD), attention-deficit/hyperactivity disorder (ADHD), intellectual disability (ID), epilepsy, and schizophrenia-affect approximately 10-15% of children worldwide. Emerging evidence suggests that NDDs arise from complex interactions between genetic susceptibility and environmental insults, particularly during perinatal periods. This narrative review aims to synthesize current evidence on the molecular and circuit-level mechanisms linking perinatal insults and genetic vulnerabilities to NDD pathoge

BACKGROUND: Despite many reported imaging markers for bipolar disorder (BD), systematic assessment across diverse brain imaging-derived phenotypes (IDPs) is lacking. We used epidemiological and genetic analyses to progress from association to causality and identify candidate biomarkers and potential pathways of BD.
METHODS: We conducted an Exposome-Wide Association Study (ExWAS) in 35,680 UK Biobank participants to assess associations of 686 brain IDPs with BD. Genetic correlations were estimated via linkage disequilibrium score regression (LDSC). Bidirectional two-sample Mendelian randomizat

BACKGROUND: Obsessive-compulsive disorder (OCD) commonly begins during adolescence, yet its neurobiological basis remains unclear. Beyond the classical cortico-striatal framework, the cerebellum has been implicated in cognitive, affective, and motor regulation, relevant to OCD. However, it is unknown whether adolescent OCD involves abnormal dynamic coordination of large-scale cerebellar networks.
METHODS: Thirty-eight adolescents with OCD and 29 matched controls underwent dynamic functional network connectivity (dFNC) analysis of cerebellar networks using a sliding-window approach and k-means

Precerebellar structures, mainly studied in weakly electric fish, act to remove the effects of the fish's own movements on the electrical signals they receive, but do not directly control movement, nor do they have a clear error signal. The cerebellum does directly control movement - arguably the most prominent difference in function between the two structures. The most pronounced anatomical difference between precerebellar structures and the cerebellum itself is that the latter possesses an inferior olive. This conjunction suggests that the inferior olive carries out a function that is necess

Polycystic kidney disease (PKD) represents one form of chronic kidney disease (CKD) affecting the pediatric population. CKD in early life has been associated with neurocognitive deficits and disturbances in iron metabolism. Excess iron accumulation may exacerbate inflammatory and oxidative stress pathways linked to neurocognitive decline in CKD. No animal studies have yet investigated brain iron metabolism in early-life CKD, including PKD, or confirmed these findings through histological analysis. Utilizing a conditional Pkd2 knockout mouse, we investigated the effect of kidney disease on earl

INTRODUCTION: Patients with diabetes may develop cognitive dysfunction, particularly at advanced stages of the disease, manifested by impairments in learning, memory, language, and judgment. Previous studies have reported abnormal brain functional alterations in patients with diabetic retinopathy (DR), which may be associated with cognitive impairment in diabetes; however, the molecular correlates of these brain alterations remain poorly understood.
METHODS: In this study, we performed a coordinate-based neuroimaging meta-analysis to identify convergent resting-state brain functional alterati

Upper-extremity motor control has been widely studied with hand-task functional magnetic resonance imaging (fMRI) in healthy and disease states. However, proximal-arm motor tasks, such as at the shoulder, are uncommon during fMRI due to practical limitations, though they are critical to a complete understanding of upper-extremity motor control. Previous fMRI studies of proximal upper-extremity control have generally used unconstrained movements or a single low force or torque condition, leaving unknown how distributed supraspinal brain activity scales with increasing proximal motor output. CUB and Sushi domains-containing CSMD proteins are involved in neuron development. Variants in the CSMD1-3 paralogs were GWAS-associated with cognition, attention-deficit hyperactivity disorder (ADHD), schizophrenia and brain structure, suggesting that these genes could be linked to neurodevelopmental disorders (NDD). Indeed, rare biallelic variants in all three CSMD paralogs were recently associated with epilepsy, while rare biallelic variants in CSMD1 were also linked with intellectual disability and cortical malformations. We identified nine individuals from eight families carrying CSMD2 bi

BACKGROUND: The cerebellum is increasingly recognized as a key contributor to cognitive reserve and network organization in Parkinson's disease (PD). However, the relationship of cerebello-cortical and cerebello-basal ganglia connectivity with cognitive status and nigrostriatal integrity remains incompletely understood.
METHODS: Resting-state functional magnetic resonance imaging (fMRI) data from 172 participants with PD in the Parkinson's Precision Medicine Initiative were analyzed. Cerebello-basal ganglia and cerebello-cortical connectivity were investigated using region of interest (ROI)-t

Upper-limb motor symptoms in spinocerebellar degeneration (SCD) and Parkinson's disease (PD) frequently overlap, making clinical differentiation challenging in mild cases using the finger-to-nose test (FNT) or the diadochokinesis test (DDKT). This study aimed to identify quantitative kinematic markers that distinguish mild SCD from mild PD using three-dimensional motion capture. Twenty-six individuals per group-mild SCD, mild PD, and normal controls (NCs)--underwent motion analysis during FNT forward, FNT backward, and DDKT. Key parameters including segmental velocity ratios during FNT and rot

Post-stroke aphasia is a disabling disorder of language and communication for which speech-language therapy remains the standard of care. Because behavioural treatment yields variable gains and many people retain chronic communication limitations, non-invasive neuromodulation has been investigated to increase the responsiveness of residual language networks to therapy. This narrative review critically integrates evidence available through July 2026 for repetitive transcranial magnetic stimulation (rTMS), theta-burst stimulation, transcranial direct current stimulation (tDCS), and emerging appr

UNLABELLED: The cerebellum plays an emerging, but understudied, role in neurodevelopmental disorders (NDDs). Mutations in CHD8 cause a monogenic NDD (CHD8-NDD) characterized by autism and other manifestations, and prior work in Chd8 mutant mice reported cerebellar abnormalities, raising the possibility of cerebellar contribution to CHD8-NDD. To comprehensively characterize the impact of Chd8 haploinsufficiency on cerebellar development and function, we profiled the postnatal day 12 (P12) cerebellum of germline Chd8 +/- mice using transcriptomic, morphological, and electrophysiological approach

BACKGROUND: Atlanto-occipital (AO) joint instability and arthrosis are significant contributors to cervicogenic headaches and related disability. Injection and radiofrequency treatment of the AO joint are innovative approaches. However, the literature providing guidance on safe access to these joints remains sparse. The AO joint presents a high-risk target for interventions due to its proximity to the vertebral artery (VA), potentially compromising vascular supply to the cord, brainstem, cerebellum, and occipital lobe.

BACKGROUND: Acrylamide (ACR) is a common industrial substance that is used in the manufacture of many laboratory materials, formed during food processing, and is known to cause neurotoxicity and brain damage. Protective substances such as Nano-hydroxyapatite and vitamin B12 (Vit.B12) may play a modulatory role in mitigating this neural damage. The current study aimed to investigate the neurotoxicity and brain damage induced by ACR exposure and the ameliorative effects of nano-hydroxyapatite and Vit.B12 against these neural toxic effects.

This study proposes a spatiotemporal connectome-based framework to characterize the human brain's action network. Unlike temporal connectivity (TPC), this approach leverages full functional connectivity profiles derived from large-scale neural wave dynamics, termed spatial connectivity (SPC). We applied SPC to map the action network for the first time in a non-Western young adult cohort. Our results delineate the network's detailed functional architecture across the cerebral cortex, cerebellum, and subcortical nuclei. Compared to TPC, SPC is more robust to global signal regression when charact

Powassan viruses (POWVs) are tick-borne orthoflaviviruses that cause lethal encephalitis and long-term neurologic sequelae. POWVs are divided into two lineages that share 86% genetic identity. POWV-LB (Lin I) was isolated from a fatal case of encephalitis, while POWV-LI9 (Lin II) is a tick-derived isolate that causes age-dependent lethality in mice. Domain III of orthoflavivirus envelope proteins (EDIII) directs cell attachment and determines murine neuropathology and lethality. Here, we developed POWV LB reverse genetics, analyzed brains of LB- and recLB mutant-infected mice, and assessed the

KCND3 encodes the voltage-gated potassium channel Kv4.3, and pathogenic variants in KCND3 are associated with spinocerebellar ataxia type 19/22 (SCA19/22) and related neurological phenotypes. Here, we report a Japanese patient harboring a novel KCND3 missense variant, classified as a variant of uncertain significance, who presented with a clinical phenotype compatible with SCA19/22, including slowly progressive cerebellar ataxia beginning in childhood, accompanied by asymmetric parkinsonism. Dopamine transporter single-photon emission computed tomography demonstrated reduced striatal uptake. P
