Robot-assisted Axillary Lymph Node Dissection for Axillary Lymph Node Metastasis in Breast Cancer.

Current experience with robotic-assisted axillary lymph node dissection (R-ALND) is limited. Axillary lymph node dissection is essential for patients with pathological N2-3 disease, but conventional open surgery carries a risk of injury to the intercostobrachial nerve, leading to sensory deficits, pain, and reduced quality of life. R-ALND may enhance precision through minimally invasive techniques, potentially reducing complications. Patients with breast cancer who underwent R-ALND at our institution between March 2024 and March 2025 were enrolled and analyzed. The procedure used the da Vinci

Assessing Nodal Positivity in Men 70 Years of Age and Older With Clinically Node Negative Hormone-Receptor Positive Breast Cancer to Guide Axillary Intervention.

BackgroundChoosing Wisely Guidelines recommend against routine sentinel lymph node biopsy (SLNB) for early-stage, hormone-receptor positive (HR+), clinically node negative (cN0) breast cancer (BC) in elderly women. To determine the applicability of this guideline to men, we evaluated pathologic nodal positivity (pN+) between men and women with low-risk HR+, cN0 BC.MethodsThe National Cancer Database was queried for patients ≥70 years who underwent surgical resection for low-risk, HR+, cN0 BC. Low-risk was defined as grade 1, cT1mi-T1c or grade 2, or cT1mi-T1b. pN+ was evaluated and compared by

Clinical efficacy of gasless trans-subclavian approach and trans-axillary approach endoscopic thyroidectomy for papillary thyroid carcinoma: a prospective randomized study.

BACKGROUND: Endoscopic technology continues to evolve, expanding the clinical application spectrum of various endoscopic thyroidectomy approaches. However, comparative studies between gasless trans-subclavian approach endoscopic thyroidectomy (GTSA-ET) and gasless trans-axillary approach endoscopic thyroidectomy (GTAA-ET) remain notably scarce in the current literature. In this study, we aimed to compare the clinical efficacy of GTSA-ET and GTAA-ET in the treatment of papillary thyroid carcinoma (PTC). METHODS: This study was conducted on 160 patients with cN0-stage PTC who were enrolled betwe

Marking Techniques for Target Lymph Nodes in Node-Positive Breast Cancer Treated With Neoadjuvant Therapy in the AXSANA/EUBREAST-03/AGO-B-053 Study.

PURPOSE: Surgical axillary staging in patients with node-positive breast cancer (BC) who converted to clinical node negativity through neoadjuvant chemotherapy (NACT) has changed significantly in recent years. Targeted axillary dissection (TAD) and target lymph node (TLN) biopsy (TLNB) became increasingly popular. However, data comparing marking techniques for the TLN are limited. Here, we evaluate marking techniques in the largest prospective cohort worldwide. MATERIALS AND METHODS: Among patients from the ongoing prospective multicenter AXSANA (EUBREAST-03) study who received TLN marking an

Diagnostic accuracy of FDG-PET-CT to predict axillary lymph node response after neo-adjuvant chemotherapy in lymph node-positive breast cancer patients.

BACKGROUND: In certain cases, some institutions use fluorodeoxyglucose positron emission tomography combined with CT (FDG-PET-CT) scans to assess response to neo-adjuvant chemotherapy (NAC) in lymph node-positive breast cancer (cN+), to determine the extent of surgery and subsequently radiotherapy. In this study, we assessed the diagnostic accuracy of FDG-PET-CT to determine the axillary response to NAC using the histopathology results as the golden standard. METHODS: Between 2016 and 2022, all women with cN+ breast cancer receiving NAC and axillary surgery were retrospectively identified. Pat

[Metastases of endometrioid adenocarcinoma of the endometrium in the mammary gland and axillary lymph nodes].

Breast tumors are one of the most common locations of malignant neoplasms. Every year, new aspects of the pathogenesis of breast cancer are discovered; along with genetic, hormonal and other factors, the issue of implantation and metastatic damage to the organ by tumors of other localizations is being increasingly discussed. With evolving diagnostic methods, more and more reports are emerging indicating that sometimes a breast tumor is not a primary lesion, but a site of metastasis. In this paper, the authors present a clinical case of metastasis of endometrioid adenocarcinoma of the endometri

A clinicopathological and spatial feature-based nomogram for individualized prediction of axillary lymph node involvement in breast cancer.

OBJECTIVES: This study sought to identify key independent determinants of axillary lymph node metastasis (ALNM) in breast cancer, focusing on tumor spatial distribution and clinicopathological characteristics. A preoperative model was established to predict ALNM risk and guide individualized axillary treatment strategies. To assess the model’s clinical practicality, we validated it further via decision curve analysis (DCA) and clinical impact curves (CIC), with results highlighting its relevance for daily clinical work. METHODS: We retrospectively included 446 breast cancer patients who receiv

Predictive value of node-RADS scoring system for axillary lymph node metastasis in breast cancer.

BACKGROUND: We aimed to evaluate the predictive value of preoperative Node-RADS determination at imaging for axillary lymph node (ALN) involvement in cases of breast cancer. MATERIALS AND METHODS: Node-RADS was determined in all cases using preoperative breast magnetic resonance imaging (MRI) by two radiologists blinded to pathologic results. The ROC curves and AUCs depicted the overall diagnostic performance of the Node-RADS score for lymph node involvement. In addition, the presence of a flare-like perinodal signal was also evaluated as an imaging feature suggestive of extranodal extension.

Axillary staging in post-neoadjuvant chemotherapy breast cancer patients: prognostic analysis and validation of axillary nodal metrics and comparison of the pathological node staging versus the lymph node ratio systems in a 169-case series.

BACKGROUND: Neoadjuvant chemotherapy (NAC) is considered disruptive for axillary nodal staging in breast cancer patients, by altering the total number of excised nodes, and the number of positive and negative nodes. We aimed to identify the axillary variable having the strongest association with survival, and also to compare the prognostic efficacy of the pathological node (pN) staging versus the lymph node ratio (LNR) systems in post-NAC cases. METHODS: In this retrospective single-center study of 169 post-neoadjuvant chemotherapy breast cancer patients, three absolute axillary nodal counts (
