Malignant Tracheal Stenosis: A Narrative Review of Current Diagnostic and Therapeutic Approaches.

Background: Malignant tracheal stenosis (MTS) is a rare, life-threatening condition caused by primary tracheal tumor, mainly adenoid cystic carcinoma (ACC) and squamous cell carcinoma (SCC), and more rarely other primary neoplasms such as lymphomas, or local extension/metastasis from adjacent malignancies (thyroid, esophageal, lung, or mediastinal cancers). Accounting for <0.1% of respiratory tumors, MTS presents with progressive respiratory failure, requiring urgent multidisciplinary care involving thoracic surgery, interventional pulmonology, oncology, and intensive care. Diagnosis relies on

A multicenter prediction model for false-positive clinical nodal disease in < 4-cm NSCLC: clinical implications for upfront surgery versus neoadjuvant therapy.

BACKGROUND: Perioperative immunochemotherapy is recommended for resectable non-small cell lung cancer (NSCLC) with ≥ 4-cm tumors or clinical nodal involvement (cN(+)). Therefore, accurate preoperative nodal assessment is crucial in < 4-cm tumors, where false-positive cN staging should be carefully considered. This multicenter study aimed to develop and validate a prediction model to identify patients at high risk of cN(+)pN(-) disease. METHODS: The development cohort included 251 patients with tumors < 4 cm who were diagnosed as cN(+) using positron emission tomography and underwent curative

Lymph node yield and station count in early robot-assisted versus established video-assisted resection for non-small cell lung cancer: higher total node counts reflect yield per sampled station.

Whether robot-assisted thoracic surgery (RATS) improves nodal staging over video-assisted thoracoscopic surgery (VATS) early in adoption is unknown. We compared occult nodal upstaging and lymph node (LN) retrieval between early RATS and an established VATS practice by one surgeon, and asked whether total LN count reflects the number of stations sampled. Retrospective single-surgeon cohort of consecutive lung cancer resections with systematic LN dissection (2023-2026). Primary outcome: occult nodal upstaging (pathologic N + in clinically N0 patients). Secondary outcomes: LN yield, distinct stat

Balanced double aortic arch with late-onset symptoms detected on 18F-FDG PET/CT in an octogenarian with advanced prostate cancer: A case report.

RATIONALE: Double aortic arch (DAA) is a rare congenital vascular anomaly usually diagnosed in infancy. Adult or elderly presentation is uncommon, and balanced-type DAA is relatively rare. On oncologic 18F-fluorodeoxyglucose (FDG) positron emission tomography (PET)/computed tomography (CT), a mediastinal tubular structure with blood-pool-level FDG uptake may provide an important clue to a vascular anomaly. PATIENT CONCERNS: An 80-year-old man with biopsy-proven advanced prostate cancer underwent 18F-FDG PET/CT for systemic staging. He had dysuria and back pain for more than 1 year, with mild

Direction of discordance between PET/CT, EBUS-TBNA, and multidisciplinary tumor board nodal assignment in non-small cell lung cancer: A retrospective observational study.

Mediastinal nodal classification in non-small cell lung cancer (NSCLC) at the N0/N1-N2/N3 treatment-defining boundary guides therapeutic strategy and is commonly finalized in multidisciplinary tumor board (MDT) meetings. We compared positron emission tomography/computed tomography (PET/CT) and endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) against MDT assignment and quantified discordance at this threshold. We retrospectively analyzed 114 consecutive NSCLC patients who underwent both PET/CT and EBUS-TBNA before MDT review. Nodal stage was dichotomized as N0/N1 vs

Despite improved overall stratification, the 9th edition stage IIIA category remains prognostically heterogeneous: insights from a T3N2a subgroup analysis.

BACKGROUND: The 9th edition TNM staging system refines N2 subclassification into single-station (N2a) and multi-station (N2b) metastases, leading to substantial stage migration within the previously heterogeneous stage IIIA category. While the 9th edition has demonstrated improved overall prognostic performance, it remains unclear whether the newly defined stage IIIA category is prognostically homogeneous. This study aimed to address this question in a real-world cohort of resected occult N2 NSCLC. METHODS: Data of NSCLC patients who underwent surgery at the Department of Thoracic Surgery, Fo

Primary pulmonary mucosa-associated lymphoid tissue lymphoma presenting as a right hilar mass: A case report.

RATIONALE: Primary pulmonary lymphoma (PPL) is an extremely rare malignant lung tumor that is frequently misdiagnosed as pneumonia or lung cancer due to its atypical radiological and clinical presentation. The rationale of this report is to elucidate the diagnostic value of combining computed tomography (CT) and 18F-fluorodeoxyglucose positron emission tomography/computed tomography (18F-FDG PET/CT) in evaluating a rare presentation of PPL. PATIENT CONCERNS: A 44-year-old male was admitted after an incidental lung lesion was discovered during routine screening. He was completely asymptomatic,

Spontaneous regression of colorectal liver metastasis with coexisting IgG4-rich inflammatory pseudotumor-like changes: a case report.

Spontaneous regression of colorectal liver metastasis (CRLM) is an extremely rare phenomenon with only a few reported cases. However, the underlying mechanism remains unclear. Infection-triggered immune reactivation and inflammation have also been proposed to be potential contributors. IgG4-related disease (IgG4-RD) is a systemic fibroinflammatory condition characterized by IgG4-positive plasma cell infiltration and fibrosis, and rarely manifests in the liver as an inflammatory pseudotumor (IPT). We herein report a rare case of a 71-year-old man with possible spontaneous regression of CRLM acc

Segmental sternal resection for metastatic ovarian cancer: surgical technique and clinical outcome.

Isolated sternal recurrence from ovarian cancer is exceptionally rare, and its optimal surgical management remains poorly defined. In selected patients with a prolonged disease-free interval and a solitary metastatic lesion, radical resection may provide durable local control while preserving chest stability and respiratory function. This video tutorial describes the surgical technique and clinical outcome of segmental sternectomy performed for a single-site sternal recurrence of ovarian cancer occurring after a disease-free interval of 12 years. Following comprehensive multidisciplinary evalu

Patterns of Lymph Node Metastases After Multimodality Therapy in Esophageal Carcinoma Treated with Robot-Assisted Minimally Invasive Esophagectomy (RAMIE).

BACKGROUND: Lymph node metastases (LNMs) in esophageal carcinoma demonstrate extensive and variable dissemination. Previous mapping studies are limited to region-based reporting, heterogeneous classification systems, and absence of contemporary multimodality-treated cohorts. This study aimed to characterize the station-level distribution of LNMs after standardized robot-assisted minimally invasive transthoracic esophagectomy (RAMIE) with two-field lymphadenectomy and to evaluate patterns in relation to key preoperative parameters. METHODS: A prospective, single-center observational cohort of

Optimizing Mediastinal Lymph Node Dissection by Integrating Tumour Location, Consolidation Tumour Ratio, and Size for Lung Cancer.

OBJECTIVES: Several mediastinal lymph node dissection strategies have been proposed for non-small-cell lung cancer based on prognostic analysis. However, as the lymph metastatic pattern of non-small-cell lung cancer has not been fully revealed, there are still needs for optimization of lymph node management. METHODS: Data on 7067 invasive non-small-cell lung cancer patients who underwent pulmonary resection with systematic lymph node dissection were retrospectively analysed. The tumour size, location, and consolidation tumour ratio (CTR) were identified with computed tomography scans. Other c
