High-dose chemotherapy followed by autologous stem-cell transplantation versus non-myeloablative consolidation in primary CNS lymphoma (MATRix/IELSG43): a randomised phase 3 trial.

BACKGROUND: Consolidation therapy for primary CNS lymphoma (PCNSL) includes high-dose chemotherapy with autologous stem-cell transplantation (HCT-ASCT), yet its efficacy compared with non-myeloablative chemoimmunotherapy remains uncertain. This study aimed to provide randomised comparative evidence on the efficacy and safety of thiotepa-based HCT-ASCT versus non-myeloablative R-DeVIC consolidation after uniform MATRix induction in newly diagnosed PCNSL. METHODS: This open-label, randomised, phase 3 trial was conducted across 56 university and academic non-university hospitals with established

[Recent advances in the treatment of chronic myeloid leukemia].

The introduction of tyrosine kinase inhibitors (TKIs) has substantially improved outcomes for chronic-phase chronic myeloid leukemia (CML-CP). Furthermore, the development of ABL myristoyl pocket inhibitors that target specific molecular pathways has enabled adoption of treatment strategies with fewer adverse events. However, the clinical significance of somatic mutations in genes other than ABL1 in treatment resistance is attracting increasing attention. Additionally, novel TKIs and STAMP inhibitors are under development, and are expected to further improve CML management. This article summar

Acquired Pseudo-Pelger-Huet anomaly in a patient with chronic lymphocytic leukemia treated with acalabrutinib.

We report the case of an 82-year-old male acalabrutinib-treated patient with chronic lymphocytic leukemia (CLL) who was admitted to hospital because of a crural ulceration. Examination of a peripheral blood smear revealed 81% of the neutrophils as hypolobulated in addition to the leukemic lymphoma- and smudge cells. The nuclei of the abnormal neutrophils showed a markedly clumped chromatin and appeared as unilobular (round or oval) in 51%, bilobular in 13% and peanut- and band-shaped in 17%, respectively. A suspected diagnosis of a drug-induced acquired Pseudo-Pelger-Huet anomaly (PPHA) was ma

SOHO State of the Art Updates and Next Questions | Beyond Romidepsin: Novel Therapeutic Targets and Emerging Strategies for Relapsed and Refractory Peripheral T-Cell Lymphoma.

Peripheral T-cell lymphomas (PTCLs) represent heterogeneous aggressive non-Hodgkin lymphomas with dismal outcomes in the relapsed/refractory (R/R) setting. Following romidepsin's withdrawal from the R/R PTCL indication in 2021, the only FDA-approved agents are belinostat and pralatrexate, and Brentuximab Vedotin. Median overall survival following relapse is approximately 5.8 months, highlighting the urgent need for effective therapies. Novel agents targeting distinct molecular pathways demonstrate promising activity. PI3K inhibitors, particularly duvelisib, achieved an ORR of 48% (complete res

Degron-independent recruitment of KAT2A expands the target space of CRBN molecular glues.

Lysine acetyltransferases (KATs) cooperate with oncogenes such as c-Myc, estrogen receptor, and lysine methyltransferase 2A (KMT2A) fusions to sustain malignant programs. Targeting of KAT proteins has shown clinical efficacy; however, achieving homolog selectivity for most KATs remains a major challenge. By extending cereblon (CRBN)-based molecular glues beyond the canonical degron space, we developed an exquisitely selective degrader of KAT2A. Cryo-electron microscopy revealed that CRBN recruits KAT2A independently of a degron; instead, the molecular glue engages a surface-exposed tyrosine, m

Gastric Antral Vascular Ectasia in a Patient with Chronic Myeloid Leukemia on Imatinib Treatment: A Rare Case Report.

Imatinib is the standard first-line therapy for chronic myeloid leukemia (CML) and is generally well tolerated. Gastrointestinal adverse effects are common; however, gastric antral vascular ectasia (GAVE) is an extremely rare complication. We report a 77-year-old woman with chronic-phase CML who developed severe upper gastrointestinal bleeding 1 month after the initiation of imatinib therapy. Endoscopy revealed classic features of GAVE, and no alternative etiology was identified. Discontinuation of imatinib resulted in complete clinical and endoscopic resolution. Clinicians should consider thi

Isolated bilateral breast myeloid sarcoma: A rare case report.

Myeloid sarcoma (MS) is a rare extramedullary tumor composed of immature myeloid cells that is most associated with acute myeloid leukemia (AML), whereas isolated bilateral breast involvement without bone marrow disease is exceptionally uncommon. We report the case of a 38-year-old woman who presented with bilateral breast masses, both classified as BI-RADS 5 on imaging. Ultrasound-guided core needle biopsies of both breast lesions established the diagnosis of myeloid sarcoma, with immunohistochemical positivity for MPO, CD34, CD56, and CD117. Peripheral blood and bone marrow examinations show

Genetic Diversity of the BLV env Gene and gp51 Mutations in Genotypes G4 and G7 Circulating in Dairy Cattle in the Novosibirsk Region (Western Siberia, Russia).

Bovine leukemia virus (BLV) is an oncogenic retrovirus and the etiological agent of enzootic bovine leukosis (EBL), which is spread worldwide. This study presents data on the genetic diversity of BLV in the Novosibirsk region of Russia. ELISA-positive samples were selected from six districts of the Novosibirsk region (Dovolnoye, Barabinsk, Tatarsk, Toguchin, Bolotnoye, and Kochenyovo districts). To assess the diversity of circulating BLV genotypes, samples were collected from settlements and districts that were geographically distant from each other and had no shared pasture lands. In total, 1

Pediatric Acral Site Cutaneous T-Cell Lymphoma Imitating Acromegaly.

Cutaneous T-cell lymphoma (CTCL) is an infrequent non-Hodgkin lymphoma of the skin, with mycosis fungoides (MF) as the most common subtype. MF in palms and soles is an exceptionally rare presentation, particularly in pediatric patients. We report a case of a 17-year-old boy presenting with progressive enlargement of his hands and feet with thickening of palms and soles, initially suspected to have acromegaly. Laboratory and imaging workup ruled out endocrine and genetic causes of acromegaly. A skin biopsy confirmed a diagnosis of MF. This case illustrates the diagnostic challenges of pediatric

Ponatinib in CML and Ph+ ALL: balancing high efficacy with vascular safety.

Chronic myeloid leukemia (CML) and Philadelphia chromosome-positive acute lymphoblastic leukemia (Ph+ ALL) are driven by the BCR:ABL1 fusion and remain highly responsive to tyrosine kinase inhibitors (TKIs). Ponatinib, a third-generation TKI, uniquely retains activity against the T315I 'gatekeeper' mutation and most other BCR:ABL1 variants, producing deep cytogenetic and molecular responses in resistant CML and improving survival in Ph+ ALL. Across the PACE and OPTIC programs, frontline Ph+ ALL combinations (hyper-CVAD- and blinatumomab-based), and real-world cohorts, ponatinib demonstrates ro

Patient-Centric First-in-Human Dose Selection: An Ex Vivo Minimal Anticipated Biological Effect Level Approach for T-Cell Engagers in Multiple Myeloma.

The minimal anticipated biological effect level (MABEL) approach to first-in-human (FIH) dose calculation can lead to long dose-escalation periods before observing clinical activity. We used a novel ex vivo MABEL approach to calculate the FIH starting dose for forimtamig, a T-cell bispecific antibody under investigation for the treatment of relapsed/refractory multiple myeloma (RRMM). We used in vitro and ex vivo MABEL approaches to calculate the FIH dose for a Phase 1 trial (NCT04557150) in patients with RRMM. In vitro assays used the OPM-2 MM cell line and peripheral blood mononucleocytes fr

Cutaneous involvement by myeloid leukaemias: challenging cases with important implications.

Myeloid leukaemias and precursors involving the skin are often challenging to diagnose with varied and overlapping clinical and histopathological features. Nonetheless, they are important for pathologists to recognize as they have significant implications for patient care. This article discusses cutaneous involvement by myelodysplastic syndrome (myelodysplasia cutis), acute myeloid leukaemia (myeloid sarcoma), and blastic plasmacytoid dendritic cell neoplasm. For each entity, the clinical and histopathological features are reviewed, along with key genetic features and a concise discussion of e

Arterial stiffness in patients with chronic myeloid leukemia receiving tyrosine kinase inhibitors: An exploratory prospective observational study.

Arterial stiffness, measured by pulse wave velocity (PWV), is a validated surrogate of vascular damage and CV risk, but evidence on TKI-specific patterns of arterial stiffness and their prognostic relevance in chronic myeloid leukemia (CML) is limited. We primarily evaluated the longitudinal association between TKI type and arterial stiffness, and secondarily explored the association between arterial stiffness and CV adverse events (CVAEs). In this prospective cohort study, 69 patients with CML receiving imatinib (n = 30), dasatinib (n = 18), or nilotinib (n = 21) underwent non-invasive assess

Preparation, characterization and application of feline CD3-specific single-domain antibodies.

Feline lymphoma, a prevalent feline malignancy, responds poorly to conventional chemotherapy-a setting that underscores the urgent need for targeted therapeutic alternatives. The stable expression of CD3 on T-cell lymphoma cells positions this molecule as an ideal immunotherapeutic target. Compared with conventional antibodies, single-domain nanobodies derived from camelid heavy-chain antibodies offer distinct advantages, including smaller size, enhanced tissue penetration, and superior stability, thereby rendering CD3-targeted nanobodies particularly promising for managing feline T-cell lymph

Advancing mediastinal sampling in non-metastatic lymphadenopathy.

Despite advances in PET/CT, tissue confirmation remains essential for mediastinal lymphadenopathy. EBUS-TBNA is minimally invasive but limited by cytology, particularly in benign disease and lymphoma. Emerging EBUS-guided techniques, notably transbronchial mediastinal cryobiopsy, improve diagnostic yield where tissue architecture is required. The multicenter randomized study1 strengthens evidence for its added value, while underscoring the need for selective application and standardized, indication-specific trials.

Secondary peripheral T-cell lymphoma in a patient with DLBCL harboring BLM mutation following CD19/CD22 bispecific CAR-T cell therapy.

We describe a case of secondary peripheral T-cell lymphoma (PTCL) arising from infused CD19/CD22-4-1BB-CD3ζ-lenti bispecific chimeric antigen receptor (CAR)-T cells in a patient with relapsed or refractory B-cell lymphoma who carried a pathogenic germline BLM p.L107Ffs*36 variant. Integrated genomic and molecular analyses confirmed that the PTCL was clonally derived from the infused CAR-T product and revealed a multistep oncogenic process involving pre-existing driver mutations (BLM), somatic TET2 mutations (p.R1261H and p.E1755*), and genomic instability. Clonal CAR vector integration events
