Non-classical immune checkpoint CD137/CD137L and CD200/CD200R expressions are regulated by the tumor immune microenvironment in lymph node aspirates from lung cancer patients.

BACKGROUND: Immune escape is a hallmark of lung cancer, and limited responsiveness to PD-1/PD-L1 blockade highlights the need to identify additional immunoregulatory mechanisms in the mediastinal tumor microenvironment (TME). Non-classical immune checkpoints, including CD137/CD137L and CD200/CD200R, may interact with classical pathways but remain insufficiently defined in metastatic lymph nodes (LNs). We evaluated their expression on tumor cells and lymphocytes and their associations with PD-L1 and PD-L2. METHODS: LN aspirates obtained during transbronchial needle aspiration biopsy (EBUS/TBNA

Di̇agnosti̇c yi̇eld of EBUS i̇n medi̇asti̇nal stagi̇ng of extrapulmonary mali̇gnanci̇es; si̇ngle center cli̇ni̇cal experi̇ence.

BACKGROUND: Accurate mediastinal staging is essential for treatment planning and prognostic assessment in patients with extrapulmonary malignancies. Endobronchial ultrasonography (EBUS) has emerged as a minimally invasive modality for mediastinal evaluation; however, evidence regarding its diagnostic performance in extrapulmonary malignancies remains limited. METHODS: Patients with known extrapulmonary malignancies who underwent EBUS for mediastinal staging were retrospectively analyzed. Demographic characteristics, radiological findings, sampled lymph node stations, and pathological results w

Lymph Node Dissection of the Upper Mediastinum in Distal Oesophageal and Oesophagogastric Junction Cancer: Indicated in All Patients? Review.

OBJECTIVES: The role of upper mediastinal lymph node dissection (UMLND) in distal oesophageal and oesophagogastric junction (AEG) cancers remains debated, requiring a balance between potential oncologic benefit and surgical risk. This review provides an updated perspective on its role. METHODS: We analysed current evidence from retrospective studies, meta-analyses, and ongoing clinical trials, focusing on histology, tumour location, staging, neoadjuvant therapy response, and surgical outcomes. RESULTS: Routine UMLND is not supported for all distal oesophageal and AEG cancers. A standard 2-fi

Upfront surgery for clinical N1 stage II non-small cell lung cancer.

PURPOSE: To identify the high-risk subgroup of outcomes following upfront surgery for clinical (c)-N1 stage II non-small cell lung cancer (NSCLC). METHODS: Between 2009 and 2022, 140 patients underwent upfront lung resection of more than one lobe with mediastinal lymphadenectomy for cN1 stage II NSCLC. The subjects of this retrospective study were 129 of these patients, whose cN1 node was confirmed pathologically as metastasis after surgery. “Continuous cN1” was an N1 node directly involving the primary tumor. Disease-free survival (DFS) and overall survival (OS) were estimated using log-rank

MT-IDS: A multi-task information decoupling strategy for identifying lymph node metastasis in the mediastinal region.

Accurately identifying the mediastinal regions where metastatic lymph nodes are located is critical for the staging diagnosis of lung cancer. This identification task involves two distinct detection dimensions: mediastinal region identification and lymph node metastasis assessment. Traditional single-task image classification algorithms struggle to manage the interference between different classification dimensions within a single task. Existing multi-task learning methods struggle to balance the relationship between shared and task-specific features, and often fail to effectively fit the unde

Axillary metastases following total laryngectomy: an under-recognised pathway.

OBJECTIVES: Axillary lymph node metastases represent a rare and under-recognised pattern of disease progression in laryngeal squamous cell carcinoma, with limited data regarding their timing and clinical significance. METHODS: We retrospectively reviewed all patients treated for advanced-stage laryngeal squamous cell carcinoma at two tertiary medical centres between 2017-2022, identifying cases that subsequently developed axillary lymph node metastases. RESULTS: Five patients (3.6 per cent) developed axillary lymph node metastases, all following total laryngectomy, four after salvage surgery

Challenging uniform size criteria for EBUS-TBNA sampling: Comparative analysis by lymph node station.

PURPOSE: Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is essential for mediastinal staging in non-small cell lung cancer (NSCLC), yet uniform lymph node (LN) size criteria for sampling remain unvalidated. METHODS: This single-center retrospective study evaluated consecutive patients who underwent EBUS-TBNA for initial staging of NSCLC with available PET-CT were included. Nonparametric tests compared LN sizes across mediastinal stations 4R, 4L, and 7. RESULTS: Among 205 patients (median age 69 years), 1306 LNs were analyzed, including 770 nodes ≥5 mm and 449 me

Palatine tonsil metastasis from small cell lung cancer.

INTRODUCTION: Palatine tonsil metastasis is an uncommon manifestation of disseminated malignancy and is exceptionally rare in small-cell lung cancer (SCLC). Because it may mimic benign tonsillar disease or a primary oropharyngeal tumor, diagnosis can be delayed. CASE SUMMARY: A 71-year-old man was admitted after pertrochanteric hip fracture. Preoperative evaluation revealed a left tonsillar mass and a left hilar pulmonary lesion. Computed tomography showed a 3-4cm lesion involving the left palatine tonsil, a 7.5cm left hilar mass with mediastinal lymphadenopathy and multiple brain metastases.

Thoracotomy Versus Uniportal and Multiportal Video-Assisted Thoracoscopic Approaches for Lymph Node Dissection in Lung Cancer: A Propensity-Matched Analysis.

OBJECTIVE: Video-assisted thoracoscopic surgery (VATS), via multiportal and uniportal approaches, has largely replaced thoracotomy for lung cancer surgery. However, the comparative effectiveness of these techniques in optimal lymph node dissection remains under active investigation. METHODS: A retrospective review included 763 patients who underwent lung resection for primary lung cancer between October 2001 and January 2024. Demographic, clinical, and pathological data were collected. Patients were stratified into 3 VATS learning phases: early (2001 to 2016), intermediate (2017 to 2020), and

From metabolic to pathologic complete response: case report of Neoadjuvant Lorlatinib in Stage IIIB ALK-positive NSCLC combined with uniportal VATS.

While stage IIIB non-small cell lung cancer (NSCLC) is generally considered unresectable, a subset of patients may benefit from neoadjuvant therapy following comprehensive evaluation. We report a 35-year-old non-smoking female diagnosed with stage IIIB (cT1bN2bM0) adenocarcinoma of the left lower lobe with multiple-station mediastinal lymph node metastasis. Based on multidisciplinary evaluation of multimodality imaging and ALK-positive status, the patient with high tumor burden yet no significant comorbidities received neoadjuvant lorlatinib. The patient achieved a complete metabolic response

Athletic Intervention After Thoracic Surgery for Lung Cancer: Rationale and Design of the ATHENA Study.

BACKGROUND: The optimal therapeutic approach for early-stage NSCLC (I-II, and, in some specific cases, IIIa) is lung resective surgery with mediastinal lymphadenectomy. However, it should be remarked that lung resection surgery further negatively affects respiratory muscle function and health-related quality of life (HRQoL). It is well known that exercise interventions may improve pulmonary function but the optimal approach has not been determined yet. AIMS: The aim of the present study is to investigate the results obtained by administering a specific athletic/physical protocol to a cohort o

Case Report: Long-term maternal and neonatal outcomes after nivolumab therapy in metastatic tonsillar sarcomatoid squamous cell carcinoma.

BACKGROUND: Immunotherapy with checkpoint inhibitor (ICI) has revolutionized the treatment of cancer including squamous cell carcinoma. Nevertheless, there is an unmet need to gain a better understanding of the effect of these therapies on pregnancy and fertility. Treatment with anti-PD-1 therapy decreases fetal-maternal tolerance and increases the risk of pregnancy loss in animal studies. However, few data are currently available in humans. CASE PRESENTATION: We report the case of conception and pregnancy, with successful maternal and fetal outcomes, in a young woman with metastatic squamous

Prognostic impact of the 9th vs 8th TNM classification in small-cell lung cancer. a population-based study.

BACKGROUND: Small cell lung cancer (SCLC) is an aggressive malignancy with poor prognosis. The traditional limited and extensive-stage classification does not adequately capture disease heterogeneity. The tumor-node-metastasis (TNM) system has prognostic value in SCLC and is increasingly used, but the relevance of the 9th TNM edition in SCLC remains uncertain. MATERIALS AND METHODS: This retrospective, population-based study included patients with pathologically confirmed SCLC treated at Karolinska University Hospital, Sweden, between 2008 and 2022. Patients were reclassified according to the

Atypical Carcinoid Tumor Metastatic to the Parotid Gland: A Case Report and Review of the Literature.

Metastatic carcinoid tumor to the salivary glands is an extremely rare occurrence. The authors present a case of a 65-year-old female with a remote history of pT1aN0 atypical carcinoid tumor of the lung status-post resection and class 3 heart failure that was found to have locally advanced disease recurrence on imaging performed for heart transplantation evaluation. Fine-needle aspiration cytology (FNA) performed on mediastinal lymph nodes revealed metastatic atypical carcinoid tumor. This prompted a CT scan of the head for non-small cell lung cancer staging, which showed a parotid nodule that

Rare Right Lung Tumor with Intracardiac Extension: A Case of Successful Multidisciplinary Management.

We present a challenging case of a high-grade spindle cell sarcoma originating in the right lung with intracardiac extension to the left atrium and left ventricle via the pulmonary veins, complicated by aorto-iliac thromboembolism. An 18-year-old male presented with sudden-onset bilateral lower limb weakness, low-grade fever, cough, and breathlessness. Computed tomography imaging revealed a large mediastinal mass with intracardiac extension and an infrarenal aortic thrombus. A multidisciplinary team performed a successful single-stage procedure involving excision of the intracardiac tumor via
