Case Report: Filum terminale extradural arachnoid diverticulum associated with tethered cord syndrome in a French Bulldog.

Extradural arachnoid diverticula and tethered cord syndrome are rare disorders in dogs, and their simultaneous occurrence with a possible mechanistic association has not been previously reported. A 2-year-old male intact French Bulldog was evaluated for chronic lumbar pain, presumed secondary aggression, and subtle pelvic limb neurological deficits. Magnetic resonance imaging (MRI) revealed a well-defined, extradural, CSF-isointense, cyst-like lesion located at the origin of the filum terminale externum and dural sac termination. An additional intradural, elongated fluid-filled lesion position

Diffuse Large B-Cell Lymphoma Arising from Cauda Equina: A Rare Case Report and Review of the Literature.

Background: Malignant lymphoma is the most common hematological malignancy; however, primary central nervous system lymphoma accounts for only a small percentage of non-Hodgkin lymphoma (NHL). Among these, primary cauda equina lymphoma (CEL) is extremely uncommon. Its rarity and atypical clinical presentation often make diagnosis challenging. Case Presentation: An 80-year-old man presented with progressive gait disturbance, lower-extremity weakness, and numbness. MRI revealed diffuse swelling and homogeneous gadolinium enhancement of the cauda equina at T12-L1; additionally, CSF cytology ident

Impact of Surgical Timing on Neurological Outcomes in Acute Lumbar Disc Herniation: Ultra-Early Versus Delayed Microdiscectomy.

OBJECTIVE: To compare neurological and functional outcomes after ultra-early microdiscectomy (≤12 h) versus later surgery (12-24 h, 24-48 h, >48 h) in adults with acute lumbar disc herniation presenting with motor weakness and/or cauda equina syndrome. METHODS: We retrospectively studied 300 consecutive adults undergoing emergency microdiscectomy (2015-2025) at a tertiary center. Patients were grouped by time from presentation to surgery (≤12 h, 12-24 h, 24-48 h, >48 h). Primary outcomes were motor recovery (Medical Research Council scale) and pain improvement; secondary outcomes were complic

Gadolinium enhancement of the cauda equina in a case of familial ALS with p.S135G SOD1 mutation.

Amyotrophic lateral sclerosis (ALS) is a progressive neurodegenerative disorder characterized by motor neuron degeneration. Gadolinium enhancement of the cauda equina is typically associated with inflammatory diseases. We report a case of familial ALS with a Cu/Zn superoxide dismutase (SOD1) gene mutation showing marked gadolinium enhancement of the lumbar nerve roots. To date, only a few cases of ALS with gadolinium enhancement of the nerve roots have been reported. To our knowledge, this is the first reported case of ALS with an p.S135G SOD1 mutation exhibiting gadolinium enhancement in the

A case report: Guillain-Barré syndrome probably associated with TNF inhibitor in Blau syndrome.

BACKGROUND: Blau syndrome (BS) is an early-onset autoinflammatory disease, typically characterized by a clinical triad of granulomatous polyarthritis, uveitis and skin lesion. Tumor necrosis factor (TNF) antagonists (TNF inhibitor, TNFi) are used routinely according to treatment guidelines for several refractory BS therapy. However, peripheral nervous system demyelination related to TNFi is being increasingly recognized. Here, we report a case of Guillain-Barré syndrome probably secondary to an TNFi, Yisaipu, one kind of etanercept biosimilars, which is a rare and usually unpredictable adverse

Luton cauda equina syndrome pathway: a pragmatic approach to achieve a national target of time to scan in UK district general hospitals.

AIMS: Cauda equina syndrome (CES) is a rare neurosurgical emergency where diagnosis and treatment by emergency spinal decompression is time critical. A national CES pathway from Getting It Right First Time (GIRFT) suggested development of 24/7 MRI services at district general hospitals (DGHs). It gave a national target of time to scan time between MRI scan request and actual scan of less than four hours. The Luton CES pathway was introduced in 2021 at Luton and Dunstable University Hospital, UK, to improve the time (in hours) to a MRI service, as well as to streamline patient care for suspecte

A Prospective Clinical and Radiological Study of Symptomatic Upper Lumbar Disc Herniation in the Indian Population.

Introduction The upper lumbar spine is anatomically predisposed to neural compression due to a relatively narrow spinal canal, the presence of the conus medullaris and proximal cauda equina, and reduced segmental mobility. This distinct anatomy can lead to a wide range of clinical presentations, from nonspecific polyradiculopathy to cauda equina syndrome. The variability in symptoms and the unique anatomical features often complicate surgical decision-making. The primary objective of this study was to evaluate the association between upper lumbar disc level (L1-L2, L2-L3, L3-L4) and neurologic

Brachial plexopathy in juvenile-onset Krabbe disease: A rare case.

Krabbe disease is an autosomal recessive leukodystrophy where a deficiency of the galactosylceramide beta-hydrolase enzyme leads to accumulation of toxic substances which cause demyelination in both central and peripheral nervous systems. It is classified into early infantile, late infantile, juvenile and adult types based on the age of presentation. MRI of the brain in patients with Krabbe disease shows bilaterally symmetrical T2 hyperintense signal in the periventricular and deep white matter and in the corticospinal tracts. Patients can have peripheral neuropathy, which, on imaging, is comm

The Suspected Cauda Equina Syndrome Score (SuCESS) : development and validation of a clinical triage tool.

AIMS: Cauda equina syndrome (CES) is a rare but critical spinal emergency. However, over 80% of urgent MRIs for suspected CES show no cauda equina compression. This diagnostic uncertainty places strain on imaging services and delays care. We aimed to develop and validate a clinical triage tool, the Suspected Cauda Equina Syndrome Score (SuCESS), to safely exclude CES and improve the accuracy of triage. METHODS: We retrospectively developed the SuCESS score using the records of 259 patients who had been referred to a UK tertiary spinal unit between 2016 and 2018. Six clinical variables - saddl

Clinical Insights and Statistical Analysis of Failed Back Surgery Syndrome: A Multicentric Retrospective Review.

INTRODUCTION: Failed back surgery syndrome (FBSS) is a well-documented yet underinvestigated phenomenon, with prevalence estimates ranging from 10 to 40% among all spine surgeries. FBSS contributes significantly to chronic pain and disability, leading to socioeconomic burdens. Studies have emphasized the multifactorial etiology of FBSS, including inadequate decompression, misdiagnosis, iatrogenic injury, and psychosocial contributors. However, there is a scarcity of large-scale, real-world clinical datasets offering multidimensional analysis of FBSS. MATERIALS AND METHODS: This retrospective

Delayed cauda equina compression caused by BioGlue®-induced granulomatous reaction following spinal dural repair: A case report.

BACKGROUND: Adjunctive dural sealants such as BioGlue® are widely used to reinforce dural closure in cranial and spinal surgery. While generally effective, BioGlue® has been associated with inflammatory foreign-body reactions that may lead to delayed mass formation and neural compression. These delayed complications can mimic the original pathology. Here, a 66-year-old male underwent primary dural closure that included the use of BioGlue, which, 6 months later, caused the delayed onset of a cauda equina syndrome. CASE DESCRIPTION: A 66-year-old male presented with sciatica secondary to focal

Cauda equina syndrome caused by intradural migration of a bullet: A rare case presentation.

Intradural migration of a bullet within the spinal canal represents an exceptionally uncommon clinical scenario and carries the potential for progressive or delayed neurological decline. Although such fragments may initially remain silent, their subsequent movement within the spinal canal can create significant diagnostic and therapeutic challenges. In this report, we present an unusual and instructive case of cauda equina syndrome that developed secondary to the delayed caudal migration of an intradural bullet fragment. We further provide a detailed discussion of the diagnostic evaluation, su

Effect of standardized reporting template on turnaround times of on-call radiology residents' preliminary interpretation of spinal cord or cauda equina compression.

PURPOSE: Prompt and accurate diagnosis of spinal cord compression (SCC) or cauda equina compression (CEC) is of great clinical importance. The purpose of this study is to evaluate the effect of a standardized reporting template (SRT) on report turnaround time (RTAT) and performance of on-call radiology residents’ preliminary interpretation (PI) for the detection of SCC/CEC on after-hour spine MRI studies. METHODS: We retrospectively reviewed a total of 266 spine MRI studies performed after hours for SCC/CEC, before and after the implementation of a SRT to be used by on-call radiology residents

Transforaminal endoscopic discectomy with temporary percutaneous pedicle screw fixation as a fusion-sparing approach for giant recurrent L4/5 disc herniation and iatrogenic pars fracture in a young female: a case report.

BACKGROUND: Reoperation after lumbar discectomy for recurrent disc herniation can be technically challenging due to surgical scarring and altered anatomy, especially if postoperative complications are present. Iatrogenic pars fractures are a rare but significant post-operative complication that can complicate management with potential to cause persistent back pain, radiculopathy, and progression to instability, particularly in young active patients if not treated appropriately. CASE DESCRIPTION: The authors present the case of a 37-year-old female who presented with cauda equina syndrome foll

Rapid spontaneous migration and resolution of an acute subdural hematoma: A case report and comprehensive literature review.

This case report details a 32-year-old male with a known history of seizures who presented with an acute subdural hematoma (SDH) after a low-energy, low-velocity head injury, giving the impression of a relatively minor trauma mechanism. Although his initial Glasgow Coma Scale (GCS) score was 7, the depressed level of consciousness was considered disproportionate to the imaging findings and potentially related to a postictal state. Interestingly, the hematoma, after initially affecting the cranial region, later migrated to the lumbar spine, precipitating cauda equina syndrome. Remarkably, throu
