Case Report: Pulsed electric field treatment of metastatic liver disease demonstrating a possible abscopal response.

Adrenocortical carcinoma (ACC) is a rare and aggressive endocrine malignancy, typically presenting at an advanced stage with distant metastases. We report a unique case of a 49-year-old female with metastatic ACC in the liver who exhibited an abscopal effect following pulse electric field (PEF) ablation. The patient initially presented with hypertension, male-patterned hair loss, voice hoarseness, and weight gain and was subsequently diagnosed with a large left adrenal mass along with liver and pulmonary metastases. Following surgical interventions-including left salpingo-oophorectomy, hepatic

Primary epithelioid angiosarcoma of thyroid: A case report and review of literature.

Epithelioid angiosarcoma (EA) is a malignant tumor of endothelium origin that most commonly arises in the deep soft tissues of extremities but may occasionally be primary in skin, adrenal gland, and bone. A 72-year-old male presented with a painless enlargement of his thyroid for more than 10 days before hospitalization. A walnut-sized mass in the right thyroid was found simultaneously by palpation and Color Doppler Ultrasound. After a total thyroidectomy was performed, a mass with a size of 4.5 cm × 3.5 cm was found at the lower pole of the right thyroid gland. Histologically, the tumor was d

Differentiation of Fat-poor and Atypical Adrenal Adenomas from Metastases: MRI-based Radiomic, Radiologic, and Radiomic-radiologic Machine Learning Models.

INTRODUCTION: The accurate differentiation of fat-poor and atypical adrenal adenomas from metastases remains a diagnostic challenge. This study aimed to evaluate the predictive value of MRI-based radiomic, radiologic, and combined radiomic-radiologic machine learning (ML) models. METHODS: This single-center retrospective study included 37 patients with 44 adrenal masses (19 adenomas; 25 metastases). Data were split into training and testing sets (2:1). To expand the training set, data augmentation was performed by multiple sampling (56 labeled slices from 30 masses). Radiomic features were ex

Differentiation of adrenal metastases and adenomas based on clinical characteristics, deep learning features, and radiomics features derived from ultrasound imaging.

BACKGROUND: Accurate diagnosis of adrenal incidentalomas is crucial in patients with extra-adrenal malignant tumors. This study aims to develop a nomogram integrating clinical features, deep learning-derived imaging features, and ultrasound radiomics characteristics to distinguish adrenal metastases from adrenal adenomas. METHODS: A retrospective analysis was conducted on 449 cases, including 228 cases of adrenal metastases and 221 cases of adrenal adenomas, divided into training and testing cohorts at a 7:3 ratio. Patient clinical data and ultrasonographic images were collected, with regions

BUB1 as a candidate non-oncogene addiction vulnerability in metastatic phaeochromocytoma/paraganglioma.

Phaeochromocytomas and paragangliomas (PPGLs) are rare neuroendocrine tumours with limited treatment options in the metastatic setting. We performed integrative transcriptomic analyses of TCGA and COMETE datasets and identified BUB1, a mitotic checkpoint kinase and prototypical non-oncogene addiction (NOA) factor, as significantly upregulated in metastatic PPGLs. We validated BUB1 overexpression in primary tumour tissues using RT-qPCR and subsequently conducted functional studies in the human phaeochromocytoma-derived hPheo1 cell line. Genetic silencing of BUB1 or its pharmacological inhibitio

Approach to the patient with metastatic pheochromocytoma and paraganglioma: advances in systemic therapy.

Pheochromocytomas and paragangliomas (PPGLs) are rare tumors of the adrenal and extra-adrenal chromaffin cells. Although most PPGLs remain localized, approximately 25% develop metastatic disease (MPPGL), leading to substantial morbidity due to tumor burden and catecholamine excess. These tumors have a heterogeneous biologic activity, with some having an aggressive course requiring intensive treatment, and others behaving indolently, not requiring treatment over many years. Because there are no sufficiently accurate predictors of future behavior, all PPGLs are considered to have the potential f

Gastric Metastasis from Small Cell Lung Cancer: An Unusual Site of Metastatic Spread: A Case Report.

Background Small Cell Lung Cancer (SCLC) is a lung neuroendocrine tumor with high invasiveness and early extensive metastasis. The most common metastatic sites of SCLC include the brain, bone, liver, and adrenal gland, but gastric metastasis is uncommon. We report a case of SCLC metastasizing to the stomach and discuss its imaging findings, in order to enhance clinical understanding and emphasize the key role of imaging in identifying such uncommon metastasis. Case Presentation The patient was a 70-year-old male with no smoking history and was hospitalized due to dizziness and fatigue. Cranial

Mechanisms and therapeutic targets in distant metastasis of lung adenocarcinoma.

Distant metastasis, characterized by organotropism, is a major cause of mortality in lung adenocarcinoma (LUAD). In this study, digital spatial profiling (DSP), multiplex immunofluorescence (mIF), and clinical data from 52 LUAD patients were integrated to develop organ-specific metastasis risk models, and the molecular mechanisms underlying metastatic organotropism were investigated. Random forest models based on primary tumor spatial transcriptomics accurately predicted metastasis to the brain (AUC = 0.974), liver (AUC = 0.975), adrenal gland (AUC = 0.929), and bone (AUC = 0.907). Adult adrenal ganglioneuroblastoma with an extensive metastasis of the skull: case report and review of the literature.

BACKGROUND: Ganglioneuroblastoma (GNB) is a malignant neoplasm of the autonomic nervous system on the spectrum of peripheral neuroblastic tumors (PNT), exceedingly rare in adults with only a limited number of cases documented in literature. CASE PRESENTATION: We present the case of a 57-year-old male who was found to have a large nodular lesion in the left adrenal gland, which was later confirmed as a ganglioneuroblastoma following adrenalectomy, with multiple skeletal metastases and, most notably, a large, growing and destructive metastasis in the right frontoparietal region of the skull wit

Safety and Survival Outcomes of Neoadjuvant Chemoradiotherapy Combined with Immunotherapy in Locally Advanced Esophageal Squamous Cell Carcinoma: A Multicenter Real-World Analysis (NEO-EC-01).

BACKGROUND: Neoadjuvant chemoradiotherapy (nCRT) is the standard treatment for patients with locally advanced esophageal squamous cell carcinoma (LA-ESCC), although perioperative immunotherapy has shown considerable promise. The integration of immunotherapy with nCRT (nICRT) is an innovative treatment paradigm and early-phase studies have reported enhanced pathological complete response (pCR) rates. However, comprehensive evaluations of safety, long-term prognosis, and recurrence patterns are lacking. In this multicenter real-world study, the clinical efficacy, recurrence patterns, and potenti

Outcomes and opportunities: evaluating an unselected cohort of patients with adrenal incidentaloma at a tertiary hospital in New Zealand.

BACKGROUND: The prevalence of adrenal incidentalomas is increasing, along with the incidence of functional or malignant lesions. This study assessed how imaging of adrenal incidentalomas at a tertiary hospital in Auckland, New Zealand, without a dedicated adrenal incidentaloma service, conformed to the American College of Radiology (ACR) recommendations for radiological screening, as well as how using a non-washout imaging approach affected clinical and service outcomes. Secondary outcomes included determining how biochemical screening conformed to the European Society of Endocrinology (ESE) g

Safety of biopsy in phaeochromocytoma and paraganglioma: an international, multicentre, retrospective cohort study.

BACKGROUND: Biopsy of phaeochromocytomas and paragangliomas (PPGLs) is discouraged due to the perceived risk of catecholamine-related complications. However, a systematic review showed that this recommendation is primarily supported by case reports. We aimed to assess the safety of percutaneous biopsy in patients with PPGLs. METHODS: This international, multicentre, retrospective study included patients of any age with PPGLs referred to participating centres who had had percutaneous core-needle biopsy or fine-needle aspiration. Biopsies of head and neck paragangliomas were excluded. Participa

Efficacy of iodine-131-metaiodobenzylguanidine therapy for multiple endocrine neoplasia type 2A-associated metastatic pheochromocytoma: case report and literature review.

Malignant transformation in multiple endocrine neoplasia type 2 (MEN2)-associated pheochromocytoma is rare, and there have been few reports of treatment with 131I-metaiodobenzylguanidine (MIBG) therapy. Here, we describe a 47-year-old woman diagnosed as having bilateral pheochromocytoma with medullary thyroid carcinoma. Genetic testing confirmed the MEN2A subtype with the pathogenic variant p.C634Y in the RET oncogene. She underwent total thyroidectomy due to medullary thyroid carcinoma with elevated calcitonin levels as high as 1,380 pg/mL, classified as pT2N0M0, stage I. Plasma noradrenaline

Focal False-Positive 18 F-MFBG Uptake in the Kidney Attributable to Renal Infarction in a Pediatric Patient With Pheochromocytoma.

A 10-year-old boy was enrolled in our clinical trial. He underwent 18 F-MFBG PET/CT imaging for the differential diagnosis of a right adrenal lesion, which revealed moderate 18 F-MFBG uptake in the lesion with a central photopenic defect. Concurrently, focal increased 18 F-MFBG uptake was observed in the lower pole of the right kidney. Retrospective review of prior abdominal contrast-enhanced CT demonstrated imaging features consistent with renal infarction. Surgical pathology confirmed a right adrenal pheochromocytoma. This case illustrates that focal renal 18 F-MFBG uptake may arise secondar

An unexpected finding of apparent bilateral aldosterone suppression after superselective adrenal venous sampling: a case report.

INTRODUCTION: In adrenal venous sampling (AVS) for patients with primary aldosteronism (PA), apparent bilateral aldosterone suppression (ABAS) is occasionally experienced, but the underlying causes remain poorly understood. CASE PRESENTATION: A 40-year-old Chinese man with a right adrenal adenoma presented with hypertension and refractory hypokalaemia. Initial AVS revealed no evidence of unilateral aldosterone excess and demonstrated ABAS. Six months later, repeat AVS was performed, during which two distinct venographic patterns-a 'triangular pattern' and a 'delta pattern'-were observed in th

Preoperative Risk Assessment of Adrenal Metastases in a Multicenter Study: Development of a Robust Federated Learning Model.

RATIONALE AND OBJECTIVES: In clinical practice, the preoperative risk assessment of adrenal metastases versus benign adrenal lesions carries a substantial risk of misdiagnosis. The artificial intelligence technology holds promise for reducing misdiagnosis rates. However, due to the problem of data privacy protection and non-independent and identically distributed of multi-center data, the performance of artificial intelligence models is significantly affected. MATERIALS AND METHODS: We retrospectively collected 1187 adrenal lesions from 1100 patients who underwent three phase Computed tomogra
