Lymph Node Metastasis and Recurrence Patterns in Clinical Stage IA Lower-Lobe Non-Small Cell Lung Cancer: Toward an Optimal Surgical Strategy for Superior Segment (S6) Tumors.

PURPOSE: The superior segment (S6) may differ from the basal segments (BSs) in lymphatic spread, affecting surgical strategy and mediastinal lymph node dissection (LND). We aimed to define lymphatic spread patterns and to guide surgical strategy in S6 non-small cell lung cancer (NSCLC). METHODS: We reviewed 375 patients with cT1a-cT1c N0 lower-lobe NSCLC (S6, 168; BS, 207) who underwent segmentectomy or lobectomy (2012-2024). We analyzed nodal metastasis and recurrence by station. RESULTS: Segmentectomy was more frequent in S6 than in BS. pN1 and pN2 incidence was 8.3% and 2.4% in S6 and 4.3

A New N2 Descriptor for Resectable Non-Small Cell Lung Cancer: The Classification Based on Skip Metastasis and the Number of N2 Station Involvement.

BACKGROUND: Patients with non-small cell lung cancer (NSCLC) are a heterogeneous group with varied patterns of disease. The skip mediastinal lymph node metastasis is quite frequent and a prognostic factor for patients with N2 lung cancer. The aim of this study is to assess the clinical significance and prognostic value of a new N2 descriptor based on skip N2 disease and ninth edition N2 classification. METHODS: A retrospective review of 533 patients with stage pN2 NSCLC was undertaken. Patients were finally classified into three categories (skip N2a, skip N2b/nonskip N2a, and nonskip N2b). Th

Hemoperitoneum in a dog with cranial mediastinal lymphoma.

A 2-year-old neutered male terrier-mix dog was presented to a multispecialty hospital because of acute onset of lethargy and a single episode of vomiting. The dog was unstable on admission and had a moderate hemoperitoneum as determined by point-of-care ultrasonography. Additional diagnostic tests revealed a cranial mediastinal mass and scant pleural effusion. After the dog was stabilized, a computed tomographic scan confirmed a cranial mediastinal mass with bicavitary effusion as well as bicavitary lymphadenopathy. Cytologic examination of thoracic fluid was most consistent with intermediate-

Clinical features and imaging findings for a mediastinal hemangiosarcoma in a dog.

An 8-year-old neutered male mixed-breed dog weighing 29.7 kg was presented because of peracute restlessness. Abdominal radiographs revealed mild gastric gas dilation. Thoracic radiographs demonstrated a mild caudodorsal mediastinal mass effect with moderate cranial mediastinal widening and mild accompanying pleural effusion. Pleural fluid cytology was consistent with hemorrhage (packed cell volume: 58%). Computed tomography revealed findings consistent with active pancreatitis and a noninvasive, pre-contrast hyperattenuating, non-enhancing caudodorsal mediastinal mass suggestive of an acute he

Introduction to the ninth edition TNM staging of non-small cell lung cancer: A surgical perspective.

PurposeThe ninth TNM staging of non-small-cell lung cancer (NSCLC) was introduced in 2023 to replace the eighth edition that had been in use since 2017. It provides a standardised universal language to discuss management of NSCLC patients. Outcomes across different patient populations can be better evaluated and this process is undertaken by the International Association for the Study of Lung Cancer Lung Cancer Staging Project team to further refine the staging classification past and present.Changes in descriptorsThere is no change to the size-based T-descriptors. Mediastinal nodal N-descript

Percutaneous Cryoablation for Sternal Bone Metastases: A Retrospective Study.

Sternal metastases are uncommon and challenging to treat with ablation due to adjacent mediastinal structures and severe pain. This retrospective study conducted between 2017 and 2025 reviewed 14 patients (10 females and 4 males; mean age, 62 years; range, 39-79 years) treated with percutaneous cryoablation, which offers a less invasive option for palliation. Primary tumors included breast (n = 9), lung (n = 3), renal cell carcinoma (n = 1), and myxoid liposarcoma (n = 1). Twelve patients underwent palliative cryoablation, and 2 were treated with curative intent. Procedures were performed with

From One Cancer to Two: [¹⁸F]FES PET/CT Redirected Diagnosis and Therapy in a Metastatic Breast Cancer Patient.

A 59-year-old woman with an ulcerated right breast lesion was diagnosed with ER-positive invasive ductal carcinoma. Staging [¹⁸F]FDG PET/CT revealed intense uptake in the breast, axillary nodes, a large hypermetabolic pulmonary mass, mediastinal lymphadenopathies, and bone lesions. Because of this metabolic pattern, [¹⁸F]FES PET/CT was performed, demonstrating [¹⁸F]FES-avid bone metastases but no uptake in the lung mass. Biopsy confirmed a BRAF V600E-mutated lung adenocarcinoma. [¹⁸F]FES PET/CT proved crucial in differentiating synchronous primary malignancies from metastatic spread and guidin

Carcinoma of unknown primary with a lung cancer profile in mediastinal and/or hilar lymph nodes: EGFR-mutant cases and a literature review of actionable drivers.

We report two cases of carcinoma of unknown primary (CUP) with a lung cancer profile harboring epidermal growth factor receptor (EGFR) mutations. Case 1 was node-limited and achieved a durable partial response for at least 15 months with osimertinib. Case 2 harbored an EGFR exon 20 insertion and showed disease progression despite therapy. While responses to first- and second-generation EGFR tyrosine kinase inhibitors have been reported in EGFR-mutant CUP, evidence for the third-generation EGFR-TKI osimertinib remains limited. These cases highlight that early molecular profiling can identify ac
