Tuberculous brain abscess mimicking a neoplastic lesion in a patient with well-controlled human immunodeficiency virus infection: illustrative case.

BACKGROUND: Tuberculous brain abscess is a rare manifestation of CNS tuberculosis, typically occurring in patients with significant immunosuppression. These lesions may present as solitary ring-enhancing masses and can radiographically resemble neoplastic processes, creating diagnostic and management challenges. OBSERVATIONS: A 43-year-old man with well-controlled human immunodeficiency virus (HIV) infection and a remote history of treated pulmonary tuberculosis presented with progressively worsening unilateral headaches. MRI demonstrated a small, lobulated, peripherally enhancing posterior t

When the Clinical Picture Demands More: Dual Diagnosis of Neurofibromatosis Type 1 and Auriculocondylar Syndrome 2A in a Pediatric Case.

INTRODUCTION: Neurofibromatosis type 1 (NF1) and auriculocondylar syndrome 2A (ARCND2A) are distinct rare genetic disorders, each with characteristic clinical features. The simultaneous occurrence of these two syndromes is exceptionally rare and poses significant diagnostic challenges, especially in pediatric patients with complex phenotypes. CASE PRESENTATION: We report a 3-year-old Turkish girl who presented with multiple café-au-lait macules and dysmorphic facial features. Initial single-gene sequencing revealed a variant in the NF1 gene; however, the presence of severe craniofacial anomal

Two reading routes, two white matter tracts: Evidence from dyslexia following brain tumors.

Cognitive models propose that reading aloud relies on two routes, a lexical route for familiar word recognition and a sublexical route for graphene-to-phoneme conversion. Selective disruption of these routes results in surface or phonological dyslexia, respectively. Although the dual route model is well supported behaviorally, its white matter substrates remain incompletely characterized. This study examined the anatomical correlates of acquired lexical and sublexical reading impairments in patients with high-grade glioma in the dominant hemisphere. Thirty-seven patients underwent diffusion te

Brain lesions associated with abnormal muscle tone during the swing phase in post-stroke hemiparesis.

BACKGROUND: Post-stroke spasticity may involve dynamic abnormalities that impair voluntary movement, such as muscle co-contraction during gait, which may be present even in patients without apparent spasticity at rest. This study aimed to identify brain lesion locations associated with abnormal lower-limb muscle co-contraction during walking using support vector regression lesion-symptom mapping. METHODS: Thirty-two ambulatory patients with first-ever supratentorial stroke were included. Lower-limb motor impairment was defined as a Fugl-Meyer Assessment lower-extremity score of < 34. Surface

POWDR: Pathology-Preserving Outpainting with Wavelet Diffusion for 3D MRI.

Background: Medical imaging datasets often suffer from class imbalance and limited availability of pathology-rich cases, which limit the performance of machine learning models for segmentation, classification, and vision-language tasks. We propose POWDR, a pathology-preserving outpainting framework for 3D MRI that uses real pathological regions as conditioning evidence while generating anatomically plausible surrounding tissue. Methods: Our approach leverages wavelet-domain conditioning to enhance high-frequency detail and mitigate blurring common in latent diffusion models. We introduce a ran

BG-YOLO11s: Boundary-Guided YOLO11 with Bézier Contour Augmentation for Brain Tumour Segmentation in T1-CE MRI.

Background/Objectives: Accurate delineation of brain tumours on contrast-enhanced MRI remains difficult because lesions can be small, irregular, and weakly separated from adjacent tissue. This study developed BG-YOLO11s, a boundary-guided single-stage instance-segmentation model for T1 contrast-enhanced MRI. Methods: The public Figshare/Cheng dataset, comprising 3064 slices from 233 patients, was converted to YOLO polygon annotations and evaluated using a fixed 70/15/15 image-level split (2144/459/461 slices). Because patient identifiers were not retained in the exported image-and-polygon data

Case Report: DICER1-mutant primary intracranial sarcoma with concurrent TP53, PDGFRA, and KEAP1 somatic mutations in a 5.5-year-old boy.

DICER1-mutant primary intracranial sarcoma (DICER1-mutant PIS) is a rare and aggressive central nervous system (CNS) malignancy primarily affecting pediatric patients. We report the case of a 5.5-year-old boy who presented with non-specific prodromal symptoms, including recurrent dizziness and progressive vomiting. Brain neuroimaging revealed a cystic-solid mass in the frontal lobe, characterized by heterogeneous contrast enhancement, significant perilesional edema, and intratumoral hemorrhage. Histopathological examination of the tumor demonstrated marked cellular pleomorphism, atypical mitos

Adaptive multi-stage domain unlearning for white-matter lesion segmentation.

INTRODUCTION: Inter-scanner variability in magnetic resonance imaging (MRI) adversely affects the diagnostic and prognostic quality of scans and necessitates the development of models that are robust to domain shift arising from the unseen scanner data. A review of recent advances in domain adaptation and domain generalization showed that the efficacy of strategies involving modifications or constraints on the latent space appears to be contingent upon the level and/or depth of supervision during model training. METHODS: We propose an adaptive multi-stage domain unlearning (ADMU) technique to

Automatic extraction of structured information from brain MRI reports using an open-weight large language model.

OBJECTIVES: Automatic data extraction from free-text radiology reports enables large-scale research, but few studies assessed the performance of large language models (LLMs) on Dutch neuroradiology reports. MATERIALS AND METHODS: We analyzed 947 brain MRI reports from a tertiary memory clinic (2016-2021), authored by consultant neuroradiologists. Trained medical students annotated thirty variables; 100 reports were double annotated to assess inter-rater reliability. We evaluated the performance of the open-weight LLM LLaMA 3.1 using different languages (Dutch vs English translation) and few-s

Comparative analysis of candidate gene expression in Asian atopic dermatitis and psoriasis: identification of shared and distinct molecular features.

BACKGROUND: Atopic dermatitis (AD) and psoriasis are chronic inflammatory skin diseases with distinct molecular profiles, but their clinical differentiation in Asian patients is often difficult due to overlapping or atypical features. This study aimed to identify differentially expressed genes (DEGs) and pathways in lesional skin to explore potential diagnostic and therapeutic targets. METHODS: To identify molecular differences, we performed transcriptomic analysis of lesional skin from patients with AD, psoriasis, and healthy controls (HCs). DEGs were identified by comparing each disease gro

Plasma proteomics of cerebrovascular disease, cognitive decline, and clinical outcomes.

INTRODUCTION: The plasma proteomic signatures underlying cerebrovascular disease (CeVD) remains poorly understood. METHODS: A total of N = 2534 participants across two independent longitudinal Southeast-Asian cohorts were included. We profiled 1441 baseline plasma proteins in a memory-clinic cohort (N = 518), followed-up for 4 years. Proteins associating with baseline and longitudinal CeVD lesions (i.e., white matter hyperintensity volume, lacunes, cerebral microbleeds, and cortical infarcts) were reported. The prognostic value of CeVD-associated proteins was evaluated for incident major card

Spatial pattern-driven interpretable model and biological correlates in brain glioblastoma-lymphoma differentiation.

Glioblastoma (GBM) and primary central nervous system lymphoma (PCNSL) often exhibit overlapping appearances on routine MRI, complicating pre-treatment diagnosis. In 1,109 patients from five centers, we constructed standard-space tumor probabilistic maps and derived atlas-anchored spatial features to augment conventional radiomics. The spatial radiomics classifier outperformed radiomics alone (external test area under the ROC curve [AUC], 0.98) with acceptable calibration and decision curve benefit, and SHapley Additive exPlanations (SHAP)-enabled anatomy-grounded interpretation. Aligning tumo

An Adaptive, Time-Informed, Phenotype-Guided Pharmacological Framework for Severe Traumatic Brain Injury : A Hypothesis-Driven Narrative Review.

Severe traumatic brain injury (TBI) remains difficult to treat because successful control of mass lesions, intracranial pressure, cerebral perfusion, oxygenation, and systemic instability does not ensure that secondary cellular injury has stopped. PubMed/MEDLINE and ClinicalTrials.gov were searched through July 28, 2026, and representative candidates were organized across five overlapping pathophysiological nodes. Evidence was described by study source, purpose, and result direction rather than assigned a formal quality grade. Human evidence was heterogeneous. Glibenclamide/glyburide and hyper

Neural structures associated with auditory short-term memory and sentence repetition following chronic left hemisphere stroke.

INTRODUCTION: Prior evidence suggests auditory short-term memory (STM) and repetition share several processing elements. However, it remains elusive whether auditory STM capacity and repetition ability show overlapping neural correlates. We sought to identify the brain structures underpinning STM capacity and repetition outcomes in patients with chronic left hemisphere stroke and aphasia. METHODS: Fifty-four left hemisphere stroke patients with aphasia and 50 demographically matched healthy controls underwent digit span and language tasks. Structural and diffusion images were acquired in all

Case Report: Myelin oligodendrocyte glycoprotein antibody-associated cortical encephalitis masked by peri-ictal magnetic resonance imaging abnormalities.

One phenotype of cerebral cortical encephalitis in myelin oligodendrocyte glycoprotein (MOG) antibody-associated disease (MOGAD) is FLAMES (fluid-attenuated inversion recovery-hyperintense lesions in anti-MOG-associated encephalitis with seizures). FLAMES is characterized by its imaging features and epileptic seizures. We present a case in which these findings were masked by peri-ictal magnetic resonance imaging abnormalities (PMAs) associated with status epilepticus, making differentiation difficult. A middle-aged woman presenting with persistent right upper limb weakness and aphasia was admi

Ovarian Teratoma After Treatment for Coexisting Anti-NMDA Receptor and Myelin Oligodendrocyte Glycoprotein Antibody-Associated Encephalitis.

Anti-N-methyl-d-aspartate receptor encephalitis (NMDARE) is an autoimmune encephalitis often associated with ovarian teratomas in adults but rarely in children. Myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) typically occurs in pediatric populations and may occasionally coexist with NMDARE. We report the case of a 6-year-old girl diagnosed with concurrent NMDARE and MOGAD. She presented with psychiatric symptoms and oral dyskinesia, and her magnetic resonance imaging findings were compatible with both NMDARE and MOGAD. Anti-NMDAR antibodies were detected in the cerebro

Comparison of volume- and surface-based magnetic resonance imaging morphometry algorithms in the detection of focal cortical dysplasia.

OBJECTIVE: Detection of focal cortical dysplasia (FCD) remains a major challenge in presurgical epilepsy diagnostics. Magnetic resonance imaging (MRI) morphometry increasingly improves lesion detection and postsurgical outcomes. The volume-based Morphometric Analysis Program, version 2018 (MAP18) with integrated artificial neural network and surface-based Multi-centre Epilepsy Lesion Detection (MELD; MELD Graph) MRI morphometry algorithms have demonstrated diagnostic utility, but their performance has not been directly compared within a single cohort of surgically treated patients with histopa
