The effects of TMS stimulus direction on corticospinal and cortico-cortical excitability of the lower limb in healthy adults.

Transcranial magnetic stimulation (TMS) studies in upper limb muscles have shown that stimulus direction can influence corticospinal and intracortical excitability; however, limited evidence exists for lower limb muscles. This study investigated the effect of TMS stimulus direction on corticospinal and intracortical excitability of the tibialis anterior (TA) muscle using paired-pulse TMS. Nineteen healthy adults (10 males, 9 females; mean age 41.8 ± 23 years) participated. Single- and paired-pulse TMS was delivered using a figure-of-eight coil oriented to induce anterior-to-posterior (AP) or p

Differences in knee axis behaviour and muscle activity between open- and closed-chain knee exercises.

Understanding knee mechanics and joint loading is vital for optimizing training and rehabilitation. This study analysed knee axis behaviour and lower limb muscle activity during a closed-chain exercise (Bilateral Squat, BLS), compared with an open-chain exercise (Seated Leg Extension, SLE) using time-series analysis of electromyographic (EMG) data and motion capture to assess knee axis inclination and translations through 90 degrees of flexion-extension. EMG data were filtered and normalized to maximum activity, and kinematic curves were analysed across the full movement cycle (0-100%) employi

Differences in Clinical Presentation of Chronic Pelvic Pain Caused by Iliac Vein Obstruction Impacts Referral Pathway, Time to Diagnosis, and Post-stenting Pain Resolution.

OBJECTIVE: Iliac venous stenting improves chronic pelvic pain (CPP) caused by iliac vein obstruction (IVO). Signs and symptoms vary, but this pain is reproducible and easily identifiable. However, patients can experience significant delays in diagnosis and treatment. This study aimed to determine associations between the referral history, clinical presentation, pain characteristics, and stenting outcomes among women with CPP due to IVO. METHODS: This was a retrospective analysis of prospectively collected data of women who were referred to a vascular surgeon with signs and symptoms of IVO, in

Bilateral lower-limb asymmetry and phase-specific trunk acceleration during the Timed Up and Go in older women: an analysis based on muscle morphology and antagonist coactivation.

The Timed Up and Go (TUG) test is widely used to assess mobility and fall risk in older adults, but total completion time does not capture phase-specific motor control. This study quantified bilateral lower-limb asymmetry in muscle morphology and neuromuscular control and examined its association with phase-specific trunk acceleration during the TUG. Twenty-seven healthy older women (78.96 ± 2.44 years) participated. Bilateral muscle thickness was measured by B-mode ultrasound, and bilateral surface electromyography and L4 trunk acceleration were recorded during a five-phase instrumented TUG.

Multi-dataset benchmarking and preliminary transfer learning for lower-limb EMG classification toward adaptive prosthesis control.

PURPOSE: Reliable decoding of lower-limb motor intent from surface electromyography signals remains challenging due to electrode placement variability, muscle fatigue, inter-subject differences, and differences between able-bodied and amputee populations. This study evaluates the robustness of commonly used machine-learning approaches for lower-limb electromyography classification under these sources of variability. METHODS: A unified benchmarking analysis was conducted across four lower-limb electromyography datasets: UCI lower limb electromyography, vastus medialis electromyography, K2MUSE,

Effects of Integrative Neuromuscular Training on Unilateral and Bilateral Lower-Extremity Sports Performance in Adolescent Female Sprinters: A Randomized Controlled Trial.

BACKGROUND: Adolescent female athletes often exhibit more pronounced lower-limb asymmetry due to physiological differences, increasing injury risk. Despite documented efficacy in team sports for injury mitigation and performance enhancement, integrative neuromuscular training (INT) effects on unilateral and bilateral lower-limb capabilities in trained adolescent female sprinters remain undetermined. HYPOTHESIS: We hypothesized that an 8-week INT program would significantly improve unilateral and bilateral lower-extremity performance measures in adolescent female sprinters compared with tradit

Use of Biodegradable Temporizing Matrix for Donor and Recipient Site Wound Healing in Delayed Reverse Sural Flap: A Case Report.

INTRODUCTION: Reconstruction of distal third leg soft tissue defects is challenging, particularly in patients with multiple co-morbidities. Whilst the reverse sural artery (RSA) flap remains a viable reconstructive option, flap failure due to torsion, congestion, and necrosis persists. CASE PRESENTATION: This case report presents a patient who underwent a staged RSA flap surgery, with initial application of a biodegradable temporising matrix (BTM) over both the donor site and the heel defect. This was performed with three aims: to create a physical interface as an adjunct to surgical delay; t

[Attenuated X-linked hypophosphatemia: late diagnosis in an adult patient].

Although patients with X-linked hypophosphatemia (XLH) usually present a phenotype characterized by rickets in children or osteomalacia in adults, short stature, lower limb deformities, and dental abnormalities, this condition can manifest with a wide phenotypic spectrum. In attenuated cases, diagnosis is often delayed, preventing early treatment. We present the case of a 50-year-old woman who was referred to our clinic by her son's pediatrician, who had been diagnosed with XLH through molecular testing. Manifestations suggesting attenuated XLH (mild valgus curvature of the lower limbs, bilate

[Livedoid vasculopathy: A case report].

Livedoid vasculopathy (LV) is a painful and thrombotic obliterative vascular disorder characterized by spontaneous thrombus formation within skin capillaries or small veins, ultimately leading to localized hypoxia and the development of skin ulcers. LV is a rare condition predominantly affecting young women, with an incidence rate of approximately 1/100 000. Clinically, it manifests as recurrent reticular green spots and painful ulcers on the distal lower extremities, which subsequently heal, leaving behind white and atrophic scars, termed "white atrophy". The skin manifestations of the diseas
