Venous thoracic outlet syndrome in the presence of cervical rib: report of two cases.

Thoracic outlet syndrome is a rare condition caused by the compression of neurovascular structures in the region between the neck and chest. The venous form accounts for 2% to 3% of cases, with deep vein thrombosis most often caused by compression of the subclavian-axillary vein segment between the first rib and the clavicle. The occurrence of a cervical rib as a cause of venous deep vein thrombosis is even rarer. In this report, we present the diagnosis, treatment, and outcome of 2 cases of venous deep vein thrombosis resulting from 2 different mechanisms. The approach was based on a review o

Upper-limb motor-cognitive dual-task performance and incident disabling dementia among older adults.

BACKGROUND: Upper-limb motor-cognitive dual-task performance may help identify older adults at elevated risk of dementia; however, longitudinal evidence remains limited. In this prospective study, we examined associations of Digital Trail Making Peg Test Part A (DTMP-A) and Part B (DTMP-B) completion times with incident disabling dementia. Corresponding associations were examined for related motor- and cognitive-oriented measures. METHODS: This community-based longitudinal study included 274 Japanese adults aged 65 years or older from the Kasama Study who completed the DTMP assessment in 2020

Pituitary Apoplexy Presenting as a Transient Ischemic Attack: An Endocrine Emergency.

Pituitary apoplexy (PA) is an endocrine emergency caused by hemorrhage or infarction of a pituitary adenoma and occurs more frequently in males and patients with macroadenomas. It typically presents acutely with headache, visual disturbances, or impaired consciousness. Focal neurological deficits are uncommon and may be mistaken for a primary cerebrovascular event. Acute secondary adrenal insufficiency is a major contributor to hemodynamic instability and mortality. A 67-year-old man with arterial hypertension, type 2 diabetes mellitus, dyslipidemia, and chronic obstructive pulmonary disease p

The effects of TMS stimulus direction on corticospinal and cortico-cortical excitability of the lower limb in healthy adults.

Transcranial magnetic stimulation (TMS) studies in upper limb muscles have shown that stimulus direction can influence corticospinal and intracortical excitability; however, limited evidence exists for lower limb muscles. This study investigated the effect of TMS stimulus direction on corticospinal and intracortical excitability of the tibialis anterior (TA) muscle using paired-pulse TMS. Nineteen healthy adults (10 males, 9 females; mean age 41.8 ± 23 years) participated. Single- and paired-pulse TMS was delivered using a figure-of-eight coil oriented to induce anterior-to-posterior (AP) or p

Postoperative Changes in Line-Drawing Trajectories After Vim Thalamotomy for Essential Tremor: An Exploratory Analysis of Functional Correlates.

INTRODUCTION: Ventral intermediate nucleus (Vim) thalamotomy effectively suppresses essential tremor (ET); however, improvements in tremor severity do not necessarily translate into enhanced upper-limb dexterity or quality of life (QOL). In this study, we aimed to characterize postoperative changes in line-drawing trajectories and explore their functional correlates following Vim thalamotomy. METHODS: Thirty patients with ET who underwent unilateral Vim thalamotomy and 13 healthy controls (HC) were included. The ET group was assessed preoperatively and at 1 week and 1 month postoperatively us

Seven-day Accelerometer-derived Wrist Activity after Acute Stroke Stratified by Hemiparesis Severity: A Prospective Observational Pilot Study.

OBJECTIVES: This hypothesis-driven prospective pilot study characterized 7-day trajectories of bilateral wrist activity after acute stroke by baseline Fugl-Meyer Assessment-Upper Extremity (FMA-UE) severity. We hypothesized that mild impairment would be associated with greater paretic-side activity and more symmetric activity, whereas bilateral balance would change little despite clinical improvement. METHODS: Patients who began rehabilitation within 3 days after stroke wore bilateral wrist accelerometers for 7 days. Exploratory strata were mild (FMA-UE >45) and severe (≤45), based on an infl

Case Report: McLeod syndrome caused by a nonsense variant c.484C > T (p.Gln162Ter) in the XK gene in a Chinese Han family.

INTRODUCTION: McLeod syndrome (MLS) is a rare X-linked neuroacanthocytosis caused by pathogenic variants in XK, and is characterized by hyperkinetic movements, peripheral neuropathy and myopathy with hyperCKemia, acanthocytosis and cardiomyopathy. Here, we report a mainland Chinese Han family with McLeod syndrome carrying the XK variant c.484C > T, which resulted in a premature stop codon at position 162 (p.Gln162Ter), which, to our knowledge, has not been reported in association with McLeod syndrome in Asian populations; notably, the variant is not listed in the ISBT online Kx allele table at

Pharmacological targeting of FKBPL with rapamycin sensitizes triple-negative breast cancer to ferroptosis.

Triple-negative breast cancer (TNBC) remains a clinically formidable subtype, while resistant to conventional apoptotic cell death pathways, it exhibits a unique and exploitable vulnerability to ferroptosis. Here, we identify the FK506-binding protein-like (FKBPL) as a novel scaffold that dictates ferroptosis sensitivity and promotes TNBC progression. Bioinformatic and clinical analyses revealed that FKBPL was significantly overexpressed in TNBC and correlated with inferior patient prognosis. Functional characterization demonstrated that FKBPL knockdown markedly impaired TNBC cell proliferatio

Triage models for surgical decision-making in patients with shoulder pain: an integrative review.

Shoulder pain is a major cause of work absenteeism, affecting 1 in 3 adults. Although nonsurgical treatments like physical therapy are recommended as the first line of management, 40% of patients are referred for surgery before exploring these options. This review aimed to identify existing triage models used for shoulder pain referrals for surgery, guidelines for shoulder pain management that include criteria for surgical referral, and indications for surgery. An integrative review approach was used to comprehensively search major databases (Medline, PubMed, Embase, CINAHL Plus) and gray lite

Comparative outcomes of central venous access via open jugular and ultrasound-guided subclavian approaches in low-weight pediatric patients: a study with secondary outcomes.

BACKGROUND: The necessity of this study stems from the lack of comparative evidence on the outcomes of central venous catheterization via the open internal jugular and ultrasound-guided subclavian approaches in low-weight pediatric patients. Its innovation lies in the simultaneous evaluation of both primary and secondary outcomes of these two techniques in this vulnerable population. METHODS: This prospective randomized clinical trial at Mardani Azar Children's Hospital (winter 2022-2023) included 92 pediatric patients under five kilograms weight, randomly assigned to either open internal jug

Overcoming Subacromial Impingement, as a Term and Concept: Rationale and Proposals.

Background/Objectives: Subacromial impingement syndrome has traditionally been described as pathological contact between the rotator cuff and the anterolateral aspect of the acromion and has long been regarded as a primary cause of shoulder pain and tendon injury. Despite a substantial body of literature questioning the very existence of subacromial impingement as a pathological entity, the diagnosis continues to be widely used in medical and rehabilitation settings as a predominantly biomechanical explanation for shoulder pain. The objective of this review is to determine whether the concept

A GRAPH CONVOLUTION BASED DUAL-BRANCH NETWORK FOR DECODING OF UPPER-LIMB MOTOR IMAGERY EEG SIGNALS.

BACKGROUND: Analysing motor imagery EEG signals using conventional Euclidean feature computation is challenging due to low spatial resolution, non-stationarity, and complex inter-channel dependencies, which limit robust subject-independent generalization in brain-computer interface systems. NEW METHOD: This study proposes a graph-based convolutional framework for subject-independent decoding of upper-limb motor imagery. The model integrates graph convolution with skip connections and handcrafted time-frequency features, together with parallel spatial and temporal convolutional pathways. Graph

The Reliability and Validity of Active Range of Motion Measurement for Fingers Using Three-Dimensional Motion Analysis.

The Fugl-Meyer Assessment (FMA) and Range of Motion (ROM) are well-known measures for evaluating finger motor function. However, the finger FMA has a narrow scoring range, and goniometer ROM measurements lack reliability. Therefore, we investigated ROM measurement using a 3D motion capture system (Ultraleap Stereo IR 170, Ultraleap Ltd., Bristol, UK, hereinafter referred to as Ultraleap). This study aimed to evaluate the intra- and inter-rater reliability of finger Active ROM (A-ROM) using Ultraleap, and its criterion-related validity compared to a goniometer, in stroke patients with upper lim

Concurrent Validity and Agreement of Viewpoint-Dependent Markerless Motion Tracking in Upper-Limb Assessment: A Validation Against Optoelectronic Motion Capture.

Quantitative upper-limb assessment in rehabilitation traditionally relies on expensive optoelectronic reference tracking. While single-camera markerless systems offer an accessible alternative, their viewpoint-dependent accuracy and depth ambiguity remain critical concerns. This study systematically validates an integrated MediaPipe tracking pipeline (Pose and Hands) against an 8-camera optoelectronic system. Ten healthy males performed four clinical upper-limb tasks, recorded simultaneously across three spatial camera viewpoints (frontal, oblique elevated, and oblique side). To isolate spatia
