Optic nerve sheath meningioma exhibits neural niche-associated transcriptomic features and rare copy number variation-linked evolution.

Optic nerve sheath meningioma (ONSM) is a rare tumor that arises from the meninges enveloping the optic nerve. Although the genetic landscape of meningiomas has been extensively studied, the molecular alterations underlying ONSM remain poorly understood. We retrospectively analyzed consecutive patients surgically treated for ONSM between 2000 and 2025 at our institution, with available histological specimens. Intracranial meningiomas secondarily extending into the optic canal were excluded. Fresh-frozen tumor samples were subjected to whole-exome sequencing, and transcriptomic analyses were co

The Definitive-Intent 10-Fraction Radiotherapy Protocol ('The Zurich Protocol') Yields Lasting Tumour Control and Survival in Dogs With Intracranial Meningiomas.

Moderately hypofractionated definitive-intent radiotherapy protocols can be used for canine intracranial meningiomas to shorten overall treatment time while maintaining tumour control and limiting normal tissue toxicity. This retrospective study evaluated oncologic outcomes, toxicity, and radiation dose-volume-related prognostic factors in dogs with imaging-based intracranial meningiomas treated with a 10-fraction definitive-intent protocol. Medical records from 2015 to 2025 were reviewed. All dogs received 10 daily fractions of 4 Gy delivered with a homogeneous dose prescription, risk-adapted

Risk factors for postoperative motor complications in middle-third parasagittal meningioma surgery.

BACKGROUND: Middle one-third parasagittal meningiomas are surgically demanding due to their proximity to the motor cortex and complex venous anatomy. Preservation of neurological function while achieving maximal safe resection remains a key challenge. This study aimed to identify clinical, radiological, and intraoperative predictors of postoperative motor weakness in patients with middle one-third parasagittal meningiomas. METHODS: A retrospective review was conducted on 103 consecutive patients who underwent microsurgical resection for middle one-third parasagittal meningiomas at a tertiary

Association between persisting peritumoral brain edema and seizures as well as worse functional outcome after meningioma surgery: a retrospective study of 218 patients.

OBJECTIVE: Peritumoral brain edema (PTBE) of intracranial meningioma (IM) often persists after gross-total resection (GTR) and likely represents reactive gliosis. The authors investigated whether persisting PTBE (i.e., FLAIR hyperintensity) is associated with postoperative seizures and functional outcomes, hypothesizing that it is associated with a higher likelihood of seizures and worse outcomes. METHODS: The authors retrospectively reviewed all patients (age ≥ 18 years) with IM who underwent GTR at their institution between 2000 and 2020. Included patients had a preoperative MRI study showi

Significance of Positive Cerebrospinal Fluid Cytology (Leptomeningeal Metastasis) in Central Nervous System Metastasis - A Multicentre Clinicopathological Review.

Leptomeningeal metastases (LM) is a rare but clinically important subtype in central nervous system (CNS) metastasis. Multicentre data was reviewed to detail the epidemiology, clinicopathological associations among patients with positive cerebrospinal fluid cytology (CSF) cytology. Patients with CNS metastasis and clinical data were identified through the Hong Kong Hospital Authority Clinical Data Analysis and Reporting System (CDARS). In total, through 1st January 2000 to 31st December 2024, 38,893 patients with CNS metastasis were retrieved, 1503 had CSF cytology. The most common primary mal

An unusual lesion of cerebellopontine angle as a pathological and diagnostic enigma: a comprehensive case report.

Cerebellopontine angle (CPA) tumours comprise a pathologically varied group of tumours owing to the origin of the tumour and effect on neurovascular structures, leading to a myriad of clinical manifestations. These tumours are benign, sluggish, with a low potency for malignant transformation. Malignant peripheral nerve sheath tumours (MPNST) are a malignant neoplasm of the peripheral nerves. It may develop from a pre-existing neurofibroma or may arise sporadically. Malignant peripheral nerve sheath tumours have a poor overall prognosis owing to their aggressive nature and limited therapeutic o

A sonoelectric artificial meningeal device for enhanced implantable electric field ablation of glioblastoma.

Electric field tumor therapy has emerged as a promising non-invasive treatment for glioblastoma (GBM), but its clinical efficacy is severely limited by skull-induced attenuation of electric field intensity. To overcome this limitation, we developed a multilayer thin-film sonoelectric meningeal device (MF-SMD) consisting of an electrospun PLGA artificial meningeal scaffold integrated with a microneedle-based triboelectric film. This device converts externally applied low-intensity ultrasound into intracranial electric fields, enabling transcranial, localized, and controllable electric ablation

Low-input CSF cfDNA shallow whole-genome sequencing for CNV-based diagnosis and monitoring of leptomeningeal metastasis in lung cancer.

Leptomeningeal metastasis (LM) is a life-threatening complication of lung cancer for which magnetic resonance imaging (MRI) and cerebrospinal fluid (CSF) cytology may miss disease and provide limited molecular guidance. Because the low cfDNA input typical of CSF constrains deep sequencing and mutation-focused approaches may underrepresent the copy-number-rich genomic architecture of CNS metastases, we developed a low-input shallow whole-genome sequencing approach to quantify genome-wide copy-number changes from trace CSF cfDNA. In 57 patients with lung cancer and CNS metastases (28 with LM and

Biophysical Diffusion MRI Models Better Identify White Matter Tracts in Edema.

BACKGROUND/OBJECTIVES: White matter (WM) tract detection is critical in the presurgical planning of tumor resection. However, standard-of-care imaging techniques including T1-weighted, T2-weighted, and Diffusion Tensor Imaging (DTI) often fail to identify WM tracts within edematous regions. In T1/T2-weighted imaging, edema increases extracellular water and reduces tissue contrast, and in diffusion-weighted imaging, edema elevates isotropic diffusion, reducing sensitivity to anisotropic diffusion along WM tracts. Advanced biophysical diffusion modeling techniques such as Neurite Orientation Dis

Effects of Galectin-3 inhibitors paired with direct current stimulation in preclinical models of aggressive meningioma.

Purpose Galectin-3 (Gal-3), a multifunctional β-galactoside-binding lectin, is more highly expressed in atypical and anaplastic meningioma subtypes than in normal brain tissue. The mechanism(s) by which Gal-3 may contribute to the aggressive growth of these tumors remains unclear. Methods We evaluated the effects of the Gal-3 inhibitor TD139 combined with either low-intensity direct current electrostimulation (DCES) on meningioma cells in a human co-culture model, or non-invasive transcranial direct current stimulation (tDCS) in an orthotopic preclinical model. Results In vitro, DCES treatment

Intracranial Breast Cancer Brain Metastases Mouse Model for Immune and Stromal Cells Characterization in Meninges and Olfactory Bulb.

The central nervous system (CNS) is surrounded by specialized immune interfaces, including the meninges and the olfactory bulb, which can serve both as permissive routes for metastatic colonization and as entry points for anti-tumor immune responses. Understanding how immune cell dynamics evolve within these structures during metastatic progression is essential for identifying mechanisms that either support or restrict tumor growth in the brain. This protocol details the establishment of a breast cancer brain metastases (BCBM) mouse model through intracranial injection of E0771 breast cancer c

Meningeal immunity and "Interstitial" therapy: a new paradigm for immunotherapy in glioblastoma.

Glioblastoma (GBM) is one of the most aggressive brain tumors, and its immunotherapy faces significant challenges, including the physical barrier of the blood-brain barrier, the tumor immunosuppressive microenvironment, and the difficulty of effective immune cell infiltration into tumor tissue. In recent years, the anatomical and functional basis of the meningeal immune system has been gradually revealed, particularly the discovery of the dural lymphatic system and tissue-resident border-associated macrophages (rBAMs), providing new insights for GBM immunotherapy. "Interstitial" immunotherapy,

Surgical Outcomes of Canine Intracranial Meningioma With or Without Radiation.

Meningioma is the most common primary brain tumor in dogs, and recurrence after treatment presents a significant clinical challenge. This retrospective study evaluated prognostic factors in 21 dogs with intracranial meningioma treated with surgical resection alone or with postoperative radiation therapy (RT). Recurrence rate, progression-free survival (PFS), median survival time (MST), and prognostic indicators were assessed. Of the 21 dogs, 13 received adjuvant RT. Local recurrence was noted in six dogs (28%). Overall MST was 845 days, and PFS was 771 days. By treatment group, MST was 886 day

Enhancing decision-making in surgery for a large temporocorneal meningioma through an explainable human-AI collaboration: a case report.

BACKGROUND: Meningiomas, particularly large temporocorneal meningiomas, pose significant surgical challenges due to their proximity to critical brain structures. Achieving optimal tumor resection while minimizing neurological deficits requires advanced decision-making strategies. This case report explores the integration of an explainable artificial intelligence (AI) system into the neurosurgical workflow to enhance preoperative planning, intraoperative decision-making, and postoperative outcome prediction. CASE PRESENTATION: MAIN SYMPTOMS AND CLINICAL FINDINGS: We report the case of a 48-yea

Distinct molecular subgroups in pediatric and young-onset meningiomas require age-adapted risk stratification.

Meningiomas in pediatric and adolescent/young adult patients are poorly characterized biologically and clinically, and risk stratification is largely extrapolated from adult tumors. We analyze 293 tumors from patients aged 0-39 years using integrated histopathological and molecular profiling. Youth-onset meningiomas are enriched for NF2 and SMARCE1 alterations and exhibit a gain-dominated copy-number landscape, including recurrent chr17q gain, whereas canonical adult high-risk features, such as chr1p loss, lack prognostic significance. Adult-derived prognostic frameworks, including WHO grade,
