Staging pleural mesothelioma revitalized.

Malignant pleural mesothelioma (PM) is the most common primary tumour of the pleura. Accurate staging is essential for treatment selection and prognostication. The 9th edition of the TNM classification introduces validated, imaging-based updates to tumour (T), node (N), and metastasis (M) staging. This article explores the scientific rationale and clinical utility of TNM 9, with a focus on imaging techniques, automated tumour volume assessment, and future developments in imaging and artificial intelligence.

Initial Results and Clinical Experiences of [177Lu]Lu-FAP-2286 for Pleural Metastases: A Single-Center Retrospective Study.

Cancer metastasis, particularly to the pleura, signifies advanced disease and poor prognosis. Fibroblast activation protein (FAP), which is highly expressed on cancer-associated fibroblasts, is a promising therapeutic target. This retrospective study investigates the efficacy and safety of the FAP-targeting radiopharmaceutical therapy [177Lu]Lu-FAP-2286 in patients with pleural metastases. Methods: Seventeen patients with FAP-positive pleural metastases from various carcinomas, all previously treated with standard therapies, received a median of 2 cycles of [177Lu]Lu-FAP-2286. Treatment respon

Safety and Efficacy of CT-Guided Lung Biopsy in Elderly Patients age 75 Years and Older: A Single-Centre Retrospective Comparative Study.

IntroductionCT-guided biopsy has good diagnostic accuracy, but adverse events such as pneumothorax are common. There are few reports on the safety and efficacy of CT-guided biopsy in the elderly.MethodsThis was a retrospective single-centre cohort study. Patients who underwent CT-guided lung biopsy between February 2017 and August 2024 were included. Patient background, disease background, examination status, and adverse events were ascertained. Elderly were defined as those aged 75 years and older. The primary outcome was the incidence of all adverse events, and the secondary outcomes were th

Safety and Survival Outcomes of Neoadjuvant Chemoradiotherapy Combined with Immunotherapy in Locally Advanced Esophageal Squamous Cell Carcinoma: A Multicenter Real-World Analysis (NEO-EC-01).

BACKGROUND: Neoadjuvant chemoradiotherapy (nCRT) is the standard treatment for patients with locally advanced esophageal squamous cell carcinoma (LA-ESCC), although perioperative immunotherapy has shown considerable promise. The integration of immunotherapy with nCRT (nICRT) is an innovative treatment paradigm and early-phase studies have reported enhanced pathological complete response (pCR) rates. However, comprehensive evaluations of safety, long-term prognosis, and recurrence patterns are lacking. In this multicenter real-world study, the clinical efficacy, recurrence patterns, and potenti

Surgical resection of pleural lipoma after 9 years of serial imaging.

Pleural lipomas are rare benign intrathoracic mesenchymal neoplasms. Although typically asymptomatic, they can enlarge and cause symptoms secondary to compression of surrounding intrathoracic structures. There is minimal reporting of pleural lipoma observed with computed tomography throughout prolonged periods of surveillance; therefore, differentiating well-differentiated malignant liposarcoma from imaging alone remains a diagnostic challenge. We present the case of a 74 year-old male with tobacco use and asbestos exposure who underwent surveillance of a pleural neoplasm over 9 years. Continu

Efficacy and safety of radial probe endobronchial ultrasound-transbronchial lung biopsy in peripheral pulmonary lesions without guide sheath and fluoroscopy.

BACKGROUND: Radial probe endobronchial ultrasound-transbronchial lung biopsy (rEBUS-TBLB) enables accurate and safe sampling of small peripheral pulmonary lesions (PPLs) inaccessible to conventional flexible bronchoscopy. Although widely adopted and effective, the standard approach using fluoroscopic guidance and a guide sheath (GS) has limitations, including reduced instrument flexibility, greater procedural complexity, and higher demands for equipment and radiation safety. OBJECTIVE: This study aimed to evaluate the diagnostic efficacy and safety of rEBUS-TBLB technique performed without th

Single-cell and spatial transcriptomic profiling reveal CST6 + epithelial-SPP1+ macrophage crosstalk driving lung adenocarcinoma metastasis.

Metastasis remains the primary cause of mortality in lung adenocarcinoma (LUAD), yet the microenvironmental drivers of this progression are not fully elucidated. In this study, we performed single-cell transcriptomic profiling on primary LUAD and multi-site metastatic lesions (lymph nodes, pleura, and brain), complemented by spatial transcriptomics and multiplex immunohistochemistry (mIHC) to map the tumor microenvironment (TME). Single-cell analysis identified a conserved subset of CST6 + epithelial cells and SPP1 + macrophages significantly enriched at metastatic sites. Integrated spatial an

Unexpected localized pleural metastasis without pathologically confirmed nodal involvement in a central ALK-rearranged lung adenocarcinoma: a case report.

BACKGROUND: Lymphatic dissemination is widely recognized as an important pathway in the progression of non-small cell lung cancer (NSCLC), and nodal involvement often accompanies more advanced thoracic spread (Rawal N. Eur J Anaesthesiol. 2016;33(3):160-71). Pleural involvement represents a distinct form of intrathoracic dissemination and is generally classified as M1a disease; when a primary tumor extends to or invades the pleura, tumor cells may gain access to the pleural cavity (Gehling M, Tryba M. Anaesthesia. 2009;64(6):643-51). Nevertheless, pleural lesions may occasionally be discovered

Impact of Heart Dose on Survival in Palliative Radiotherapy for Bone Metastases: Propensity Score-matched Analysis.

BACKGROUND/AIM: Although the effects of varying heart doses on overall survival (OS) in curative thoracic radiotherapy have been investigated, their impact in palliative settings remains underexplored. This study aimed to examine the impact of heart dose on OS in patients with bone metastases treated with palliative radiotherapy over a three-year follow-up period. MATERIALS AND METHODS: This study included 303 patients who underwent palliative radiotherapy for bone metastases between 2013 and 2022. The primary endpoint was OS, which was evaluated over a fixed three-year follow-up period. To a

Preoperative computed tomography findings of chest wall or mediastinal invasion in pulmonary metastases among children, adolescents, and young adults.

BACKGROUND: Pulmonary metastasectomy can improve the survival of children with pulmonary metastases from solid tumors. However, preoperative imaging often suggests chest wall and/or mediastinal invasion, raising concerns about resectability. Thus, we aimed to investigate associations between computed tomography (CT) findings and intraoperatively assessed chest wall or mediastinal invasion in children, adolescents, and young adults. METHODS: We retrospectively reviewed data on pulmonary metastasectomies performed between January 2015 and December 2024. Tumors considered radiographically suspic

Thromboprophylaxis, pain and organization: How do expert centers manage PIPAC's perioperative care? An international survey.

INTRODUCTION: Pressurized Intraperitoneal Aerosol Chemotherapy (PIPAC) is an emerging therapeutic option for peritoneal carcinomatosis, offering improved drug distribution and tissue penetration. Although clinical outcomes have been encouraging, international guidelines for perioperative management - including pain control and thromboprophylaxis - are still lacking. MATERIALS AND METHODS: A global online survey was distributed to PIPAC practitioners to corresponding authors of published PIPAC studies and members of the International Society for the Study of Pleura and Peritoneum (ISSPP). Pleural Metastasis as the Only Presentation of Recurrent Cervical Carcinoma on 18 F-FDG PET/CT.

A 60-year-old woman with cervical squamous cell carcinoma received 18 F-FDG PET/CT for tumor surveillance because of increased serum CA-125. PET/CT images showed multiple left pleural foci and nodular thickening of the pleura at the corresponding sites. Then, the pathology after CT-guided biopsy confirmed that the pleural foci were metastases from the cervical carcinoma.

International practice patterns and perceptions of pressurised intraperitoneal aerosol chemotherapy for management of gastric cancer: A global clinician survey.

BACKGROUND: Gastric cancer with peritoneal metastases remains a major clinical challenge, with poor prognosis and limited benefit from systemic therapy. PIPAC is an emerging locoregional option used mainly in the palliative setting, however international practice remains heterogenous, particularly with respect to patient selection, chemotherapy regimens, and response assessment. This study aimed to capture international practice patterns and identify areas of consistency, variation and evidence gaps in the use of PIPAC for gastric cancer. METHODS: A 44-question online survey was distributed e

Subacute normotensive cardiac tamponade as the initial presentation of metastatic lung adenocarcinoma: a case report.

BACKGROUND: Malignant pericardial effusion as the initial presentation of lung cancer is rare, and its diagnosis can be challenging, particularly when clinical features mimic common infectious or cardiogenic conditions. This case highlights the value of echocardiography and pericardial fluid cytology in identifying occult malignancy in patients with persistent cardiorespiratory symptoms and inconclusive initial workup. It also draws attention to the atypical occurrence of lung adenocarcinoma in a female patient, in whom breast cancer is more commonly associated with malignant pericardial effus

Cytological Diagnosis of a Rare Case of Buccal Mucosa Squamous Cell Carcinoma with Malignant Pleural Effusion and Review of Literature.

Oral squamous cell carcinoma (OSCC) is a major global health concern, especially in India, where it is one of the primary causes of cancer-related fatalities. OSCC is notorious for its propensity to spread to distal sites such as the lungs, bones, and liver, but malignant pleural effusions resulting from OSCC are extremely uncommon. This case report details an unusual presentation of OSCC in a 61-year-old male presenting with bilateral malignant pleural effusion from a primary buccal mucosa squamous cell carcinoma with no lung involvement. Through cytological analysis and immunocytochemistry,

Recurrent lung cancer with multi-organ metastases showing remarkable regression after two weeks of targeted therapy: a case report.

BACKGROUND: Recurrent pulmonary malignancy with multi-organ metastasis remains one of the most refractory scenarios in thoracic oncology. Despite advances in systemic therapy, the prognosis for such cases remains poor. We report a case of disease recurrence with multiple metastatic lesions detected 34 months following initial surgical resection. After repeat surgical intervention, targeted therapy was initiated, resulting in complete resolution of brain and pericardial metastatic foci within two weeks-a response rarely observed in clinical practice. This case underscores the critical role of s

Thermal ablation of lung metastases in children: what every paediatric radiologist should know.

Thermal ablation is increasingly used as a lung-sparing local treatment for selected children with pulmonary metastases, and diagnostic paediatric radiologists play a central role in lesion detection, procedural planning support, and post-treatment interpretation. This review focuses on the imaging appearance of pulmonary metastases before, during, and after percutaneous thermal ablation, highlighting the practical findings that every diagnostic radiologist should recognise. Pre-procedural imaging assessment is discussed with emphasis on lesion size, location, morphology, proximity to pleura,
