Lymphatic Mapping to the Inguinal Region in Patients with Lower Extremity Melanoma Undergoing Radical Inguinal Lymph Node Dissection: A Retrospective Cohort Study.

BACKGROUND/OBJECTIVES: The anatomical distribution of inguinal lymph node (ILN) metastases may inform research on the extent of therapeutic dissection for lower-limb melanoma. METHODS: Fifty-eight patients undergoing radical inguinal lymph node dissection (rILND) were retrospectively evaluated using five anatomical zones. Zone-specific node counts were reaggregated, and exploratory binary logistic regression assessed multiple-zone involvement. RESULTS: The cohort included 48 clinically node-positive and 10 sentinel lymph node biopsy (SLNB)-positive patients. Among 816 mapped ILNs, 149 were p

[Diagnosis and Intraoperative Decision-making in Pulmonary Sclerosing Pneumocytoma with Atypical Imaging Findings: A Case Report and Review of the Literature].

Pulmonary sclerosing pneumocytoma (PSP) is a rare benign pulmonary tumor that predominantly occurs in middle-aged women. Its preoperative diagnosis remains challenging, and it is frequently misdiagnosed as a malignant tumor. Herein, we report a case of PSP with atypical imaging manifestations and review the current literature on this entity. A 68-year-old woman presented with chest tightness. Chest computed tomography (CT) revealed a lobulated solid nodule in the right middle lobe measuring approximately 26 mm × 22 mm, accompanied by multiple enlarged lymph nodes in the bilateral hilar, medias

[Advanced Enteric-type Thymic Adenocarcinoma with Axillary Lymph Node
Oligometastasis: A Case Report].

Enteric‑type thymic adenocarcinoma (ETAC) is a rare subtype of thymic carcinoma. It shares highly similar histological and immunophenotypic features with intestinal adenocarcinoma, and is therefore prone to being misdiagnosed as metastatic carcinoma of gastrointestinal origin. Herein, we report a case of advanced ETAC with axillary lymph node oligometastasis. Positron emission tomography/computed tomography (PET/CT) and gastrointestinal endoscopy were performed to rule out primary gastrointestinal malignancy. The patient underwent median sternotomy with extended thymectomy, which effectively r

Integrative multi-omics analysis identifies CCDC69 as a marker associated with germinal center B-cell infiltration in TNBC: development and validation.

Although coiled-coil domain containing 69 (CCDC69) has been implicated in tumor biology, its role in triple-negative breast cancer (TNBC) and immune regulation remains unclear. Bulk transcriptomic data, single-cell RNA sequencing datasets, and spatial transcriptomics were used to investigate the role of CCDC69 in TNBC. Germinal center B-cell (GCB) signatures were quantified using single-sample gene set enrichment analysis (ssGSEA), and their associations with CCDC69 expression were assessed using correlation, ROC, and logistic regression analyses. An independent external TNBC cohort (GSE58812)

Case Report: Atypical cat-scratch disease presenting as referred shoulder pain and deep lymphadenopathy in a pediatric patient.

BACKGROUND: Cat-scratch disease (CSD) is a zoonotic infection attributable to Bartonella henselae. Typical clinical manifestations include regional lymphadenopathy accompanied by primary cutaneous lesions that develop following exposure to cats. Although most patients present with typical regional lymphadenopathy, a subset of patients may develop atypical manifestations, which can complicate diagnosis. Owing to the nonspecific clinical features, CSD is prone to misdiagnosis or diagnostic omission. This report describes a pediatric case of atypical cat-scratch disease characterized by initial s

A Rare Case of Large Cell Neuroendocrine Carcinoma of the Thymus Presenting as a Lower Neck Mass in a Young Male Patient.

Large cell neuroendocrine carcinoma (LCNEC) of the thymus is a strikingly unusual and high-grade thymic tumor with a poor prognosis. Studies with large series of patients on the clinical, epidemiological, and biological behavior of the disease are not available due to its low prevalence and short survival. We report here such a rare case of LCNEC in a 33-year-old male patient who primarily presented with a mass in the suprasternal and left supraclavicular regions with multiple bilateral cervical lymph nodes. Fine needle aspiration cytology from the neck mass demonstrated hypercellular smears w

Omission of Axillary Surgery in Patients with Invasive Lobular Carcinoma of the Breast: Rates of Nodal Involvement in Clinically Node Negative Patients and Impact on Recurrence in a Single Institution Analysis.

BACKGROUND: Recent randomized trials such as INSEMA and SOUND identified patients with breast cancer for whom axillary surgery omission appears to have no detrimental oncologic impact. However, some have questioned the safety of omitting axillary surgery in patients with invasive lobular carcinoma (ILC), the most common special histology type. METHODS: We analyzed a prospectively maintained institutional ILC database containing 1029 tumor records. We identified those meeting INSEMA and SOUND criteria for axillary surgery omission (clinically node negative with axillary ultrasound, cT1-2 for I

Lymphovascular Invasion in SOUND-Eligible Breast Cancer: Implications for Occult Axillary Nodal Metastasis.

BACKGROUND: The SOUND randomized controlled trial demonstrated that omitting sentinel lymph node biopsy (SLNB) is noninferior to performing SLNB in early-stage breast cancer. Lymphovascular invasion (LVI), however, is an adverse pathologic feature associated with axillary nodal metastasis. In this study, we evaluated the association of LVI with occult axillary nodal metastasis in early-stage, SOUND-eligible breast cancer. PATIENTS AND METHODS: We retrospectively reviewed patients with cT1N0 breast cancer from 2011 to 2025 who underwent upfront breast-conserving surgery and lymph node surgery

Nodal Pathologic Response After Neoadjuvant Systemic Therapy in Clinically Advanced Node-Positive (cN2 and cN3) Breast Cancer.

BACKGROUND/OBJECTIVE: Patients with clinically advanced node-positive (cN2/3), non-metastatic breast cancer have traditionally undergone axillary lymph node dissection (ALND) after neoadjuvant systemic therapy (NST). Randomized de-escalation trials have excluded cN2/3 disease. However, some of these patients achieve nodal pathologic complete response (ypN0), suggesting potential for less extensive axillary surgery. We aimed to determine ypN0 rates and identify factors associated with nodal response. METHODS: The National Cancer Database (NCDB, 2004-2022) was analyzed. Female patients with cN2

Cheek Merkel Cell Carcinoma Is Associated With Increased Nodal and Parotid Involvement.

BACKGROUND: Merkel cell carcinoma (MCC) is a rare and highly aggressive neuroendocrine tumor that frequently arises in the head and neck region. While known risk factors include advanced age, Merkel cell polyomavirus infection, immunodeficiency, and ultraviolet exposure, the impact of anatomic subsite on disease behavior and prognosis remains poorly defined. METHODS: We performed a retrospective cohort study of 46 patients with head and neck MCC treated by head and neck surgeons at Roswell Park Comprehensive Cancer Center between 2008 and 2022. Patients were identified using ICD-9 and ICD-10

Prospective Clinical Trial on Expanding the Eligibility Criteria for Targeted Axillary Dissection after Neoadjuvant Systemic Therapy in Breast Cancers with Advanced Nodal Disease.

BACKGROUND: Targeted axillary dissection (TAD) is an established axillary restaging approach in breast cancer after neoadjuvant chemotherapy (NAC), but evidence to date has focused largely on clinical N1 disease with limited nodal involvement. For higher nodal burden patients, axillary dissection (AD) is still frequently performed despite high axillary pathologic complete response (pCR) rates. We hypothesized that TAD is noninferior to AD for nodal staging in patients with advanced nodal disease. PATIENTS AND METHODS: Patients with higher nodal burden breast cancer planned for NAC followed by

Assessing the performance of deep learning and hand-crafted radiomics models using MRI and ultrasound in predicting axillary lymph node status in breast cancer: a systematic review and meta-analysis.

BACKGROUND: Axillary lymph node metastasis (ALNM) is a critical prognostic factor in breast cancer. While sentinel lymph node biopsy remains the gold standard, conventional imaging relies on operator expertise with variable diagnostic accuracy. This meta-analysis evaluates the diagnostic performance of deep learning (DL) and hand-crafted radiomics (HCR) models using MRI and ultrasound (US) for ALNM prediction. METHODS: Literature search was conducted across four databases up to November 2024. Studies assessing DL or HCR models for ALNM prediction using MRI or US, with histopathological confir

Motivating and enabling factors supporting targeted improvements to hospital-SNF transitions.

OBJECTIVES: Older adults with Alzheimer disease and related dementias (ADRD) experience hospital-to-skilled-nursing-facility (SNF) transitions at disproportionate rates, yet it is unclear whether investments in information sharing practices are equitably distributed across SNFs that care for more of these patients. The purpose of this study was to characterize and compare hospital-SNF dyads according to the proportion of patients they share who have diagnosed ADRD and to analyze whether specific motivating (ie, historical readmission rates) and/or enabling (ie, health information exchange [HIE

Axillary Surgery Omission Versus SLNB Versus ALND in Early Breast Cancer: Survival and a Prediction Tool.

INTRODUCTION: Randomized trials support omission of selected axillary procedures in carefully defined early breast cancer populations. Whether these trial findings translate to broader real-world practice remains uncertain, particularly among patients who do not undergo any axillary surgery. METHODS: We analyzed women aged 18-85 y with surgically treated T1/T2N0M0 invasive breast cancer in the Surveillance, Epidemiology, and End Results database from 2018 to 2020. Patients were classified according to the number of lymph nodes examined as no axillary surgery (NAS; 0 nodes), sentinel lymph nod
