Predictors of fluoroquinolone-resistant bacterial colonization prior to prostate biopsy: a secondary analysis of two randomized trials.

BACKGROUND: The prevalence of fluoroquinolone resistant (FQR) rectal bacterial colonization ranges from 15-20%, and FQR colonization increases the risk of prostate biopsy infectious complications. FQR bacteria have been identified in 50% of prostate biopsy infectious complications. However, predictors of FQR remain unclear, so we aimed to elucidate these characteristics. METHODS: A secondary analysis of two multicenter clinical trials (NCT04815876 and NCT04843566) comprising 11 sites was conducted on men who underwent rectal swab cultures prior to prostate biopsy. Rectal swab cultures were ex

Anterior tumour location as a risk factor for local recurrence after laparoscopic surgery for stage I ultra-low rectal cancers: exploratory analysis of the Ultimate trial.

BACKGROUND: Anterior tumour location is an established risk factor for local recurrence (LR) in advanced rectal cancer, but its significance in stage I ultra-low rectal cancer remains unclear. METHODS: This exploratory supplemental analysis used data from Ultimate trial, a multicentre prospective phase II study across 47 Japanese institutions (2014-2017). Patients with cT1-2 N0 M0 ultra-low rectal cancer (≤ 5 cm from the anal verge) underwent laparoscopic resection. Tumour location was categorized as anterior (10-2 o'clock) or non-anterior (2-10 o'clock). The primary endpoint was LR; secondar

Risk Factors for Fluoroquinolone Resistance on Pre-Prostate Biopsy Rectal Swab.

INTRODUCTION: Prostate cancer (PCa) is usually diagnosed through transrectal prostate needle biopsy (PNB). Targeted prophylaxis with pre-PNB rectal swab and tailored antibiotics, often fluoroquinolones, can decrease post-PNB urosepsis risk, but fluoroquinolone resistance (FQR) is rising. The 2016 AUA white paper discusses risk factors for post-PNB sepsis, but more contemporary evidence is needed. Here, we evaluated FQR rate across a multihospital system and identify FQR risk factors. METHODS: We report a retrospective cohort study of patients who underwent pre-PNB rectal swab between 2013 and

Prophylactic wound closure after ESD significantly reduces post-ESD bleeding: stratified and meta-regression analyses by tumor size and rectal proportion.

BACKGROUND: Colorectal endoscopic submucosal dissection (ESD) enables curative resection but carries a risk of delayed bleeding. The efficacy of prophylactic wound closure remains uncertain. We conducted a systematic review and meta-analysis to evaluate closure versus non-closure and identify high-risk subgroups. METHODS: Following PRISMA 2020 guidelines, databases were searched to September 2025 for randomized and comparative cohort studies of closure after colorectal ESD. Primary outcome was delayed bleeding; secondary outcomes were perforation and post-ESD coagulation syndrome (PECS). Pool

Number of risk factors versus infection after transrectal prostate biopsy: a nationwide population-based study.

OBJECTIVE: It is unknown how risk factors for infection after transrectal prostate biopsy interact. We designed a study to evaluate this. METHODS: We identified biopsy procedures from 2006 to 2020 in the Swedish nationwide database PCBaSe. Primary outcome was post-biopsy infection, defined as a dispensed prescription of a urinary tract antibiotic and secondary outcome was inpatient care for infection both within 30 days. Risk factors were age, diabetes, medical treatment of lower urinary tract symptoms (LUTSs), prostate enlargement, immunosuppressives, corticosteroids, and defined antibiotic

The LDR BURST trial: a randomised controlled trial comparing the side effects of low-dose-rate brachytherapy against ultra-hypofractionated radiotherapy using spacer gel treatment in men with Cambridge Prognostic Groups 1-3 prostate cancer.

INTRODUCTION: Long-term follow-up after low-dose rate brachytherapy and ultra-fractionated radiotherapy for localised prostate cancer has demonstrated impressive survival statistics with low toxicity profiles. Superiority between the two treatments has not been shown, with a lack of randomised data comparing the two. Emerging studies have shown a benefit of adding a rectal spacer gel to sit between the prostate and anterior rectal wall to further reduce toxicity following radiotherapeutic treatments. Further randomised studies are, however, recommended to support this data. To the best of our

[Rectal neuroendocrine tumors of the rectum: update on endoscopic and surgical treatment].

Rectal neuroendocrine tumors (rNET) are rare but increasingly more common entities, which are usually an incidental finding during routine colonoscopy. The rNETs are usually well-differentiated with low metastatic potential. Thus, staging is only required in high-risk situations (size ≥ 10 mm, invasion of the muscularis propria, grading > G1, L1, V1). Endoscopic resection is the most frequently used treatment. Based on the depth of invasion, endoscopic mucosal resection (EMR), endoscopic submucosal dissection (ESD) or endoscopic full-thickness resection (EFTR) are applied. In cases of R1 resec

Robotic versus laparoscopic TME for rectal cancer: meta-analysis of pathological quality indicators.

BACKGROUND: Total mesorectal excision (TME) quality is a key determinant of oncological outcomes in rectal cancer. While robotic surgery offers technical advantages over laparoscopy in the confined pelvis, its superiority regarding pathological outcomes remains debated. We conducted a meta-analysis comparing robotic and laparoscopic TME focusing on quality indicators of TME and risk factors for incomplete TME. METHODS: A PROSPERO-registered systematic search of PubMed and EMBASE up to May 2025. Comparative studies reporting pathological outcomes of robotic versus laparoscopic TME were include

Resection quality and oncologic outcomes after robotic versus laparoscopic total mesorectal excision for mid and low rectal cancer: a systematic review and meta-analysis of randomised trials.

Total mesorectal excision (TME) for mid and low rectal cancer is technically demanding, and resection quality strongly influences local control and survival. Robotic platforms may offer technical advantages over conventional laparoscopy, but oncologic benefits remain uncertain. This meta-analysis evaluates resection quality and early oncologic outcomes using evidence exclusively from randomised trials. A PRISMA aligned systematic review of MEDLINE, Embase, CENTRAL and Google Scholar was performed to 22 September 2025. Eligible studies were parallel-group randomised controlled trials comparing

A Prospective Trial on Operability and Safety of a Simplified Robot-Assisted System (FASTER) for Colorectal Endoscopic Submucosal Dissection.

BACKGROUND AND AIMS: One of the key challenges in endoscopic submucosal dissection (ESD) is achieving effective intraoperative mucosal traction. Although numerous countertraction technologies have been developed, there is still room for optimization. This randomized controlled trial was designed to assess the comparative effectiveness of a novel robotic assistance system (FASTER) versus conventional ESD techniques for managing colorectal neoplasia. METHODS: A total of 24 ESD procedures were performed on 6 different anatomic sites (rectum and sigmoid colon) of 4 pigs. Twelve procedures were pe

Systematic review and Bayesian network meta-analysis comparing multiple anastomotic techniques after total mesorectal excision in rectal cancer.

BACKGROUND: Although several studies have performed pairwise comparisons of individual anastomotic techniques after total mesorectal excision (TME) for rectal cancer, there is a lack of comprehensive analyses, which simultaneously evaluate multiple techniques. This study aims to perform a systematic review and Bayesian network meta-analysis to compare the various anastomotic methods used after TME. METHODS: This systematic review and Bayesian network meta-analysis included both randomized controlled trials (RCTs) and non-RCTs which evaluated outcomes of various anastomotic techniques in patie

Single-stapled versus non-single-stapled colorectal anastomosis in minimally invasive anterior resections: a systematic review and meta-analysis.

BACKGROUND: Anastomotic leak remains a major cause of morbidity after anterior resection for rectal cancer. The single-stapled technique (SST) has been proposed to eliminate crossover of intersecting staple lines in conventional non-single-stapled techniques (NSST). However, evidence comparing the two approaches in minimally invasive anterior resections remains limited. METHODS: A systematic review and meta-analysis was conducted according to PRISMA 2020 guidelines (PROSPERO CRD420251053461). Unusual presentation of Currarino syndrome in an infant with right gluteal abscess.

Currarino syndrome is a rare condition with an incidence of approximately 1 in 100,000. It is characterised by the triad of a sacral defect, a presacral mass and anal atresia or stenosis, and it is commonly associated with genetic mutations in the MNX1 gene.We report a case of an infant presenting with recurrent right gluteal abscess secondary to an infected sacrococcygeal teratoma, associated with anal stenosis. Extensive workup was performed to investigate for the source of her recurrent gluteal abscess and anal stenosis, leading to the diagnosis of Currarino syndrome. She underwent multiple
