Retrospective Analysis of T2-Hyperintense Lesions in Children With Neurofibromatosis Type 1.

OBJECTIVE: The aim of this study is to determine whether a previously reported three-tiered classification scheme for T2-hyperintense brain lesions in children with neurofibromatosis type 1 (NF1) was associated with distinct radiographic or clinical characteristics after 10 years of real-world follow-up data were obtained. METHODS: In this single-center retrospective study, we analyzed T2-hyperintense lesions identified on brain magnetic resonance imaging (MRI) of 58 children with NF1. Lesions were categorized as "Low Likelihood of Tumor," "Intermediate Likelihood of Tumor," or "High Likeliho

Neuronal intranuclear inclusion disease: a diagnostic pitfall for MELAS.

BACKGROUND: Neuronal intranuclear inclusion disease (NIID) is a rare neurodegenerative disease characterized by eosinophilic hyaluronan inclusions in the nervous system and internal organs. Mitochondrial encephalomyopathy with lactic acidosis and stroke-like episodes (MELAS) is the most common neurological mitochondrial disease involving multiple organs. The complex and overlapping clinical manifestations of both diseases pose a significant risk for misdiagnosis. CASE PRESENTATION: We present a case of NIID that closely mimicked the phenotype of MELAS. A 60-year-old Chinese man presented with

Comparative Analysis of Magnetic Resonance Spectroscopy and Post-contrast Magnetic Resonance Imaging in the Evaluation of Intracranial Ring-Enhancing Lesions.

Background Intracranial ring-enhancing lesions are diagnostically heterogeneous because infective, neoplastic, demyelinating, and vascular etiologies may show overlapping conventional magnetic resonance imaging (MRI) appearances. This study evaluated and compared the diagnostic performance of post-contrast MRI and magnetic resonance spectroscopy (MRS) in the etiological characterization of intracranial ring-enhancing lesions. Materials and methods This hospital-based prospective observational, non-interventional study included 45 patients with intracranial ring-enhancing lesions. All patients

A new era of genome-wide association studies in the field of Alzheimer's disease and overlapping co-pathologies: lessons learned from a neuropathology-centered approach.

Alzheimer's disease (AD) and overlapping pathologies represent a growing worldwide health concern. With the first disease-modifying treatments on the rise, it becomes increasingly important to move research in this area forward. Genetic research is excellent at discovering novel contributors to disease mechanisms, which has been demonstrated by the discovery of over 75 disease loci associated with AD. However, classical large-scale genome-wide association studies (GWAS) use cohorts of individuals that have been assigned a case or a control status based on a clinical diagnosis. For AD, this can

Genome wide association study meta-analysis of neuropathologic lesions of Alzheimer's disease and related dementias in a multi-site autopsy cohort.

Understanding the genetic foundations of dementia is critical to unraveling its complex molecular basis. Given that a clinical diagnosis of Alzheimer's disease (AD) dementia often results from interplay between multiple underlying neuropathologic co-morbidities, previous genome-wide association studies (GWAS) of clinically diagnosed AD are restricted in their ability to translate genetic associations to potential targeted therapeutics. The current study seeks to address these limitations by presenting the largest GWAS to date (n = 12,509) of neuropathologic hallmarks of AD and AD related demen

Midbrain dopamine loss drives parvalbumin interneuron vulnerability through tissue plasminogen activator-linked perineuronal-net breakdown and hippocampal disinhibition.

Midbrain dopaminergic degeneration is an early feature of Alzheimer's Disease (AD), dementia with Lewy bodies (DLB), and AD-Parkinson's disease overlap (AD-PD). However, its direct contribution to the failure of hippocampal inhibitory-circuits, a pathological feature shared across these conditions, remains unresolved. Parvalbumin-positive interneurons (PV-INs) regulate hippocampal excitation-inhibition balance and are directly modulated by dopamine (DA). These neurons are protected by perineuronal nets (PNNs), extracellular-matrix structures supporting fast GABAergic signaling and neuronal res

Posterior reversible encephalopathy syndrome (PRES): A narrative review of pathophysiology, clinical insights, and advances in management.

BACKGROUND: Posterior reversible encephalopathy syndrome (PRES) is an acute neurological condition characterized by a spectrum of clinical and radiological features. It is most often associated with severe hypertension, renal dysfunction, autoimmune disease, sepsis, and exposure to cytotoxic or immunosuppressive agents. Although increasingly recognized in emergency and critical care practice, its pathophysiology remains incompletely understood and diagnostic challenges frequently delay treatment. Reported mortality ranges from 10 to 19%, and up to 40–44% of patients experience persistent neuro

Attentional dysfunction in persistent postural-perceptual dizziness: a narrative review of mechanistic evidence.

Persistent postural-perceptual dizziness (PPPD) is a common chronic functional neuro-otological disorder characterized by persistent non-spinning dizziness, unsteadiness, and heightened sensitivity to upright posture, motion, and visually complex environments. Several conceptual models have been proposed to explain its persistence, including failure of readaptation after a destabilizing event, maladaptive multisensory integration with excessive visual or somatosensory weighting, and prediction-actual sensory mismatch with amplified prediction errors. This narrative review synthesizes neuroimag

Comparative Evaluation of Single-Voxel and Multi-Voxel Proton Magnetic Resonance Spectroscopy Using Small-Sized Voxels in Healthy Beagle Dogs: A Pilot Study.

Magnetic resonance imaging has limitations in definitively diagnosing canine brain diseases due to overlapping lesion morphologies. Magnetic resonance spectroscopy (MRS) can complement conventional MR imaging by evaluating brain metabolites at a molecular level, though voxels under 1 cm3 have not been routinely evaluated in veterinary medicine. This study evaluated the usefulness of single-voxel spectroscopy (SVS) using small-sized voxels (0.125 cm3) by comparing it with multi-voxel spectroscopy (MVS). Both techniques were performed on the white matter and gray matter of the frontoparietal lob

Anti-NMDA-receptor encephalitis and MOGAD associated optic neuritis: a case series.

INTRODUCTION: Anti-N-methyl-D-aspartate receptor (NMDAR) encephalitis is a well-recognized autoimmune condition that often presents with neuropsychiatric symptoms and seizures. Myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) often manifests as optic neuritis and, less frequently, as acute demyelinating encephalomyelitis or transverse myelitis. The co-occurrence of anti-NMDAR encephalitis and MOGAD is becoming increasingly recognized, but clinical series remain limited. DESCRIPTION OF CASES: We present three patients with anti-NMDAR encephalitis and MOGAD optic neuritis

Myelin Oligodendrocyte Glycoprotein Antibody-Associated Disease Mimicking Central Nervous System Infection in an Adolescent: A Case Report.

Myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) can resemble central nervous system infection in children because fever, headache, vomiting, cerebrospinal fluid pleocytosis, and multifocal brain lesions may coexist. We report a previously healthy 13-year-old boy who developed headache, fever, vomiting, and binocular horizontal diplopia. Examination showed transient exotropia with apparent adduction limitation but no encephalopathy, seizures, limb weakness, pupillary abnormality, optic neuritis, or spinal cord lesion. Cerebrospinal fluid showed mononuclear-predominant pl

Presence of Antinuclear Antibodies in the Cerebrospinal Fluid and in the Serum in a Patient With Sjögren's Syndrome and Systemic Lupus: A Case Report.

Systemic lupus erythematosus (SLE) may overlap with Sjögren's syndrome (SS), leading to complex systemic involvement, including neurological and pulmonary manifestations. We report a 60-year-old woman with a 10-year history of SLE-SS overlap presenting with headaches and bilateral visual impairment. Hydroxychloroquine (HCQ) had been discontinued 4 months prior due to HCQ-related optic neuropathy confirmed on optical coherence tomography (OCT). Ophthalmologic evaluation revealed bilateral optic neuritis. Immunological testing showed high-titre antinuclear antibodies (ANAs) in both serum (1:640)

Pediatric hemophagocytic lymphohistiocytosis with predominant CNS involvement: MRI features and differential diagnosis.

BACKGROUND: Hemophagocytic lymphohistiocytosis (HLH) is a rare, life-threatening hyperinflammatory disorder resulting from immune dysregulation that leads to uncontrolled macrophage activation. The diagnosis of HLH is often challenging, as its clinical manifestations can mimic various systemic conditions, including sepsis, autoimmune disorders, and malignancies. This overlap, combined with the rapid progression of the disease, may delay treatment and increase morbidity and mortality. Neurological involvement is particularly frequent in pediatric cases. Neuroimaging plays a critical role in rai

Enlarged Perivascular Spaces in Patients with Lacunar Infarctions.

INTRODUCTION: Lacunar infarction (LI) and enlarged perivascular spaces (EPVSs) are key MRI findings in cerebral small vessel disease. The underlying causes of LI and EPVS share similarities. To explore their relationship, we examined the association between lacunar infarction (LI) and enlarged perivascular spaces (EPVS). METHODS: We examined subjects with no history of brain disease (control) and patients with LIs requiring acute hospitalization. EPVS severity (graded 0-4) in the centrum semiovale (CSO) and basal ganglia (BG), along with other radiological findings such as white matter lesion

Astrocytopathy in Wernicke Encephalopathy and Neuromyelitis Optica Spectrum Disorder. Pathogenic Differences With Occasional Clinical and Neuroimaging Overlap. A Review.

PURPOSE OF THE REVIEW: There is an increasing number of single case reports describing patients with findings compatible with thiamine deficiency, in whom rapid replacement of thiamine fails to induce improvement. Further work-up identifies positive aquaporin-4 antibodies (AQP-4) leading to a final diagnosis of neuromyelitis optica spectrum disorder (NMOSD). Occasionally, NMOSD is the initial diagnosis, but it turns out that the patient has thiamine deficiency. Similarly, these two diagnoses may overlap, particularly with area postrema lesions and in other cases of protracted vomiting. RECENT

Real-World Challenges in the MRI Detection and Classification of Amyloid-Related Imaging Abnormalities.

Anti-amyloid monoclonal antibody therapies (AATs) are increasingly used for the treatment of early symptomatic Alzheimer disease. Safe implementation of AATs relies on accurate assessment for exclusionary findings on baseline brain MRI and reliable longitudinal detection of amyloid-related imaging abnormalities (ARIA), including ARIA-H (hemosiderin or hemorrhage) and ARIA-E (edema or effusion). In routine clinical practice, MRI findings encountered during baseline screening and ARIA surveillance sometimes fall into borderline or ambiguous categories that do not clearly conform to trial-defined

Duodenal Lymphocytosis and B1 Deficiency: Unveiling the Overlap Between Gut and Brain.

Thiamine (vitamin B1) deficiency is a reversible, yet potentially fatal condition that affects both the central and peripheral nervous systems. Although commonly linked to chronic alcoholism, non-alcoholic causes, including malabsorption syndromes, are increasingly being recognized. We report the case of a 22-year-old woman who presented with progressive lower limb weakness, gait disturbances, and confusional symptoms, following a history of intermittent diarrhea and vomiting. Neurological examination revealed ascending motor deficits, areflexia, cerebellar syndrome, and internuclear ophthalmo

Intrasellar symptomatic salivary gland choristoma: a case report and literature review.

BACKGROUND: Symptomatic ectopic salivary tissue in the sellar region is extremely rare. A few cases were reported in the literature. Its radiological features overlap with those of other sellar lesions, making a definitive preoperative diagnosis difficult. This case report presents the youngest treated symptomatic patient who was diagnosed with ectopic salivary tissue in the sellar region. CASE DESCRIPTION: A 6-year-old symptomatic girl presented with chronic headache, polyuria, polydipsia, and short stature. The hormonal assay revealed hypopituitarism with low growth hormone.
