Enhanced prognostic score for breast cancer brain metastases incorporating contemporary clinical features and leptomeningeal disease: Development and validation.

PURPOSE: Prognosis of breast cancer brain metastases (BCBM) remains heterogeneous. The current Breast-Graded Prognostic Assessment (GPA) lacks leptomeningeal disease (LMD) or contemporary clinical features. We aimed to develop and externally validate an enhanced prognostic scoring system integrating clinical factors, cancer subtypes, and LMD. METHODS: In this retrospective multi-institutional study, 207 patients with newly diagnosed BCBM treated with stereotactic radiosurgery between 2015 and 2021 formed the development cohort. 240 patients formed the external validation cohort. Clinical vari

Intrathecal nivolumab in metastatic solid tumors with leptomeningeal disease: dose escalation part of the multicenter IT-PD1/NOA-26 phase 1 trial.

Leptomeningeal metastatic disease (LMD) of solid tumors represents a cancer stage with high unmet therapeutic need. Here we report results from the dose escalation part of a multicenter phase 1 trial investigating intraventricular nivolumab, now continuing in the expansion part. Eligible participants had LMD from tumors with an approval for intraveneous PD1/PDL1 therapy or high tumor mutational burden. The primary endpoint was safety across four dose levels (20, 30, 40 and 50 mg) with each cohort reviewed by an independent data safety monitoring board before escalation. The secondary endpoint

Furmonertinib-based triplet therapy improves survival in EGFR-mutant NSCLC with leptomeningeal metastases: a large-scale multicenter retrospective study (FURMO-LM).

BACKGROUND: Leptomeningeal metastases (LM) in epidermal growth factor receptor mutant (EGFRm) non-small-cell lung cancer (NSCLC) represent a major therapeutic challenge. Despite the improved central nervous system penetration of third-generation EGFR tyrosine kinase inhibitors (TKIs), key uncertainties persist regarding the efficacy of combination regimens with antiangiogenic therapy and intrathecal chemotherapy and optimal dosing because of the limited evidence. PATIENTS AND METHODS: In this large-scale, multicenter, retrospective study, 618 patients with EGFRm NSCLC and LM who were treated

Deep learning for H&E-based meningioma molecular classification and outcome prediction: a retrospective cohort study.

BACKGROUND: The introduction of genomic profiling as a tool for molecular classification and clinical outcome prediction has revolutionised the care of patients with brain tumours. Artificial intelligence (AI) provides advanced avenues to convert complex genomic information into routinely available patient-level information. In this study, we aimed to evaluate whether deep learning could robustly characterise molecular subtypes, predict risk of recurrence, and identify salient chromosome copy number alterations in haematoxylin and eosin (H&E)-stained tissue samples of the most common brain tum

Prognostic impact of concomitant leptomeningeal disease in breast cancer patients with brain metastases: a large single-center cohort study.

BACKGROUND: Central nervous system (CNS) metastases are a frequent and ominous scenario in patients with breast cancer. CNS metastases (CNSm) are typically classified as brain metastases (BrMs) or leptomeningeal disease (LMD). Although the coexistence of BrMs and LMD (BrMs + LMD) is commonly encountered in clinical practice, its true prevalence and prognostic implications remain insufficiently characterized. This study aimed to characterize the distribution of CNSm and compare outcomes in patients with breast cancer BrMs with and without LMD. METHODS: We retrospectively analyzed a prospective

Genomic hallmarks of depot medroxyprogesterone acetate-associated meningiomas.

BACKGROUND: Population-based studies have linked progestin exposure to increased meningioma risk. However, the molecular basis of meningiomas associated with depot medroxyprogesterone acetate (DMPA)-a common injectable contraceptive-remains undefined. METHODS: We performed an integrated clinicopathologic and genomic analysis of meningiomas from 10 women with long-term DMPA exposure. Tumors underwent histopathological analysis, targeted sequencing, and DNA methylation profiling. Data were integrated with reference cohorts (Baylor and Heidelberg) and analyzed through classifier assignment, cons

Antibody-drug conjugates in breast cancer brain and leptomeningeal metastases: mechanistic insights and therapeutic progress.

Antibody-drug conjugates (ADCs) are targeted therapies that employ monoclonal antibodies to deliver potent cytotoxic agents to cancer cells. The treatment of breast cancer with brain and leptomeningeal metastases has long been limited by the assumption that the blood-brain barrier (BBB) and blood-cerebrospinal fluid barrier (BCSFB) restrict the penetration of large molecules into the central nervous system (CNS). However, recent evidence from several trails (DESTINY-Breast12, DEBBRAH, TUXEDO-3, etc.) challenges this paradigm. In these studies, trastuzumab deruxtecan, a HER2-targeted ADC, demon

Spatially resolved single-cell analyses of human meningioma identify novel cell states influencing tumor microenvironment and progression.

Recent advances in our understanding of the molecular landscape of meningioma have generated new insights into the biology and heterogeneity of this disease, with demonstrated clinical value. However, there remains a need to understand tumor-intrinsic heterogeneity at single-cell resolution to inform potential therapeutic avenues. In this study, we examined the breadth of cell types and states in meningioma using a large cohort profiled with single-nuclear RNA sequencing and high-resolution spatial transcriptomics, as well as bulk DNA methylation and RNA sequencing (n = 712), bulk proteomics (

FOXC1 as a molecular predictor of postoperative peritumoral edema resolution in meningiomas.

BACKGROUND: Peritumoral brain edema (PTBE) is an important prognostic factor in meningiomas and may persist after tumor resection. While clinical and radiological predictors of postoperative PTBE resolution are known, the impact of molecular markers remained unclear. The aim of this study was to investigate the association of Forkhead box C1 (FOXC1) expression, as well as radiological and clinical parameters with postoperative PTBE resolution. MATERIALS AND METHODS: We conducted a retrospective single-center study with 75 histopathologically confirmed meningiomas. Pre- and postoperative PTBE

Engineered lymphatic stroma model applications in central nervous system leukemia.

Central nervous system (CNS) involvement frequently occurs in acute lymphoblastic leukemia (ALL), but many questions remain about how leukemia cells access, persist in, and exploit the CNS. The CNS is protected by the meninges, which are fluid-filled membranes that surround the brain. Within the meninges, meningeal lymphatic vessels, located within the dura mater, drain cerebrospinal fluid to the deep cervical lymph nodes for immunosurveillance, providing a potential pathway for leukemia cells to enter or exit the CNS. Here, we utilized tissue engineered models of the meningeal lymphatics and

Primary intracranial Ewing-like sarcoma presenting with rapid bilateral blindness: A case report.

Primary intracranial Ewing sarcoma / Ewing-like sarcoma is an exceptionally rare malignant small round cell tumor. Its radiological appearance is often non-specific and may mimic other aggressive intracranial neoplasms, leading to diagnostic delay. We report the case of a young adult presenting with progressive headaches and recurrent generalized tonic-clonic seizures for six months, followed by rapid bilateral visual loss. Neurological examination revealed papilledema and decreased visual acuity, prompting brain imaging. MRI demonstrated a large aggressive frontal mass with heterogeneous enha

KRT18 promotes the high-grade meningioma proliferation by interacting with LDHA to activate glycolysis and the PI3K/AKT signaling pathway.

Meningioma is the most common central nervous system tumor and high-grade tumors frequently show progressive growth or recurrence, posing substantial therapeutic challenges. However, the lack of effective chemotherapy and targeted therapies contributes to poor prognoses in patients with high-grade disease. In this study, we evaluated KRT18 expression in meningiomas and examined its correlation with patient prognosis using RNA sequencing and immunofluorescence staining. The oncogenic role of KRT18 in high-grade meningioma was further investigated using IOMM-Lee and CH157-MN cells, xenograft mou

Extradural anterior transpetrosal approach: how I do it.

BACKGROUND: Petroclival and brainstem lesions remain challenging to access due to their deep-seated location and proximity to critical neurovascular structures. METHOD: We describe the step-by-step technique of the standard extradural anterior transpetrosal approach, and its extensions, illustrated through cadaveric and surgical illustrations. A surgical video demonstrates its application in a patient with a petroclival meningioma with posterolateral extension. CONCLUSION: The extradural anterior transpetrosal approach provides a direct « looking-down» trajectory to petroclival lesions with

What is the best surgical approach for Pineal Region Tumors? A systematic literature review and anatomical comparative study.

PURPOSE: Pineal region tumors pose significant surgical challenges due to their deep location and proximity to critical neurovascular structures. Despite multiple described techniques, no consensus exists on the optimal surgical approach, with the supracerebellar infratentorial (SCIT) and occipital interhemispheric transtentorial (OITA) approaches representing the most adopted corridors. METHODS: A systematic literature review was conducted to evaluate surgical approaches for pineal region tumors and their associated complication profiles. In addition, anatomical dissections combined with a t
