Non-invasive Neuromodulation Targeting Approach by Mapping Stimulations and Lesions That Modify Visual Memory.

Therapeutic brain stimulation is believed to target specific networks, but targeting approaches for memory remain debated. For other symptoms, neuromodulation targets have been localized by mapping connectivity of lesions and stimulation sites to specific symptoms. This approach has yielded networks for global memory, but it remains unclear whether it applies to specific types of memory. Here, we mapped connectivity of stimulation sites, lesions, and atrophy patterns associated with different memory types. We included 544 individuals across three datasets: transcranial magnetic stimulation (N=

Recognizing, managing, and preventing cognitive sequelae after stroke. A narrative review.

Post-stroke cognitive impairment (PSCI) affects up to one-third of stroke survivors and is a major contributor to long-term disability and reduced quality of life. Its heterogeneous phenotype reflects the interplay of acute cerebrovascular injury with chronic vascular pathology, inflammation, blood-brain barrier dysfunction, and, in many patients, coexisting neurodegeneration. Clinical manifestations vary according to lesion location and most commonly involve executive and attentional deficits, with variable impairment of memory, language, visuospatial function, and neuropsychiatric domains. Structural neuroanatomy of semantic retrograde memory in older adults.

Studies of brain lesions or volumes indicate that the integrity of medial and lateral temporal lobe structures is important for news event memory accuracy, but the relationship between cortical thickness and news event memory accuracy has not yet been investigated in older adults. In a mixed sample of 70 older adults with variable cognitive abilities but without dementia, we investigated the relationship between cortical volume, hippocampal volume, and cortical thickness with news event recognition memory accuracy across the entire adult lifespan using the Retrograde Memory News Events Test. Clinical Reasoning: A 28-Year-Old Man With Seizures and Thalamic Lesions.

This case involves a 28-year-old Black man who presented with new-onset seizures and bilateral medial temporal lobe hyperintensities on brain MRI, initially raising concern for viral encephalitis. CSF analysis revealed mild lymphocytic pleocytosis, but extensive infectious testing was negative. One year later, he developed acute right-sided weakness, and an MRI showed an enhancing FLAIR/T2 hyperintense mass-like lesion in the left thalamus. Stereotactic biopsy demonstrated reactive changes and Periodic acid-Schiff diastase (PASD)-positive macrophages, prompting consideration of atypical infect

Successful use of ruxolitinib in steroid-refractory chronic GvHD affecting central nervous system: a case report.

Chronic graft-versus-host disease (cGvHD) is a major complication of allogeneic hematopoietic stem cell transplantation (allo-HSCT). Beyond its classical manifestations, cGvHD may involve additional organs, including the central nervous system (CNS), where involvement remains poorly characterized, and therapeutic options are undefined, particularly in steroid-refractory cases. We report the case of a 50-year-old woman who underwent allo-HSCT for myelodysplastic syndrome and subsequently developed classical acute followed by chronic GvHD-overlap syndrome. At 2 years and 9 months post-transplant

Effect of vascular lesion preprocessing on Brain Intensity AbNormality Classification Algorithm (BIANCA) white matter hyperintensity segmentation.

BACKGROUND: White matter hyperintensity (WMH) segmentation using BIANCA (Brain Intensity AbNormality Classification Algorithm) in stroke populations is complicated by vascular lesions that share T2-hyperintense signal characteristics with WMH. Whether preprocessing decisions in the treatment of lesions affect segmentation accuracy has not yet been systematically evaluated in cerebrovascular cohorts involving multiple scanners. METHODS: We compared fixed probability thresholds and locally adaptive thresholding with LOCATE (LOCally Adaptive Threshold Estimation), and three lesion-handling appro

Dysfunctional Brain Circuits Overlap in Lesional and Idiopathic Obsessive-Compulsive Disorder.

BACKGROUND: Obsessive-compulsive disorder (OCD) may develop following brain lesions, but lesion distribution and connectivity patterns are unknown. METHODS: Cases of OCD associated with focal brain lesions were identified using a systematic literature search and compared with control lesions (n = 608). Connectivity with each lesion location was computed using normative functional connectivity (n = 1000), and a network specific to OCD lesions was identified. The relevance of this network for primary OCD was explored. RESULTS: Among 129 cases of lesion-associated OCD, 40 had clearly defined lo

Differential Cranial Nerve Susceptibility to Compression by a Giant Cerebellopontine Angle Epidermoid Cyst: A Case Report and Literature Review.

Giant cerebellopontine angle (CPA) epidermoid cysts may envelop multiple cranial nerves, yet patients often present with clinically isolated trigeminal neuralgia. The anatomical basis for this selective vulnerability is incompletely appreciated in routine clinical practice. A 48-year-old man presented with a five-year history of progressive left-sided trigeminal neuralgia involving the maxillary and mandibular divisions. Pain had become severe and refractory to high-dose carbamazepine, with substantial functional and psychological decline. Neurological examination was otherwise unremarkable, w

Magnetic resonance imaging characteristics of brain metastases from lung cancer.

BACKGROUND: One of the most common primary tumor sources of brain metastases (BMs) is lung cancer. As certain magnetic resonance imaging (MRI) features overlap between the pathological and genetic subtypes of lung cancer BMs, directly determining the primary site based on these features remains a challenge. Thus, identifying the MRI features of different subtypes of lung cancer BMs is crucial in order to facilitate early diagnosis and treatment. This study aimed to characterize the MRI characteristics distinct to the various subtypes of lung cancer BMs in order to inform clinical decision-maki

[Infection or Autoimmunity: Key Imaging Points for Differential Diagnosis].

Differentiating infectious encephalitis from autoimmune encephalitis remains a major diagnostic challenge in clinical practice because their magnetic resonance imaging (MRI) findings frequently overlap. Moreover, prior treatment with antimicrobial agents or corticosteroids may alter imaging appearances, obscure typical radiological patterns and further complicate interpretation. This review summarizes the key MRI features of infectious and autoimmune encephalitis, with emphasis on imaging patterns before treatment. Infectious encephalitis typically demonstrates meningeal enhancement, diffusion

Converging metabolic and functional networks for tremor expression and deep brain stimulation-mediated control.

Emerging evidence indicates that movement disorders arise from symptom-specific rather than disease-specific brain network dysfunctions that can be influenced through targeted neuromodulation. Such networks are widely mapped using normative connectome analyses from lesion and stimulation sites. Here, we used [18F]-fluorodeoxyglucose (FDG)-PET in 14 essential tremor patients undergoing thalamic deep brain stimulation (DBS) to identify stimulation-induced and tremor related regional metabolic changes in a within-subject design and combined this with normative connectome results. Stimulation incr

No link between piriform cortex subregion resection and seizure freedom in two cohorts with temporal lobe epilepsy.

BACKGROUND: Postoperative seizure freedom in temporal lobe epilepsy (TLE) has been linked to the extent of piriform cortex resection. Earlier work used manual piriform cortex segmentations truncated at the limen insulae and overlapping the amygdala. We re-evaluated this association using an investigator-independent, connectivity-based segmentation. METHODS: Patients with TLE who underwent mesiotemporal epilepsy surgery at a single center with > 1-year postoperative follow-up were included retrospectively (discovery cohort). Postoperative images and connectivity-defined piriform cortex subregi

Meaning for reading pseudowords: errors reveal semantic influences on pseudoword reading after stroke.

Impaired reading, i.e., alexia, is common after left hemisphere stroke. The most common deficit in alexia is a difficulty reading aloud pronounceable novel words, also called pseudowords. While semantic and phonological processes both subserve reading real words, pseudoword reading deficits in alexia are typically ascribed to phonological deficits alone. Some theories, however, suggest that pseudoword reading relies in part on lexical-semantic knowledge, such that semantic deficits could also contribute to poor pseudoword reading in alexia. Leveraging a large sample of left-hemisphere stroke s

Perineurioma-like EMA-positive calvarial neoplasms: clinicopathological study of eight cases.

BACKGROUND: Incidental skull lesions are increasingly detected due to the widespread use of brain imaging. We have encountered an unusual cohort of unclassifiable calvarial spindle cell lesions showing phenotypic overlap with perineurioma and meningioma, which we sought to further characterize. MATERIAL AND METHODS: Cases of unclassifiable low-grade spindle cell lesions arising in the calvarium were collected. Clinical, radiological and pathological findings were reviewed. All cases were uniformly stained for a panel of meningothelial and perineurial immunomarkers. RESULTS: Eight cases were

Gradual Occlusion of an Anterior Choroidal Artery Aneurysm Using Overlapping Flow Diversion and Short-term Anticoagulation: A Case Report.

Anterior choroidal artery aneurysms, in which the anterior choroidal artery arises from the aneurysmal sac, pose significant therapeutic challenges, as treatment risks include branch occlusion and devastating ischemic complications, including hemiplegia and hemianopia. We report a patient in their 70s with an incidentally diagnosed 12.2 mm unruptured right anterior choroidal artery aneurysm, in which three-dimensional angiography demonstrated that the anterior choroidal artery originated from the center of the aneurysm dome. During conservative observation, the aneurysm enlarged to 14.1 mm, ne

Significant aggravation of pre-existing myalgic encephalomyelitis/chronic fatigue syndrome following proton beam therapy for sphenoid wing meningioma: case report.

BACKGROUND: Myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS) is a debilitating multisystem disorder characterized by profound fatigue, post-exertional malaise (PEM), immune dysregulation, and mitochondrial dysfunction. While radiation exposure has been linked to fatigue syndromes with overlapping pathophysiology, no previous reports have described the effects of therapeutic radiation, including proton beam radiotherapy (PBRT), in patients with ME/CFS. CASE PRESENTATION: We report the case of a 46-year-old woman with a pre-existing, clinically confirmed diagnosis of ME/CFS (Bell sco

Cloud-to-Edge Deployment of Optimized nnU-Net for Ischemic Stroke Lesion Segmentation on Resource-Constrained Embedded Devices.

Ischemic stroke remains a leading cause of global mortality and long-term neurological disability, where the "Time is Brain" paradigm dictates that rapid and accurate lesion assessment is fundamental for effective clinical intervention. While the nnU-Net v2 framework has established a new state of the art in medical image segmentation, its high computational demands and reliance on data-center-grade GPUs hinder its translation into real-time, point-of-care clinical workflows. This study presents a technical feasibility analysis of a Cloud-to-Edge optimization pipeline designed to transfer a 3D

Systemic lupus erythematosus complicated by hydrocephalus and neuromyelitis optica spectrum disorder presenting with complex neuropsychiatric manifestations: a case report.

A 59-year-old woman with 45 years history of systemic lupus erythematosus (SLE) and antiphospholipid syndrome was admitted to our hospital with progressive disturbances in consciousness, cognitive impairment and paraparesis following pneumonia and cerebral infarction. Diagnostic workup revealed anti-aquaporin-4 antibody positivity and a longitudinally extensive transverse myelitis lesion, leading to a diagnosis of neuromyelitis optica spectrum disorder (NMOSD). Additionally, ventricular enlargement and cerebrospinal fluid (CSF) findings raised suspicion of normal pressure hydrocephalus (NPH).

Does timing matter? Unlocking the value of repeated MRI in the early phases after severe traumatic brain injury.

BACKGROUND: Cerebral MRI after severe traumatic brain injury (sTBI) may refine lesion characterization and support prognosis, but the optimal timing within the early course remains uncertain. METHODS: We retrospectively analyzed 17 adult sTBI patients (GCS ≤ 8) who underwent two clinically indicated 3 T MRI examinations: an early scan (≤ 72 h) and a subacute scan (day 12-14). Three board-certified neurointensivists, blinded to scan timing, independently rated lesion burden across standard sequences (T1, T2/FLAIR, SWI, DWI) and evaluated (i) comparative clinical utility of early vs subacute MR
