Rapidly progressive glomerulonephritis as an unusual type of renal involvement in sarcoidosis: a case report.

INTRODUCTION: Sarcoidosis is a multisystem inflammatory disease of unknown etiology characterized by the formation of noncaseating epithelioid granulomas. Clinically significant renal involvement is rare in sarcoidosis. It most commonly manifests as chronic tubulointerstitial nephritis and nephrocalcinosis with nephrolithiasis. Further, glomerular involvement is observed sporadically, mainly membranous glomerulopathy or focal segmental glomerulosclerosis. CASE PRESENTATION: We describe the clinical case of a 49-year-old patient of Caucasian ethnicity with a history of sarcoidosis of the lungs

The evolving role of MRI in pulmonary sarcoidosis: Comparative analysis with PFTs and progression markers.

BACKGROUND: Sarcoidosis is a systemic illness with unclear etiology that commonly affects the lungs and intrathoracic lymph nodes. Chest radiography, CT, and, more recently, MRI can be used for assessment, with bilateral hilar lymph node enlargement and interstitial lung disease being common observations. OBJECTIVES: In adults diagnosed with pulmonary sarcoidosis, how do chest radiography, CT, and MRI compare in the assessment of disease progression? STUDY DESIGN AND METHODS: This is a prospective study of 77 adults diagnosed with pulmonary sarcoidosis. Each patient underwent pulmonary funct

Sarcoidosis: Solitary Pulmonary Mass Masquerading As Lung Cancer.

Sarcoidosis is a multisystem inflammatory disease of unknown cause with a wide variety of presentations. It primarily involves the lungs and intrathoracic lymph nodes, as well as the eyes, skin, spleen, and liver. Very rarely, it can present as a solitary lung mass. We present a case of a 60-year-old female who presented with an erythematous plaque on her face associated with irritation. Initial imaging detected a well-defined lobulated mass in the right middle and lower lobes of the lung. A biopsy of the mass revealed non-caseating granulomas, suggestive of sarcoidosis.

Osseous Sarcoidosis: Three Typical Presentations of an Atypical Disease.

Sarcoidosis is a systemic inflammatory disease characterized by the formation of non-necrotizing granulomas in affected tissues. While it most commonly involves the lungs and intrathoracic lymph nodes, it can affect virtually any organ system. Osseous sarcoidosis is a less common manifestation, and its prevalence may be underrepresented, as many patients remain asymptomatic. When symptoms do occur, they may include bone pain, reduced exercise capacity, or localized swelling. The spine is the most frequently involved site. In this case series, we present three distinct patients with osseous sar

A unique case of extrapulmonary sarcoidosis on F18 FDG PET/CT: "Tiger man".

Sarcoidosis is a systemic granulomatous disease in which muscle involvement is a rare extrapulmonary manifestation. We present the case of a 34-year-old man with shortness of breath lasting for three months.  Computerized tomography showed intrathoracic lymph nodes and perihilar consolidations in both lungs. She received treatment for pneumonia, but because of insufficient regression, she was referred to our hospital's chest diseases department for further examination. Laboratory results showed an elevated calcium. We conducted an F-18 FDG PET CT scan because the clinicians suspected cancer. P

Intestinal Sarcoidosis: A Diagnostic Conundrum.

Sarcoidosis is a systemic granulomatous disorder of unknown cause, most commonly affecting the lungs and intrathoracic lymph nodes. We present the case of a 51-year-old woman in whom intestinal sarcoidosis was incidentally discovered alongside pulmonary involvement. The diagnosis was made during evaluation of trauma-related findings and confirmed through PET-CT, colonoscopy, and biopsy. GI sarcoidosis can closely mimic other granulomatous diseases, such as Crohn's disease or certain infections, making diagnosis particularly challenging. In this case, the presence of mediastinal lymphadenopathy

PET-CT-guided characterisation of progressive, preclinical tuberculosis infection and its association with low-level circulating Mycobacterium tuberculosis DNA in household contacts in Leicester, UK: a prospective cohort study.

BACKGROUND: Incipient tuberculosis, a progressive state of Mycobacterium tuberculosis infection with an increased risk of developing into tuberculosis disease, remains poorly characterised. Animal models suggest an association of progressive infection with bacteraemia. Circulating M tuberculosis DNA has previously been detected in pulmonary tuberculosis by use of Actiphage, a bacteriophage-based real-time PCR assay. We aimed to investigate whether serial [18F]fluorodeoxyglucose ([18F]FDG)-PET-CT could be used to characterise the state and progressive trajectory of incipient tuberculosis, and e

Molecular analysis of endobronchial ultrasound needle aspirates in patients with non-small cell lung cancer: Results from the SCOPE database.

OBJECTIVE: Molecular subtyping of non-small cell lung cancer (NSCLC) is critical in the diagnostic evaluation of patients with advanced disease. This study aimed to examine whether samples from endobronchial ultrasound transbronchial needle aspiration (EBUS-TBNA) of intrathoracic lymph nodes and/or lung lesions are adequate for molecular analysis across various institutions. METHODS: We retrospectively reviewed all cases of linear EBUS-TBNA with a final bronchoscopic diagnosis of NSCLC entered in the Stather Canadian Outcomes registry for chest ProcEdures database. The primary outcome was spe

An Unusual Etiology of Fluorodeoxyglucose Avid Intrathoracic Lymph Nodes.

A middle-aged man in his 50s, active smoker, presented to the pulmonary office for lung cancer evaluation. On a low-dose computed tomography for lung cancer screening, he was found to have an 8 mm endobronchial lesion in the right main stem bronchus. A PET-CT revealed no endobronchial lesion, but incidentally, fluorodeoxyglucose (FDG) avidity was present in the right hilar (SUV 13.2) and paratracheal lymph nodes (LNs). He underwent bronchoscopy and EBUS-TBNA of station 7 and 10 R LNs. The fine needle aspiration (FNA) revealed necrotizing epithelioid granuloma. The acid-fast bacilli (AFB) and G

Development of Pseudochylothorax in a Patient with Lung Squamous Cell Carcinoma and Rheumatoid Arthritis.

Pseudochylothorax is a rare disease entity. It is characterized by a milky white pleural fluid with a cholesterol/triglyceride ratio >1. According to published reports, most patients had associated diseases, including rheumatoid arthritis and tuberculosis, with only one patient having oropharynx carcinoma associated with lung metastases. Herein, we describe the development of pseudochylothorax in a patient with rheumatoid arthritis and lung squamous cell carcinoma. Cancer cells were confirmed in the pleural fluid, but there was no thoracic duct damage caused by cancer invasion or tuberculosis-

Radiomicsmetabolic signature profiles for advanced non-small cell lung cancer with chemoimmunotherapy by reflecting biological function and survival.

BACKGROUND: Resistance to chemoimmunotherapy in patients with advanced non-small cell lung cancer (NSCLC) necessitates effective prognostic biomarkers. Although 18F-fluorodeoxyglucose (FDG) positron emission tomography (PET)/computed tomography (CT) has shown potential for efficacy assessment, it has been mainly evaluated in immuno-monotherapy setting, lacking elaborations in the scenarios of immunotherapy combined with chemotherapy. To tackle this dilemma, we aimed to build a non-invasive PET/CT-based model for stratifying tumor heterogeneity and predicting survival in advanced NSCLC patients

Diagnostic Value of Imaging and Serological Biomarkers in Pulmonary Sarcoidosis.

Sarcoidosis is a multisystem granulomatous disease of an unknown aetiology. It can exist in many organs. Pulmonary and intrathoracic lymph nodes are most commonly involved. Lung sarcoidosis is uncommon in Asia. However, due to the large population of our country and the development of bronchoscopy, percutaneous lung puncture, and other medical technologies, the number of pulmonary sarcoidosis patients is on the rise. Pulmonary sarcoidosis patients have no obvious symptoms in the early stage, and the clinical manifestations in the later stage may vary from person to person. Eventually, the dise

Lung fibrosis in sarcoidosis. Is there a place for antifibrotics?

Sarcoidosis, an enigmatic disease with unknown etiology, is characterized by inflammation and the potential involvement of various organs, predominantly the lungs and intrathoracic lymph nodes. Non-caseating granulomas can resolve spontaneously in approximately 60% of cases within 2-3 years. However, sarcoidosis-related mortality has increased. Lung fibrosis, affecting up to 20% of sarcoidosis patients, stands out as a primary cause of mortality. Traditionally, fibrosis is viewed because of prolonged inflammation, necessitating anti-inflammatory treatment with systemic steroids, immunosuppress

A case report: Endobronchial ultrasound guided biopsy of radiographically normal size thoracic lymph nodes supporting diagnosis of cardiac sarcoidosis.

Sarcoidosis is a rare chronic granulomatous disease with unknown etiology. Definite diagnosis of cardiac sarcoidosis (CS) is especially difficult to establish. Several guidelines exist to make a diagnosis of CS but those have not been clinically validated. Despite the high specificity of endomyocardial biopsy, its sensitivity is low, and the severity of potential complications is high. Thus, we present 63-year-old male who was diagnosed with CS with the endobronchial ultrasound (EBUS) transbronchial needle aspiration (TBNA) of intrathoracic lymph nodes which were non-enlarged without PET avidi

Briefing of pulmonary sarcoidosis: Reduction-oxidation, misleading and possibilities.

Sarcoidosis is an inflammatory disease with limited treatment strategies and is characterized by the presence of abnormal lumps (granulomas) of the inflammatory cells. Among the types, pulmonary sarcoidosis most commonly occurs (about 90%), affecting the lungs and intrathoracic lymph nodes. Although the cause of its occurrence is still unknown, perhaps microbes and chemical exposures, as well as genetic history, may trigger the disease occurrence. The updated scenario also depicted the interconnection between oxidative stress and pulmonary sarcoidosis. Thus, the therapeutic value of the geneti

Unique Profile of Proinflammatory Cytokines in Plasma of Drug-Naïve Individuals with Advanced HIV/TB Co-Infection.

HIV-1 infection is characterized by aberrant immune activation, and infection with M. tuberculosis by an unbalanced production of proinflammatory cytokines. The expression of these cytokines in HIV-1/TB coinfection is still understudied. Here, we aimed to compare the production of proinflammatory cytokines in drug-naive patients coinfected with HIV-1 and M. tuberculosis (HIV/TB) compared to patients with respective monoinfections. Plasma samples of patients with HIV/TB coinfection (n = 36), HIV-1 monoinfection (n = 36), and TB monoinfection (n = 35) and healthy donors (n = 36) were examined fo
