Impact of intraoperative ultrasound staging on surgical strategy and recurrence after resection for intrahepatic cholangiocarcinoma.

BACKGROUND: Intraoperative ultrasound (IOUS) is central to liver surgery, but its incremental staging value in intrahepatic cholangiocarcinoma (ICC) remains poorly defined. METHODS: We retrospectively analyzed a prospectively maintained database of patients considered for curative-intent liver resection for suspected ICC from January 2004 to December 2024. The primary endpoint was the incremental intraoperative staging yield of IOUS/laparoscopic ultrasound (LUS), defined as the proportion of resected patients with at least 1 additional pathologically confirmed ICC nodule. Secondary endpoints

A stress granule-associated RNA-binding protein FAM120A drives cisplatin resistance in non-small cell lung cancer.

Cisplatin-based chemotherapy is a standard treatment for non-small cell lung cancer (NSCLC), but drug resistance poses a major clinical challenge. Stress-adaptive mechanisms, such as stress granule (SG) formation, are increasingly recognized alternative pathways that facilitate cancer cell survival. Here, we identify the RNA-binding protein, family with sequence similarity 120A (FAM120A), as an SG-associated factor that drives cisplatin resistance in NSCLC. FAM120A expression was markedly elevated in cisplatin-resistant NSCLC cell lines and clinical tumor specimens and was essential for SG for

Enozertinib Is a Selective, Brain-Penetrant EGFR Inhibitor for Treating Non-Small Cell Lung Cancers with EGFR Exon 20 and Atypical Mutations.

UNLABELLED: EGFR mutations are common oncogenic drivers in non-small cell lung cancer (NSCLC), and approximately half of patients develop brain metastases over the course of their disease. Patients with nonclassic EGFR mutations, such as insertions in exon 20, are a high unmet need with a worse prognosis compared with patients with classic EGFR mutations. Here, we describe the discovery and development of enozertinib (formerly ORIC-114), a highly brain-penetrant, orally bioavailable, irreversible inhibitor that targets EGFR exon 20 mutations with unparalleled kinome selectivity. Preclinical st

CD146+ pericyte-like lung cancer brain metastatic stem cells promote tumor angiogenesis through dual regulatory effects on the VEGF/VEGFR axis.

Rationale: Anti-angiogenic therapy is indispensable for the treatment of non-small cell lung carcinoma (NSCLC)-derived brain metastasis (BrM). Targeting vascular endothelial growth factor (VEGF) and its receptor (VEGFR) is the most effective strategy against angiogenesis. However, bevacizumab (Bev) shows limited therapeutic effects on NSCLC BrM. The plasticity of cancer stem cells (CSCs) has been found to drive therapeutic resistance via the "mimicry" behavior. Methods: CD146+ BrM-CSCs were validated in clinical tissues and organoids using immunostaining assays. The ability of CD146+ BrM-CSCs

Aggregation of Platelets and Human EGFR Mutant Lung Adenocarcinoma Cells Under Flow Is Governed by Shear Accumulation.

BACKGROUND: Circulating tumor cells (CTCs) utilize platelets to withstand hemodynamic stress and evade immune clearance; however, the mechanisms driving platelet-cancer cell aggregation under flow remain unclear. OBJECTIVE: We investigated which physical quantity-shear rate, exposure time, or an integrated metric-governs the aggregation and identified the key adhesion molecules involved. METHODS: Human platelets and EGFR-mutant lung adenocarcinoma PC-9 cells were fluorescently labeled, mixed at a 10:1 ratio, and subjected to mechanical stimulation using a vortex mixer or a TA ARES G2 rheomet

Generative AI-based low-dose digital subtraction angiography for intra-operative radiation dose reduction: a randomized controlled trial.

Digital subtraction angiography (DSA) devices guide procedures across numerous diseases, performed on more than 100,000 patients daily worldwide. However, these procedures expose patients and healthcare providers to radiation, increasing the risk of health issues. Despite many low-dose DSA imaging methods proposed, none have been prospectively clinically validated. In this study, 46,829 patients (over 5 million DSA images) from 70 centers were used to iterate our previously developed generative artificial intelligence system (named GenDSA-V2). A total of 1,068 patients (533 in intervention arm

TGFβ induces an atypical EMT to evade immune mechanosurveillance in lung adenocarcinoma dormant metastasis.

Different forms of epithelial-to-mesenchymal transition (EMT) manifest during tumor progression. Little is known about the mechanistic basis and functional role of these distinct EMTs. We explored this question in lung adenocarcinoma (LUAD) primitive progenitors, which are competent to enter dormancy in response to transforming growth factor-β (TGFβ) upon metastatic dissemination. The TGFβ response in these cells includes growth arrest and a full EMT that subsequently transitions into an atypical mesenchymal state of round morphology and lacking actin stress fibers. TGFβ drives this transition

APMAP regulated by YY1 transcription mediates ferroptosis and metastasis in non-small cell lung cancer through the PI3K/AKT/mTOR pathway.

Non-small cell lung cancer (NSCLC) is a leading cause of cancer-related deaths worldwide. Adipocyte Plasma Membrane Associated Protein (APMAP) is a glycosylated type II transmembrane protein, and APMAP induces malignant metastasis of colorectal cancer. However, the role and mechanism of APMAP in NSCLC remain unknown. We aimed to study the APMAP regulation on NSCLC. APMAP mRNA levels in NSCLC tissues were tested by quantitative reverse transcription-PCR (qRT-PCR). APMAP protein levels in NSCLC tissues and cells were determined via Western blot. Also, the association between APMAP expressions an

Interrelationship Between Patient and Family Caregiver Outcomes Following Lung Cancer Surgery: A Secondary Analysis of a Randomized Controlled Trial.

INTRODUCTION: While surgery is considered a standard treatment for patients diagnosed with early-stage lung cancer, it often brings significant challenges for patients including functional limitations and post-operative symptoms. Family caregivers play a critical role in care coordination, post-operative management, and addressing emotional distress often with limited preparation and support. Despite their importance, few studies have assessed the interrelationship between patient and caregiver outcomes during the peri-operative period. METHODS: This study is a secondary analysis of data from
