Therapeutic and biomarker potential of erythropoietin in neurodegenerative diseases: a systematic review.

BACKGROUND: Neurodegenerative diseases present an escalating global health burden with limited disease-modifying therapies. Erythropoietin (EPO), well-known for its hematological effects, has demonstrated neuroprotective properties in preclinical studies, but its clinical utility in neurodegeneration remains unclear. Therefore, this study aims to systematically appraise clinical and translational evidence on the efficacy and safety of EPO and its analogues in AD, PD, and dementia syndromes. METHODS: A systematic review was conducted following JBI and PRISMA guidelines. PubMed and Scopus were s

Altered cerebrospinal fluid-based clearance mechanisms in aging autistic adults.

BACKGROUND: Autistic adults demonstrate a 4-6-fold increased risk of unspecified dementia compared with the general population; however, the neurobiological substrates underlying this elevated risk remain unexplored. Alterations in cerebrospinal fluid-based mechanisms involved in brain metabolic waste clearance may represent a shared neuropathological pathway between autism spectrum disorder and dementia. Specifically, developmental deviations in cerebrospinal fluid-related imaging markers have been consistently reported in autistic infants, children, and adolescents, and brain amyloid and oth

Why dementia is often not diagnosed by subtype: care pathways and imaging access in the United Arab Emirates.

BACKGROUND: Whether a person with dementia receives a specific subtype diagnosis rather than a nonspecific label is a marker of diagnostic completeness. Subtype shapes safe management, including psychotropic prescribing, behavioural and psychological symptom management, and eligibility for Alzheimer's disease therapies, yet nonspecific labels remain common where specialist assessment is unevenly accessed. We examined which patient, clinical, and service-pathway factors were associated with specific subtype assignment in a dementia cohort in the United Arab Emirates (UAE), and whether an explor

Neonatal Presentation of Suspected Vici Syndrome: A Case Report with Multisystem Features.

Vici syndrome is a rare autosomal recessive multisystem disorder caused by biallelic mutations in the EPG5 gene. It is classically characterized by agenesis of the corpus callosum, oculocutaneous hypopigmentation, combined immunodeficiency with recurrent infections, cataracts, and cardiomyopathy. We report the case of a newborn born to consanguineous parents, with a family history of a sibling death of unspecified cause, presenting with hypopigmentation, generalized hypotonia, agenesis of the corpus callosum, severe recurrent infections, developmental delay, and cleft palate. The infant also d

The role of noninfectious comorbidities in the association between severe infections and risk of dementia in Finland: A nationwide registry study.

BACKGROUND: Severe infections have been linked to an increased risk of dementia, but both conditions often coexist with other illnesses that may confound this association. Using nationwide Finnish health registry data, we examined the role of noninfectious mental and physical illnesses in the association between severe infections and dementia. METHODS AND FINDINGS: This register-based study included 62,555 individuals aged 65 or older in Finland in 2016 who were diagnosed with late-onset dementia between 2017 and 2020 and 312,772 dementia-free controls matched for year of birth, sex, and the

Long-term risk of dementia following encephalitis: a large-scale retrospective cohort study of electronic health records.

BACKGROUND: Encephalitis is an acute inflammation of the brain parenchyma, typically caused by microbial infections or autoimmune mechanisms, and represents a potentially life-threatening neurological condition affecting thousands of individuals each year in the United States. Although survivors may experience persistent neurological sequelae, long-term association of encephalitis and subsequent dementia and related diagnoses across different age groups and encephalitis etiologies remains poorly characterized. This study aimed to quantify the association between encephalitis and the risk of de

Association between leukocyte telomere length and neurodegenerative diseases: a prospective cohort in the UK Biobank.

BACKGROUND: This study aims to investigate the association between leukocyte telomere length (LTL) and the risk of incident NDDs using a large-scale cohort from the UK Biobank. METHODS: Data from 459,902 subjects were analyzed using Cox proportional hazards models and machine learning (ML) algorithms to assess LTL's association with NDD risk. RESULTS: Shorter LTL was associated with an increased risk of NDDs, including Alzheimer's disease (HR: 0.52, 95% CI 0.40-0.67, P < 0.001), dementia in AD (HR: 0.53, 95% CI 0.39-0.73, P < 0.001), unspecified dementia (HR: 0.74, 95% CI 0.58-0.95, P < 0.05

Exclusion of patients with brain metastases from early-phase systemic therapy trials: a cross-sectional analysis of eligibility criteria.

PURPOSE: Patients with brain metastases (BrM) are frequently excluded from early-phase clinical trials, limiting evaluation of central nervous system (CNS) efficacy of novel therapies. We aimed to determine the proportion of phase I systemic therapy trials for patients with metastatic solid tumours that permit enrolment of patients with BrM and to characterise BrM-specific eligibility criteria. METHODS: ClinicalTrials.gov was searched in July 2024 for phase I systemic therapy trials for patients with advanced solid tumours and publicly available protocols. Trial characteristics and BrM eligib

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Data-driven neuroanatomical subtypes of primary progressive aphasia.

The primary progressive aphasias are rare, language-led dementias, with three main variants: semantic, non-fluent/agrammatic and logopenic. Although the semantic variant has a clear neuroanatomical profile, the non-fluent/agrammatic and logopenic variants are difficult to discriminate from neuroimaging. Previous phenotype-driven studies have characterized neuroanatomical profiles of each variant on MRI. In this work, we used a machine learning algorithm known as SuStaIn to discover data-driven neuroanatomical 'subtype' progression profiles and performed an in-depth subtype-phenotype analysis t

Biopsy location and tumor-associated macrophages in predicting malignant glioma recurrence using an in-silico model.

Predicting the biological behavior and time to recurrence (TTR) of high-grade diffuse gliomas (HGG) after maximum safe neurosurgical resection and combined radiation and chemotherapy plays a pivotal role in planning clinical follow-up, selecting potentially necessary second-line treatment and improving the quality of life for patients diagnosed with a malignant brain tumor. The current standard-of-care (SoC) for HGG includes follow-up neuroradiological imaging to detect recurrence as early as possible and relies on several clinical, neuropathological, and radiological prognostic factors, which

What would be pre-modern human cognition?

Stibbard-Hawkes's detailed demonstration that in the case of hunter-gatherer artifacts, absence of evidence is not evidence of absence must never be forgotten. The belief that there is a single coherent "human cognitive capacity" difference between modern humans and some unspecified earlier form should be rigorously re-examined.

Biological signatures in the Alzheimer's continuum discriminate between diagnosis-related and -unrelated associations to ATN categories.

Alzheimer's disease and related dementias have a multifactorial aetiology and heterogeneous biology. The current study aims to identify different biological signatures in a deeply phenotyped memory clinic patient population. In this cross-sectional study, we analysed 49 pre-specified proteins using a multiplex antibody-based suspension bead array in 278 CSF samples from the real-world research database and biobank at the Karolinska University Hospital Memory Clinic, Solna, Sweden. Patients with a clinical diagnosis of subjective cognitive decline (N = 151), mild cognitive impairment (N = 61),

Co-occurrence of congenital anomalies and childhood brain tumors in 22 million live births.

BACKGROUND: Children born with a congenital anomaly have a higher risk of developing a brain tumor during childhood or adolescence, but the co-occurrence between specific types of congenital anomalies and specific types of childhood brain tumors (CBTs) is not well described. This study characterized the associations between specific congenital anomalies and CBTs. METHODS: We leveraged a population-based registry linkage study of births (1990-2018), congenital anomalies, and cancer from 9 states (n = 22,599,099 births). Congenital anomalies were classified as major structural without a known c

Examining the Impact of a Developed Family Intervention Program on Children with ADHD.

ADHD is a brain development disorder that typically starts in early childhood, making it difficult for children to function effectively in daily life. The course of ADHD is negatively influenced by a dysfunctional family environment. Since ADHD affects development early on, involving parents in treatment is crucial for helping children with this condition. This study aims to explore how a group program designed to help families function better can improve the lives of children with ADHD and their parents. The correlational, causal-comparative, and quasi-experimental designs were utilized withi

Ten-year patterns of emergent concussion injuries among various martial arts disciplines.

BACKGROUND: Martial arts participation appears to be increasing in the US, and along with benefits to health and wellbeing, the sport carries musculoskeletal and neurological injury risks which differ among disciplines. Evidence is limited regarding the burden of concussion injuries in many martial arts disciplines, particularly among female athletes and outside of competitions. The purposes were: 1) to characterize injury patterns over 2013-2022 for concussion injuries treated in emergency departments (ED) related to martial arts; and 2) to compare the number of concussions by mechanism, circ

Asthma causally affects the brain cortical structure: a Mendelian randomization study.

OBJECTIVE: The potential causal relationship between asthma and brain structures remains uncertain. We performed a two-sample Mendelian randomization to investigate the causal effects of various asthma phenotypes - unspecified asthma, moderate-to-severe asthma, childhood-onset asthma, and adult-onset asthma (AOA) - on cerebral cortex structure. METHODS: We utilized phenotype data derived from genome-wide association studies (GWASs). The ENIGMA Consortium GWAS provided outcome variables for surface area (SA) and thickness across the whole brain and 34 region-specific areas of the cerebral cort

Semiparametric joint modeling for biomarker trajectory before disease onset.

Understanding how biomarkers change in relation to disease pathogenesis is a key area in biomedical research. We propose a semiparametric joint model to analyze the temporal evolution of biomarkers prior to the onset of disease. The model allows for a flexible biomarker trajectory that depends on two time scales: a natural time scale such as age and time to disease onset. In practice, the natural time scale often differs from time-on-study, leading to analytical challenges such as left-truncation bias. We introduce a profile kernel estimating equation approach to estimate regression coefficien
