Cheek Merkel Cell Carcinoma Is Associated With Increased Nodal and Parotid Involvement.

BACKGROUND: Merkel cell carcinoma (MCC) is a rare and highly aggressive neuroendocrine tumor that frequently arises in the head and neck region. While known risk factors include advanced age, Merkel cell polyomavirus infection, immunodeficiency, and ultraviolet exposure, the impact of anatomic subsite on disease behavior and prognosis remains poorly defined. METHODS: We performed a retrospective cohort study of 46 patients with head and neck MCC treated by head and neck surgeons at Roswell Park Comprehensive Cancer Center between 2008 and 2022. Patients were identified using ICD-9 and ICD-10

Can supermicrosurgery treat Alzheimer's disease? - Current evidence and hope.

Alzheimer’s disease is characterized by a loss of temporal and spatial orientation and a progressive loss of cognitive and locomotor function. Therapeutic drug options are currently limited. There is evidence that degradation products of Alzheimer’s diseases such as the tau protein or amyloid beta in Alzheimer’s disease could be removed by the glymphatic system (lymphatic system of the brain), as these drain into the deep cervical lymph nodes. The microsurgical creation of lympho-venous anastomoses could improve this drainage and improve cognition in Alzheimer’s patients (probably also for oth

Outcomes of Mohs Micrographic Surgery Compared to Wide Local Excision for Malignant Melanoma and Melanoma In Situ.

Mohs micrographic surgery (MMS), traditionally used for non-melanoma skin cancers, has been used for some cases of melanoma. Using the TriNetX (TriNetX, LLC, Cambridge, MA, USA) database, two analyses were performed. The first compared wide local excision (WLE) and MMS for the management of malignant melanoma (MM) of the face, scalp, and neck. The second analysis compared WLE and melanoma in situ (MIS) of the same anatomical areas. WLE was associated with a significantly higher incidence of secondary and unspecified malignant neoplasms of the lymph nodes of the head and neck and secondary mali

Compartmental surgery for squamous cell carcinoma of the buccal mucosa: description of a new surgical technique.

BACKGROUND: Oral squamous cell carcinoma (OSCC) is the sixth most common cancer globally. Patient survival varies depending on tumour stage and oral cavity subsites. Buccal mucosa neoplasia is rare and burdened by worse prognosis than other oral subsites, showing a high rate of loco-regional relapses within six months after treatment. According to NCCN guidelines, the gold standard treatment is radical surgery. In the oral cavity, the buccal mucosa subsite lacks anatomical barriers opposing neoplastic growth. At this level, the tumour cells could hypothetically spread along the fibres of the p

Delayed regional metastasis from small face/scalp cutaneous squamous carcinoma: outcomes and predictors.

OBJECTIVE: CSCC (cutaneous squamous cell carcinoma) of the face/scalp generally has a favorable outcome. However, patients with delayed regional metastasis from scalp/facial CSCC do very poorly. Historically, prophylactic treatment of the N0 neck for the T1-T2 (as staged in the AJCC 7th edition) face/scalp cancers has not been recommended. This study aims to determine predictors of delayed regional metastases in < 4 cm CSCC of the face and scalp. METHODS: We retrospectively analyzed medical records of patients diagnosed regional metastasis of CSCC of the scalp/face at least 6 months after the

Metastatic basal cell carcinoma: Case series and literature review.

OBJECTIVE: To present a metastatic head and neck basal cell carcinoma (BCC) series treated by a university teaching service between 1993 and 2021. METHODS: A review of medical records of 500 patients with high-risk BCC in the head and neck was performed. Tumors with a diameter greater than or equal to 6 mm in the "H" area of the face or greater than 10 mm in other head and neck areas; recurrences; sclerodermiform, micronodular and metatypical subtypes; and perineural invasion were considered factors indicating high-risk. RESULTS: Seven cases of metastatic head and neck BCC were diagnosed. Th

Fasciocutaneous Vascularized Lymph Node Transfer for Head and Neck Lymphedema: A Case Report.

Head and neck lymphedema is a common complication of head and neck cancer treatment. Lymphovenous bypass is a promising surgical treatment but may not be an option for all patients after radiotherapy due to the obliteration of local lymphatic targets for bypass. We aim to present vascularized lymph node transfer (VLNT) as a solution to this problem. We report the case of a 38-year-old patient with Stage 2 face and neck lymphedema after treatment for T4aN1oral squamous cell carcinoma, who had no targets for lymphovenous bypass. The patient was treated with a 6 by 14 cm fasciocutaneous VLNT base

Tumor budding predicts lymph node metastasis in squamous cell carcinoma of the lip.

BACKGROUND: Squamous cell carcinoma of the lip (LSCC) is a relatively rare malignancy. The criteria for performing a neck dissection remain unclear, as reliable predictors for lymph node metastasis (LNM) have not been fully established. This study aimed to identify risk factors for LNM to guide the indication for elective neck dissection. METHODS: A total of 57 patients with LSCC were evaluated based on 81 clinical and pathological parameters, including three previously published pathological grading systems. Statistical analyses focused on identifying the most relevant and independent predic

Comparison of deep learning networks for fully automated head and neck tumor delineation on multi-centric PET/CT images.

OBJECTIVES: Deep learning-based auto-segmentation of head and neck cancer (HNC) tumors is expected to have better reproducibility than manual delineation. Positron emission tomography (PET) and computed tomography (CT) are commonly used in tumor segmentation. However, current methods still face challenges in handling whole-body scans where a manual selection of a bounding box may be required. Moreover, different institutions might still apply different guidelines for tumor delineation. This study aimed at exploring the auto-localization and segmentation of HNC tumors from entire PET/CT scans a

Trichilemmal carcinoma metastatic to parotid gland: First report of fine needle aspiration cytology features.

Trichilemmal carcinoma is a rare malignant cutaneous adnexal neoplasm arising from the outer root sheath of the hair follicle. Majority of cases occur on sun-exposed sites such as the face, scalp and neck, making them easily amenable to being biopsied and subjected to histological examination for definitive diagnosis. Thus, cytological features of trichilemmal carcinoma have not been described till date. Trichilemmal carcinoma is a low-grade malignancy, albeit with potential to metastasize to regional lymph nodes and distant sites. We report the case of trichilemmal carcinoma of scalp that met

Lymphadenopathy and lymph node rejection following facial vascularized composite allotransplantation.

BACKGROUND: Apart from the skin, little is known about the immunological processes in deeper tissues, which are typically not accessible to biopsy and inspection, of vascularized composite allografts (VCAs). Face transplant patients develop prominent adenopathy shortly after transplantation that resolves over time. The mechanisms underlying this process are not understood. MATERIALS AND METHODS: A retrospective cohort study was conducted on 9 patients who underwent 10 facial VCAs at the Brigham and Women's Hospital, Boston, MA, between April 2009 and July 2019. Merkel Cell Carcinoma on the Face: Case Report.

Merkel cell carcinoma (MCC) is a rare primary neuroendocrine skin tumor that presents as a flesh-colored or bluish-red nodule on the face, neck, or head. Long-term ultraviolet radiation exposure and Merkel cell polyomavirus are associated with MCC pathogenesis. We present a case of MCC on the right cheek in a male patient aged 87 years. Our primary goal in presenting the case is to bring MCC, which is a diagnostic challenge, to the notice of dermatologists and oncologists, as early detection and prompt treatment are important. The patient had a significant past medical history, including diabe

Bone Scan With SPECT/CT Demonstrated Phthisis Bulbi.

A 46-year-old man has a history of right eye vision loss from childhood due to an injury, right hypopharyngeal cancer with metastasis to right neck lymph nodes treated with concurrent chemoradiotherapy, and left tonsillar cancer after resection and adjuvant chemotherapy. Whole-body bone scan showed abnormal uptake to the right eye region, whereas SPECT/CT imaging localized this uptake to sharply defined, round to oval bone density foci of calcium scattered in the sclera and surrounding tissues, indicative of phthisis bulbi with posttraumatic calcification. Phthisis bulbi represents a shrunken
