Adenocarcinoma of unknown primary origin (ACUP) is a common oncologic problem for which there is no standard therapy. Forty-two patients with metastatic tumor were identified as having ACUP after extensive evaluation failed to reveal a primary site of disease. They were treated with an investigational chemotherapy regimen consisting of vindesine and doxorubicin. Of the 38 evaluable patients, six (16%) had major responses to chemotherapy. Using the data accumulated in the Osaka Cancer Registry, we conducted a study on the effect of radiotherapy for cervical cancer in causing second primary cancers. Two groups of patients with cervical cancer, 1,767 who were given radiotherapy and 1,377 who were not, were followed for 7-9 years on average. 34 patients suffering from a metastatic cervical neoplasm with unknown origin of the primary tumour were examined between 1977 and 1981. The site of the primary tumour was detected in 61% (= 21 cases) of the patients. In the literature (4158 cases, 61 authors) a 32% success rate only was stated. Diagnostic improvement was achieved via "panendoscopy". This method allows a complete systematic evaluation of the upper respiratory and gastrointestinal tract.

Among 120 cases of cerebral metastases, 62 primary sites were found. A retrospective review of 245 patient with metastatic adenocarcinomas of unknown primary site (ACUP) seen at The Johns Hopkins Hospital from 1965--1979 was undertaken. Twelve prognostic features of melanoma were examined in a series of 185 patients with nodal metastases (Stage II), who underwent surgical treatment at our institution during the past 20 years. A study of 48 patients with metastatic carcinoma in the neck from an unknown primary site has revealed several facts. Among patients with squamous cell carcinoma, the three-year survival rate was 40%. The clinical course of melanoma of the skin diagnosed during pregnancy was compared with a control population of women with melanoma that occurred during the childbearing years, as recorded by the Connecticut Tumor Registry. In order to determine the natural history and results of treatment of intracerebral metastases in solid-tumor patients, the records of 191 patients with an antemortem diagnosis of intracerebral metastasis made during the period from August 1974 to November 1978 were reviewed.
