Case Report: Atypical cat-scratch disease presenting as referred shoulder pain and deep lymphadenopathy in a pediatric patient.

BACKGROUND: Cat-scratch disease (CSD) is a zoonotic infection attributable to Bartonella henselae. Typical clinical manifestations include regional lymphadenopathy accompanied by primary cutaneous lesions that develop following exposure to cats. Although most patients present with typical regional lymphadenopathy, a subset of patients may develop atypical manifestations, which can complicate diagnosis. Owing to the nonspecific clinical features, CSD is prone to misdiagnosis or diagnostic omission. This report describes a pediatric case of atypical cat-scratch disease characterized by initial s

Omission of Axillary Surgery in Patients with Invasive Lobular Carcinoma of the Breast: Rates of Nodal Involvement in Clinically Node Negative Patients and Impact on Recurrence in a Single Institution Analysis.

BACKGROUND: Recent randomized trials such as INSEMA and SOUND identified patients with breast cancer for whom axillary surgery omission appears to have no detrimental oncologic impact. However, some have questioned the safety of omitting axillary surgery in patients with invasive lobular carcinoma (ILC), the most common special histology type. METHODS: We analyzed a prospectively maintained institutional ILC database containing 1029 tumor records. We identified those meeting INSEMA and SOUND criteria for axillary surgery omission (clinically node negative with axillary ultrasound, cT1-2 for I

Lymphovascular Invasion in SOUND-Eligible Breast Cancer: Implications for Occult Axillary Nodal Metastasis.

BACKGROUND: The SOUND randomized controlled trial demonstrated that omitting sentinel lymph node biopsy (SLNB) is noninferior to performing SLNB in early-stage breast cancer. Lymphovascular invasion (LVI), however, is an adverse pathologic feature associated with axillary nodal metastasis. In this study, we evaluated the association of LVI with occult axillary nodal metastasis in early-stage, SOUND-eligible breast cancer. PATIENTS AND METHODS: We retrospectively reviewed patients with cT1N0 breast cancer from 2011 to 2025 who underwent upfront breast-conserving surgery and lymph node surgery

Nodal Pathologic Response After Neoadjuvant Systemic Therapy in Clinically Advanced Node-Positive (cN2 and cN3) Breast Cancer.

BACKGROUND/OBJECTIVE: Patients with clinically advanced node-positive (cN2/3), non-metastatic breast cancer have traditionally undergone axillary lymph node dissection (ALND) after neoadjuvant systemic therapy (NST). Randomized de-escalation trials have excluded cN2/3 disease. However, some of these patients achieve nodal pathologic complete response (ypN0), suggesting potential for less extensive axillary surgery. We aimed to determine ypN0 rates and identify factors associated with nodal response. METHODS: The National Cancer Database (NCDB, 2004-2022) was analyzed. Female patients with cN2

Prospective Clinical Trial on Expanding the Eligibility Criteria for Targeted Axillary Dissection after Neoadjuvant Systemic Therapy in Breast Cancers with Advanced Nodal Disease.

BACKGROUND: Targeted axillary dissection (TAD) is an established axillary restaging approach in breast cancer after neoadjuvant chemotherapy (NAC), but evidence to date has focused largely on clinical N1 disease with limited nodal involvement. For higher nodal burden patients, axillary dissection (AD) is still frequently performed despite high axillary pathologic complete response (pCR) rates. We hypothesized that TAD is noninferior to AD for nodal staging in patients with advanced nodal disease. PATIENTS AND METHODS: Patients with higher nodal burden breast cancer planned for NAC followed by

Assessing the performance of deep learning and hand-crafted radiomics models using MRI and ultrasound in predicting axillary lymph node status in breast cancer: a systematic review and meta-analysis.

BACKGROUND: Axillary lymph node metastasis (ALNM) is a critical prognostic factor in breast cancer. While sentinel lymph node biopsy remains the gold standard, conventional imaging relies on operator expertise with variable diagnostic accuracy. This meta-analysis evaluates the diagnostic performance of deep learning (DL) and hand-crafted radiomics (HCR) models using MRI and ultrasound (US) for ALNM prediction. METHODS: Literature search was conducted across four databases up to November 2024. Studies assessing DL or HCR models for ALNM prediction using MRI or US, with histopathological confir

Axillary Surgery Omission Versus SLNB Versus ALND in Early Breast Cancer: Survival and a Prediction Tool.

INTRODUCTION: Randomized trials support omission of selected axillary procedures in carefully defined early breast cancer populations. Whether these trial findings translate to broader real-world practice remains uncertain, particularly among patients who do not undergo any axillary surgery. METHODS: We analyzed women aged 18-85 y with surgically treated T1/T2N0M0 invasive breast cancer in the Surveillance, Epidemiology, and End Results database from 2018 to 2020. Patients were classified according to the number of lymph nodes examined as no axillary surgery (NAS; 0 nodes), sentinel lymph nod

Can Axillary Positivity Be Predicted Using Hemogram Parameters in Patients with Breast Cancer at the Time of Diagnosis?

BACKGROUND: Axillary lymph node status is a key prognostic factor in breast cancer and guides treatment decisions. Sentinel lymph node biopsy (SLNB) is the standard staging method in clinically node-negative patients; however, it remains invasive. Inflammatory indices derived from complete blood counts, including neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), and lymphocyte-to-monocyte ratio (LMR), have been investigated as potential markers of nodal involvement. AIM: This study aimed to evaluate the predictive performance of NLR, PLR, and LMR for axillary lymph nod

Pain relief in plantar fasciitis within 6-8 weeks using (ortho)manual therapy of the foot joints: a prospective cohort study.

The sentinel lymph node biopsy (SLNB) procedure is an alternative method for assessing the axillary lymph node (ALN) status in patients with breast cancer. The SLNB carries the risk of a false-negative result, with patients harboring positive ALNs in the face of a negative SLNB examination. In addition, the significance of a SLNB with cells identified only with keratin or with deposits less than 0.2 mm remains unresolved. We analyzed our SLNB data over the past 5 years in order to determine the relationship between SLN tumor burden and ALN tumor burden. Pathology files for the past 5 years at

Oncological outcomes with and without axillary lymph node dissection in patients with residual micrometastases after neoadjuvant chemotherapy (OPBC-07/microNAC): an international, retrospective cohort study.

BACKGROUND: Despite the paucity of outcome data, axillary lymph node dissection (ALND) is increasingly being omitted in patients with positive sentinel lymph nodes after neoadjuvant chemotherapy, particularly in those with low-volume residual disease. We investigated oncological outcomes in patients with breast cancer and residual micrometastases in the sentinel lymph nodes treated with or without ALND. METHODS: OPBC-07/microNAC was a retrospective cohort study, using data obtained from the institutional databases of 84 cancer centres in 30 countries. Patients aged 18 years or older with clin

Real-world Outcomes of Postoperative Management for Sentinel Lymph Node-positive Breast Cancer Without Intraoperative Assessment: A Single-institution Retrospective Study.

BACKGROUND/AIM: Sentinel lymph node biopsy (SLNB) has become the standard procedure for axillary staging in clinically node-negative breast cancer. Traditionally, axillary lymph node dissection (ALND) has been performed when intraoperative pathological assessment revealed sentinel lymph node (SLN) metastasis. However, growing evidence suggests that postoperative radiotherapy and systemic therapy may safely replace ALND in selected patients, challenging the clinical necessity of intraoperative SLNB assessment. PATIENTS AND METHODS: We retrospectively reviewed 540 patients with stage I-III prim

Preoperative assessment of axillary lymph node tumor burden in cT1-2N0 breast cancer patients with a modality-adaptive network based on sentinel lymph node ultrasound images.

To determine a direct method for diagnosing axillary lymph node (ALN) tumor burden preoperatively in cT1-2N0 breast cancer patients, we developed and validated a deep learning (DL) model based on ultrasound (US) images of sentinel lymph nodes (SLNs) detected by contrast-enhanced lymphatic ultrasound (CEUS). Women with cT1-T2N0 breast cancer who received CEUS were enrolled prospectively from Peking Union Medical College Hospital between April 2020 and July 2021 and from Sichuan Cancer Hospital between April 2022 and July 2022. Heavy ALN tumor burden was defined as > 2 metastatic lymph nodes acc

Robot-assisted Axillary Lymph Node Dissection for Axillary Lymph Node Metastasis in Breast Cancer.

Current experience with robotic-assisted axillary lymph node dissection (R-ALND) is limited. Axillary lymph node dissection is essential for patients with pathological N2-3 disease, but conventional open surgery carries a risk of injury to the intercostobrachial nerve, leading to sensory deficits, pain, and reduced quality of life. R-ALND may enhance precision through minimally invasive techniques, potentially reducing complications. Patients with breast cancer who underwent R-ALND at our institution between March 2024 and March 2025 were enrolled and analyzed. The procedure used the da Vinci
