NeuroMetBench: patient-aware evidence bounds for transcript-derived metabolic representations across cancer proteogenomic contexts.

Transcript-derived metabolic representations connect gene expression with pathway state, but interpretation depends on sampling unit, endpoint, cohort, and validation evidence. NeuroMetBench is a patient-aware methodology that keeps target quantity, independent unit, endpoint mapping, coverage, uncertainty, transport, numerical diagnostics, and provenance separate rather than collapsing them into a universal score. A frozen three-gene serine score showed RNA-protein rank concordance in 89 lung squamous cell carcinoma (LSCC) patients (Spearman [Formula: see text], 95% confidence interval (CI) 0

THINKERS-X: multisequence MRI radiomics in a mixture-of-experts model for outcome prediction and exploratory dose-scenario evaluation in lung cancer brain metastases treated with radiosurgery.

We developed THINKERS-X: Lung, the eXtended multimodal evolution of THINKERS-Lung, to determine whether multisequence MRI radiomics provides incremental value for lesion-specific outcome prediction and exploratory dose-scenario evaluation after Gamma Knife radiosurgery (GKRS) for lung cancer brain metastases. We performed a retrospective single-center study of 767 patients with 3,728 lung cancer brain metastases treated with GKRS. THINKERS-X preserved the original mixture-of-experts and discrete-time survival architecture while adding a dedicated computational neuroradiology expert. Approximat

MSC C1: A Potential Hub Linking Stromal Plasticity, Immune Regulation, and Patient Survival in Lung Adenocarcinoma.

Bone metastasis of lung adenocarcinoma (LUAD) directly leads to poor prognosis. This study aimed to dissect the stromal landscape of LUAD with bone metastases. Unsupervised clustering of scRNA-seq data from LUAD patients with bone metastasis identifies stromal subsets. Pseudotime analysis combined with CytoTRACE stemness scoring reconstructs mesenchymal stromal cell (MSC) differentiation trajectories, while CellChat characterizes cell-cell communication networks among MSC subpopulations. Differentially expressed genes (DEGs) specific to functionally distinct MSC subsets are subjected to LASSO-

Multimodality salvage treatment incorporating ivonescimab plus chemotherapy for EGFR-mutated NSCLC with leptomeningeal metastases after dose-escalated third-generation EGFR-TKI failure: a retrospective study.

BACKGROUND: Leptomeningeal metastasis (LM) is among the most devastating modes of progression in EGFR-mutated non-small cell lung cancer (NSCLC). Although dose escalation of third-generation EGFR-TKIs has been an important central nervous system (CNS)-directed salvage strategy, therapeutic options after progression on this intensification approach remain limited. METHODS: This single-center retrospective study included consecutive patients treated between September 2024 and December 2025. Eligible patients were adults (≥18 years) with histologically confirmed lung adenocarcinoma, a documented

PRKAR2A/RIIα as a candidate spatial cAMP-PKA signal router in triple-negative breast cancer metastasis: evidence boundaries, competing models and decisive experiments.

Metastatic relapse remains the principal lethal event in triple-negative breast cancer (TNBC), yet the contribution of PRKAR2A, which encodes the type IIα regulatory subunit (RIIα) of protein kinase A (PKA), remains unresolved. RIIα creates a mechanistic duality: it restrains catalytic PKA within the holoenzyme while recruiting type II PKA to A-kinase anchoring protein (AKAP) microdomains. A targeted critical narrative search identified no eligible published study in the searched sources that causally linked PRKAR2A to a TNBC metastasis endpoint. Direct evidence is limited to four association

Extracellular vesicles and immune-glial remodeling in lung cancer brain metastasis: potential intersections with exercise-related myokines.

Lung cancer brain metastasis develops through reciprocal interactions between metastatic tumor cells and a brain-specific vascular, glial, and immune microenvironment. Tumor-derived extracellular vesicles (EVs) can participate in this process by altering blood-brain barrier (BBB) properties, influencing brain-directed distribution, and transferring regulatory cargo to endothelial and myeloid recipient cells. This review integrates direct evidence from non-small cell lung cancer (NSCLC) brain or central nervous system metastasis with related lung cancer EV studies, while distinguishing these fi

Oncogenic mutations in non-small cell lung cancer: from molecular mechanisms to clinical practice.

Targeted therapy has revolutionized the treatment of non-small cell lung cancer (NSCLC) patients harboring oncogenic alterations, and next-generation tyrosine kinase inhibitors (TKIs) significantly increase blood-brain barrier (BBB) penetration, leading to improved disease control and survival benefits. However, several issues remain to be addressed, including the selection of more appropriate and individualized agents, the emergence of TKI resistance, and the challenging management of central nervous system (CNS) progression. In this review, we comprehensively summarize the efficacy and safet

Metastasis Extraction From Non-Small Cell Lung Cancer Clinical Notes Using Large Language Model-Based Classification: Retrospective Comparative Study.

BACKGROUND: Identification of metastasis status in non-small cell lung cancer (NSCLC) is a critical part of understanding disease prognosis, treatment courses, trial eligibility, and population-level cancer surveillance. However, metastasis status is inconsistently recorded in structured cancer registry fields, since manual abstraction of clinical notes is often an error-prone process. This challenge highlights an opportunity for leveraging large language models (LLMs) to conduct high-scale metastasis extraction from real-world clinical documentation. OBJECTIVE: We conducted a retrospective,

Malignant Tracheal Stenosis: A Narrative Review of Current Diagnostic and Therapeutic Approaches.

Background: Malignant tracheal stenosis (MTS) is a rare, life-threatening condition caused by primary tracheal tumor, mainly adenoid cystic carcinoma (ACC) and squamous cell carcinoma (SCC), and more rarely other primary neoplasms such as lymphomas, or local extension/metastasis from adjacent malignancies (thyroid, esophageal, lung, or mediastinal cancers). Accounting for <0.1% of respiratory tumors, MTS presents with progressive respiratory failure, requiring urgent multidisciplinary care involving thoracic surgery, interventional pulmonology, oncology, and intensive care. The cerebrospinal fluid immune microenvironment in lung cancer leptomeningeal metastasis: cellular ecosystems and therapeutic possibilities.

Leptomeningeal metastasis (LM) is a devastating form of central nervous system (CNS) dissemination in lung cancer, particularly non-small cell lung cancer (NSCLC). Current treatment frameworks emphasize CNS drug penetration, oncogenic driver evolution, and intrathecal therapy. Emerging evidence supports viewing the cerebrospinal fluid (CSF) compartment as an active tumor-immune space, but the field remains early, with limited lung cancer-specific mechanistic and single-cell datasets. Lung cancer cells in CSF appear to adapt to low protein content, limited nutrients, fluid shear, weak matrix su
