ArtAssist Sequential Compression Biomechanical Device as a Cost-Dominant Limb-Salvage Strategy for No-Option Chronic Limb-Threatening Ischaemia: A Decade and Half, Single-Centre Cohort Study with Health Economic and Q-TWiST Analysis.

BACKGROUND: Major lower limb amputation for no-option chronic limb-threatening ischaemia (CLTI) costs the Irish Health Service Executive (HSE) a median €88,820 per patient; long-term health economic and quality-adjusted survival evidence for ArtAssist SCBD is lacking. METHODS: Longitudinal cohort analysis of 428 patients with non-reconstructible CLTI managed with ArtAssist SCBD at a single tertiary centre (January 2008-June 2025). Primary endpoints were limb salvage, cost-effectiveness versus primary amputation, and Q-TWiST. Kaplan-Meier analysis of limb salvage, multivariable Cox regression

Cilostazol and Risk of Major Adverse Limb and Cardiovascular Events after Revascularisation for Peripheral Artery Disease.

OBJECTIVE: Patients with peripheral artery disease (PAD) remain at high risk of major adverse limb events (MALE) and cardiovascular events (MACE) after revascularisation. Cilostazol, a phosphodiesterase-3 inhibitor with antiplatelet and antiproliferative effects, reduces re-stenosis but its real world impact on clinical outcomes is uncertain. This retrospective, nationwide, propensity score matched cohort study evaluated the effectiveness and safety of cilostazol on MALE, MACE, and all cause mortality after lower limb revascularisation for PAD. METHODS: Using the Taiwan National Health Insura

Presence of lower limb joint arthritis moderates the interrelationship between vision impairment and fear of falling avoidance behavior in older adults: A cross-sectional study.

Falling is a common concern for older adults. Previous studies showed that mobility performance is significantly lower in those with age-related eye diseases or lower limb arthritis, contributing to their increased fall risk. However, it is currently unknown how the co-presence of these 2 age-related sensorimotor impairments interacts to affect fear of falling (FOF) and associated activity-avoidance behaviors. A total of 200 participants recruited from an outpatient regional medical center (127 women, 73 men, age = 65.5 ± 8.4 years) participated in this study. The presence of age-related, unco

Temporal synchronization during bilateral foot-tapping and its relationship with gait disturbance in patients with Parkinson's disease.

OBJECTIVE: Previous studies using finger-tapping tasks have shown that patients with Parkinson's disease (PD) exhibit a deficit of temporal processing linked to gait disturbance. In this study, we aimed to investigate abnormalities in lower-limb rhythmic control and their association with freezing of gait in PD using a bilateral foot-tapping task. METHODS: Fifteen patients with PD and 25 healthy controls performed foot-tapping tasks synchronized with auditory tones at fixed intervals (500-2,400 ms). The tasks included unilateral and bilateral foot-tapping, with bilateral movements performed e

Biomechanics of knee osteoarthritis from local knee loading to whole kinetic chain imbalance: a narrative review.

The onset and progression of knee osteoarthritis (KOA) are closely associated with abnormal mechanical loading. Previous research has focused largely on local biomechanical factors, such as lower-limb alignment and the knee adduction moment; however, these factors alone may not fully explain the discordance between radiographic degeneration and clinical manifestations such as pain and functional impairment. From a whole-kinetic-chain perspective, the knee functions as an intermediate link within the continuous spine-to-foot chain. Multisegmental malalignment involving the lumbar spine, pelvis,

Feasibility and safety of a 6-month outpatient rehabilitation program combining therapeutic electrical stimulation using belt electrodes and nutritional management in patients with rheumatoid arthritis: a pilot study.

BACKGROUND: Although resistance training with nutritional intervention is recommended for sarcopenia, effective programs are often difficult to implement in patients with rheumatoid arthritis (RA). This pilot study aimed to assess the feasibility and safety of a 6-month outpatient rehabilitation program combining therapeutic electrical stimulation (TES) using belt electrodes with nutritional management in patients with RA. METHODS: This pilot study was conducted as a prospective observational study with a single-arm before-after framework and enrolled outpatients with RA in remission or with

Circulating Natural Killer Cells in Patients With Type 2 Diabetes and Peripheral Neuropathy Are Altered in Their Frequency and Exhibit Impaired Functions and Angiogenic Activities.

Patients with type 2 diabetes mellitus (T2DM) and critical limb-threatening ischemia (CLTI), a severe vascular complication associated with amputation and high mortality, exhibit chronic inflammation and endothelial dysfunction. We previously reported an increased frequency of natural killer (NK) cells in advanced CLTI; however, whether NK cell alterations are already present in patients with peripheral neuropathy, which often precedes diabetic foot lesions with or without distal macroangiopathy, remains unclear. We conducted an observational pilot study to assess circulating NK cell frequency

Evaluation of Methylmercury-Induced Motor Dysfunction and Proteomic Analysis of Cerebrospinal Fluid in Common Marmosets (Callithrix jacchus).

Methylmercury (MeHg) poisoning can lead to Minamata disease, often manifesting as motor dysfunction. We aimed to establish a quantitative evaluation method for MeHg exposure-associated motor dysfunction in the common marmoset (Callithrix jacchus). Three male marmosets were orally administered MeHg (1.5 mg Hg/kg/day) for 2 weeks, followed by a 2-week non-treatment period; three control marmosets received vehicle. Walking and spontaneous behavior were recorded weekly. After marking body segments, bounding gait videos were analyzed to assess locomotion speed, hip joint position, knee angle, dorsa

Disseminated MDR tuberculosis manifesting as Pott's disease with tuberculous septic ankle arthritis: an uncommon sequential dual-site involvement.

Disseminated osteoarticular TB is uncommon, and the occurrence of multidrug-resistant (MDR) strains impacting many skeletal locations in an immuno-competent individual is very rare. A 25-year-old male from a TB-endemic rural region is presented, having a three-month progression of worsening low back pain, subsequently accompanied by lower limb weakness. GeneXpert from both sites detected Mycobacterium tuberculosis that was resistant to rifampicin. The line probe assay detected resistance to rifampicin, high-level resistance to isoniazid, and resistance to ethambutol. After surgical procedures

Data-driven progression trajectories across clinical subtypes of amyotrophic lateral sclerosis.

BACKGROUND: Amyotrophic lateral sclerosis (ALS) exhibits substantial clinical heterogeneity; however, conventional classifications based on the predominant site of involvement may not fully capture patterns of disease progression. This study aimed to identify data-driven subtypes of ALS and reconstruct their temporal progression using item-level scores from the ALS Functional Rating Scale-Revised (ALSFRS-R). METHODS: Baseline data from 3067 participants in the Pooled Resource Open-Access ALS Clinical Trials database were analyzed. Subtype and Stage Inference (SuStaIn) was applied to ALSFRS-R

Blood culture-positive Aspergillus niger native aortic valve endocarditis presenting as acute lower limb ischemia: a case report.

Aspergillus endocarditis is a rare, life-threatening infection in which blood cultures are almost invariably negative. We report an exceptional case of blood culture-positive native aortic valve endocarditis caused by Aspergillus niger, presenting with acute limb ischemia. Concordant isolation of A. niger from peripheral blood cultures, embolic material, and resected valve tissue provided robust microbiological confirmation. Early surgical valve replacement combined with susceptibility-guided antifungal therapy enabled prompt targeted management and a favorable clinical outcome.

Retrieval of Thrombosed Inferior Vena Cava Filter and Mechanical Thrombectomy of Associated Caval Thrombosis Using FlowTriever and ClotTriever Systems With the ClotTriever as Embolic Protection.

A 55-year-old Chinese male with anti-phospholipid syndrome presented with left lower limb deep venous thrombosis (DVT) on warfarin and had an inferior vena cava filter (IVCF) implanted before a radical resection of a perineal sarcoma. The inferior vena cava (IVC) filter removal was attempted 2 weeks after the surgery, but thrombus was found at the filter. It was decided to leave the filter and to continue with anticoagulation. He presented with progressive right lower limb swelling, erythema and pain, with a CT venogram showing caval thrombosis extending from the IVC filter to the right below-

Additive effects of active vitamin D3 or vitamin E administration on therapeutic lower-limb home exercise for knee osteoarthritis: A pilot prospective randomized controlled trial.

BACKGROUND: Quadriceps weakness, inflammation, and reduced physical function are common in knee osteoarthritis (OA), and lower limb exercise therapy is an established non-pharmacological treatment. Vitamin D3 (vit D3) and vitamin E (vit E) have been proposed as adjunctive therapies; however, clinical findings are inconsistent. We aimed to examine whether combining lower limb home exercises with oral active vit D3 or vit E supplementation provides additive benefits compared with home exercise alone in women with symptomatic knee OA. METHODS: Eighty-eight postmenopausal women with knee OA (Kell

Tuberculous spinal epidural abscess: a cautionary case report of permanent paraplegia due to delayed diagnosis and management.

BACKGROUND: Spinal epidural abscess caused by tuberculosis is a rare but serious complication of spinal tuberculosis. Its atypical presentation predisposes to diagnostic delay and a poor prognosis. This case not only documents the clinical and radiologic evolution of tuberculous spinal epidural abscess but also suggests that delays in diagnosis and treatment may jointly contribute to serious neurological outcomes. CASE PRESENTATION: A 39-year-old man initially underwent acupotomy for recurrent low back pain with recent exacerbation. Subsequent imaging revealed a paraspinal abscess within the

Intravascular Ultrasound-Guided Iliofemoral Venous Stenting: In-Stent Mural Thrombus as a Possible Early Sign of Occult Malignancy.

Dedicated venous stents became available in Japan only recently, and reports of their use and follow-up in Japanese practice remain scarce. A 77-year-old woman developed acute left iliofemoral deep vein thrombosis (DVT) after prolonged recumbency and was treated with edoxaban. Eighteen days later she underwent endovascular therapy of the right lower limb for chronic limb-threatening ischaemia, and aspirin was added. Limb swelling and pain persisted despite three months of anticoagulation and graduated compression, and imaging then showed chronic occlusion of the left common and external iliac

Rapidly Progressive Acute Limb Ischemia With Irreversible Gangrene and Major Amputation in a Resource-Limited Setting: A Case Report.

Acute limb ischemia (ALI) is a vascular emergency in which delayed recognition and treatment can result in irreversible tissue necrosis, gangrene, limb loss, and potentially life-threatening systemic complications. We present the case of a 63-year-old woman with atrial fibrillation and heart failure who developed acute lower-limb ischemia with progressive discoloration and tissue necrosis. Lower-extremity arterial Doppler ultrasonography demonstrated extensive arterial occlusive disease with absent flow across multiple major arterial segments, confirming severe limb ischemia.
