A Comprehensive Landscape of B Symptoms in Mantle Cell Lymphoma Reveals Heterogeneity Associated With Different Primary Sites.

IntroductionMantle cell lymphoma (MCL) constitutes an aggressive subtype of B-cell lymphoma and demonstrates significant clinical and biological heterogeneity. B symptoms are important clinical indicators in tumors. In the present study, we investigated the frequency and prognostic significance of B symptoms among different primary sites of MCL.MethodWe conducted an observational study of 2,025 MCL patients from the SEER database. We analyzed the frequency of B symptoms at different primary sites and evaluated their impact on prognosis.ResultsThe highest incidence of B symptoms was observed in

A case report of tuberculous fibrosing mediastinitis: focusing on the diagnostic value of three-dimensional reconstruction from pulmonary contrast-enhanced CT and bronchoscopic features.

BACKGROUND: Fibrosing mediastinitis (FM) is a rare disease characterized by excessive fibrotic tissue proliferation in the mediastinum, often leading to compression of vital structures. Tuberculosis is a common cause in certain regions. Diagnosis is challenging due to non-specific symptoms. This report explores the integrated diagnostic value of advanced three-dimensional (3D) computed tomography (CT) reconstruction and bronchoscopy in tuberculous fibrosing mediastinitis (TFM). CASE DESCRIPTION: A 69-year-old female presented with a 2-year history of progressive exertional dyspnea. Chest cont

The unusual first sign of presentation of hepatocellular carcinoma: a rare case report.

BACKGROUND: Hepatocellular carcinoma (HCC) is the most common primary liver malignancy, characterized by aggressive biological behavior and a high propensity for extrahepatic spread. The most common sites of metastasis are the lungs, regional lymph nodes, bones, adrenal glands, and peritoneum. In contrast, the paranasal sinuses are an unusual site for metastatic HCC, and involvement of the sphenoid sinus represents an even rarer event. CASE DESCRIPTION: We report the case of a 67-year-old male patient who presented with transient diplopia, left eyelid ptosis, and refractory epistaxis as the i

SMARCB1 (INI-1)-deficient carcinoma with eosinophilia and cervical lymph node metastasis of unknown primary: a case report and literature review.

BACKGROUND: Cancer of unknown primary (CUP) represents a heterogeneous group of metastatic malignancies in which the primary site remains unidentified despite comprehensive clinical, laboratory, radiological, and pathological evaluation. CUP is generally characterized by aggressive biological behavior, poor prognosis, and the absence of standardized treatment strategies. Tumor-associated blood eosinophilia (TABE) is relatively uncommon and typically occurs after tumor dissemination, often indicating an unfavorable prognosis. SMARCB1 (INI-1) is a core subunit of the switch/sucrose non-fermentab

Torsion of the Vermiform Appendix in an 18-Day-Old Neonate: A Case Report from Romania and Review of the Literature.

Background and Clinical Significance: Torsion of the vermiform appendix is a rare condition with a clinical presentation closely resembling acute appendicitis, while preoperative investigations are of limited value in distinguishing between the two entities. In most cases, the definitive diagnosis is made incidentally during surgery. Case Presentation: The authors present the case of an 18-day-old female neonate who presented with marked abdominal distension, diffuse spontaneous and palpation-induced abdominal pain, guarding, and signs of peritoneal irritation. The clinical manifestations and

Spontaneous malignant trichoepithelioma in a female CD1/ICR mouse.

A spontaneous malignant trichoepithelioma is diagnosed in a CD1/ICR outbred, six-month-old, intact, nulliparous female mouse belonging to a breeding colony. Postmortem examination revealed a solitary, locally extensive, tumour mass, 3.5 × 3,2 centimetres in size, affecting the skin and subcutaneous tissues on the left side of the neck, shoulder, and chest. The overlying skin was focally ulcerated. At sagittal section, the mass was pseudolobulated, grayish pink in colour with soft consistency and adhered to deeper tissues. Regional lymph nodes were not enlarged, and no other lesions were found

Mandibular lymph node tuberculosis in an 11-year-old child managed without surgery: a case report.

INTRODUCTION AND IMPORTANCE: Tuberculosis (TB) is a chronic infectious disease that can affect multiple anatomical sites. Although involvement of the maxillofacial region is rare, mandibular lymph node TB is particularly uncommon and may pose diagnostic challenges. CASE PRESENTATION: An 11-year-old male patient presented with a persistent swelling on the right side of the mandible for 8 months. Initial clinical assessment suggested a benign condition, and multiple courses of antibiotics failed to produce improvement. Histopathological examination following surgical intervention revealed granu

Clinical Cognition and Practice in Anti-Interferon-γ Autoantibody-Associated Immunodeficiency Syndrome.

Adult-onset immunodeficiency syndrome mediated by anti-interferon-gamma autoantibody (AIGA) is a rare disorder that has garnered increasing recognition in recent years. The condition is most prevalent among adults in Southeast Asia. It is characterized by recurrent disseminated opportunistic infections, frequently involving multiple organ systems such as the lungs, lymph nodes, bones, and skin. The pathogenesis is primarily attributed to the presence of neutralizing AIGAs, which impair the IFN-γ-mediated JAK-STAT1 signaling pathway, leading to compromised host defense against intracellular pat

Primary pancreatic histiocytic sarcoma with peritoneal sarcomatosis in a cat.

A 12-year-old neutered male Domestic Shorthair Cat, negative for feline immunodeficiency virus and feline leukaemia virus, was presented to the veterinary hospital with a 3-week history of anorexia, lethargy, abdominal pain and intermittent vomiting. Abdominal ultrasonography revealed a heterogeneous, poorly demarcated mass in the right mesogastric region, involving the pancreas and adjacent organs. Cytological evaluation of the abdominal fluid revealed a neoplastic effusion consistent with carcinomatosis. Given the poor prognosis, the cat was euthanized and submitted for necropsy, which revea

Uncovering Sex and Gender Differences in Sarcoidosis: A Systematic Review of Current Evidence.

Introduction: Sarcoidosis is a systemic granulomatous disorder classified among interstitial lung diseases (ILDs). While the lungs and intrathoracic lymph nodes are most affected, the disease can involve multiple organs. The heterogeneity of clinical presentation arises from complex interactions between environmental exposures and immune responses in genetically susceptible individuals. Sex-dependent genetic variations are associated with differences in phenotype and organ localization. Gender-related factors also influence the impact of sarcoidosis on quality of life and health perception, co

Coexisting Sarcoid Reaction and Sarcoidosis Requiring Differentiation from Pancreatic Adenocarcinoma Recurrence: A Case Report.

INTRODUCTION: Pancreatic ductal adenocarcinoma (PDAC) is one of the most severe malignant tumors. The recurrence rate is high, even after radical surgery. A sarcoid reaction is a histological finding that occurs in the lymph nodes in response to malignant tumors, infections, or foreign substances. Its histopathology is similar to that of sarcoidosis. Here, we report a case of coexisting sarcoid reaction and sarcoidosis that required differentiation from PDAC recurrence. CASE PRESENTATION: A 65-year-old woman was diagnosed with locally advanced PDAC. She underwent modified FOLFIRINOX (folinic

Case Report: Benign colonic schwannoma with restricted diffusion mimicking malignancy.

INTRODUCTION: Colonic schwannoma is a rare gastrointestinal mesenchymal tumor that is usually benign but may be difficult to distinguish from malignancy before surgery. This case is notable because a benign colonic schwannoma showed both pronounced restricted diffusion on MRI and prominent mesocolic lymphadenopathy, creating a strong radiologic impression of malignancy. CASE PRESENTATION: A 43-year-old woman presented with a 2-day history of abdominal pain accompanied by altered bowel habits. Colonoscopy revealed a large spherical submucosal mass in the transverse colon. Biopsy suggested a ne

Small-cell neuroendocrine carcinoma of the descending colon in a young adult: a case report.

Neuroendocrine neoplasms (NENs) are a heterogeneous group of tumors arising from neuroendocrine cells with malignant potential. NENs of the colorectum are uncommon, and colonic neuroendocrine neoplasms (CNENs) are particularly rare. Here, we report the diagnostic and therapeutic course of a man in his 25-year-old with small-cell neuroendocrine carcinoma (SCNEC) of the descending colon. The patient presented with sudden, severe abdominal pain accompanied by nausea and vomiting. Computed tomography (CT) revealed circumferential wall thickening of the descending colon, blurring of the surrounding

Metastatic Medullary Thyroid Carcinoma in Multiple Endocrine Neoplasia Type 2B (MEN 2B) With RET M918T Mutation: Challenges in Long-Term Management and Targeted Therapy.

Multiple endocrine neoplasia type IIB (MEN2B) is a rare hereditary cancer syndrome characterized by medullary thyroid carcinoma (MTC), pheochromocytoma, and distinctive mucosal neuromas. MEN2B-associated MTC is most often caused by RET M918T mutations and confers an earlier and more aggressive disease progression. The aggressive nature of RET M918T-mutated MEN2B emphasizes the necessity of vigilant lifelong surveillance. It also highlights the real-world challenges of maintaining continuity of targeted therapy, where treatment interruptions may compromise disease control. This reports the case

Off the Map: A Ten-Year Review of Coccidioidomycosis in Southeastern Michigan.

Recent studies suggest coccidioidomycosis can be found outside regions not considered to be endemic for Coccidioides species. We reviewed our experience with coccidioidomycosis at a large quaternary medical center in southeastern Michigan, an area not typically considered to be endemic for this infection. In the last decade, we cared for 18 patients with proven (11) or probable (7) coccidioidomycosis. All patients had a history of travel or prior residence in areas known to be endemic for Coccidioides species; for 16, the presumed source of exposure to Coccidioides was in southern Arizona. The
