Can brain-to-tumour outer interface radiomics improve the efficiency of MRI in predicting the brain invasion of meningiomas?

BACKGROUND: Brain invasion serves as an independent diagnostic criterion for the diagnosis of WHO grade 2 meningiomas. This study aimed to evaluate the value of radiomic models constructed with features extracted from different regions of the brain-to-tumor interface to predict potential brain invasion of meningiomas. METHODS: The cohort of this retrospective multicenter study included 915 consecutive patients from three centers with histopathologically confirmed meningiomas. Brain invasion status was determined separately using predefined criteria based primarily on histopathological finding

Breaking barriers in leptomeningeal disease: A Society for Neuro-Oncology and The American Society of Clinical Oncology consensus review on clinical trial design and basic science integration.

Leptomeningeal disease (LMD) is a devastating complication of cancer, and its incidence is rising. The inaugural LMD Think Tank Meeting took place in 2023 during the Society for Neuro-Oncology (SNO) and American Society of Clinical Oncology (ASCO) Central Nervous System (CNS) annual meeting, convening an international group of clinicians and scientists dedicated to understanding and treating LMD in a multidisciplinary fashion. While this meeting laid the foundation for a SNO/ASCO consensus review on the clinical management of LMD, the frequent exclusion of LMD patients from trials was identifi

Peritumoural brain edema is associated with seizure-related presentation in intracranial meningiomas.

PURPOSE: Peritumoural brain edema (PTBE) is associated with preoperative seizures in meningioma, but its association with seizure-related rather than other symptomatic presentation is less clear. We examined this relationship and associations of PTBE with tumour volume and anatomical location. METHODS: We retrospectively analysed 213 patients with surgically treated intracranial meningiomas. PTBE was classified on preoperative T2-weighted MRI by two independent neurosurgical reviewers, with disagreements resolved by consensus. The primary analysis compared seizure-related with other symptomat

Relationship Between Benign Intracranial Tumor Location and the Side of Spontaneous Cerebrospinal Fluid Leak.

AIM: To present cases of spontaneous cerebrospinal fluid (CSF) rhinorrhea and otorrhea, and to conduct a systematic literature review examining the relationship between benign tumor location and the leak side. MATERIAL and METHODS: A PRISMA-based systematic review was conducted to evaluate studies on spontaneous CSF leaks caused by benign intracranial tumors, alongside three newly reported cases. RESULTS: All three cases exhibited CSF leaks ipsilateral to the tumor: Case 1 involved a pituitary ACTH-secreting macroadenoma with rhinorrhea; Case 2, a pineocytoma with hydrocephalus and nasal encep

NGRA in Cosmetic Safety: Moving From the Frameworks and Case Studies to Wide Adoption.

Hirudin is a specific thrombin inhibitor which acts independent of antithrombin III and is able to bind to fluid phase and clot bound thrombin. It has become available for clinical use by production with recombinant techniques just recently. Hirudin produces a relatively stable level of anticoagulation when compared with heparin. However, hirudin is not as effective as heparin in blocking thrombin generation, but is more effective than heparin in inhibiting thrombin activity. In patients with unstable angina, hirudin was more effective than heparin in preventing ischaemic events in the acute p

Cell atlas of the developing human meninges reveals a dura-like nature of meningiomas.

The vertebrate central nervous system is enveloped by the meninges, consisting of the pia, arachnoid and dura layers. The arachnoid is hypothesized to give rise to the most common primary intracranial tumours, meningiomas. However, supporting molecular evidence is lacking. There are no effective medical therapies to treat meningiomas that are resistant to local interventions, encumbered by our limited understanding of their cellular origin. Here, to advance our understanding of meningioma biology, we generated a comprehensive reference single-cell and spatial transcriptomic atlas of human feta

Clinical Outcomes and Prognostic Factors Following Surgery for Supratentorial Intraventricular Tumours: A 15-year Experience.

OBJECTIVES: Supratentorial intraventricular tumours (SIVTs) represent rare and surgically demanding subset of intracranial neoplasms, characterized by heterogeneous pathology, deep anatomical location, and significant perioperative risk. Due to their low prevalence, robust outcome-data and evidence-based surgical guidelines remain scarce. This study evaluated clinical characteristics, surgical outcomes, and predictors of adverse events in a large single-center cohort. METHOD: A retrospective study was conducted on patients operated for SIVTs between April 2010 - April 2025. The data was colle

Epidemiological and clinical data link depot medroxyprogesterone acetate to meningioma.

Meningiomas are among the few intracranial tumors that are more common in females than males. The majority of meningiomas express progesterone receptor (PR), and differences in exposure to progestogens are thought to contribute to sex-based disparity in meningioma. Previous epidemiological and clinical data have demonstrated that multiple progestins, such as cyproterone acetate (CPA), are linked to meningioma. More recently, international epidemiological studies have found a significantly increased risk of meningioma with depot medroxyprogesterone acetate (DMPA), a widely used injectable contr

Preclinical evaluation of trastuzumab deruxtecan in a new HER2+ leptomeningeal colonization model.

Leptomeningeal metastases are devastating complications of advanced HER2+ breast cancer, with limited therapeutic options. We developed a xenograft model of human HER2+ breast cancer cell line JIMT1-BR3-LM4 leptomeningeal colonization by four iterative cycles of intrathecal injection. The model reliably produced leptomeningeal lesions in brain and spinal cord and tumor cells in the CSF, as confirmed by endpoints of BLI, MRI, pathologic analysis and immunofluorescent staining. Upon RNA-seq, the LM model exhibited significant transcriptional changes as compared to the starting brain-tropic line.

[177Lu]Lu-DOTATATE for Recurrent Meningioma (LUMEN-1, EORTC-2334-BTG): Study Protocol for a Randomized Phase II Trial.

There are no established treatment options for patients with meningioma recurring after surgery and radiotherapy. Somatostatin receptor type 2 (SSTR2) is highly expressed in meningiomas, and SSTR2-targeting radionuclide therapy with [177Lu]Lu-DOTATATE has shown potential activity in the treatment of meningioma in uncontrolled and small studies. Methods: EORTC-2334-BTG (LUMEN-1, NCT06326190) is a randomized, multicenter, phase II trial in patients with recurrent World Health Organization (WHO) grade 1, 2, or 3 meningioma. In total, 136 patients will be randomized in a 2:1 ratio to [177Lu]Lu-DOT

Characterization of focal leptomeningeal extension in patients with brain metastases: A novel entity.

BACKGROUND: Classical leptomeningeal disease (cLMD) impacts the entire craniospinal axis, but isolated parenchymal metastases sometimes display focal leptomeningeal extension (fLME) without diffuse LMD elsewhere. We assessed whether patients with fLME can be managed with stereotactic radiation (SRS/SRT) as opposed to whole brain/craniospinal radiation without excess development of marginal recurrences or subsequent cLMD. METHODS: We identified 796 patients with 2,354 newly-diagnosed brain metastases (BrM) without cLMD at diagnosis managed at a tertiary center between 2007 and 2022. Each metas

Validation of an immunohistochemistry-based molecular group stratification of intracranial meningiomas.

We sought to validate previously proposed immunohistochemistry (IHC) markers that classify meningiomas into 4 molecular groups (MG) with superior predictive ability of progression compared to World Health Organization (WHO) grade. IHC for target proteins S100B, SCGN, ACADL, and MCM2 corresponding to molecular groups 1-4, respectively, was performed on 85 surgically resected primary meningiomas across WHO grades 1-3. Additional IHC for FOXM1 was included as a potential predictor of aggressiveness. All slides were digitally analyzed for percent of cells staining positive. The primary outcome mea

Assessment of molecular tools in pediatric, adolescent, and young adult meningioma highlights the need for lifespan precision in neuro-oncology.

BACKGROUND: Adolescent and young adult (AYA) patients remain underrepresented in neuro-oncology research. Despite being the second most common primary brain tumor in this population, meningiomas have not been studied using age-specific molecular analyses. DNA methylation-based classification and prognostic tools have transformed meningioma care. This study aimed to evaluate the performance of these tools across age groups. METHODS: We analyzed 1,568 meningiomas with DNA methylation and clinical data, including 18 pediatric patients (<15 years), 195 AYA patients (15-39 years), and 1,355 adult

A framework for using DNA methylation-based modelling for the clinical management of cranial meningioma.

BACKGROUND: DNA methylation profiling can be used to robustly predict postsurgical outcomes and response to radiotherapy (RT) for meningioma patients. To allow for seamless integration of these complementary models into clinical practice, a practical framework is needed. METHODS: We leveraged a cohort of nearly 2000 surgically-treated meningiomas with DNA methylation profiling and clinical outcomes data. Existing methylation-based prediction models were dichotomized to yield four risk groups: low and high recurrent risk, each with RT sensitive and resistant subgroups. Risk groups were correla

Cerebrospinal fluid metabolomics and machine learning identify novel biomarkers for lung cancer leptomeningeal metastasis.

BACKGROUND: Lung cancer leptomeningeal metastasis (LC-LM) severely impacts patient survival and quality of life, yet current diagnostic methods lack sufficient sensitivity and specificity, particularly for early detection. Cerebrospinal fluid (CSF) metabolomics may reveal specific biomarkers reflecting brain metastasis. METHODS: We performed untargeted metabolomic profiling of CSF samples by high-resolution mass spectrometry in a cohort of 218 participants, including 99 samples from LC-LM (with cancer cells detected in the CSF), 12 samples from the lung cancer parenchymal brain metastases (wi

The glymphatic system and glioblastoma.

The discovery of the glymphatic system and meningeal lymphatic vessels revolutionized the understanding of brain waste clearance. These findings challenged the long-held notion of CNS immune privilege and provided insights into how the brain maintains homeostasis despite lacking conventional lymphatic drainage. However, the impact of pathological conditions, particularly CNS cancers, on the glymphatic system and vice versa remains underexplored. Glioblastoma (GBM) is the most common and aggressive primary adult CNS cancer. Its presence results in glymphatic dysfunction with significant clinica
