The Efficacy of Calcium Dobesilate Alone or with Vascularized Lymph Node Transfer in Lymphedema Treatment: A Preliminary Study.

BACKGROUND: Lymphedema remains an uncurable disease, despite recent advancements in pharmacological research and microsurgical techniques. Calcium dobesilate is an angioprotective and antioxidant drug. This study aimed to compare the use of calcium dobesilate alone or in combination with vascularized lymph node transfer in the treatment of lymphedema. METHODS: Twenty-four rats were divided into four groups. In all groups, a lymphedema model was created in the left lower limb by excision of the inguinal and popliteal lymph nodes, followed by irradiation. In group 1, no treatment was performed.

Clinical value of MRI in central lymphatic vessel evaluation and grading in primary lower-extremity lymphedema.

Introduction: The manifestations of the central lymphatic vessels in primary lower-extremity lymphedema (LEL) remain incompletely understood. Advanced magnetic resonance imaging (MRI) techniques offer the potential to noninvasively evaluate central lymphatic vessels and their relationship with disease severity. Our objective was to investigate the differences in imaging performance of central lymphatics and inguinal lymph nodes in primary LEL of different clinical stages, and to assess the ability of MRI to grade primary LEL. Methods: Patients were staged according to the International Society

A New Pearl in Chronic Venous Disease Pathophysiology-The Duplex Ultrasound and the Elastographic Features of Lymph Nodes Varicose Veins in the Groin.

Background/Objectives: Chronic venous disease (CVD) is a prevalent condition marked by valve dysfunction and increased pressure in lower limb veins. The trans-nodal veins in the inguinal region and Scarpa triangle, which connect the superficial and deep venous systems, provide new insight into venous insufficiency pathways. While they function normally in healthy individuals, they can become dilated in chronic venous disease or following surgery. The purpose of this study was to provide an ultrasonographic anatomical description of intranodal varicose veins and to assess possible changes in th

A Practical Algorithm for Donor- and Recipient-site Selection in Vascularized Lymph Node Transfer in Extremity Lymphedema.

BACKGROUND: Vascularized lymph node transfer (VLNT) involves the autologous transplantation of functioning lymph nodes to a lymphedematous region. Despite various donor and recipient sites, comparative effectiveness and complication rates remain understudied. This study aims to develop an evidence-based algorithm for donor- and recipient-site selection in VLNT. METHODS: A prospectively maintained database was reviewed for patients who underwent unilateral VLNT for chronic extremity lymphedema between January 1, 2016, and December 31, 2023. Surgical effectiveness was assessed using circumferen

Real-World Outcomes of Mycosis Fungoides and Site-Specific Nodal Involvement: A Registry-Based Study of 25 467 Patients.

Mycosis fungoides (MF) is a rare extranodal T-cell lymphoma with primary cutaneous involvement. While it is the most common primary skin lymphoma, large-scale real-world data defining survival benchmarks and the prognostic significance of specific nodal sites remain limited. To address this, we conducted a retrospective cohort study using the TriNetX global health research network and identified 25 467 patients with MF. The 5- and 10-year OS for the overall cohort was 83.4% and 74.7%, respectively. Patients with disease limited to the skin had 5- and 10-year OS rates of 86.1% and 78.0%, respec

Merkel Cell Carcinoma of the Thigh Presenting as a Hemorrhagic Mass: A Rare Case Report and Literature Review.

BACKGROUND: Merkel cell carcinoma (MCC) is a rare, aggressive primary cutaneous neuroendocrine carcinoma with a marked propensity for early regional lymph node metastasis. Although MCC most often arises on sun-exposed head and neck skin in older adults, tumors of the lower extremity are uncommon and may be mistaken for benign hemorrhagic lesions. CASE PRESENTATION: A 54-year-old woman developed a rapidly enlarging, hemorrhagic mass in the left suprapatellar thigh. Magnetic resonance imaging demonstrated an extracompartmental subcutaneous soft-tissue mass without quadriceps muscle invasion. Wi

Vascularized Lymphatic Tissue Transfer for Treatment of Lower Limb Lymphedema: Does the Recipient Site Matter?

INTRODUCTION: Vascularized lymph node transfer (VLNT) is a pivotal component in lower limb lymphedema (LLL) treatment. We aimed to assess the association of using the groin and ankle as single or multilevel recipient sites on both limb volume reduction and recipient site morbidity. METHODS: A total of 51 patients with LLL received VLNT between 2016 and 2023. We assessed volume preoperatively and postoperatively over a 12-mo period. We compared limb volume changes in primary lymphedema (PLE) and secondary lymphedema (SLE) subgroups depending on recipient sites: groin, ankle, and multilevel (gr

A focused systematic review and meta-analysis of robot-assisted inguinal lymphadenectomy versus open inguinal lymph node dissection in penile cancer.

This focused systematic review and meta-analysis evaluated robot-assisted inguinal lymphadenectomy (RAIL) versus open inguinal lymph node dissection (OILND) in penile cancer, aiming to provide robot-specific comparative estimates rather than a broad minimally invasive synthesis. PubMed, EMBASE, Web of Science, and the Cochrane Library were searched through December 2025 for comparative studies. All meta-analyses were conducted using STATA 18, employing random-effects modeling. Binary outcomes were summarized using odds ratios (ORs), and continuous outcomes were presented as weighted mean diffe

Primary Tuberculous Intramuscular Abscess in a Diabetic Elderly Male: An Unusual Case Report.

INTRODUCTION: Tuberculosis (TB) primarily affects the lungs, but extrapulmonary manifestations, such as musculoskeletal TB, account for 15-20% of cases. Isolated intramuscular TB abscesses are exceedingly rare cases of extrapulmonary TB. Predisposing factors, such as diabetes mellitus, immunosuppression, and advanced age, increase the risk of such atypical presentations. This report presents a rare case of a primary tuberculous intramuscular abscess in an elderly diabetic male, emphasizing diagnostic challenges and the importance of a multidisciplinary approach. Minimizing Lymphatic Morbidity: Incidence of Lower Extremity Lymphedema After vNOTES-Assisted Sentinel Node Mapping in Endometrial Cancer.

Background: Endometrial cancer is the most common gynecologic malignancy in developed countries. Sentinel lymph node (SLN) mapping has emerged as a less invasive alternative to systematic lymphadenectomy and is increasingly incorporated into surgical staging algorithms. Vaginal natural orifice transluminal endoscopic surgery (vNOTES) provides transvaginal access to the retroperitoneum and may facilitate SLN mapping while potentially reducing postoperative morbidity, including lower extremity lymphedema (LEL). Objective: This study aimed to evaluate the feasibility of vNOTES hysterectomy with b

Long-Term Follow-Up of Single-Step Free Vascularized Lymph Node Transfer for the Management of Combined Genital and Lower Extremity Lymphedema: A Case Report.

The occurrence of genital lymphedema with lower extremity involvement is rare. There is no standard approach in the management of combined genital and lower extremity lymphedema (CGLL). The limited literature available on the management of CGLL reveals the use of multiple procedures, including vascularized lymph node transfer (VLNT), lymphovenous anastomosis (LVA), and debulking. These approaches individually target the lower extremity or genital region. There is no single-step procedure for managing the CGLL, which involves two different anatomical regions. In this case report, we describe a

Amyotrophic lateral sclerosis in a patient with chronic lymphocytic leukaemia and drug related sarcoid-like reaction.

Sarcoid-like reaction is an immunological reaction that can affect lymph nodes and organs but does not meet the diagnostic criteria for systemic sarcoidosis. Anti-CD20 auto-antibodies have been reported to be responsible for such reactions. There are several reported associations between Chronic lymphocytic leukaemia (CLL), Amyotrophic lateral sclerosis (ALS) and Sarcoid-like reactions (SLR). We report a case of ALS developing in a patient with treated CLL and drug related SLR one day after exposure to Venetoclax and Rituximab. Detection of Efferent Lymphatic Vessels Using Gamma Probe Guidance for Efferent Lymphatic Vessel Anastomosis.

Efferent lymphatic vessel anastomosis (ELVA) is a microsurgical technique used to anastomose efferent lymphatic vessels from the inguinal lymph nodes (LNs) to the veins in patients with lower extremity lymphedema (LEL). Intraoperative detection of inguinal LNs is important when performing ELVA. Identifying inguinal LNs in patients with mild LEL is easy on palpation, ultrasonography, and indocyanine green (ICG) lymphography because the LNs are large and have good ICG uptake. In contrast, inguinal LNs of patients with advanced LEL are small and have minimal to no ICG uptake owing to lymphatic de
