Multi-omics insights into metastatic pheochromocytomas and paragangliomas: Mechanisms, signatures, and prediction models.

Pheochromocytomas and paragangliomas (PPGLs), rare neuroendocrine tumors, have been generally considered, since the 2017 WHO classification, as malignant neoplasms with various metastatic potential. Approximately 10-25% of PPGLs will develop metastases, with a varying 5-year-overall-survial of 40%-77%. Previous studies have extensively elucidated the mechanisms by which SDHB mutations promote tumor metastasis through inducing the pseudohypoxic and hypermethylation phenotype. Recent multi-omics studies further profiled the additional landscape of metastatic PPGLs, involving ATRX/TERT alteration

Tumor stage-dependent expression of autophagy proteins in adrenocortical carcinoma.

INTRODUCTION: Adrenocortical carcinoma (ACC) is a rare endocrine malignancy with poor yet heterogeneous prognosis, mostly due to its complex and incompletely understood molecular background. Autophagy is a multi-step catabolic process with a dual role in tumorigenesis, acting either as a tumor suppressor or promoter in a context-dependent manner. Therefore, our aim was to investigate the expression of key proteins involved in different autophagy steps in both adrenocortical adenomas (ACA) and ACC, to further understand the role of autophagy in this type of tumors, particularly its involvement

Hypokalaemic paralysis as a presenting feature of primary aldosteronism.

A woman in her 30s presented with acute flaccid paraparesis secondary to severe hypokalaemia. She had a known history of hypertension controlled on amlodipine and bisoprolol. Biochemical workup revealed metabolic alkalosis, renal potassium loss, suppressed plasma renin activity and markedly elevated aldosterone levels. CT imaging identified a left adrenal adenoma consistent with an aldosterone-producing adenoma. The patient underwent laparoscopic adrenalectomy, after which she no longer required antihypertensive medication and maintained normal serum potassium. This case highlights a reversibl

Diagnostic accuracy of third-generation dual-source dual-energy CT virtual non-contrast vs true non-contrast imaging in adrenal gland masses: a comparative study following the ACR guidelines.

PURPOSE: To assess whether virtual non-contrast (VNC) images are diagnostically equivalent to true non-contrast (TNC) images in differentiating adrenal adenomas from metastases. MATERIALS AND METHOD: Consecutive oncologic patients who underwent staging CT examinations performed on a third-generation dual-source CT scanner between January 2016 and February 2023 with adrenal lesion with histopathologic or MRI confirmation were retrospectively included. Adrenal lesion attenuation values were measured on TNC and VNC reconstructions and directly compared. Subsequently, the diagnostic accuracy of b

Radiotherapy for Adrenal Metastases from Hepatocellular Carcinoma: A 20-Year Bi-Institutional Experience.

BACKGROUND: To evaluate tumor response, local control (LC), survival, and toxicity after radiotherapy for adrenal metastases from hepatocellular carcinoma (HCC), we conducted a retrospective, bi-institutional study spanning two decades. METHODS: Twenty patients received radiotherapy for adrenal metastases (2005-2025). Techniques included three-dimensional conformal radiotherapy, intensity-modulated radiotherapy, stereotactic body radiotherapy, and MR-guided radiotherapy. Tumor response was assessed using Response Evaluation Criteria in Solid Tumors version 1.1. LC, overall survival (OS), and

Study protocol: targeted delivery of interleukin-12 in combination with hepatic artery infusion pump therapy for patients with adrenocortical carcinoma liver metastases.

BACKGROUND: Adrenocortical carcinoma (ACC) is a rare, aggressive malignancy often detected at an advanced stage with limited effective treatment options beyond first-line chemotherapy. The liver is a common site of metastasis, and patients frequently present with substantial hepatic disease burden that precludes complete resection. Locoregional therapies, including hepatic artery infusion pump (HAIP) delivery of floxuridine, have not been thoroughly evaluated for treatment of ACC liver metastasis, likely due to its rare incidence and highly aggressive nature. Habitat-radiomics combining multichannel 2.5D deep learning for differentiating adrenal adenomas from metastases using automatic segmentation: a multicenter study.

BACKGROUND: The qualitative diagnosis of lipid-poor adrenal adenomas and metastases presents challenges, yet there is a significant difference in their treatment principles and prognosis. MATERIALS AND METHODS: A total of 390 patients from two hospitals were divided into training, internal validation, and external test sets. Lesion segmentation was performed automatically using the large segmentation model Medical SAM. The 2.5D deep learning model was constructed using the DenseNet-121 architecture. Habitat-radiomics employed the K-means clustering algorithm; both habitat-radiomics and conven

Macroscopic venous tumour thrombus in paediatric adrenocortical tumours.

INTRODUCTION: Macroscopic venous tumour thrombus is recognized in adult adrenocortical tumours (ACT), but its incidence, surgical implications, and prognostic impact in children remain poorly defined. MATERIALS AND METHODS: We retrospectively analysed prospectively collected registry data from 104 children and adolescents with adrenocortical carcinoma or tumours of uncertain malignant potential treated between 1997 and 2025. Patients were classified by vascular status: no invasion, microscopic invasion, macroscopic venous tumour thrombus, or unresectable disease. The study was non-randomized;

Adrenocortical oncocytoma manifesting as primary aldosteronism: a case report.

BACKGROUND: Adrenocortical oncocytomas are rare adrenal neoplasms and are most often reported as non-functioning adenomas. A minority, however, exhibit steroid hormone hypersecretion and may present with Cushing's syndrome, pheochromocytoma-like syndromes, feminisation in males or virilization in females. Adrenocortical oncocytoma presenting with primary aldosteronism is an exceptionally uncommon entity. CASE PRESENTATION: We report a 44-year-old man with severe hypertension and hypokalaemia, accompanied by a right adrenal lesion on computed tomography. A comprehensive renin-angiotensin-aldos

Metabolic splicing subtypes elucidate metastatic trajectories and clinical biomarkers in adrenocortical carcinoma.

BACKGROUND AND OBJECTIVE: Adrenocortical carcinoma (ACC) is a rare but highly aggressive malignancy, and alternative splicing (AS) serves as a key post-transcriptional regulatory mechanism involved in its malignant progression. A prominent feature of ACC is metabolic reprogramming, which is closely associated with poor prognosis. However, the mechanism by which AS mediates metabolic abnormalities to promote ACC invasion and metastasis remains unclear. METHODS: We identified three splicing subtypes based on metabolism-related splicing events, which were derived by correlating metabolic flux sc

Endocrine hypertension: the role of imaging in diagnosis and management.

Endocrine hypertension is an uncommon but treatable cause of secondary hypertension. It results from excessive hormone production by the endocrine glands or due to ectopic hormone production. The causes of abnormal hormonal production can be congenital or acquired. Specific syndromes can also predispose to the development of endocrine hypertension. Extensive catecholamine production can occur due to pheochromocytomas and paragangliomas (PPGLs). Excessive aldosterone secretion by the adrenal cortex commonly occurs due to idiopathic (bilateral) adrenal hyperplasia or aldosterone-producing adrena

Bilateral giant myelolipomas in a female patient with untreated 21-hydroxylase deficiency.

Adrenal myelolipomas are benign neoplasms of the adrenal gland composed of mature adrenal and adipose tissue as well as myeloid elements. Although infrequent in the general population, their prevalence is markedly increased in patients with poorly controlled classical congenital adrenal hyperplasia (CAH), most likely attributable to chronic ACTH-mediated adrenal stimulation. We report the case of a 59-year-old female with a history of classic CAH due to 21-hydroxylase deficiency who did not receive treatment for over 30 years and had developed bilateral giant myelolipomas (left side: 24.5 cm ×

Metastatic Disease in Phaeochromocytomas and Paragangliomas in Various Genotypes-A Systematic Review and Meta-analysis.

BACKGROUND: Paragangliomas and phaeochromocytomas (PPGLs) are rare neuroendocrine tumours with 10% to 20% of patients developing metastatic disease. The tumors exhibit a high degree of heritability, and pathogenic genetic variants have been associated with metastases. OBJECTIVE: We aimed to investigate the association between the genotype of PPGL patients and their risk of metastatic disease, adjusting for time. METHODS: A systematic search was conducted in PubMed and Embase. The primary outcome was the rate of metastatic disease per 100 followed patient-years analyzed through meta-analyses.

Management Approach of Adrenal Incidentaloma.

BACKGROUND: The prevalence of adrenal incidentalomas is increasing substantially with cross-sectional imaging, with reported prevalence from 1% to 5% on computed tomography and up to 8.7% at autopsy. While the majority of incidentalomas are benign nonfunctional adenomas, clinicians face the critical challenge of identifying the rare adrenocortical carcinoma (0.5-2 per million annually), which carries an extremely poor prognosis with 5-year survival of only 50% and <15% if metastatic. Functioning adrenal tumors occur in 27.5% of incidentalomas, including mild autonomous cortisol secretion (MACS

A Rare Presentation of Tuberculosis: Dilated Cardiomyopathy Secondary to Myocarditis with a Dual-secretory Adrenal Pheochromocytoma.

We describe an unusual case involving a 25-year-old male who presented with breathlessness, cough, chest pain, abdominal pain, and significant weight loss. Detailed evaluation revealed pulmonary tuberculosis, an adrenal mass with biochemical evidence of a dual-secretory pheochromocytoma and severe dilated cardiomyopathy with profound left ventricular dysfunction. The coexistence of these conditions is extremely rare and highlights several clinical challenges - both in diagnosis and management. This case report emphasizes the importance of considering uncommon etiologies when confronted with mu

Residential radon in Peru: challenges for health surveillance and lung cancer prevention.

The Journal of Genetics, started by William Bateson in 1910, played a distinguished role in the early years of genetics. However, it stopped publishing in 1978. The Indian Academy of Sciences revived it in 1985, and has published it regularly since then. To commemorate this landmark, I highlight one of the 17 articles published that year.
