Contrasting Shamblin classification with tumor volume in predicting morbidity and mortality in carotid body tumor resection.

BACKGROUND: The Shamblin classification is the standard for predicting carotid body tumor (CBT) surgical outcomes; however, its subjective nature limits prognostic accuracy. This study compared the predictive value of preoperative tumor volume vs Shamblin classification for perioperative morbidity and mortality in CBT resection at a tertiary center. METHODS: A single-center retrospective cohort study analyzed data from 24 consecutive CBT resections performed at an Academic Hospital (January 2015-2025). Tumor volumes were calculated from preoperative contrast-enhanced computed tomography using

Targeting pulmonary and arterial chemoreceptors via inhaled therapeutics: A novel pathway to blunt hypoxia-induced pulmonary hypertension?

BACKGROUND: Group-3 hypoxia-induced pulmonary hypertension (HPH) associated with obstructive sleep apnea (OSA) and high-altitude exposure is characterized by a maladaptive chemoreflex phenotype with hyper-reactive intrapulmonary and arterial oxygen sensors. At present, non-selective systemic vasodilator therapy often cannot reverse vascular remodeling and carries the risk for systemic hypotension and worsening of ventilation-perfusion mismatch. OBJECTIVE: To suggest a conceptual therapeutic approach to transfer HPH treatment from nonspecific vasodilation to precision-targeted inhaled chemosen

The carotid body role in oxygen sensing: Anatomy, neurovascular organization, and structural bases of chemotransduction.

BACKGROUND: The carotid body is a small, highly vascularized organ located at the bifurcation of the common carotid artery and serves as the primary peripheral detector of arterial oxygen tension in mammals. Although its physiological role in ventilatory and autonomic reflexes has been extensively investigated, these functions are fundamentally dependent on its specialized anatomical organization. Objective: This review provides a comprehensive overview of the anatomical organization of the carotid body, highlighting how its gross morphology, microvascular architecture, cellular composition, a

Cell-based regenerative interventions in Parkinson's disease: Overcoming pharmacological limitations.

Parkinson's disease (PD) is a progressive neurodegenerative disorder characterized by the loss of dopaminergic (DA) neurons and the pathological aggregation of α-synuclein. While current pharmacological therapies provide symptomatic relief, they do not halt or reverse disease progression. Cell-based regenerative strategies have emerged as promising approaches to restore DA function and target the complex, multifactorial pathophysiology of PD. This review critically examines current approaches, including transplantation of fetal ventral mesencephalic tissue, pluripotent stem cell-derived midbra

Influence of low-dose dopamine on exercise in fibrosing interstitial lung disease.

Fibrotic interstitial lung disease (FILD) is associated with dyspnoea and exercise intolerance. In other cardiorespiratory conditions, heightened carotid body (CB) chemoreflex sensitivity is associated with reduced exercise capacity. We tested the hypothesis that CB chemoreflex inhibition would improve exercise endurance time (EET) and reduce dyspnoea in FILD. In this randomised, single-blind study, 10 FILD patients (four women, 64 ± 8 years) completed the experimental procedures. Low-dose dopamine infusion (2 µg/kg/min) was used to inhibit the CB chemoreflex, and its effectiveness verified by

Molecular changes in hypoxia-induced central neural circuits and nuclei.

FactsHypoxia activates a multi-level central regulatory network involving the carotid body and multiple brainstem nuclei that coordinate autonomic and cardiopulmonary responses.Acute hypoxia induces rapid compensatory responses, while chronic hypoxia causes persistent central remodeling including neuronal injury and pathological hypertension.Understanding molecular changes and neural circuit mechanisms provides a framework for treating hypoxia-related clinical diseases.Open QuestionsThe dynamic interactions between different central nuclei and their coordination mechanisms during hypoxic respo

Course of arterial hypertension after open and endovascular procedures for significant carotid artery stenosis: single-center randomized study.

BACKGROUND: Arterial hypertension is present in most patients with ischemic stroke and is frequently associated with significant internal carotid artery (ICA) stenosis. Carotid endarterectomy (CEA) and carotid artery stenting (CAS) are the primary techniques for carotid revascularization; however, their long-term effects on blood pressure control remain debated, particularly given the role of the carotid body in regulating sympathetic activity. Objective - to assess how the chosen method of carotid reconstruction influences the long-term course of arterial hypertension. METHODS: This single-c

Surgical Evolution and Long-Term Control in Carotid Body Tumors. An Institutional Experience over Two Decades.

BACKGROUND: Carotid body tumors (CBTs) are rare neoplasms for which universally accepted diagnostic and management guidelines are lacking, complicating the development of standardized treatment pathways. We present a 22-year single-center experience that longitudinally evaluates operative strategy, perioperative risk mitigation, and long-term outcomes, and we describe an institutional care bundle derived from this experience. METHODS: We conducted a retrospective cohort study with prospective follow-up of all patients who underwent open resection for CBT at a tertiary vascular surgery unit be

The Role of Carotid Graft Interposition During Elective and Emergent Interventions: Protocol for a Scoping Review.

BACKGROUND: In a significant proportion of carotid interventions, carotid graft replacement is required to achieve a successful outcome either as a primary method or as a bailout solution. Whether carotid grafting should be performed, the indications for its use, and other technical aspects remain insufficiently addressed in the literature. An exhaustive mapping of the sparse and heterogeneous evidence available in the literature may provide a more comprehensive understanding of this topic. OBJECTIVE: This scoping review aims to examine and summarize the scientific evidence on the indications

Oxygen extremes, inflammation, and neonatal breathing.

Infants born prematurely often develop apnea of prematurity (AOP), characterized by periodic apneas with intermittent hypoxia (IH) and commonly treated with supplemental oxygen. On the other hand, infants born at high altitude experience sustained hypoxia from birth. Thus, during critical developmental period, many infants experience abnormal oxygen environments, including hypoxia or hyperoxia. Emerging evidence indicates that disrupted neonatal oxygen homeostasis can produce long-lasting effects on cardiorespiratory function. Preterm infants also exhibit systemic inflammation, and elevated in

Pathophysiology of silent hypoxia in COVID-19: Truth and mystery.

Coronavirus disease 2019 (COVID-19) is a highly infectious viral disease caused by the SARS-CoV-2. COVID-19 commonly leads to mild flu-like illness in most of cases. However, severe COVID-19-induced pneumonia and associated acute lung injury (ALI) and acute respiratory distress syndrome (ARDS) may lead to ventilation-perfusion mismatch and the development of silent hypoxia. Silent hypoxemia is observed in critically COVID-19 patients without signs of subjective dyspnea that is often linked with poor clinical outcomes. Proposed mechanisms include impairment of peripheral carotid chemoreceptors,

Targeting the Carotid Body Function With Big-K+-Channel Blocker ENA-001 in Opioid-Induced Respiratory Depression: A Scoping Review.

Opioids, most intravenous anesthetics and various illicit substances can cause fatal respiratory depression by depressing central respiratory networks. ENA-001, a ventilatory modulator targeting the carotid bodies, has emerged as a potential countermeasure without impairing analgesia or hypnosis. We conducted a literature review to evaluate the efficacy and safety of ENA-001. This scoping review summarizes the current evidence base for ENA-001 from in vitro, animal experiments, and human volunteer studies. A comprehensive search was conducted across several electronic databases to identify all

Treatment Outcomes of Radiation Therapy of Paragangliomas of the Head and Neck at a Single Institution.

BACKGROUND/OBJECTIVES: Stereotactic radiosurgery and photon therapy for the locoregional management of head and neck paragangliomas (HNPGLs) have been shown to provide long-term tumor control and symptomatic relief. We report proton therapy (PT) outcomes for HNPGLs at a single institution and describe stereotactic radiosurgery (SRS) and intensity-modulated radiation therapy (IMRT) outcomes as institutional context for modality selection. The primary goal is to report outcomes including tumor control, toxicity, and overall recovery, while evaluating differences across radiation modalities. MAT

Carotid body dysfunction contributes to respiratory instability in Rett syndrome.

Rett syndrome (RTT) is a neurodevelopmental disorder caused by mutations in MECP2. Disordered breathing is a hallmark feature of RTT that negatively impacts quality of life and can be life-threatening. Despite this, the mechanisms underlying disordered breathing in RTT are poorly understood. Although RTT is primarily considered a central nervous system disorder, we found that increased drive from peripheral respiratory chemoreceptors may contribute to unstable breathing in MeCP2-deficient mice. Consistent with this, breathing pure oxygen to limit the output of peripheral chemoreceptors stabili

The non-selective cation channels TRPC3 and TRPC6 in the human carotid body in the elderly.

BACKGROUND: The carotid body is important in cardiovascular (patho)physiology as well as Canonical Transient Receptor Potential channel 3 and 6 (TRPC3, TRPC6). However, TRPC3 and TRPC6 were found in the rodent carotid body, whereas their distribution in the human carotid body has yet to be described. Thus, the present exploratory study aims to describe the protein distribution of TRPC3 and TRPC6 in the elderly. METHODS: The carotid body of six body donors (median age 86.5 years; 1 male, 5 female) was assessed. Hematoxylin-eosin and Movat-Pentachrome staining were applied. TRPC3 and TRPC6 were

Major advances in carotid body research: Then and Now.

The carotid body (CB) was anatomically described in the 18th century, and by the early 20th century, it was established as the principal peripheral chemoreceptor that senses arterial blood PO₂, PCO₂, and [H⁺], thereby mediating cardiorespiratory reflex responses. However, the absence of modern electrophysiological and molecular techniques limited the mechanistic insight into stimulus transduction. For decades, the research was dominated by unresolved debates over the mechanisms underlying O₂ and CO₂-H⁺ sensing and the role of putative excitatory transmitters such as dopamine, acetylcholine, an
