Serial prenatal MRI evaluation of valacyclovir-treated vs untreated fetuses with congenital cytomegalovirus (cCMV) infection.

OBJECTIVES: To compare serial prenatal MRI findings in valacyclovir-treated vs untreated fetuses with confirmed congenital cytomegalovirus (cCMV) infection, focusing on the occurrence and timing of minor brain lesions. MATERIALS AND METHODS: In this retrospective single-center cohort study, fetuses with confirmed cCMV infection and at least two prenatal brain MRI examinations were included. Fetuses with major brain malformations at baseline were excluded. Treated fetuses received oral valacyclovir (8 g/day) from diagnosis to delivery. Two senior neuroradiologists assessed MRI examinations in

Deciphering Freezing of Gait: What Neuropathology Reveals About an Episodic Phenomenon.

Freezing of gait (FoG) is an episodic, debilitating motor phenomenon defined as paroxysmal episodes wherein there is an inability to step effectively, despite attempting to do so. Although FoG is most commonly associated with Parkinson's disease, it manifests across a spectrum of progressive neurodegenerative and potentially transient non-neurodegenerative conditions that affect overlapping locomotor circuits. The episodic nature of FoG implies transient disruptions of a distributed locomotor network rather than fixed structural lesions, rendering its neuropathological basis particularly chall

Neuropsychiatric-pharmacologic synergy: interaction between psychiatric polypharmacy and cancer treatment-related neurotoxicity.

Altered mental status (AMS) in oncology populations frequently reflects complex interactions among medical, pharmacologic, and neuropsychiatric factors. Accurate identification of the primary contributors is crucial to minimize morbidity. We report the case of a man in his 60s with a relapsed plasma cell malignancy, status post autologous stem-cell transplant, who experienced recurrent AMS during ongoing combination chemotherapy and psychiatric polypharmacy. Neuroimaging excluded structural or vascular lesions. Laboratory evaluation revealed intermittent mild renal dysfunction without metaboli

Personalized tremor control targeting for MR-guided focused ultrasound using tractography.

Magnetic Resonance guided Focused Ultrasound Surgery (MRgFUS) is a non-invasive, innovative technology that induces a thermal lesion to treat tremor in medication-resistant patients diagnosed with essential tremor or Parkinson's disease. The ventral intermediate nucleus of the thalamus (VIM) is a well-established neuromodulation target for tremor reduction. The standard VIM detection method is based on anatomical landmarks, specifically the anterior and posterior commissure. However, it is correlated with suboptimal tremor control up to 20%, inconsistency of the treatment's outcome, leading to

Assessment of diverse deep brain stimulation targets uncovers a common neural pathway for instantaneous antidepressant effects in rats.

Deep brain stimulation (DBS) is a promising therapeutic modality for managing treatment-resistant depression. Most DBS research has focused on single brain regions resulting in unclear optimal stimulation targets and vague mechanisms. Here, we introduce an experimental paradigm in which multiple graphene fiber stimulating electrodes were implanted in various brain regions of the same depressive animal for behavioral testing and DBS-functional MRI studies. We observed an instantaneous alleviation of depressive-like symptoms with a high response rate in Wistar-Kyoto rats following DBS at the med

Frontal brain injury alters human risky choices in self and other contexts.

Frontal lobe integrity is crucial for risky decision-making. We investigated how frontal lesions affect computational mechanisms underlying risk when decisions impact oneself versus others. Twenty patients with frontal damage and 20 matched controls accepted or rejected mixed-outcome gambles for themselves or an anonymous other. Patients accepted more disadvantageous gambles than controls in the self condition and showed greater caution for another person; controls showed no self-other difference. Formal model comparison favored a four-parameter Prospect Theory model. Relative to controls, pat

ROS-Responsive Quercetin Nanoparticles Improve the Prognosis of Traumatic Brain Injury by Inhibiting Aberrant Nrf2-Keap1 Signaling Pathway Activation.

Traumatic brain injury (TBI) is one of the leading causes of mortality and disability worldwide, with secondary injury recognized as a critical therapeutic target. Quercetin (QR), a natural flavonoid, exerts antioxidant and anti-inflammatory effects by modulating the Nrf2-Keap1 pathway and shows neuroprotective potential in various neurological disorders. In this study, network pharmacology analysis identified 496 overlapping targets of QR and TBI, further highlighting the pivotal role of the Nrf2-Keap1 pathway in TBI treatment. However, the poor blood-brain barrier (BBB) permeability and low

Language network functional connectivity varies by aphasia type and severity.

Aphasia is increasingly understood as a disorder of disrupted language networks rather than damage to isolated language regions. However, most resting-state functional connectivity studies treat people with aphasia as a single group or classify individuals by overall severity, potentially obscuring qualitative differences in network organization across aphasia types. The present study examined how resting-state functional connectivity varies across aphasia types and how aphasia severity relates to network organization following left-hemisphere stroke. Resting-state fMRI data were analyzed from

SparseMed3D: Foundation Models for Sparse Instance Medical Segmentation.

Vision foundation models excel at general segmentation but underperform sharply when positive instances occupy a vanishing fraction of the input-a regime that arises across small-lesion and rare-event medical segmentation, with neonatal hypoxic-ischemic encephalopathy (HIE) as a representative extreme case (lesions <1% of brain volume; positive-to-negative voxel ratio >99:1). We present SparseMed3D, a framework for adapting vision foundation models to this extreme-sparsity regime through three coupled components: (i) parameter-efficient multi-channel patch-embedding adaptation; (ii) diffusion-

Convergent Network Localization of Brain Stimulation Targets for Trait Anxiety.

OBJECTIVE: Transcranial magnetic stimulation (TMS) and deep brain stimulation (DBS) can modulate brain circuits, and certain stimulation targets may relieve or exacerbate anxiety. The objective of this study was to localize circuit-based stimulation targets that modify anxiety by integrating causal sources of information, including lesion, TMS, and DBS datasets. METHODS: The authors leveraged a series of natural experiments across multiple datasets (N=936). First, patients with incidental brain lesions (N=451) and TMS-treated individuals with incidental variability in stimulation location for

Symptom-Only Localization of Brainstem Ischemia Using Large Language Models Versus Neurologists in Diffusion-Weighted Imaging-Positive Cases: Retrospective Single-Center Study.

BACKGROUND: Symptom-based localization of brainstem ischemia is challenging because of the anatomical complexity of the brainstem and the nonspecific overlap of clinical syndromes. Whether large language models (LLMs) can meaningfully assist in this task remains uncertain. OBJECTIVE: This study aimed to compare the performance of several OpenAI LLMs and neurologists in localizing diffusion-weighted imaging (DWI)-confirmed brainstem ischemic lesions based on symptom descriptions alone. METHODS: In this retrospective single-center study, 109 patients with DWI-confirmed acute brainstem ischemia

The structural grammar of integration and competition in the human connectome.

INTRODUCTION: Brain function emerges from coordinated activity across anatomically connected regions, where structural connectivity (SC)-the network of white matter pathways-provides the physical substrate for functional connectivity (FC), defined as the correlated activity between brain areas. While structural and functional networks exhibit substantial overlap, their relationship involves complex, indirect mechanisms, including the dynamic interplay of direct and indirect pathways. To systematically untangle how structural architecture shapes functional patterns, this work aims to establish

The European Association for Neuro-oncology (EANO) Consensus Statement on Radiation Necrosis.

INTRODUCTION: Radiation necrosis (RN) complicates neuro-oncological care, mimicking tumor recurrence and lacking high-level evidence for standardized management. METHODS: A European Association for Neuro-Oncology (EANO) expert panel utilized a three-round Delphi process to create a comprehensive expert opinion document based on the available current scientific evidence. A series of statements, derived from the published literature were created by the experts in each field. Consensus was defined as ≥ 80% agreement using a 5-point Likert scale. RESULTS: After three rounds among 20 experts that

Brain Abscess Likely Secondary to Infective Endocarditis Mimicking High-Grade Glioma in a Patient with Pyelonephritis.

Brain abscesses are serious central nervous system infections that often present with nonspecific infectious and neurologic symptoms and may mimic intracranial neoplasms on imaging. They can develop rapidly or insidiously, and diagnosis is often delayed when symptoms overlap with concurrent systemic infections. We report the case of a 66-year-old woman admitted for pyelonephritis who initially improved with intravenous antibiotics but later developed persistent fever, worsening occipital headache, and acute neurologic deterioration. Initial imaging demonstrated an irregular ring-enhancing lesi

Clinical and MRI factors influencing interobserver delineation variability of brain metastases before stereotactic radiotherapy.

BACKGROUND: Innovations in systemic therapies have prolonged survival for many cancers, increasing the number of patients living with brain metastases. In stereotactic radiotherapy, steep dose gradients and small target margins mean that delineation differences may affect target coverage and the dose to adjacent normal brain. We aimed to identify lesion- and MRI-related factors associated with interobserver variability in brain metastasis gross tumor volume delineation. METHODS: We retrospectively included 149 patients (354 metastases) treated with stereotactic radiotherapy at the University

Beyond the Brain: Circulating Tumor Cells as a Tool for Diagnosis and Monitoring.

PURPOSE: Glioblastoma (GBM) is an aggressive brain tumor characterized by extensive heterogeneity and inevitable recurrence. Although extracranial metastases are rare, circulating tumor cells (CTCs) are consistently detectable in patients with GBM, challenging the idea of GBM as a strictly localized disease and suggesting CTCs as a promising liquid biopsy tool. METHODS: We analyzed genomic and phenotypic features of bulk CTCs (BCTCs) and single CTCs in 27 patients with GBM. Whole-exome sequencing of BCTCs and matched tumors confirmed tumor origin and assessed shared mutational and copy number

A large-scale comparative study of YOLO-based detectors for ischemic stroke lesion localization on diffusion-weighted imaging.

Ischemic stroke lesion localization on diffusion-weighted imaging (DWI) remains challenging because acute lesions may be small, faint, irregular, or multifocal. This study presents a controlled lesion-level benchmark of recent YOLO-based detectors for ischemic stroke localization on DWI. A private clinical cohort of 300 patients and approximately 2,200 lesion-containing axial trace-weighted DWI images was annotated with expert-consensus bounding boxes. Twenty-four detector configurations from YOLOv10, YOLO11, YOLOv12, YOLOv13, and YOLO26 were trained and evaluated under a unified protocol usin

Imaging MRI biomarkers for diagnosing brain radionecrosis: focus on incomplete ring enhancement sign.

PURPOSE: Differentiating brain radionecrosis from recurrent metastases after radiotherapy remains challenging because no single imaging technique is definitive. This study evaluated the diagnostic accuracy of the incomplete ring enhancement sign (IRES) on contrast-enhanced 3D T1-weighted gradient-echo MRI (CE 3D T1 GRE) as a diagnostic sign of radionecrosis and analyzed associated morphologic features. METHODS: In this retrospective case-control study, 45 cases of radionecrosis were identified from an institutional database of 1,179 brain metastases consecutively treated with stereotactic rad

Bridging the Gap: Neurocircuitry Behind Mutism: Insights From a Midbrain Lesion.

Acquired mutism, including akinetic mutism (AM) and postoperative pediatric cerebellar mutism syndrome (ppCMS), arises from damage to specific brain regions and is characterized by an inability to produce verbal communication. AM is associated with damage to fronto-subcortical motivational circuits and-in addition to mutism-presents with akinesia, whereas ppCMS involves disruption of (non)motor cerebello-cerebral circuits and is further characterized by emotional lability and a typically delayed onset. We report a case of a 10-year-old patient who developed mutism after surgical resection of a
