A total of 83 patients with papillary tumour in the renal pelvis have been observed in 25 years. The peak incidence was in the age group between 51 and 70 years. The first symptom was almost always haematuria without any other symptom. The therapy is surgical, complemented with radiotherapy and/or chemotherapy. The problem of the involvement of the bladder is discussed.

Acute renal failure developed in a 47-year-old man during the second course of a high-dose methotrexate sodium (MTX) treatment for chondrosarcoma of the pelvis with pulmonary metastasis. Uninterrupted peritoneal dialysis for seven days had a negligible effect on plasma MTX. Although high-dose leucovorin calcium prevented severe myelosuppression and mucositis, the patient died 12 days after receiving MTX. Renal histological studies showed severe tubulointerstitial damage consistent with MTX toxicity. Better alternatives for removing MTX in patients with renal failure are clearly needed.

30 female Wistar rats weighing between 70 and 90 g were fed for 8 months with 0,188% N-[4-(5-nitro-2-furyl)-2-thiazolyl] Formamide (FANFT). Sarcomas of the kidney are highly malignant neoplasms. Abnormal vascularity can be detected in almost all types of sarcomas, but arteriovenous shunting, pooling of contrast material, and tumor staining are seldom present. Extension to renal vein and inferior vena cava may occur. While angiographic findings in our seven cases of renal sarcomas were nonspecific, this diagnosis should be considered in a relatively hypovascular neoplasm especially when the capsular arteries contribute considerably to the vascular supply. From 1964 to 1974, 1,668 patients with renal carcinoma were seen at the Mayo Clinic. Bone metastases were present in 167 of these patients (only lung metastatic involvement ranked higher in frequency). More than one-third of the patients with bone metastasis from renal carcinoma had this as a presenting lesion of occult renal tumor. The first case of simultaneous presentation of a bladder and renal pelvic malignant fibrous histiocytoma is reported. Although only a small percentage of fibrous histiocytomas are malignant the lesion is extremely aggressive. Radiotherapy and chemotherapy have been used but only sporadic remissions have been reported. If the tumor is localized wide surgical excision is recommended.

Twelve patients with metastatic transitional cell carcinoma from the urinary bladder were treated with a combination chemotherapy program consisting of cisplatinum, cyclophosphamide, and doxorubicin hydrochloride (CISCA). Bone scan findings (using 99mTc-stannous pyrophosphate) in five patients with diffuse metastatic carcinoma of the axial skeleton are reviewed. Although there were few visually recognizable asymmetries of tracer localization, the diffuse involvement was diagnosed through abnormally elevated counting rates in the axial skeleton, decreased visualization of the kidneys, and faint or absent visualization of the appendicular skeleton.

In a series of 81 patients with a diagnosis of Wilms' tumour seen at the Hospital for Sick Children, Great Ormond Street, between 1960-1972, two patients developed histologically proven bone metastases.
