Predicting time to local failure after gamma knife radiosurgery for melanoma brain metastases using survival machine learning.

BACKGROUND: Melanoma brain metastases treated with Gamma Knife radiosurgery show heterogeneous local control trajectories. Accurate prediction of time to local control loss could support risk stratification and follow-up planning using routinely available clinical and treatment variables. METHODS: A retrospective dataset was constructed from a melanoma brain metastasis radiosurgery cohort. The endpoint was time to local control loss with right censoring for lesions maintaining local control at last follow-up. Clinical, demographic, and treatment features were included, focusing on age, sex, r

Missed Opportunities for Neoadjuvant Immunotherapy in Melanoma: Is it Time to Consider Ultrasound Staging for Newly Diagnosed Patients?

BACKGROUND: Melanoma treatment for patients with clinical adenopathy has improved outcomes with neoadjuvant immunotherapy (NAI). Some sentinel lymph node (SLN)-positive patients have larger pathologic metastases that might be identified on ultrasound (US) preoperatively. We sought to identify SLN-eligible patients with larger nodal metastases and identify characteristics to estimate patients who might have nodal metastases identified by staging US. PATIENTS AND METHODS: Patients with clinical stage IB/II melanoma who underwent a SLNB for which the pathological size of the metastases were retr

[Challenges and opportunities in breast cancer prevention in Mendoza].

INTRODUCTION: Mendoza has a medium-high breast cancer mortality rate, the fifth highest in Argentina. Awareness and prevention are essential to reduce its mortality, both closely linked to knowledge. Primary prevention aims to avoid risk factors, while secondary prevention focuses on early detection. MATERIALS AND METHODS: This study assessed the level of knowledge about breast cancer and its preventive strategies through a survey conducted via a Google Form, addressed to women over 18 years of age, answered by 760 women. RESULTS: Only 37.4% identified mammography as the most effective detec

Correspondence of Papillary Thyroid Cancer With Cutaneous Metastasis in Thyroidology.

The recent report by Gao and colleagues highlights an infrequent yet clinically significant manifestation of papillary thyroid carcinoma, PTC: isolated cutaneous metastasis appearing 8 years post-resection. While the rarity of the case is undeniable, this critique explores the nuances of the diagnostic discordance observed-specifically the thyroglobulin, Tg, negativity in the metastatic lesion despite PAX-8 and TTF-1 positivity-and challenges the authors' hypothesis regarding the role of thyrotropin suppression. Furthermore, we examine the therapeutic decision to pursue conservative local exci

Structural and Quantitative Surface N-Glycomic Profiling of Cisplatin-Treated Melanoma Cells with Distinct Metastatic Potential.

Melanoma is a highly aggressive skin cancer with strong metastatic potential and frequent chemoresistance. Aberrant N-glycosylation plays a critical role in tumor progression and drug resistance; however, cisplatin-induced alterations in melanoma surface N-glycans remain poorly understood. Thus, this study aimed to perform comprehensive structural and quantitative profiling of surface N-glycans in G-361 (low metastatic potential), A375P (moderate), and SK-MEL-28 (high) melanoma cells following cisplatin treatment using LC-MS/MS. A total of 25, 39, and 28 surface N-glycans were identified in un

Stereoselective Determination of Omeprazole and 5-Hydroxy-Omeprazole in Dried Blood Spots for CYP2C19 Activity Assessment.

We previously proposed stereoselective 5-hydroxy-omeprazole/omeprazole cut-offs for R- and S-omeprazole to differentiate between rapid, normal, and poor CYP2C19 metabolisers. However, the performance of these stereoselective CYP2C19 phenotyping approaches has never been compared with conventional racemic omeprazole phenotyping. Therefore, dried blood spots from healthy, genetically predicted rapid (n = 15), normal (n = 15), and intermediate (n = 14) CYP2C19 metabolisers were stereoselectively reanalysed after administration of omeprazole alone (10 mg) and following CYP2C19 inhibition with vori

Masked pressure lesion on the tip of the toe, a diagnostic challenge : Case report.

Amelanotic acral melanoma is an uncommon melanoma subtype often associated with delayed diagnosis due to its nonpigmented and clinically inconspicuous presentation. We report the case of a 61-year-old man presenting with a painful erythematous nodule on the plantar aspect of the second toe persisting for 1 year and initially presumed benign. Histopathological evaluation confirmed acral amelanotic melanoma (pT3a) with micrometastatic sentinel lymph node involvement. The patient underwent partial toe amputation followed by adjuvant immunotherapy with nivolumab. This case underscores the importan

Mast cell phenotypic remodeling toward a PD-1-positive, Chymase-negative phenotype is associated with metastatic melanoma and poor survival.

Mast cells (MCs) display phenotypic heterogeneity across tissues and disease contexts, yet their relevance in melanoma remains unclear. Here, we investigated MC phenotypic programs associated with melanoma progression using high-dimensional phenotyping of MCs co-cultured with melanoma spheroids, trajectory inference, and transcriptomic analyzes. Human melanoma tissues showed differences in MC phenotypic composition despite comparable abundance between outcome groups, with metastatic melanomas enriched for Tryptase⁺ Chymase- MCs. Melanoma exposure induced MC phenotypic remodeling, characterized

Cutaneous Neoplasms with Sebaceous Differentiation: A Comprehensive Update on Clinical, Histopathological and Molecular Classification.

Cutaneous sebaceous neoplasms are rare adnexal tumours differentiated towards sebaceous glands, spanning a biological spectrum from benign (sebaceous adenoma, sebaceoma, nevus sebaceus) to outright malignant (sebaceous carcinoma). Most tumours are sporadic and solitary, with a predilection for the head and neck of elderly adults, but a clinically important subset is observed in the setting of Lynch syndrome/Muir-Torre syndrome. Recent large-scale genomic and transcriptomic studies have provided insights into the pathogenesis of these unique neoplasms, revealing a genetically heterogeneous land

Study on the migration of bacteria in transoral endoscopic thyroidectomy vestibular approach and development of a comprehensive oral management process.

BACKGROUND: The transoral endoscopic thyroidectomy vestibular approach (TOETVA) utilizes an intraoral incision, raising concerns about surgical-site infections (SSI) originating from the oral cavity. The types and sources of bacteria that may cause TOETVA infection are not clear, and the optimal antibiotic prevention plan is still unclear, lacking targeted perioperative management. MATERIALS AND METHODS: In this prospective, non-randomized controlled experimental study, drainage-fluid from 52 TOETVA and 15 open surgery patients (March - December 2025) was analyzed using 16S rDNA sequencing an

Spatial Transcriptomic and Proteomic Analysis Identifies Novel Immune-Associated Biomarkers of Anti-PD1 Response in Metastatic Melanoma.

Resistance to anti-PD1 therapy remains a major challenge in metastatic melanoma, despite long-term benefit in some patients. Since a functional dendritic cell (DC)-CD8+ T cell axis is essential for anti-PD1 response, biomarkers associated with this axis may help characterize underexplored immune states linked to resistance. Here, we further characterized poor survival predictor (PSP) biomarkers previously negatively correlated with DC-CD8+ T cell axis in melanoma, evaluating their associations with anti-PD1 response, immune infiltration, and transcriptional and proteomic signatures. Pre-treatm

Endothelial exosomes remodel chromatin accessibility and superenhancer in cancer cells to promote liver metastatic colonization.

Liver-organotropic metastasis occurs in cancers such as uveal melanoma (UM) and pancreatic ductal adenocarcinoma (PDAC) and is a leading cause of mortality at the terminal stage. Colonization is a rate-limiting and converging step in the metastatic cascade. The drivers underlying liver colonization have largely remained elusive. In this report, we hypothesized that pioneer transcription factors (TFs) increase chromatin accessibility and superenhancer (SE) remodeling, which subsequently promote colonization-associated cellular features [e.g., proliferation, survival, cancer stem cells (CSCs), m

Advancing Electrochemotherapy in Melanoma and Non-Melanoma Skin Cancer: From Clinical Practice to Emerging Frontiers.

Electrochemotherapy (ECT) is a locoregional treatment that combines poorly permeable drugs, commonly bleomycin (BLM) or cisplatin (CDDP), with electric pulses that induce electroporation. By transiently increasing cell membrane permeability, ECT promotes intracellular drug accumulation, enhancing cytotoxic effects while minimizing undesirable systemic toxicity. Beyond its direct cytotoxic effects, ECT exerts additional antitumour mechanisms that contribute to tumour control. These include the induction of immunogenic cell death (ICD), leading to activation of innate and adaptive immune respons

Immune Checkpoint Inhibitor Data in Metastatic Melanoma with Pre-Existing Autoimmune Disease: A Real World Experience.

Patients with pre-existing autoimmune disease (AID) are underrepresented in trials of immune checkpoint inhibitors (ICIs). We retrospectively compared 41 patients with stage IV melanoma and pre-existing AID with 162 contemporaneous controls without AID who received ICI treatment between November 2016 and April 2023. In the AID and control cohorts, respectively, the objective response rates were 46.3% and 32.7%, and the disease control rates were 80.5% and 51.2%. Treatment interruption attributed to worsening of the baseline AID was recorded in 11/41 patients, while 13/41 interrupted treatment

pTINCR Expression Is Associated with Tumor Aggressiveness but Does Not Independently Predict Prognosis in Head and Neck Cutaneous Squamous Cell Carcinoma.

Terminal differentiation-induced non-coding RNA (TINCR), initially classified as a long non-coding RNA, encodes a biologically active microprotein (pTINCR) involved in epidermal differentiation and tumor biology. However, the clinicopathological and prognostic relevance of pTINCR expression in cutaneous squamous cell carcinoma (cSCC) remains unclear. We conducted a retrospective case-control study including 116 patients with head and neck cSCC: 58 patients who developed nodal metastasis and 58 non-metastatic controls. pTINCR expression was assessed by immunohistochemistry and analyzed in relat

Spatial and Single-Cell Multi-Omics Analyses of T-Cell Subsets and Interactions in Melanoma Lymph Node Metastasis.

Melanoma is a highly immunogenic malignancy, yet responses to systemic therapy remain variable. The tumour-draining lymph node (TDLN), including the sentinel lymph node (SLN) during early regional spread, is not only a site of metastatic dissemination but also an important setting for T-cell priming and immune regulation. Conventional classification of nodes as tumour-positive or tumour-negative provides essential staging information but reveals little about their immune state. Single-cell immune transcriptomics, T-cell receptor sequencing and single-cell epigenomics now allow T-cell states, c

PRAME predicting outcome in uveal melanoma.

Uveal melanoma (UM) is the most common primary intra-ocular malignancy in adults and carries a high risk of metastatic disease, affecting approximately 50% of patients despite advances in local tumour control. PRAME (PReferentially expressed Antigen in MElanoma) has emerged as a marker for metastatic risk; however, evidence is largely derived from mRNA expression studies rather than routine immunohistochemistry (IHC). This study evaluates PRAME protein expression by IHC in a large UM cohort and examines its association with clinicopathological features and survival outcomes. A total of 403 UM

Engineering bispecific exosome activators of T cells to target immune checkpoint inhibitor-resistant metastatic melanoma.

Cancer immunotherapy with immune checkpoint inhibitors (ICIs) is often limited by an immunosuppressive tumor microenvironment (TME). Simultaneous targeting of the TME and immune checkpoints is a promising approach to address this limitation. Here we develop an inhalable exosome system that enables co-display of two inhibitory ligands and apply it to treat lung metastases of ICI-resistant melanoma. As immune exclusion in this context is often mediated by Wnt/β-catenin signaling, we harnessed the Alix sorting domain for tandem display of PD-1 and FZD8 to block PD-L1 and Wnt7b, which is overexpre

Impact of line-field optical coherence tomography in preoperative margin assessment of head and neck basal cell carcinoma: a cohort study.

In head-and-neck basal cell carcinoma (BCC), accurate preoperative delineation of lateral tumour margins remains challenging. Line-field confocal-optical coherence tomography (LC-OCT) offers real-time, high-resolution imaging that may improve surgical planning. This study evaluated the impact of preoperative LC-OCT on oncologic and procedural outcomes, focusing on lateral positive margins.A mixed prospective-retrospective cohort study was conducted, comparing BCC cases treated after LC-OCT implementation (2023-2024) with historical controls (2018-2021).
