Hemorrhagic lumbar synovial cyst mimicking a neoplastic process: illustrative case.

BACKGROUND: Synovial cysts with extensive hemorrhage, marked mass effect, and tumor-like appearance are rare and may mimic neoplastic processes. OBSERVATIONS: A 56-year-old man presented with progressive mechanical low back pain, neurogenic claudication, gait instability, and bilateral weakness. MRI revealed a 2.0-cm lesion arising from the right L3 facet joint, causing severe central canal and right foraminal stenosis with displacement of the cauda equina. Imaging features suggested pigmented villonodular synovitis. Intraoperatively, however, the lesion appeared fleshy, partially encapsulate

Segmental Variation in Lumbar Nerve Root Weight: A Donor-Body Study.

Introduction The cauda equina consists of lumbosacral nerve roots extending from the conus medullaris within the lumbar cistern. Although radiographic imaging is essential for evaluating lumbar spinal pathology, normal anatomical variation in the cauda equina may affect its appearance. This donor-body study quantified normalized segmental linear mass per root across the L1-L2 through L4-L5 vertebral intervals and evaluated level-specific and demographic patterns. These measurements provide exploratory anatomical data for future imaging and clinical correlation studies. Methods Thirty-two donor

Leptomeningeal metastasis of lung adenocarcinoma presenting as cauda equina syndrome: a case report and literature review.

OBJECTIVE: This study aims to improve the understanding of the imaging characteristics of leptomeningeal metastasis, particularly in patients presenting with cauda equina syndrome as the initial manifestation, in order to improve diagnostic accuracy and reduce the misdiagnosis rate. METHODS: A 69-year-old male presented with a 4-month history of lower back pain accompanied by progressive lower limb numbness and gait instability. Non-contrast lumbar MRI performed at another hospital revealed punctate abnormal signal intensity in the cauda equina region, which was overlooked. The patient was tr

[Lumbar Spinal Disease or Peripheral Nerve Disorder? The Role of Neurological Examination in Differential Diagnosis].

Lower-limb numbness, pain, and weakness are common clinical complaints that require differentiation between cauda equina or radicular lesions associated with lumbar spinal disease and peripheral nerve disorders. Because degenerative findings on lumbar imaging are common, particularly among older adults, the responsible lesion should not be determined based on imaging findings alone. In this context, neurological examination contributes to anatomical localization and is central to the differential diagnosis. Particular emphasis is placed on interpreting muscle weakness and sensory disturbance a

Imaging of Neoplastic Disorders of the Spinal Cord: Part 2.

This article addresses uncommon primary spinal cord tumors, neoplasms of the conus medullaris, filum terminale, cauda equina region, and secondary neoplastic involvement of the spinal cord and CSF spaces. Although many primary intramedullary tumors discussed are individually rare, secondary neoplastic disease, including intramedullary metastases and leptomeningeal dissemination is encountered more frequently in clinical practice and represents a major cause of rapidly progressive neurological symptoms. Imaging appearances across these entities often overlap with common primary tumors and non-n

Acute Cauda Equina Syndrome in Rapid Succession: A Case Series of Eight Consecutive Patients Undergoing Emergency Surgical Decompression Over One Week.

INTRODUCTION: Cauda equina syndrome (CES) is a rare but devastating spinal surgical emergency, most commonly caused by lumbar disc herniation. Delayed diagnosis or decompression may result in irreversible neurological and sphincter dysfunction. MATERIALS AND METHODS: This retrospective single-Center case series included eight consecutive patients with acute CES who underwent emergency surgical decompression over 1 week at a tertiary spine care center. Clinical presentation, radiological findings, surgical management, and 6-month post-operative outcomes were evaluated. RESULTS: The cohort inc

Lumbar Epidural Steroid Injection

Lumbar epidural steroid injections (LESIs) are minimally invasive, image-guided procedures used to treat lumbosacral radicular pain by depositing corticosteroid, with or without local anesthetic, into the epidural space. Radicular pain results from inflammation, irritation, or compression of a lumbar or sacral nerve root. This pain commonly presents in the low back and radiates into the buttock or lower extremity in a dermatomal distribution. Lumbar disc herniation is the most common cause; degenerative lateral recess or foraminal stenosis, spondylolisthesis, postoperative epidural fibrosis, a

MRI FINDINGS IN CAUDA EQUINA NERVE ROOTS AND ASSOCIATED CLINICAL SIGNS IN DOGS WITH DEGENERATIVE LUMBOSACRAL STENOSIS.

Magnetic resonance imaging (MRI) is the most commonly used advanced imaging modality for diagnosing degenerative lumbosacral stenosis (DLSS). However, stenosis of the central canal and intervertebral foramina does not reliably correlate with clinical signs, and additional diagnostic imaging criteria would strengthen the validity of MRI. This study aimed to evaluate the size, signal characteristics, position (nerve root sedimentation sign, NRSS), and course (redundant nerve roots, RNR) of cauda equina nerve roots in dogs with and without DLSS using high-field MRI, and to assess associations bet

Re-evaluating prone-position MRI markers in occult tethered cord syndrome: a prospective observational study.

OBJECTIVE: Occult Tethered Cord Syndrome (OTCS) is defined as a clinical syndrome of cord tension with symptomatology similar to Tethered Cord Syndrome (TCS) but lacking typical imaging features, with the conus medullaris (CM) terminating at a normal level above the second lumbar vertebral body. A posteriorly displaced filum terminale (FT) on prone MRI, termed the "sunrise sign," has been proposed as a diagnostic marker for OTCS. This study aimed to characterize the position of the CM and FT and to determine the prevalence of the sunrise sign in asymptomatic adults. METHODS: In this prospecti

Aggressive lumbar vertebral hemangioma presenting with foot drop mimicking spinal tuberculosis: A case report.

BACKGROUND: Aggressive vertebral hemangiomas are uncommon vascular lesions that may present with epidural extension and neurological deficit. In tuberculosis-endemic regions, atypical vertebral hemangiomas may mimic spinal tuberculosis, resulting in diagnostic confusion and delay in definitive management. CASE DESCRIPTION: A 20-year-old male presented with a 1-year history of left lower-limb radiculopathy followed by progressive left foot drop for 1 month. The patient had previously received empirical antitubercular therapy without clinical improvement. Magnetic resonance imaging demonstrated

Pediatric Guillain-Barré Syndrome Post Organ Transplant: Acute and Chronic Complication Requiring High Index of Suspicion.

BACKGROUND: Guillain-Barré syndrome (GBS) is an acute, immune-mediated polyradiculoneuropathy most often triggered by infections such as Campylobacter jejuni, Epstein-Barr virus (EBV), or cytomegalovirus (CMV). Although transplant-associated GBS is recognized in adults, it remains rare in pediatric recipients, and distinguishing immune-mediated disease from medication toxicity or critical illness can be challenging. METHODS: We describe two pediatric cases of GBS following organ transplantation, one after liver transplant and one after small bowel transplant. Clinical features, diagnostic eva

Natural history of filum terminale lipoma and low-set conus medullaris.

OBJECTIVE: Filum terminale lipoma and low-set conus medullaris may cause tethered cord syndrome; however, the natural course of these lesions has not yet been fully elucidated. In this study, the authors investigated the natural history of filum terminale lipoma and low-set conus medullaris. METHODS: The authors retrospectively analyzed the data of patients who were diagnosed with filum terminale lipoma or low-set conus medullaris and followed without untethering surgery. Cases with a follow-up period of < 12 months were excluded. RESULTS: From 2010 to 2021, 1070 cases were diagnosed with fi

Spinal Subdural Abscess in the Emergency Department: A Rare Diagnosis in an Immunocompetent Host.

Spinal subdural abscess is an exceptionally rare and potentially life-threatening spinal infection characterized by the accumulation of purulent material between the dura and arachnoid mater. In contrast to the more common spinal epidural abscess, spinal subdural abscesses have been reported in fewer than 80 cases, often in pediatric, postoperative, or immunocompromised patients. This case represents the fourth documented instance of a spinal subdural abscess in an immunocompetent adult without identifiable risk factors or a known source of infection and the first reported in emergency medicin

Spinal granulocytic sarcoma with both extradural and intradural involvement: a case report.

INTRODUCTION: Granulocytic sarcoma (GS) is a rare extramedullary accumulation of myeloid blasts. Cases of GS with isolated intradural presentation sparing the underlying bone are extremely rare. CASE DESCRIPTION: A 75-year-old male patient with a remote history of acute myeloid leukemia (AML) in 30-year remission presented with progressive lumbar radiculopathy. Magnetic Resonance Imaging (MRI) revealed intradural lesions spanning L2-S1, mimicking nerve sheath tumors. The patient underwent emergency laminectomy and resection of a vascular, grayish-green L2 mass adherent to the cauda equina. Im

Malignant phyllodes tumour with spinal metastasis presenting as cauda equina syndrome.

Malignant phyllodes tumours (MPTs) are rare fibroepithelial breast neoplasms. While local recurrence is relatively common, distant metastasis is uncommon. Spinal metastases are exceptionally rare, with only a handful of cases in the literature. This case describes a female in her early 50s with a prior diagnosis of borderline phyllodes tumour status post right mastectomy and adjuvant radiotherapy. Two years post treatment, she presented with progressive lower back and bilateral leg pain, later identified as severe spinal canal compression causing cauda equina syndrome secondary to an L4 verteb

[Intraspinal extradural tumors].

CLINICAL ISSUE: Intraspinal extradural lesions represent the most frequent group of spinal tumors and comprise a broad spectrum of primary and secondary neoplasms as well as intradural extramedullary tumors with secondary extradural extension. Because even small epidural lesions can cause significant neurological deficits through compression of the spinal cord or cauda equina, a prompt and accurate diagnosis is essential. RADIOLOGICAL STANDARD PROCEDURES: Contrast-enhanced magnetic resonance imaging (MRI) is the imaging modality of first choice as it enables a reliable assessment of the tumor

Percutaneous Endoscopic Interlaminar Lumbar Discectomy for Treating Acute Cauda Equina Syndrome: A Case Series of 25 Patients.

BACKGROUND: Cauda equina syndrome (CES) is caused by compression of nerves in the lumbar spine below the conus medullaris. Symptoms include progressive weakness in the lower extremities, back pain with sciatica, saddle anesthesia, and urinary or bowel dysfunction. These symptoms vary in intensity and onset, and can sometimes be an emergency. The standard surgical procedure for CES typically involves a microscope-assisted interlaminar decompression with laminotomy and sequestrectomy, with or without partial, rather than complete, discectomy. However, endoscopic spine surgery was introduced as a
