Malignant phyllodes tumour with spinal metastasis presenting as cauda equina syndrome.

Malignant phyllodes tumours (MPTs) are rare fibroepithelial breast neoplasms. While local recurrence is relatively common, distant metastasis is uncommon. Spinal metastases are exceptionally rare, with only a handful of cases in the literature. This case describes a female in her early 50s with a prior diagnosis of borderline phyllodes tumour status post right mastectomy and adjuvant radiotherapy. Two years post treatment, she presented with progressive lower back and bilateral leg pain, later identified as severe spinal canal compression causing cauda equina syndrome secondary to an L4 verteb

[Intraspinal extradural tumors].

CLINICAL ISSUE: Intraspinal extradural lesions represent the most frequent group of spinal tumors and comprise a broad spectrum of primary and secondary neoplasms as well as intradural extramedullary tumors with secondary extradural extension. Because even small epidural lesions can cause significant neurological deficits through compression of the spinal cord or cauda equina, a prompt and accurate diagnosis is essential. RADIOLOGICAL STANDARD PROCEDURES: Contrast-enhanced magnetic resonance imaging (MRI) is the imaging modality of first choice as it enables a reliable assessment of the tumor

Percutaneous Endoscopic Interlaminar Lumbar Discectomy for Treating Acute Cauda Equina Syndrome: A Case Series of 25 Patients.

BACKGROUND: Cauda equina syndrome (CES) is caused by compression of nerves in the lumbar spine below the conus medullaris. Symptoms include progressive weakness in the lower extremities, back pain with sciatica, saddle anesthesia, and urinary or bowel dysfunction. These symptoms vary in intensity and onset, and can sometimes be an emergency. The standard surgical procedure for CES typically involves a microscope-assisted interlaminar decompression with laminotomy and sequestrectomy, with or without partial, rather than complete, discectomy. However, endoscopic spine surgery was introduced as a

A Scoping Review of Traumatic Cauda Equina Injury Outcomes.

OBJECTIVES: To identify which outcomes have been described following traumatic cauda equina injury (CEI). METHODS: A search strategy was developed and the following electronic databases were searched for this scoping review: MEDLINE, Embase, CINAHL, Scopus, Web of Science, Emcare, PsycInfo, and Google Scholar. Eligible articles were first title and abstract screened, and then full text was screened by 2 independent reviewers. The data were extracted and presented in a narrative format. RESULTS: Of the 3853 articles returned in the initial search, 44 studies were included in the final review.

Case Report: Successful treatment of mixed-phenotype acute leukemia with cauda equina syndrome as the initial manifestation.

BACKGROUND: Mixed-phenotype acute leukemia (MPAL) has a poor prognosis, and initial presentation with cauda equina syndrome (CES) is extremely rare. Allogeneic hematopoietic stem cell transplantation (allo-HSCT) may be the only potentially curative modality. To our knowledge, this is the first reported case of MPAL presenting with CES at disease onset. METHODS: We reviewed the clinical data of a 21-year-old man with high-risk myeloid/B-lymphoid MPAL presenting with CES as the initial manifestation. The patient initially presented with lumbosacral radicular pain and lower limb numbness, which

Clinical management of a Veteran with chronic scan-negative cauda equina syndrome: a case report.

OBJECTIVE: To describe the clinical presentation, diagnostic considerations, and safety-first course of chiropractic care for an adult male with chronic scan-negative cauda equina syndrome. CLINICAL FEATURES: A 41-year-old male U.S. Veteran sought chiropractic care for progressively intensifying chronic low back pain and bilateral lower extremity pain along with progressive bladder dysfunction, groin hypoesthesia, and sexual dysfunction. The history and examination findings raised concern for cauda equina syndrome, but a same-day MRI and neurosurgery consult failed to yield surgical indicatio

Case Report: Neurosyphilis with a paraparetic Guillain-Barré-spectrum phenotype and CSF-detected anti-GT1a IgG: an infection-autoimmunity diagnostic challenge.

BACKGROUND: Active central nervous system (CNS) infection can resemble acute immune polyradiculoneuropathy, and the two processes may coexist. We describe a patient with neurosyphilis, rapidly progressive paraparetic weakness, and anti-GT1a IgG detected in cerebrospinal fluid (CSF) but not serum by a qualitative line immunoblot. CASE PRESENTATION: A 41-year-old man developed rapidly progressive paraparesis, urinary retention, and left abducens palsy. On hospital day 2, CSF opening pressure was 260 mmH2O, protein 1.56 g/L, white-cell count 272 × 106/L, CSF TPPA was reactive, and CSF RPR was 1:

Immune Checkpoint Inhibitor-Induced Myelitis Presenting with "Hot Cord" Sign on 18F-FDG PET/CT: A Case Report.

INTRODUCTION: Immune Checkpoint Inhibitor (ICI)-induced myelitis is a rare but potentially severe neurological immune-related adverse event. Its diagnosis can be challenging because conventional Magnetic Resonance Imaging (MRI) findings may be subtle or limited. A distinctive case was reported of ICI-induced myelitis characterized by diffuse spinal cord hypermetabolism on 18F-FDG PET/CT, demonstrating a "hot cord" sign and highlighting the potential value of metabolic imaging in neurological immune-related adverse events. CASE PRESENTATION: A 55-year-old man with clear cell renal cell carcino

Progressive Pseudorheumatoid Dysplasia Presenting with Cauda Equina Compression: A Case Report.

PURPOSE: Fresh vegetables are essential human dietary components but are increasingly implicated in foodborne infections caused by Escherichia coli. Contamination may occur during cultivation, irrigation, handling, and retail display, and the presence of pathogenic or antimicrobial-resistant E. coli poses significant public health risks. Despite widespread vegetable consumption in Ogun State, Nigeria, limited data exist on the resistance and virulence characteristics of E. coli in these foods. This study therefore assessed antimicrobial resistance profiles and virulence genes in E. coli isolat

Full-endoscopic Laminotomy Is Effective for Lumbar Spinal Stenosis with Low-grade Spondylolisthesis: A Comparative Cohort Study.

The optimal management of lumbar spinal canal stenosis with spondylolisthesremains controversial, particularly when choosing between decompression alone or decompression with fusion. Current evidence is based on conventional open or endoscopy-assisted surgeries, with limited data on full-endoscopic decompression. This study aimed to assess the impact of spondylolisthesis on outcomes after full-endoscopic laminotomy by evaluating back pain-related disability scores in patients with lumbar spinal canal stenosis, with and without spondylolisthesis. A retrospective analysis was conducted at Iwai O

Neurogenic priapism in spinal cord injury and degenerative spinal pathology: a narrative review.

INTRODUCTION: Neurogenic priapism is a rare but clinically significant manifestation of disrupted spinal autonomic control of erectile physiology. While most described following acute spinal cord injury (SCI), priapism has also been reported in association with chronic degenerative spinal pathology, suggesting distinct underlying mechanisms. AIMS: To synthesize reported cases of neurogenic priapism associated with spinal pathology and clarify how lesion level, injury chronicity, and autonomic pathway involvement influence priapism phenotype. METHODS: A narrative review of the literature was

Artificial intelligence (AI)-based multi-organ contour quality assurance with uncertainty estimation for online adaptive radiotherapy (oART).

Accurate delineation of treatment targets and organs at risk (OARs) is essential to the success of radiotherapy (RT). Although artificial intelligence (AI)-based segmentation methods have successfully automated the delineation process, a reliable and efficient quality assurance (QA) mechanism is still missing, particularly in the time-critical setting of online adaptive RT (oART). This study aims to address this unmet clinical need by introducing an AI-driven multi-organ contour QA framework with uncertainty quantification. Using MR-guided oART for prostate cancer as the testbed, we developed

[Schwannomatosis diagnosed from an approximately 30-year history of multiple mononeuropathy accompanied by multiple cauda equina nodules: a case report].

A 51-year-old woman presented with left drop foot at age 20, numbness in the right fourth and fifth fingers at age 45, and numbness in both lower extremities at age 51. Lumbar MRI revealed multiple cauda equina nodules, while ultrasonography of the peripheral nerve identified enlarged nerve bundles in several locations, including the right ulnar and left peroneal nerves. Nerve conduction studies and upper extremity MRI findings were consistent with these observations, and head MRI confirmed the absence of auditory schwannomas. Schwannomatosis (SWN) is a rare disease characterized by the format

Persistent genital arousal disorder/Genito-pelvic dysesthesia in all genders: recommendations from the Fifth International Consultation on Sexual Medicine (ICSM 2024).

INTRODUCTION: Persistent genital arousal disorder/genito-pelvic dysesthesia (PGAD/GPD), a sensory hyperactivity condition, involves a broad range of undesired, unrelenting dysesthesia(s), including genital arousal, hard flaccid syndrome, and sleep-related prolonged erection. PGAD/GPD is associated with significant, negative psychosocial consequences, including suicidal ideation. In 2021, the International Society for the Study of Women's Sexual Health developed a process of care for biopsychosocial management of PGAD/GPD in women. This article reviews the management of PGAD/GPD in all genders.

Neurapraxia in Time and Space.

Since its introduction into medical terminology by Lord Henry Cohen in 1941, the term "neurapraxia" has become established in the neurological and neurosurgical lexicon, although it is often misused or misspelled. It denotes a transient dysfunction of a peripheral nerve, as distinct from axonotmesis and neurotmesis, which describe more significant structural injuries to a peripheral nerve. To expand our understanding of neurapraxia, it is necessary to examine the phenomenon in both temporal and spatial contexts. This study examines neurapraxia in: (A) Time-(1) the origins of the term neuraprax
