Graded multisensory neurofeedback via the iNems system for comprehensive upper limb recovery after stroke: a multiple baseline study.

INTRODUCTION: Post-stroke upper-limb recovery remains challenging, especially in the presence of sensory impairments. Conventional brain-computer interfaces (BCIs) are constrained by binary feedback, single-modality stimulation, and exclusion of patients with sensory deficits. We developed the Imagery Neurofeedback-based Multi-sensory System (iNems), a neurofeedback system that provides graded multisensory (visual and electrical) feedback based on ERD magnitude. This study validated iNems' efficacy in motor function, self-reported numbness, and cognitive functions using a multiple-baseline des

Total wrist fusions alter scapular kinematics during functional task performance.

Total wrist fusion is a surgical procedure used to manage pain in late-stage arthritis, though it results in a complete loss of wrist mobility. This loss can lead to compensatory movement patterns in other upper limb joints, which may increase the risk of secondary musculoskeletal disorders. This study investigated scapular kinematics in wrist fusion patients compared to healthy controls. Eighteen participants (nine wrist fusion patients, nine age- and sex-matched controls) were recruited. Scapular motion was tracked using motion capture during seven functional tasks. Bilateral scapular kinema

Visualization of Collateral Lymphatic Pathways After Axillary Node Dissection in Rats Using Real-Time Fluorescent Lymphography.

BACKGROUND: Manual lymph drainage (MLD) is commonly used to alleviate upper limb lymphedema; however, objective evidence regarding its physiological efficacy remains limited. METHODS: This study aimed to visualize postoperative changes in lymphatic flow and to identify potential collateral drainage pathways following axillary lymph node (LN) dissection using real-time fluorescent lymphography in a rat model. Twelve Wistar rats underwent surgical excision of the right axillary LNs with careful ligation of lymphatic vessels. Indocyanine green (ICG) lymphography was performed at multiple time po

Stenosis of a novel frozen elephant trunk in a patient with chronic dissection.

A partial elephant trunk (PET), a new type of frozen elephant trunk, was designed to prevent distal stent graft-induced new entry using a thinner nitinol wire with less radial force. We report a case of PET stenosis in chronic dissection. Total arch replacement was performed using a PET. After weaning from cardiopulmonary bypass, the lower-limb blood pressure decreased to 90% of the upper-limb blood pressure. Intraoperative transesophageal echocardiography revealed incomplete PET expansion, and on postoperative day 1, the lower-to-upper body blood pressure ratio further decreased to 70% due to

Clinical phenotypes and outcomes of spinal cord stimulation in painful peripheral polyneuropathy: a cohort including diabetic, chemotherapy-induced, and idiopathic etiologies.

BACKGROUND AND OBJECTIVE: Chronic painful peripheral polyneuropathy (PPN) is a leading global cause of chronic pain and may arise from a wide range of etiologies. When pharmacological treatments fail to provide adequate pain relief, spinal cord stimulation (SCS) has demonstrated efficacy in selected subtypes. This study investigated the effectiveness of SCS in alleviating pain, neurological symptoms and signs, and other health-related outcomes in patients with diverse forms of PPN, including diabetic (DPN) and chemotherapy-induced (CIPN) peripheral neuropathies, and chronic idiopathic axonal p

Accelerated intermittent theta burst stimulation (iTBS) for the upper limb recovery after subacute stroke (ATLAS): study protocol for a multicenter randomized controlled trial.

BACKGROUND: Stroke is the second leading cause of death globally, with residual upper limb dysfunction being a major contributor to its high disability rate. Currently, there remains a lack of effective rehabilitation approaches to substantially improve upper limb dysfunction. While transcranial magnetic stimulation (TMS) has shown potential in promoting functional reorganization in stroke patients, more effective treatment protocols are still under exploration. In recent years, numerous clinical evidence has indicated that intermittent theta burst stimulation (iTBS) may offer a more efficient

A Retrospective Cohort Study of Distal Biceps Brachii Tendon Rupture and Subsequent Repair Methods: A Comparison of Two Combined Treatment Pathways.

Background Distal biceps brachii tendon rupture is an uncommon injury that is predominantly caused by excessive eccentric force to a flexed elbow. Operative repair is generally preferred to restore function, with a variety of surgical fixation techniques and postoperative rehabilitation protocols currently employed but with no clear consensus regarding the optimal approach. Comparative evidence evaluating surgical techniques remains limited. Objective The objective of this study is to compare two combined treatment pathways for distal biceps tendon repair using postoperative complications such

Childhood-onset parkinsonism, dystonia and severe muscle wasting in a Sudanese girl caused by a novel homozygous DnaJ heat shock protein family (heat shock protein 40) member C6 frameshift variant, with an incidental pathogenic breast cancer 1 gene secondary finding: a case report.

BACKGROUND: DNAJC6-related juvenile parkinsonism is a rare autosomal recessive disorder of auxilin, a co-chaperone essential for clathrin-mediated synaptic vesicle recycling. Fewer than 50 kindreds have been reported worldwide, and the condition is easily missed when first-line investigations come back normal. We describe a Sudanese adolescent in whom a previously unreported homozygous DNAJC6 frameshift variant was identified only after a prolonged diagnostic journey that included a period during which her illness was mistakenly attributed to a functional neurological disorder. Clinical whole-
