Predicting tremor control and side effects during focused ultrasound thalamotomy using MR temperature imaging.

OBJECTIVE: Focused ultrasound (FUS) is a minimally invasive treatment option for essential tremor and tremor-dominant Parkinson's disease. Because thermal lesions are irreversible, it is critical to understand how lesion placement relates to patient outcomes. The aim of this study was to determine whether patient outcomes can be predicted using real-time estimation of heating and the accumulated thermal dose derived from MR temperature imaging (MRTI). METHODS: MRTI, tremor control, and side effects were analyzed from 547 FUS thalamotomy treatments (481 patients, median age 74 years). Previous

Peritumoral Cystic Meningioma With Diagnostic Challenges: Two Case Reports and a Literature Review.

Cystic meningiomas are relatively rare and pose significant diagnostic challenges because their heterogeneous cystic architecture closely mimics gliomas or metastatic lesions. This radiological overlap creates critical diagnostic dilemmas that directly influence surgical strategy and patient counseling. We present two surgically treated cases that illustrate distinct imaging pitfalls and offer practical insights for advanced image-based differentiation. The first patient, a 45-year-old man, presented with behavioral changes and right-sided weakness; Computed tomography (CT) and magnetic resona

From maternal nutrition to neonatal neuroprotection: Passion fruit stilbenes in hypoxia-ischemia.

BACKGROUND: Neonatal hypoxia-ischemia (HI) remains a major cause of perinatal morbidity and mortality, leading to lifelong neurological sequelae. Therapeutic hypothermia is the current standard of care but provides only partial protection and must be applied within a narrow therapeutic window. Complementary or preventive neuroprotective strategies are therefore needed. Maternal supplementation with plant-derived polyphenols has shown promising effects in reducing oxidative stress, neuroinflammation, and neuronal loss in experimental models of neonatal HI. Passion fruit extract, rich in stilben

Expanding mutational and phenotypic spectrum of CST3-related leukoencephalopathy: a novel family and literature review.

CST3-related leukoencephalopathy is a recently recognized adult-onset neurodegenerative disorder caused by pathogenic CST3 variants with C-terminal truncations, frameshifts, or deletions. Owing to the limited number of reported cases, its clinical and radiological spectrum remains incompletely defined. Here, we report a Chinese family harboring a novel heterozygous nonsense variant of CST3 (c.327C > A; p.Cys109*), which represents the most 5’ pathogenic mutation identified to date. The proband, a 62-year-old woman, presented with recurrent encephalopathic episodes, progressive cognitive declin

The lesion correlates of impaired content word fluency during spoken discourse in aphasia.

Human conversations are strongly dependent on the production of informative and fluent connected speech. In people with aphasia, this skill is affected to varying degrees, which can negatively impact social communication and quality of life. However, available coding systems to assess connected speech production based on discourse tasks are labour-intensive and very time-consuming, limiting their utilization in clinical and research contexts. In this study, we investigated and further validated a recently developed, accurate, time-efficient and clinically applicable measure of content word flu

Semantic and Phonological Brain Networks in Older Adults: A Systematic Scoping Review.

Background/Objectives: Despite many neurobiological models of semantics and phonology, there is no consensus regarding their functional organization. Specifically, there is considerable debate concerning the precise functional roles of the left inferior frontal gyrus and inferior parietal lobe, where these two networks may overlap. Meta-analyses addressing this topic have focused on neurologically intact young adults, yet theories of cognitive aging suggest older adults may exhibit a different distribution of their language networks. This scoping review specifically explores the interplay betw

Do Serum Brain Biomarkers Differentiate the Hemorrhagic Head Injury Lesion Phenotypes? An Interim Analysis of an On-Going Randomized Clinical Trial.

Background: Traumatic head injury (THI) includes a diverse range of hemorrhagic brain lesions (HBL), which are distinct phenotypes with characteristic pathophysiological mechanisms. Computed tomography (CT) is the cornerstone of the initial assessment and diagnosis; however, its sensitivity is limited, especially in mild head injury. Blood-derived biomarkers, including Neuron-Specific Enolase (NSE) and S-100B, have been extensively studied; however, their efficacy in distinguishing HBL subtypes remains unclear. We evaluated whether circulating serum levels of S-100B and NSE can discriminate be

The Effects of Adenosine A1/A3 Receptors Agonist on Improving the Outcomes of TBI and Stroke: Focus on AST-004.

PURPOSE OF REVIEW: This review explores the therapeutic potential of dual adenosine A1 and A3 receptor (A1R/A3R) agonism as a neuroprotective treatment for acute ischemic stroke (AIS) and traumatic brain injury (TBI). Current AIS treatments, tissue plasminogen activator and mechanical thrombectomy, are limited by narrow therapeutic windows and strict eligibility criteria. In contrast, TBI management remains primarily supportive, with no Food and Drug Administration-approved therapies targeting the underlying molecular pathology. Dual A1R/A3R agonists offer a new approach by modulating injury p

Deep learning improves image quality in motion-robust and sedation-free pediatric brain MRI.

OBJECTIVES: Motion and limited compliance compromise diagnostic MR image quality, particularly in pediatric patients who frequently require sedation. Single-shot sequences offer a time-efficient alternative but suffer from reduced image quality. This study aimed to evaluate the diagnostic performance of a deep learning (DL) framework combining compressed sensing (CS) and convolutional neural networks (CNNs) to enhance T2-weighted single-shot MRI (T2-SSHDL) compared with conventional CS-based reconstruction (T2-SSHconv) and routinely acquired high-resolution T2-weighted sequences. Justo Gonzalo (1910-1986): a pioneer of brain dynamics.

The present work offers an overview of the pioneering contributions of the neuroscientist Justo Gonzalo to the study of the human cerebral cortex, within the historical context of his time and in relation to current research. Gonzalo initially trained under Gonzalo Rodríguez Lafora (a disciple of Santiago Ramón y Cajal) and always maintained a close relationship with him and his circle, connecting him to the Spanish Neurological School (Cajal's school). He also trained in neurology in Austria and Germany (1933-1935). The research he called brain dynamics began during the Spanish Civil War (193

CEREBLEED: Automated Quantification and Severity Scoring of Intracranial Hemorrhage on Noncontrast CT.

BACKGROUND AND OBJECTIVES: Standardized interpretation of intracranial hemorrhage (ICH) severity on noncontrast computed tomography (NCCT) is limited by the absence of objective, reproducible tools for quantifying lesion burden and its anatomic impact. We developed and externally validated a deep learning-based framework for automatic segmentation and volumetric quantification of ICH, deriving a quantitative Severity Index based on volumetric relations among hemorrhage subtypes and brain structures, and prospectively assessed its clinical applicability. METHODS: A total of 2112 NCCT scans wer

Imaging and physiology across the high-low cerebrospinal fluid pressure spectrum: Navigating diagnostic uncertainty in headache practice.

OBJECTIVE: This study was conducted to provide a clinically oriented, mechanism-based framework for interpreting neuroimaging across disorders of cerebrospinal fluid (CSF) pressure, with particular emphasis on patients who fall between classic diagnostic categories of spontaneous intracranial hypotension (SIH) and idiopathic intracranial hypertension (IIH). BACKGROUND: Headache specialists are increasingly asked to evaluate patients whose symptoms, imaging, and opening pressures do not fit neatly within International Classification of Headache Disorders, 3rd edition criteria. Successful Treatment of Overlapping Myelin Oligodendrocyte Glycoprotein Antibody-Associated Disease and Glial Fibrillary Acidic Protein Astrocytopathy With Plasma Exchange.

Myelin oligodendrocyte glycoprotein (MOG) antibody-associated disease (MOGAD) and glial fibrillary acidic protein (GFAP) astrocytopathy are distinct autoimmune inflammatory disorders of the central nervous system (CNS) that occasionally overlap. A 69-year-old man presented with acute gait disturbance, followed by progressive impairment of consciousness, myoclonus, cerebellar ataxia, and hyponatremia. Initial brain magnetic resonance imaging (MRI) revealed a high-intensity lesion in the left thalamus on diffusion-weighted imaging (DWI) and multiple white matter lesions on fluid-attenuated inver

Visual Epilepsy Mimicking Migraine Aura Leading to Delayed Diagnosis of an Occipital Cavernoma: A Case Report.

Cerebral cavernous malformations, also known as cavernomas, are uncommon vascular malformations that may present with seizures, headache, or focal neurologic deficits depending on lesion location. Occipital cavernomas are rare and may manifest with positive visual phenomena, thereby mimicking migraine aura and delaying diagnosis. We report the case of a 54-year-old woman who presented in March 2018 with recurrent brief visual episodes described as flashing lights, colored shapes, scotomas, and distortion of object contours, associated with a persistent headache. She was initially treated for m

Third ventricle cavernous malformations: Diagnostic pitfalls and optimal surgical strategies - Scoping review with institutional series.

OBJECTIVE: Third ventricle cerebral cavernous malformations (TV-CCMs) are rare lesions. They are often misdiagnosed due to imaging overlap with other TV pathologies. Moreover, they appear to exhibit more aggressive behaviour than parenchymal CCMs. The goal of this study is to evaluate diagnostic errors, the therapeutic options (conservative management vs surgery) chosen and their related outcomes. METHODS: The PRISMA-ScR (1980-2025), combined with an institutional case series, analyzes diagnostic accuracy, bleeding risk, and surgical outcomes. Literature scoping review (42 studies) and 3 inst

Impact of post-stroke cognitive impairment and brain connectivity on early versus late motor skill learning.

Cognition is more engaged during early than later motor skill learning (MSkL), yet the impact of post-stroke cognitive impairment (PSCI) and brain connectivity on early MSkL remains unclear. We hypothesized that early MSkL would be impaired in people with chronic stroke compared to healthy individuals (HI), and that this impairment would be associated with PSCI, lesion burden, and structural and functional connectivity. Fifty-three people with chronic stroke and twenty-one mean age-matched HI completed a comprehensive neuropsychological assessment and trained with their contralesional/non-domi

Target-Dependent Postoperative Weight Gain after Pallidothalamic Tractotomy versus Pallidotomy for Dystonia.

BACKGROUND: Postoperative weight gain is a recognized complication of deep brain stimulation, but whether ablative surgery produces target-dependent weight effects remains unknown. We investigated whether pallidothalamic tractotomy (PTT) causes greater weight gain than pallidotomy and explored spatial associations. METHODS: We retrospectively analyzed 79 patients with focal, segmental, or generalized dystonia who underwent unilateral PTT (n = 41) or globus pallidus internus (GPi) lesioning (n = 38). Multivariable linear regression quantified the effect of target on weight and body mass index

Sterile Vegetations in Malignancy: A Rare Case of Nonbacterial Thrombotic Endocarditis in a Patient with Metastatic Melanoma.

Background and Clinical Significance: Nonbacterial thrombotic endocarditis (NBTE) is a sterile fibrin-platelet valvular condition associated with malignancy and hypercoagulable states. It produces friable vegetations prone to systemic embolization, often presenting as multifocal ischemic stroke. While modestly linked to advanced adenocarcinomas, its association with melanoma is exceedingly rare; Case Presentation: We present a 43-year-old man with recently diagnosed metastatic melanoma who presented with fever, confusion and abdominal pain. Brain magnetic resonance imaging (MRI) revealed multi

Generative Synthesis of Fractional Anisotropy Maps from T1 MRI Using Transfer Learning for White Matter Assessment in Stroke.

Assessment of white matter integrity is critical for predicting functional recovery after ischemic stroke. However, conventional magnetic resonance imaging (MRI) cannot capture tract-specific microstructural changes, and diffusion tensor imaging (DTI) is limited by prolonged acquisition times. This study aimed to synthesize fractional anisotropy (FA) maps from routinely acquired T1-weighted (T1) MRI using 2.5D inputs within a generative adversarial network (GAN) framework. Specifically, our primary objective was to evaluate the relative efficacy of a proposed transfer learning strategy compare

Unilateral Cortical Ribboning and Corticomedullary Lesions in a Rare Case of Coexisting Anti-N-methyl-D-aspartate Receptor Encephalitis and Neuronal Intranuclear Inclusion Disease.

Anti-N-methyl-D-aspartate receptor (anti-NMDAR) encephalitis is an autoimmune disorder characterized by psychiatric manifestations, seizures, and dyskinesias. Neuronal intranuclear inclusion disease (NIID) presents heterogeneously with dementia, neuropathy, and episodic encephalitis-like events, typically showing bilateral corticomedullary hyperintensities on diffusion-weighted imaging (DWI). The simultaneous occurrence of these conditions is extremely uncommon, complicating diagnosis due to overlapping clinical and radiological features. We report the case of a 68-year-old female with a histo
