Mapping of pikachurin heparan sulfate modification to a region dispensable for synaptic localization in photoreceptors.

Rod and cone photoreceptors (PRs) are specialized neurons that detect light, then transmit information to bipolar cells at synapses in the outer plexiform layer of the retina. Pikachurin (PIKA) is a multi-domain heparan sulfate (HS) proteoglycan expressed in PRs. It is secreted into the synaptic cleft, where it binds to presynaptic dystroglycan, postsynaptic orphan receptor GPR179, and postsynaptic adhesion molecule LRRTM4. Knockout of PIKA results in ultrastructural and functional defects at rod synapses. However, the role of HS in PIKA function at synapses is unknown. Using a panel of deleti

Optic nerve sheath decompression-related perioptic pseudocysts: histo-biochemical features.

Two patients (Female; 9 and 32 years in age) with papilledema secondary to venous sinus thrombosis underwent bilateral medial transconjunctival optic nerve sheath decompression for visual loss. Postoperative imaging revealed perioptic cystic lesions in 3 eyes associated with persistent papilledema and increased retinal nerve fiber layer thickness. For treatment, the cystic lesions were excised through a transconjunctival approach after aspiration of their fluid content. The cystic fluid, analyzed by gel electrophoresis, tested positive for beta-2 transferrin, confirming its cerebrospinal fluid

Prenatal tobacco exposure and visual disorders in children.

Maternal smoking during pregnancy is associated with adverse fetal and perinatal outcomes and may also affect visual system development in offspring. Increasing evidence suggests that prenatal tobacco exposure may also be associated with abnormalities of visual system development in children. However, the extent to which these associations reflect direct effects of tobacco exposure versus indirect pathways mediated by adverse perinatal outcomes remains uncertain. This narrative review summarizes the available evidence regarding the association between prenatal tobacco exposure and visual abnor

Streamlining Liver SABR: Validation and Initial Experience of Bulk Density Override Based Single-Session, Simulation-Free Adaptive Workflow.

PURPOSE: Conventional liver stereotactic ablative radiotherapy (SABR) typically requires a CT simulation visit followed by multiple treatment fractions, which can increase patient burden and limit access. We propose an approach that consolidates imaging, planning, adaptation, and delivery into one session, utilizing an MR-guided adaptive workflow. We report here the workflow and dosimetric validation of a streamlined bulk density override (BDO) strategy and the initial clinical implementation of this simulation-free approach. METHODS AND MATERIALS: A single-institution phase II trial was desi

Hydrogel-based delivery of solanesol enhances Treg-astrocyte crosstalk and reduces scar formation following spinal cord injury.

Spinal cord injury (SCI) is a severe neurological condition that often leads to permanent disability, with glial scar formation serving as a key obstacle to functional recovery. In this study, we use transcriptomic profiling and molecular analyses to delineate the pathological signaling cascade involved in SCI. The TGFβ-integrin αvβ3-PTK2-PTEN axis promotes A1 astrocyte polarization and CS56 deposition, thereby contributing to glial scar formation. Treatment with solanesol (SOL) promoted Treg cell infiltration and elevated CCN3 secretion, which, in turn, induced the "integrin switch" from αvβ3

Therapeutic extracellular vesicles isolated from mesenchymal stem cells: properties, isolation, characterization, and applications in optic neuropathies.

Extracellular vesicles (EVs) are promising cell-free therapeutic agents because of their low immunogenicity, excellent biocompatibility, and intrinsic capacity for intercellular communication. Of these, mesenchymal stem cell-derived EVs (MSC-EVs) have attracted considerable interest for their potential in treating optic neuropathies; however, their clinical translation remains limited by standardized manufacturing, quality evaluation, and mechanistic understanding. This review summarizes key advances in therapeutic EV development, including MSC culture, EV production, isolation, and characteri

Caveolin-1 defines astrocyte mechanobiology in the glial lamina of the optic nerve head.

The optic nerve head is the principal site of retinal ganglion cell axonal injury in primary open angle glaucoma, where astrocyte mechanobiology and extracellular matrix remodeling are thought to play critical roles in disease progression. Caveolae, which are mechanosensitive plasma membrane invaginations formed by Caveolin and Cavin proteins, are key regulators of cellular responses to mechanical stress, yet their involvement in optic nerve head pathophysiology has not been defined. Here, we characterize the presence, regulation, and mechanobiological relevance of Caveolin and Cavin proteins

Case Report: Clinical characteristics and management strategies of intraspinal tophaceous gout: a case series and literature review.

BACKGROUND: Intraspinal tophaceous gout, defined as monosodium urate crystal deposition within the effective spinal canal causing neural compression, is a rare subtype of spinal gout with no unified diagnostic standard. Despite documented gout history, it is frequently misdiagnosed preoperatively. METHODS: We retrospectively analyzed five male patients with pathologically confirmed intraspinal tophaceous gout (median age 25 years, range 21-48) admitted to two tertiary hospitals between January 2018 and May 2026, alongside a systematic literature review of strictly defined cases. RESULTS: The

Restoration of volitional lower-limb movement after motor-complete spinal cord injury with brain-spine interface.

BACKGROUND: Spinal cord injury (SCI) interrupts volitional control of lower limbs. Brain-spine interfaces (BSIs), which decode cortical motor intent to trigger epidural electrical stimulation (EES), can provide an artificial pathway linking the brain to spinal motor circuits. Reported benefit, however, has been confined to participants who already retain substantial motor function, leaving unresolved whether those with little or no motor function can benefit, how soon, and how consistently. METHODS: Three individuals with motor-complete SCI were enrolled in a registered trial (ChiCTR250009535

Checkpoint kinase 1 (Chk1) inhibition promotes motor function recovery through suppressing pyroptosis signalling and attenuating microglial polarization shift to M1 phenotype in spinal cord injury mice.

BACKGROUND AND PURPOSE: Microglial polarization shifts that induce inflammatory responses play a role in the occurrence and progression of spinal cord injury (SCI). We reported that checkpoint kinase 1 (Chk1) was involved in regulating SCI-induced motor dysfunction. We aim now to determine whether the microglial polarization shift is involved in the action of Chk1 on SCI-induced motor dysfunction. EXPERIMENTAL APPROACH: The pathological SCI in vivo model was induced in mice through spinal cord contusion injury, while the in vitro model was established by exposing cultured murine BV2 cells to

Endogenous stem cell activation and voluntary exercise differentially influence improvements in motor recovery after chronic spinal cord injury.

The ependymal cell layer lining the central canal of the spinal cord forms a key endogenous neural stem cell niche in adult mammals, and enhancing its activity may promote repair following spinal cord injury (SCI). We investigated the independent and combined effects of voluntary free wheel running and PNU-120596 (PNU), a positive allosteric modulator of α7 nicotinic acetylcholine receptors, on functional recovery and endogenous plasticity after chronic thoracic contusion SCI. Four weeks post-injury, female rats received PNU or vehicle, with or without access to running wheels, for six weeks.

Epidural Spinal Cord Stimulation Promotes the Expansion of Endogenous Neural Stem Cells and Neuroprotection via the PI3K/AKT/ mTOR Pathway after Spinal Cord Injury in Rats.

Spinal cord injury (SCI) often leads to severe motor and sensory dysfunction, yet no effective treatment currently exists. Epidural spinal cord stimulation (ESCS) is a treatment for chronic neuropathic pain in the trunk and extremities, and exerts a significant effect on the recovery of motor function, although the underlying mechanisms remain unclear. The present study aimed to investigate the effects of ESCS on the endogenous neural stem cells (eNSCs) proliferation and neuroprotection as well as the underlying mechanisms following SCI in rats. In the present study, results showed that ESCS i

Semantic distance metrics in anatomy education: Toward AI-assisted mapping of anatomy-to-clinical reasoning.

Anatomy education traditionally emphasizes structural knowledge acquisition, with the implicit assumption that clinical reasoning capabilities will naturally follow. However, the cognitive pathway from anatomical understanding to clinical application is rarely linear or uniform. This article proposes a conceptual framework in which semantic distance-the conceptual separation between anatomical knowledge and clinical endpoints-operates at three analytically distinct levels: (1) the learner's internal cognitive network, shaped by prior experience and schema formation; (2) the structural distance
