Lifestyle Adjustments in Diabetic Individuals Following Lower Extremity Amputations: A Qualitative Study.

BACKGROUND: Diabetes mellitus (DM) often leads to lower extremity amputations when poorly managed. Managing DM in Ghana is difficult due to limited access to diabetic care, low public awareness, and a strong reliance on religious beliefs and traditional medicine. AIM: This study examined the lifestyle changes of patients after lower limb amputation at Sunyani Teaching Hospital (STH) in Ghana, using the biopsychosocial model. METHODS: Semi-structured interviews were conducted on eight diabetic individuals who had undergone amputations at STH, Ghana. The study was conducted from March 2024 to

Biomechanic Differences Between Anticipated and Unanticipated Volleyball Block Jump: Implications for Lower Limb Injury Risk.

Volleyball is a high-intensity sport characterized by repetitive jumping, sudden directional changes, and overhead movements, all of which significantly increase the risk of injuries, particularly to the shoulders, knees, and ankles. Despite the frequency of injuries caused by actions like blocking and spiking, there has been limited research focused on the specific biomechanical risk factors unique to volleyball. This study aimed to investigate the lower limb biomechanics during block jumps in both the dominant and non-dominant directions, under both anticipated and unanticipated conditions,

Serious Game-Based Balance Training with Augmented Kinesthetic Feedback Enhances Aspects of Postural Control in Poststroke Patients: A Randomized Clinical Trial.

Background: Poststroke hemiparesis presents with motor asymmetry and decreased postural control leading to functional limitations. Serious games (SG) for balance rehabilitation of people with stroke may improve motor recovery, and the visual and auditory feedback provided by the SGs helps to explain the therapeutic benefits. However, the contribution of SG combined with kinesthetic and verbal cues during balance training has not been investigated. The aim of this study is to compare the effects of two feedback conditions for an SG balance intervention, with or without the addition of kinesthet

Saving Legs & Lives: the efficacy of a community-based cardiovascular rehabilitation programme versus usual care on exercise capacity and quality of life in patients who have undergone lower limb revascularisation for peripheral arterial disease-protocol for a single-centre randomised-controlled trial.

INTRODUCTION: Peripheral artery disease (PAD) is an atherosclerotic condition characterised by stenosis or occlusion of the arteries in the lower limbs. Patients with PAD commonly report intermittent claudication (leg pain/discomfort) during physical activities, which significantly limits the ability to walk and perform activities of daily living. Supervised exercise training is an effective therapy that can improve walking capacity in people with PAD. Emerging evidence also suggests that supervised exercise therapy following lower limb revascularisation can further enhance walking capacity wh

Evaluation of Peripheral Circulatory Changes Following Hydrotherapy and Controlled Physical Training in Patients with Atherosclerotic Lower Limb Ischemia.

UNLABELLED: Numerous studies highlight the significant role of exercise therapy in patients with peripheral artery disease (PAD), emphasizing how regular physical exercise enhances vascular endothelial function and promotes metabolic adaptations in skeletal muscles, ultimately improving walking performance. There are currently discussions in the medical world on optimizing noninvasive therapy to prevent the development of lower limb ischemia. This study aimed to assess the impact of a supervised training program combined with whirlpool massage treatment on improving peripheral circulation and

Accelerometer-Measured Volumes of Moderate-to-Vigorous Physical Activity and Functional Fitness in Older Adults.

The objective of this study is to investigate the association between different volumes of accelerometer-measured moderate-to-vigorous physical activity (MVPA) and functional fitness in older adults in a cross-sectional study (n = 242; 66 ± 5 years; 80% women). MVPA was assessed by accelerometer for 1 week. Based on the MVPA recommendations, the participants were classified as: (a) inactive (<50 min/week), (b) somewhat active (50-99 min/week), (c) moderately active (100-149 min/week), and (d) active (≥150 min/week). These groups reflect <33%, 33%-66%, 67%-99%, and ≥100% of the MVPA recommendat

Utilization of Recipient Veins with Reflux during Supermicrosurgical Lymphaticovenous Anastomosis for Lower Limb Lymphedema.

BACKGROUND: The use of recipient veins with venous reflux for lymphaticovenous anastomosis (LVA) is discouraged because it is commonly believed that this may lead to venous-lymphatic reflux (VLR). VLR is a phenomenon in which venous blood is refluxed into the lymphatic lumen after anastomosis, potentially lowering the long-term patency rate. However, this concept is yet to be validated. This study aimed to determine the impact on outcomes when recipient veins with reflux were used for LVA in treating unilateral lower limb lymphedema. METHODS: Seventy-nine patients who underwent LVA as the pri

Effects of semi-immersive virtual reality and manipulation of optic flow speed on gait biomechanics in people post-stroke.

OBJECTIVES: To investigate how people post-stroke and healthy people experience the addition of semi-immersive virtual reality (VR) and optic flow speed manipulation while walking on a treadmill, and if optic flow speed manipulation could be used in rehabilitation to elicit changes in post-stroke gait biomechanics. METHODS: Sixteen people post-stroke and 16 healthy controls walked on a self-paced treadmill. After 2 habituation trials (without and with VR), participants walked 3 more trials under the following conditions of optic flow: matched, slow, and fast. Primary outcome measures were spa

Autonomic Responses to Cold Pain: Effect of Type, Duration, and Habituation in Cryotherapy.

PURPOSE: Cold limb immersion, a form of cryotherapy, can cause cardiovascular changes due to cold-pain induced autonomic reflexes. This cryotherapy treatment side effect has received less attention but could have direct implications for physical rehabilitation of individuals with cardiovascular comorbidities. METHODS: To test hypotheses related to the pressor effects of varied limb sites and surface areas of cryotherapy, two common lower limb injury sites (ankle and knee) were immersed into cold water (15 min, 1-3°C) and then referenced to a standard cold pressor test (CPT). Beat-by-beat arte

The characteristics of falls in an institutionalized elderly cohort in China - A Retrospective observational study.

Falls are classified by WHO as the second highest cause of accidental injuries in older adults. It remains unclear what characteristics of falls are in elderly individuals that do not present definite causes. A total of 304 institutionalized older adults presented at least one fall were recruited in this retrospective observational study. The fall incidence was significantly higher at 4am-8am, at locations involving sitting down or standing up and involving flat ground, and when individuals going to toilets, getting off bed or walking. The most frequently injured sites were head and lower limb

Prehabilitation Exercise Training to Target Improved Muscle Strength in Pretransplant Patients Diagnosed With Multiple Myeloma: Protocol for a Pilot Randomized Controlled Trial.

BACKGROUND: Muscle mass and strength are severely compromised in patients diagnosed with multiple myeloma, such that the risk of poor overall survival increases as the prevalence of low muscle mass, also known as sarcopenia, increases. Additionally, at the time of autologous stem cell transplant (ASCT), 51% of patients experience low muscle mass and strength, which can prolong hospitalization and lead to increased risk of obesity, insulin resistance, lowered physical function, and poor quality of life. OBJECTIVE: The PROTECT (Prehabilitation Exercise Training in Multiple Myeloma Patients Unde

Training in Roller Speed Skating: Proposal of Surface Electromyography and Kinematics Data for Educational Purposes in Junior and Senior Athletes.

Introduction: Roller skating shares biomechanical similarities with other sports, but specific studies on speed skaters are limited. Injuries, particularly to the groin, are frequent and related to acute and chronic muscle stress. Technology, particularly surface electromyography, can now be used to monitor performance and prevent injuries, especially those caused by muscular asymmetries. Such studies can be used to enhance training and for educational purposes. Materials and Methods: This pilot study was conducted on three subjects: two cadet-athletes and a novice, compared with the performan

Femoral vein transposition fistula with short skip incisions: an optimised minimally invasive technique.

ObjectivesFemoral vein transposition (tFV) is a complex procedure that provides high patency rates for patients with exhausted upper-limb vascular access. Traditionally, the procedure involves a long single incision in the thigh to harvest the femoral vein, but this approach is associated with increased risks of local complications such as infections and haematomas. Skip incisions have shown to lower complication rates and shorten maturation times. We present an optimised, minimally invasive technique that transposes the femoral vein through three short skip incisions on the anterior thigh.Met

Para-Anastomotic Pseudoaneurysms as a Long-Term Complication After Surgical Treatment of Peripheral Artery Disease: Clinical Characteristics and Surgical Treatment.

Background: Peripheral arterial disease (PAD) is becoming an increasingly prevalent clinical issue, leading to a growing number of patients requiring surgical interventions. Consequently, there is an increasing occurrence of para-anastomotic aneurysms as late complications following primary treatment for PAD. These aneurysms typically arise at the sites of graft implantation and necessitate individualized management strategies based on factors such as location, size, and the patient's overall condition. Materials and Methods: This five-year retrospective study, conducted at a single center, ai

DIAMACC: protocol of a prospective diagnostic accuracy study of the maximal systolic acceleration to detect peripheral arterial disease in patients with diabetes-related foot ulceration in the Netherlands.

INTRODUCTION: Foot ulcers are one of the most serious complications of diabetes, leading to significant risks on amputation and mortality. Peripheral arterial disease (PAD) is an important factor for the development and the outcome of diabetic foot ulcers (DFU). Although prompt and accurate detection of PAD is critical to reduce complications, its diagnosis can be challenging with currently used bedside tests (such as ankle-brachial index and toe pressure) due to medial arterial calcification. A new and promising bedside test for the detection of PAD is the maximal systolic acceleration (ACCma
