The use of topical chemotherapy for the treatment of upper urinary tract urothelial tumors and the selection of patients to receive this treatment remain problematic because of the relatively infrequent occurrence of these tumors compared with similar tumors within the bladder. Radionuclide bone scans performed in some patients with carcinoma of the breast showed abnormal retention of isotope in the renal pelvis which developed and progressed during the period of follow-up after mastectomy. This phenomenon was seen in 15 of 30 patients with infiltrating carcinoma of the breast (ILC) but in none of 29 with infiltrating duct carcinoma (IDC). Magnetic resonance imaging has achieved clinical importance and has become the primary tool in the investigation of head, spinal cord, pelvis, cancellous bone, pericardium (with gating), gallbladder, lymph node masses, and very soon, in the evaluation of joints. In many areas, particularly with respiratory and ECG gating, this method promises to replace all other imaging modalities with the precision of information obtained. Today the exceptions are the alimentary tube and its mesentery, between the esophagus and rectum, and the peripheral lung.

The cytologic features of a primary adenocarcinoma of the renal pelvis in a 64-year-old woman are reported. Cytologic examination of voided urine revealed an ordinary papillary pattern, characterized by clumping of cells with nuclear overlapping, eccentric position of nuclei and cytoplasmic vacuolization due to mucus production. We report on a man who had recurrent low grade noninvasive bladder tumors for 8 years and an increasing number of such tumors in both renal pelves and ureters for 3 years. Because of steady tumor progression, including obstruction of urine outflow from the right kidney, the tumors and as much urothelium as possible were removed. A sheathed (coaxial) ureteral catheter has been designed for use in fluoroscopically guided retrograde procedures. Once the catheter has been inserted into the ureter the inner section is removed, leaving a semiflexible sheath as a conduit for biopsy forceps, curved catheters and other instruments. The sheath facilitates ureteral stenting, forceps biopsy of the renal pelvis and brush biopsy of all calices. A study was made of the relationship between post-operative chemotherapy and metastasis and recurrence of bladder tumor in 9 patients with renal pelvic and ureteral tumors. Within 6 months on average, post-operative metastasis was found in 33.3% (3 out of 9 cases). Uretero calicostomy is a valuable technique in patients with uretero-pelvic obstruction, if a reconstruction to obtain anatomical drainage cannot be achieved. Ten cases of inferior and one of superior uretero-calicostomy, in a total of ten patients, are reported. The operation was performed eight times as a primary and three times as a secondary procedure. Good drainage of the kidney was achieved in eight of the 11 cases. The indication for ureterocalicostomy is discussed. It is rare especially in cases of malignancy.

A mean survival of 7.6 months in 20 patients undergoing nephrostomy for relief of ureteral obstruction from advanced pelvic malignancy was not as dismal as previous reports have indicated. Patients with bilateral ureteral obstruction from direct extension of pelvic malignancy survived considerably longer and appear to be the best candidates for nephrostomy. A malignant renal neoplasm with all the morphologic attributes of oat cell (small cell, neuroendocrine) carcinoma is presented. It metastasized widely to regional lymph nodes and resulted in the death of the patient. Ultrastructurally, the tumor contained dense-core endocrine-type secretory granules. It had a cell component which was argyrophilic and which gave a positive immunocytochemical reaction for calcitonin. To the best of our knowledge, this is the first documentation of this tumor type in the kidney.

Sonography for postoperative staging after malignant renal tumours was evaluated in this study based on multiple examinations in 63 patients. Sonography permits a critical examination of the liver, the abdomen in general, the retroperitoneal space, including the renal fossa and solitary kidney.
