Dual AAV gene therapy achieves recovery of hearing and auditory processing in a DFNB16 mouse model.

BACKGROUND: DFNB16, the second most common genetic cause of hearing loss, is caused by mutations of the STRC gene encoding stereocilin, a protein essential for the effective functioning of outer hair cells (OHCs) as cochlear amplifiers. Strc-/- mice, which lack stereocilin, display severe to profound deafness and constitute a relevant preclinical model for DFNB16. METHODS: Using Strc-/- mice, we developed a gene therapy strategy based on the use of dual AAV9-PHP.eB vectors to deliver the full-length Strc cDNA. Therapeutic efficacy was assessed by evaluating stereocilin expression, OHC bundle

Combinations of clinical factors and radiomics for the hemorrhage-free survival of brainstem cavernous malformation patients.

OBJECTIVES: This study aimed to develop a MRI-based prognostic nomogram including radiomics and clinical information for brainstem cavernous malformation patients (BSCMs). METHODS: 114 BSCMs were randomly divided into a training cohort and a validation cohort. Clinical and radiomics nomograms were constructed. Radiomics features were selected using three algorithms: univariate analysis, Pearson correlation, and the elastic net algorithm. A Cox regression model was employed to build the radiomics nomogram. The concordance index (C-index), time-independent receiver operating characteristic (ROC

Central hypoventilation and choking episodes revealing Chiari malformation type I.

A woman in her early 40s presented with a 7-year history of recurrent choking episodes affecting solids and liquids, nocturnal choking episodes occurring 3-4 times weekly, and chronic occipital headaches spanning 14 years. Home monitoring during the COVID-19 pandemic incidentally revealed episodes of severe oxygen desaturation (70%-80%) without respiratory distress. Multiple laryngoscopies and endoscopies performed to investigate the choking episodes were normal. Review of a 14-year-old CT scan of the head performed for unrelated trauma revealed a previously overlooked crowded foramen magnum.

Dicer Deletion in the Ear Can Cut Most Neurons and Their Innervation of Hair Cells to Project to the Ear and the Brainstem.

Dicer is crucial for the generation of microRNAs (miRNAs), which are essential for regulating gene expression and keeping neuronal health. Dicer's conditional deletion cuts all spiral ganglion neurons but spares a small fraction of vestibular ganglion neurons, innervating the utricle and part of the saccule. Hair cells develop in the utricle, saccule, posterior crista, and the cochlea in Pax2Cre; Dicerf/f. Cochlear hair cells develop at the base and expand the OHC and IHC in the middle, or split into a base/middle and the apex. In contrast, Foxg1Cre; Dicerf/f cuts all canal cristae and cochlea

EEFSEC deficiency underlies a human selenopathy with primary neurodevelopmental origins via midbrain-hindbrain hypoplasia.

Bi-allelic mutations in EEFSEC, a key factor in selenoprotein synthesis, cause a severe human selenopathy characterized by developmental delay, spasticity, and profound cerebellar atrophy. While previous studies in invertebrate models framed this condition as an early-onset neurodegenerative disorder, the contribution of primary developmental defects to the severe brain malformations in patients has remained a critical unanswered question. Here, we address this gap using a zebrafish model of EEFSEC deficiency. We discovered that loss of eefsec function does not impair global somatic growth but

Control of Blood Pressure Variability Across Behavioral States by Brainstem Adrenergic Neurons.

BACKGROUND: Short-term blood pressure (BP) variability is increasingly recognized as an independent predictor of cardiovascular and cerebrovascular risks, yet the central neural mechanisms that govern this variability, particularly across behavioral states, remain poorly defined. METHODS: We investigated the role of rostral ventrolateral medulla C1 (RVLMC1) neurons in short-term BP regulation during sleep-wake transitions and physical activity in freely behaving rats. Genetically targeted fiber photometry was used to record RVLMC1 neuronal activity across behavioral states. The contribution o

Bickerstaff Brainstem Encephalitis with a Decorticate-like Posture, Involuntary Movements, Impaired Visual Acuity, and Prolonged Motor Disability.

The acute immune-mediated disorder, Bickerstaff brainstem encephalitis (BBE), is characterized by consciousness disturbance, external ophthalmoplegia, and ataxia. BBE is usually readily diagnosed and patients recover well, but rare features may sometimes obscure the diagnosis, and some patients develop prolonged weakness. We herein describe a Japanese patient with BBE who presented with uncharacteristic features of a decorticate-like posture, involuntary movements, and impaired visual acuity, entered a coma requiring intubation, and later required orthosis-assisted ambulation for 10 months. Pharmacologic Inhibition of JAK1/2 Potentiates Aminoglycoside-Induced Ototoxicity.

HYPOTHESIS: In our study, we investigated the ototoxic interaction of Janus kinase (JAK) inhibition in combination with aminoglycosides. BACKGROUND: The therapeutic landscape of JAK inhibitors has undergone a significant transformation since 2018, characterized by a rapid increase in FDA approvals for this class of drugs. Initially approved for conditions like myelofibrosis and polycythemia vera, the indications have expanded to include several inflammatory conditions, such as psoriatic arthritis, ulcerative colitis, and rheumatoid arthritis, leading to a substantial increase in patients expo

Radiotherapy plus neoadjuvant and concomitant IL-13Rα2-directed immunotoxin therapy for diffuse intrinsic pontine glioma.

Radiotherapy (RT) is the standard-of-care for diffuse intrinsic pontine glioma (DIPG); however, it functions as a palliative treatment. Interleukin 13 receptor subunit alpha 2 (IL-13Rα2) is upregulated in most DIPG tumors, posing a promising therapeutic target. Immunotherapies harnessing IL-13Rα2 to selectively deliver cytotoxic payloads such as pseudomonas exotoxin A (PE) are safe in DIPG patients and efficacious in preclinical disease models. Here, we used DIPG cell lines and mouse models to compare RT alone with RT plus the IL-13Rα2-targeted PE immunotoxin GB13 (IL13.E13K-PE4E). DNA strand

Development of Insulin and Leptin Resistance in the Mouse Brainstem with Age.

Physiological aging involves a progressive deterioration of homeostatic mechanisms that cause obesity and defective glucose homeostasis, which develop age-related diseases increasing mortality risk and reducing lifespan. The brainstem is involved in glucose and metabolic homeostasis by integrating peripheral signals such as insulin and leptin. Here, we evaluated the brainstem response to intracerebroventricular administration of insulin or leptin and the relationship with physiological levels of key molecules implicated in their signal transduction pathway and inflammation in 3-, 6-, and 12-mo

Brainstem pathology in anti-IgLON5 disease: new insights into early events and tau progression.

Anti-IgLON5 disease is a rare neurological disease at the intersection of autoimmunity and neurodegeneration. It is characterized by the presence of anti-IgLON5 antibodies and the development of a brainstem-dominant tau pathology. Recent research indicates that the tau pathology may develop in a time-dependent manner. A three-staged neuropathological classification of the disease has been recently suggested, ranging from no or minimal tau (stage 1) to the characteristic brainstem tau pathology (stage 3) as originally described. This study aimed to characterize the evolution of the disease-asso

Alterations of brain activity in noise-exposed rats after transcutaneous auricular vagus nerve stimulation evaluated via fMRI.

This study aims to explore the role of the central nervous system network outside the auditory system in the development process of noise - induced central injury. A noise-exposed rat model was established with unilateral narrow-band noise. Auditory brainstem response (ABR) measured hearing thresholds at Click and 8, 16, 24, 32 kHz pre- and post-noise exposure. Experimental rats were split into transcutaneous auricular vagus nerve stimulation (ta-VNS) and sham subgroups for 2-week intervention. Resting-state fMRI (rs-fMRI) was performed on all groups (ta-VNS, sham, control) post-noise exposure

Rapidly progressing idiopathic neutrophilic vasculitis with fibrinoid necrosis of the central nervous system.

Rapid neurologic and clinical decline can have a vast differential diagnosis. In this unique case, a man in his mid-70s was found unresponsive at home and progressed to brain death within 36 hours from the initial presentation. Preliminary imaging revealed a dorsal pontine haemorrhage, and later, studies showed additional areas with haemorrhage including parenchyma, midbrain and pons. Other than respiratory failure, he did not demonstrate any other end organ dysfunction before losing remaining brain stem reflexes and expiring despite aggressive efforts. After full-body autopsy, microscopic exa

A novel brainstem-targeted G51D α-synuclein fibril-injected mouse model exhibits sequential emergence of sleep and motor dysfunction.

Abnormal accumulation of alpha-synuclein (α-syn) is a central pathologic hallmark of synucleinopathies such as Parkinson's disease (PD), with rapid eye movement sleep behavior disorder (RBD) widely recognized as a prodromal manifestation of these disorders. Although several mouse models recapitulate the α-syn pathology, most fail to reproduce the brainstem-originating pathology propagation proposed by Braak and do not exhibit the RBD-like phenotypes expected in the prodromal phase. Here, we focused on the G51D mutation of α-syn, a familial PD-associated variant that leads to early disease onse

Brainstem and Cerebellar Volume Loss and Associated Clinical Features in Progressive Supranuclear Palsy.

INTRODUCTION: Progressive Supranuclear Palsy (PSP) is a neurodegenerative 'tauopathy' with predominating pathology in the basal ganglia and midbrain. Caudal tau spread frequently implicates the cerebellum; however, the pattern of atrophy remains equivocal. We hypothesise that volume loss is appreciable in the cerebellum and brainstem regions-beyond the midbrain-in individuals with PSP and linked to motor and non-motor clinical features. METHODS: In this cross-sectional observational study, volumetric brainstem and cerebellar subsegmentation of T1-weighted magnetic resonance imaging (MRI) was

Paediatric auditory brainstem implant: How we do it.

BACKGROUND: The auditory brainstem implant (ABI) is a surgically implanted neuroprosthetic that may be used in the management of profound hearing loss in paediatric patients that are ineligible for cochlear implantation, primarily due to anatomical constraints. METHOD: We describe our technique for the implantation of a paediatric ABI. Surgical steps include post auricular incision, the creation of a periosteal flap, subperiosteal pocket and bony recess for the device, retrosigmoid craniotomy, placement of the electrodes adjacent to the cochlear nucleus and multi-layer closure. We highlight n

Deformable Point Cloud Registration-Based Bidirectional Local Distance (DPCR-BLD): A Methodology for Systematic Evaluation and Visualization of Local Disagreements in Clinical Autocontouring.

PURPOSE: Variability in autosegmentation can arise from the training data, leading to disagreements with clinical practice. The anisotropic and localized nature of disagreements between 2 contours makes them challenging to evaluate using common metrics, which only provide insights on overall and global similarity. This study aims to develop Deformable Point Cloud Registration-Based Bidirectional Local Distance (DPCR-BLD), a methodology to systematically evaluate local disagreements in autosegmentation. METHODS AND MATERIALS: Given a reference (clinically approved) and test (autogenerated) str

The impact of inhibiting the Hippo signaling pathway effector molecule YAP1 on in vitro glioblastoma and glioblastoma stem cells.

Glioblastoma, a prevalent and aggressive brain tumor in adults, recurs due to local invasiveness, radiation therapy failure, drug resistance, and cancer stem cell presence. The Hippo signaling pathway, regulating organ size and growth, features YAP as a key effector with oncogenic implications in various cancers, including glioma. This study aimed to explore YAP1 as a therapeutic target in glioblastoma by silencing it using siRNA and evaluating the potential of a new inhibitor, CA3 (CIL56), through advanced in vitro analyses. Investigations were conducted on brain cancer and stem cells, alongs
