Validation of an immunohistochemistry-based molecular group stratification of intracranial meningiomas.

We sought to validate previously proposed immunohistochemistry (IHC) markers that classify meningiomas into 4 molecular groups (MG) with superior predictive ability of progression compared to World Health Organization (WHO) grade. IHC for target proteins S100B, SCGN, ACADL, and MCM2 corresponding to molecular groups 1-4, respectively, was performed on 85 surgically resected primary meningiomas across WHO grades 1-3. Additional IHC for FOXM1 was included as a potential predictor of aggressiveness. All slides were digitally analyzed for percent of cells staining positive. The primary outcome mea

Assessment of molecular tools in pediatric, adolescent, and young adult meningioma highlights the need for lifespan precision in neuro-oncology.

BACKGROUND: Adolescent and young adult (AYA) patients remain underrepresented in neuro-oncology research. Despite being the second most common primary brain tumor in this population, meningiomas have not been studied using age-specific molecular analyses. DNA methylation-based classification and prognostic tools have transformed meningioma care. This study aimed to evaluate the performance of these tools across age groups. METHODS: We analyzed 1,568 meningiomas with DNA methylation and clinical data, including 18 pediatric patients (<15 years), 195 AYA patients (15-39 years), and 1,355 adult

A framework for using DNA methylation-based modelling for the clinical management of cranial meningioma.

BACKGROUND: DNA methylation profiling can be used to robustly predict postsurgical outcomes and response to radiotherapy (RT) for meningioma patients. To allow for seamless integration of these complementary models into clinical practice, a practical framework is needed. METHODS: We leveraged a cohort of nearly 2000 surgically-treated meningiomas with DNA methylation profiling and clinical outcomes data. Existing methylation-based prediction models were dichotomized to yield four risk groups: low and high recurrent risk, each with RT sensitive and resistant subgroups. Risk groups were correla

Cerebrospinal fluid metabolomics and machine learning identify novel biomarkers for lung cancer leptomeningeal metastasis.

BACKGROUND: Lung cancer leptomeningeal metastasis (LC-LM) severely impacts patient survival and quality of life, yet current diagnostic methods lack sufficient sensitivity and specificity, particularly for early detection. Cerebrospinal fluid (CSF) metabolomics may reveal specific biomarkers reflecting brain metastasis. METHODS: We performed untargeted metabolomic profiling of CSF samples by high-resolution mass spectrometry in a cohort of 218 participants, including 99 samples from LC-LM (with cancer cells detected in the CSF), 12 samples from the lung cancer parenchymal brain metastases (wi

The glymphatic system and glioblastoma.

The discovery of the glymphatic system and meningeal lymphatic vessels revolutionized the understanding of brain waste clearance. These findings challenged the long-held notion of CNS immune privilege and provided insights into how the brain maintains homeostasis despite lacking conventional lymphatic drainage. However, the impact of pathological conditions, particularly CNS cancers, on the glymphatic system and vice versa remains underexplored. Glioblastoma (GBM) is the most common and aggressive primary adult CNS cancer. Its presence results in glymphatic dysfunction with significant clinica

Combining Brigatinib with mTOR Inhibition to Effectively Treat NF2-SWN-Associated and Sporadic NF2-Deficient Meningiomas.

UNLABELLED: We previously generated an orthotopic, NF2-deficient meningioma model using the luciferase-expressing Ben-Men-1 cell line established from a sporadic tumor and identified the multikinase inhibitor brigatinib and the mTOR kinase inhibitor INK128 to potently impede tumor growth. In this study, we describe generation of the telomerase-immortalized AG-NF2-Men cell line from a grade-1 meningioma of a patient with NF2-related schwannomatosis (NF2-SWN). We showed that like Ben-Men-1 cells, AG-NF2-Men cells were NF2-null, expressed several NF2-regulated receptor tyrosine kinases, and respo

Meningioma cell reprogramming and microenvironment interactions underlie brain invasion.

BACKGROUND: Meningioma brain invasion encumbers surgical resection and increases the risk of tumor recurrence, but the molecular mechanisms underlying this process are poorly understood. METHODS: To identify molecular and cellular features of brain-invasive meningiomas, we (1) analyzed bulk RNA sequencing data from 199 meningiomas, including 33 brain-invasive tumors, (2) analyzed patient-matched single-cell RNA sequencing data of spatially mapped meningioma samples from the tumor core or brain-tumor interface (BTI), and (3) performed spatial transcriptomic sequencing of brain-invasive meningi

Novel insights into meningioma brain invasion with spatial transcriptomic profiling.

Brain invasion of meningioma is a controversial clinicopathological grading criterion that correlates with prognosis and is a stand-alone criterion for WHO grade 2 meningioma. Molecular correlates of meningioma cells and immune cell infiltration at the brain-meningioma border and its possible role in brain invasion are not known. We hypothesized that brain-invasive meningiomas have distinct gene expression profiles in meningioma cell populations and in tumour-associated microglia and macrophages (TAM) populations, most prominently at the brain-meningioma border. We performed spatial transcripto

Risk assessment of brain retraction injury based on quantitative evaluation of brain retraction distance during skull-base meningioma surgery.

OBJECTIVE: In skull-base surgery, excessive brain retraction may cause brain retraction injury (BRI), yet the brain retraction distance (BRD) that contributes to BRI and the potential of skull-base approaches to reduce BRD remain unclear. This study used a virtual reality (VR)-based method to quantify BRD related to BRI and to assess the impact of skull-base approaches on reducing BRD. METHODS: We retrospectively analyzed 18 patients who underwent frontotemporal craniotomy for anterior skull-base tumors. Three-dimensional models were generated from perioperative imaging, and the frontal lobe

Dexamethasone associated with lactic acidosis in a stroke patient with meningioma-associated vasogenic edema: a case report.

BACKGROUND: Corticosteroids are commonly used to manage a multitude of conditions, ranging from autoimmune to neurological diseases, including central nervous system vasculitis and vasogenic edema from large strokes. Type A lactic acidosis and type B lactic acidosis are the two forms of lactic acidosis. Medications are the most common cause of type B lactic acidosis. An elevated lactate confers a higher mortality rate in patients. Corticosteroids are not commonly known to cause elevated lactate. The relationship between corticosteroids and lactic acidosis has only been described in critically

Therapeutic progress in leptomeningeal metastasis from EGFR mutant non-small cell lung cancer: a clinical medicine review.

Leptomeningeal metastasis in patients with Epidermal Growth Factor Receptor-mutant non-small cell lung cancer carries a dismal prognosis and is associated with profound neurological morbidity. Historically, therapeutic efficacy has been severely limited, leading to poor overall survival. This review aims to synthesize the recent, albeit incremental, advances in the multidisciplinary management of this devastating complication. We delve into the evolving landscape of treatment modalities, with a particular focus on the latest generation of tyrosine kinase inhibitors and their superior central n

Impact of meningioma and glioma on whole-brain dynamics.

Brain tumors, particularly meningiomas and gliomas, can profoundly affect neural function, yet their impact on brain dynamics remains incompletely understood. This study investigates alterations in normal brain function among meningioma and glioma patients by assessing dynamical complexity through the Intrinsic Ignition Framework. We analyzed resting-state fMRI data from 34 participants to quantify brain dynamics using intrinsic ignition and metastability metrics. Our results revealed distinct patterns of disruption: glioma patients showed significant reductions in both metrics compared to con

Immune Cell Infiltration in Primary and Recurrent Glioblastoma, IDH-Wild Type: Validation of an Immunohistochemistry-Based Scoring System for Research and Clinical Practice.

Significant heterogeneity in the immune microenvironment of Glioblastoma, IDH-wildtype (GBM), has been reported, necessitating a standardized approach to evaluate immune infiltration in the context of emerging immunotherapies. To address this, we developed and validated a standardized immunohistochemistry-based scoring system for quantifying immune cell infiltration in formalin-fixed, paraffin-embedded (FFPE) tissue. Paired primary and recurrent GBM specimens from 20 adult patients were labeled for CD3, CD8, CD45, CD68, and PD-1, and scored across six anatomical regions, including intra- and p

Leptomeningeal metastasis from lung adenocarcinoma associated with Lemierre syndrome in a middle-aged man: a case report and review of the literature.

BACKGROUND: Lemierre syndrome (LS), predominantly triggered by oropharyngeal infection with Fusobacterium necrophorum, is a rare but life-threatening condition. The co-occurrence of LS with leptomeningeal metastases (LM) from lung adenocarcinoma (LADC) is exceptionally scarce, with only a handful of cases documented in the literature. CASE PRESENTATION: A 42-year-old male presented with neck pain and infection, which progressed to cervical venous thrombosis, cervical lymphadenopathy, and multiple pulmonary nodules, features consistent with a diagnosis of LS. The patient received targeted treat

Prognostic Value of Magnetic Resonance Imaging Defined Extent of Surgical Resection in Dogs With Intracranial Meningiomas.

Surgery is a common treatment for intracranial meningiomas in dogs, although the prognostic impact of the extent of resection (EOR) has not been systematically evaluated. This retrospective study identified prognostic factors associated with clinical outcomes in dogs that underwent surgery and early post-operative magnetic resonance imaging (epoMRI) to evaluate meningioma EOR. We hypothesised that gross total tumour resection (GTR) would result in longer progression free (PFS) and overall survival (OS), and superior post-operative seizure control and resolution of neurological dysfunction than

The 2024 Brain Tumor Segmentation Challenge Meningioma Radiotherapy (BraTS-MEN-RT) dataset.

Meningiomas are the most common primary intracranial tumors, frequently requiring radiotherapy as a part of management. Effective radiotherapy planning for meningiomas necessitates accurate and consistent segmentation of target volumes on MRI, a process that is complex, labor-intensive, and dependent on expert expertise. The 2024 Brain Tumor Segmentation Challenge Meningioma Radiotherapy (BraTS-MEN-RT) Dataset addresses this problem by providing the largest multi-institutional collection of systematically annotated radiotherapy planning MRIs for meningiomas. Publicly accessible, this dataset c
