From the invasive front to organotropic pre-metastatic niches: spatial immune regulatory networks governing cholangiocarcinoma dissemination and metastasis-intercepting immunotherapy.

Cholangiocarcinoma is an aggressive biliary tract malignancy in which metastatic relapse and primary or acquired resistance to immunotherapy remain major causes of mortality. Although immune checkpoint inhibitors have improved first-line treatment for advanced biliary tract cancer, most patients do not achieve durable benefit, indicating that immune failure is not explained by a single checkpoint pathway. In this Review, we propose a spatial immune-regulatory continuum for cholangiocarcinoma dissemination. Most direct single-cell and spatial evidence currently derives from intrahepatic cholang

Mortality trends in digestive system malignancies among US adults aged 45 years or older, 1999 to 2023: A CDC WONDER time-trend analysis.

Digestive system malignancies remain a major cause of cancer mortality in the United States (US). Updated evidence through 2023 is needed to characterize recent trends, disparities, and cancer-site heterogeneity among adults aged 45 years or older. We conducted an ecological time-trend secondary analysis of publicly available, deidentified, aggregate mortality data from the Centers for Disease Control and Prevention Wide-ranging Online Data for Epidemiologic Research (CDC WONDER) Underlying Cause of Death database. US residents aged ≥ 45 years who died from digestive system malignancies (Inter

Pancreaticoduodenectomy in a patient with severe portal hypertension: the role of preoperative TIPS.

The complication rate for Pancreaticoduodenectomy (PD) is 40-50% in most published series and mortality can raise up to 4-5% even in high-volume centers. Severe portal hypertension secondary to liver disease is associated to high perioperative mortality and therefore is considered a contraindication for PD. No standardized management exists for surgically resectable patients with periampullary cancer and severe portal hypertension. The aim of this case study is to analyse the treatment alternatives in patients with periampullary cancer and severe portal hypertension and focus into the surgical

Outcomes and Role of Lymphadenectomy in Hypervascular Intrahepatic Cholangiocarcinoma Based on CT-Vascularity.

BACKGROUND: This study aimed to evaluate the outcomes and role of lymphadenectomy in hypervascular intrahepatic cholangiocarcinoma (ICC) quantified using the arterial phase of contrast-enhanced computed tomography (CT). METHODS: Consecutive patients with mass-forming (MF) or predominantly MF type ICC who underwent surgical resection from 2000 to 2019 were retrospectively analyzed. Using the image of the late arterial phase, CT-vascularity was calculated by dividing the CT value of the tumor (Hounsfield units) with that of the liver parenchyma. According to the CT-vascularity, patients were di

Cholangioblastic Cholangiocarcinoma ( NIPBL :: NACC1 Cholangiocarcinoma) : Expanded Morphologic Spectrum and Further Genetic Characterization.

The cholangioblastic variant of intrahepatic cholangiocarcinoma is a distinctive neoplasm that typically affects young women without underlying liver disease. Morphologically, it demonstrates solid, trabecular, and tubulocystic architecture, biphasic small cell-large cell cytology, and immunoreactivity for inhibin, neuroendocrine markers, and biliary but not hepatocellular markers. In 2021, our group identified a characteristic NIPBL::NACC1 gene fusion in cholangioblastic cholangiocarcinoma, and since then ~20 genetically confirmed cases have been reported in the literature. We report 2 additi

Prognostic value of CT-based skeletal muscle and adipose tissue mass and quality parameters in patients with liver metastases and intrahepatic cholangiocarcinoma undergoing Yttrium-90 radioembolization.

OBJECTIVES: To investigate baseline patient characteristics associated with the risk of computed tomography (CT)-based sarcopenia and assess whether sarcopenia and other morphometric parameters influence survival outcomes in patients with liver metastases and cholangiocarcinoma after Yttrium-90 radioembolization. MATERIALS AND METHODS: We retrospectively analyzed 120 cancer patients (mean age, 62 ± 13.3 years, 61 men) who underwent preprocedural CT. Skeletal muscle index (SMI) was measured at the L3 vertebral level to identify sarcopenia. The Cox proportional hazard model was performed to ass

A Composite Endpoint of Liver Surgery (CELS): Development and Validation of a Clinically Relevant Endpoint Requiring a Smaller Sample Size.

BACKGROUND: The feasibility of trials in liver surgery using a single-component clinical endpoint is low because single endpoints require large samples due to their low incidence. The current study sought to develop and validate a novel composite endpoint of liver surgery (CELS) to facilitate the generation of more feasible and robust high-level evidence in the field of liver surgery. METHODS: Patients who underwent curative-intent hepatectomy for hepatocellular carcinoma, intrahepatic cholangiocarcinoma, or colorectal liver metastasis were identified using a multi-institutional database. Minimum Requirements to Safely Establish and Sustain New Hepatic Arterial Infusion Pump Programs: An International Expert Perspective.

Hepatic arterial infusion (HAI) pump chemotherapy is an effective therapy for colorectal liver metastases and intrahepatic cholangiocarcinoma. In the setting of recent reports suggesting favorable outcomes in these diseases with HAI, there has been a surge in interest in this treatment worldwide, prompting the opening of many new HAI programs. While significant technical expertise is required for pump implantation, this alone is insufficient to open a safe and sustainable HAI program, and numerous other factors must be considered prior to the first pump implantation. This expert perspective, e

The Influence of Tumor Burden Score and Lymph Node Metastasis on the Survival Benefit of Adjuvant Chemotherapy in Intrahepatic Cholangiocarcinoma.

INTRODUCTION: While postoperative adjuvant chemotherapy (AC) is generally recommended for intrahepatic cholangiocarcinoma (ICC), its benefit remains debated. This study aimed to identify patients that may benefit from AC following liver resection of ICC. METHODS: Patients who underwent liver resection for ICC between 2000 and 2023 were identified from an international multi-institutional database. Individual multivariable Cox models were used to evaluate the interaction between each prognostic factor and the effect of AC on survival. RESULTS: Among 1412 patients, 431 (30.5%) received AC. Bot

6-mm vs 10-mm diameter fully covered self-expandable metal stents in patients with unresectable malignant distal bile duct stricture (COSMIC UNISON): study protocol for a multicenter, randomized controlled trial.

BACKGROUND: Unresectable malignant bile duct stricture (often caused by unresectable pancreatic cancer and cholangiocarcinoma) can be drained via insertion of self-expandable metal stents (SEMS) during endoscopic retrograde cholangiopancreatography (ERCP). Because recurrent biliary obstruction (RBO) and complications following stent insertion can delay chemotherapy and other treatments, a longer time to RBO (TRBO) is desirable. Although a longer TRBO has been reported among patients who undergo insertion with larger diameter SEMS, patients who undergo insertion with smaller diameter fully cove

Platelets promote metastasis of intrahepatic cholangiocarcinoma through activation of TGF-β/Smad2 pathway.

Intrahepatic cholangiocarcinoma (ICC), an aggressive liver cancer, lacks simple and accurate clinical tests, which poses challenges to postoperative diagnosis and treatment. Recent studies have indicated that platelet levels might be relevant to the postoperative prognosis of ICC. However, their prognostic significance in ICC remains unclarified. This study included 218 ICC patients who underwent hepatic resection. Comprehensive analyses of patients' postoperative prognosis were conducted primarily focusing on their platelet levels associated with prognostic traits. To further investigate the

Three-dimensional location approach with silk thread guided hepatectomy for liver tumor.

BACKGROUND: Intraoperative determination of resection margin and adequate residual liver parenchyma are the key points of hepatectomy for the treatment of liver tumors. Intraoperative ultrasound and indocyanine green fluorescence navigation are the most commonly used methods at present, but the technical barriers limit their promotion. AIM: To evaluate the value of the three-dimensional location approach with silk thread (3D-LAST) in precise resection of liver tumors. METHODS: From September 2020 to January 2022, 8 patients with liver tumors including hepatocellular carcinoma, intrahepatic c

Intraductal Papillary Mucinous Neoplasm of the Bile Duct: A Case Report.

Intraductal papillary mucinous neoplasms (IPMNs) are a prevalent subtype of pancreatic cystic lesions, especially among individuals with liver cirrhosis. Intraductal papillary neoplasms of the bile duct (IPNBs) differ in histopathology based on the location and cellular variability in each location. Intrahepatic IPNBs are less aggressive than the extrahepatic variant, highlighting its heterogeneity and complexity. IPNBs are largely preinvasive but can develop into invasive carcinoma. Imaging typically reveals bile duct dilation with intraductal masses but may underestimate tumor burden. Intrad

Validation of the Iwate scoring system for the stratification of laparoscopic liver resections: An international multicenter study.

INTRODUCTION: The Iwate difficulty scoring system (DSS) is one of the most widely validated DSS for laparoscopic liver resection (LLR). However, these studies only validated the 4 difficulty levels and did not validate the 12-point difficulty index of the system. To address current limitations in the studies validating the Iwate difficulty scoring system (DSS), we performed an international multicenter study to validate the Iwate DSS across both its four difficulty levels and 12-point difficulty index. METHODS: A retrospective cohort study of 22,252 patients undergoing LLR across 64 centers w

Initial outcomes of a single-institution hepatic artery infusion pump program for colorectal liver metastases and intrahepatic cholangiocarcinoma: Safety, feasibility, and circulating tumor DNA tracking.

BACKGROUND: Hepatic artery infusion with floxuridine is a treatment option for patients with colorectal liver metastases or intrahepatic cholangiocarcinoma. Outcomes from newer centers are understudied. Predictive markers are needed, and quantitative circulating tumor DNA is an emerging candidate method for predicting response in patients receiving hepatic artery infusion. We aimed to describe safety, feasibility, early oncologic outcomes, and quantitative circulating tumor DNA dynamics in patients treated with hepatic artery infusion at a newly established program. METHODS: Single-institutio

[Identification and management strategies for postoperative recurrence of intrahepatic cholangiocarcinoma].

Postoperative recurrence is one of the primary factors contributing to the poor prognosis of patients with intrahepatic cholangiocarcinoma (ICC). The recurrence patterns of ICC can be classified into intrahepatic, extrahepatic, and intrahepatic-extrahepatic recurrence, while the timing of recurrence can be categorized as very early, early, and late recurrence. Common risk factors for recurrence include positive surgical margins, lymph node metastasis, multifocality, vascular invasion, large tumor size, and concomitant liver cirrhosis. The key to improving the quality of life for ICC patients with postop

External validation of plasma CSF1 as a preoperative prognostic marker in patients with resectable intrahepatic cholangiocarcinoma.

INTRODUCTION: Long-term prognosis after resection for intrahepatic cholangiocarcinoma (iCCA) remains poor and the preoperative oncological risk assessment is difficult. Two immune system-related plasma proteins, colony stimulating factor 1 (CSF1) and TNF-related apoptosis-inducing ligand (TRAIL), were previously indicated as prognostic factors in iCCA. This study aimed to externally validate CSF1 and TRAIL as preoperative prognostic markers for patients with resectable iCCA. MATERIALS AND METHODS: Preoperative plasma CSF1 and TRAIL concentrations (pg/mL) were determined from prospectively col
